RESUMO

Objective:To investigate the effect of an improved hydro-colloid dressing sticking method on preventing protective equipment induced facial pressure injuries among medical staff through a prospective, single-arm trial.Methods:According to the size of the edge of N95 mask, four pieces of hydro-colloid dressing with a size of 2 cm×5 cm were cut, and the edges of the mask were tension-free stuck. This improved sticking method was applied to the nurses working in the isolation ward. The follow-up was continued for two weeks, skin condition was reported daily, and the occurrence of pressure ulcers was analyzed after two weeks of follow-up.Results:Twenty percent (4/20) of subjects experienced increase grades of nasal and twenty five percent (5/20) in cheek pressure ulcers during a two-week follow-up. The incidence of the increased stage of cheek pressure ulcer in normal/dry skin was higher than in oily skin. Stage 2 pressure ulcers occurred in only five percent (1/20) of subjects, and no subjects had stage 3 or higher stage pressure ulcers. The incidence of increased cheek pressure ulcer grade in patients with neutral / dry skin was (5/11), higher than 0 in patients with oily skin ( χ2 value was 5.378, P = 0.02). The stage of nasal pressure ulcer was positively correlated with the disappearance time of the indentation of protective equipment ( r value was 0.615, P=0.004). The rate of increase in nasal pressure sore grade was higher in subjects who required more than 120 minutes of indentation recovery time (2/8) than in subjects who required 30 to 60 minutes 1/5 ( χ2 value was 4.382, P<0.05) and 60-120 minutes 1/5 ( χ2 value was 6.472, P<0.05). Conclusions:This improved hydro-colloid dressing sticking method can effectively prevent pressure injuries related to wearing N95 masks for a long time. However, skin evaluation of medical staff should be done properly, moisture protection for normal/dry skin should be emphasized, and skin blood flow recovery after compression should be strengthened. This method can reduce the risk of infection of medical staff, and it is worthy of clinical application.

OBJECTIVE:To evaluate the economics of alendronate sodium preventing osteoporotic fractures in postmenopausal women,and to provide reference for osteoporotic and osteopenia patients to select economical and effective therapy plan. METH-ODS:Using Markov economic model,TreeAge Pro 2011 software was used for cost-utility analysis using RCTs literature data of basic medication(calcium and vitamin D3)and basic medication combined with alendronate sodium as baseline data. Using quality adjusted life years(QALYs)and incremental cost utility ratio(ICUR)as economics evaluation indexes,the stability of established model was investigate by single factor sensitivity analysis. RESULTS:QALYs of the basic treatment group was 0.704 years,and QALYs of drug combination group was only 0.708 years. However,ICUR of two groups reached 714252.44. For the late period of hip fracture and other fractures,drug combination group was in high cost and low benefit,obviously had no economic advan-tage. For vertebral fractures,the cumulative probability of fracture in drug combination group was significantly lower than basic treatment group,and ICUR of the two groups was only 13902.17,with economic advantage. For hip fracture,the cumulative prob-ability of fracture in basic treatment group was the same as drug combination group,and ICUR was 19109.00,with economic ad-vantage. For wrist fractures,curative effect and economy needed further study. CONCLUSIONS:For the low-risk population with osteoporotic fractures,calcium and vitamin D3 are more economical in preventing osteoporosis fractures. For the high-risk popula-tion with vertebral and hip fractures,alendronate sodium combined with basic treatment is effective and economical.

To observe the potential protective effect of angiopoietin-1 [Ang-1] on rat choroidal neovascularization [CNV] leakage. The study was conducted at the Eye Institute of Shandong University of Traditional Chinese Medicine, Jinan, China from June 2012 to June 2013. Thirty CNV model rats were induced by laser. In vivo, fluorescein fundus angiography and pathological techniques were applied to detect the effect of vascular endothelial growth factor [VEGF] and Ang-1 intravitreous injection. In vitro, 3-[4, 5-dimethylthiazole-2-yl]-2, 5-biphenyl tetrazolium bromide [MTT] assay was applied to detect the proliferation of cultured bovine retinal endothelial cells [BRECs] after treatment with VEGF and Ang-1. Transmission electron microscopy [TEM] was used to detect the morphological changes under VEGF and Ang-1. In the CNV rat model, less late leakage was found in the Ang-1 group than the vehicle control or the VEGF group. The MTT assay showed Ang-1 administration inhibited the proliferation of BRECs. The VEGF promoted proliferation at low concentrations and inhibited the proliferation when its concentration reached 50 ng/ml. The administration of VEGF+Ang-1 rescued the inhibition effect of Ang-1 alone. The TEM results showed that there were less intercellular junctions in the VEGF group compared with the vehicle control. In the VEGF + Ang-1 group, the intercellular junctions were nearly normal. The Ang-1 can induce intercellular junction formation and decrease the CNV leakage

RESUMO

Objective The observation of curative effect and nursing for micro-penetration heating treating wound arms and legs fracture after the operation. Methods 76 cases of the patients suffered from wound arms and legs fracture are divided into 38 cases as the observation group and 38 cases the comparison group at random. The comparison group is treated by normal method only and the observation group is started to be treated by micro-penetration heating treating device for the ray based on the normal method only the second day after operation. Results The observation group is superior to the comparison group in pain situation, wounded infection rate and coalescence time totally (P<0.05). Conclusion The micro-penetration heating treatment can relieve the pain of wound bone fracture to prevent from wounded infection, coalescence to shorten the time of patients' treatment in hospital so that it has a very consequence for it to relieve press from the psychology and finance.

Objective To investigate the clinical effect of hyaluric acid sodium injection and functional exercises on patients with comminuted fracture of tibia plateau after reposition. Methods 134 patients with comminuted fracture of tibia plateau after reposition were randomized into the observation group and the control group with 67 cases in each group. The observation group received 2ml of hyaluric acidsodium injection into the knee joint cavity with 1% lidocaine once a week and 3 times totally. At the same time active and passive functional exercises were applied. The control group was only given routine nursing. The knee function was evaluated by Lysholm Scale 4 months after operation and the results were compared. Results The score of the observation group was higher than that of the control group(P<0.01).Conclusion The hyaluric acid sodium injection and functional exercises could improve the healing of joint cartilage and contribute to the rehabilitation of joint function in patients with comminuted fracture of tibia plateau after reposition.
