RESUMO

Objective To investigate the current situation and related risk factors of healthcare-associated infection (HAI),so as to provide evidence for making prevention and control measures of HAI.Methods On November 26, 2014,a combination method of bedside visiting and medical record reviewing was adopted to survey HAI status, pathogen examination,and antimicrobial application in all hospitalized patients in a tertiary first class military hospi-tal.Results A total of 1 657 hospitalized patients were investigated,66 patients developed 71 times of HAI,HAI rate and HAI case rate were 3.98% and 4.28% respectively.The top 4 departments with HAI prevalence rates were departments of neurosurgery (24.49%),hematology(19.05%),cadre ward(13.73%),and burn surgery (10.91 %).The top 5 HAI sites were lower respiratory tract (40.85%),urinary tract(23.94%),upper respirato-ry tract(12.68%),surgical site(9.86%),and gastrointestinal tract(5.63%).Of 66 cases of HAI,39 (59.09%) patients sent specimens for culture,a total of 48 pathogens were cultured,the major isolated bacteria was Esche-richia coli (n = 10,20.84%),followed by Staphylococcus aureus ,Klebsiella pneumoniae ,and Pseudomonas aeruginosa ,each was 7 (14.58 %)respectively.The usage rate of antimicrobial agents was 34.40%,specimen de-tection rate in patients receiving therapeutic and therapeutic+prophylactic antimicrobial agents was 59.28%.Risk factors for HAI were age 60 years,with respirator,tracheotomy,urinary tract catheterization, arteriovenous catheterization,hemodialysis,and surgery,difference was significant(all P <0.05).Conclusion Mo-nitoring on key departments and key sites of HAI should be strengthened,antimicrobial agents should be used rationally based on pathogenic detection results,specimen pathogenic detection rate should be improved,and effective prevention and control measures needs to be taken according to the risk factors of HAI.

<p><b>OBJECTIVE</b>To observe the effect of Jinmaitong capsule (JMT), a compound traditional Chinese medicine, on expressions of inducible nitric oxide synthase (iNOS) and nitro tyrosine (NT) protein in streptozocin-induced diabetic (STZ-DM) rats.</p><p><b>METHOD</b>Intraperitoneal injection of streptozocin in rats to establish a model. STZ-DM rats were randomly divided into the model control group (distilled water), the small-dose JMT group (JMT at dose of 0.45 g x kg(-1) x d(-1)), the medium-dose JMT group (JMT at dose of 0.88 g x kg(-1) x d(-1)), the large-dose JMT group (JMT at dose of 1.75 g x kg(-1) x d(-1)) and Vitamin C group (VC at dose of 0.05 g x kg(-1) x d(-1)). Ten normal rats with matching weight and age were selected as the normal control group (distilled water). After intragastric administration for 16 weeks, the expressions of iNOS and NT in sciatic nerve were detected by the immunohistochemistry method.</p><p><b>RESULT</b>The expression levels of iNOS and NT protein in diabetic rats were higher than those in normal rats (P<0.05, P<0.01). Compared with the model group, the levels of iNOS and NT protein in JMT and VC groups were significantly decreased (P<0.05, P<0.01). Particularly, the medium-dose JMT group showed a better effect than the VC group (P<0.05, P<0.01).</p><p><b>CONCLUSION</b>JMT could down-regulate the expressions of iNOS and NT protein of sciatic nerve in diabetic rats.</p>

OBJECTIVE@#To select the optimal method for developing experimental animal model of viral facial paralysis by comparing several inoculation methods.@*METHOD@#One hundred and twenty Balb/c mice were divided into 4 groups, with each group having 30 mice. Group A, the posterior auricular branch of right facial nerve were incised and inoculated with 25 microl HSV-1; group B, 25 microl HSV-1 were inoculated into the posterior aspect of the right auricle by cutaneous scarification; group C, 25 microl HSV-1 were injected into subcutaneous tissue of the posterior aspect of the right auricle; group D, 100 microl HSV-1 were inoculated in the way similar to that of group C. The symmetry of mouse face was observed, and the incidence of paralysis and death were analyzed. The temporal bones of paralyzed mice were serially sectioned and stained with hematoxylin and eosin.@*RESULT@#Thirteen (43.33%) mice developed the right facial paralysis and recovered from it 3-7 days later in group A. Six (20%) mice developed the paralysis and recovered from it 2-9 days later in group B. Group C had no signs of facial paralysis and group D had 1 paralyzed animal. Except for 12 mice in group D, there was no death in the other groups. Nerve swelling was observed in right temporal facial nerve of paralyzed mice. Facial nerve to facial canal cross-sectional area ratio (FN/FC) of the right side was much higher than that of the left side.@*CONCLUSION@#Inoculating HSV-1 into the posterior auricular branch of facial nerve can produce an acute and transient facial paralysis in mice. With the advantage of higher morbidity of facial paralysis and lower mortality in comparison to the other methods, it is an optimal method.

@# ObjectiveTo investigate the effects of Jiuqiang Naoliqing(JNQ) on the morphology of the vital organs(the brain,kidney and heart) in acute hypertension rats.MethodsThe model was established by L-NG-nitro arginine(L-NNA),which can inhibit the synthesis of nitric oxide(NO).Affected animals were administrated with high,medium or low dose of JNQ while nifedipine was used as the positive control.Effects on cardiac function and morphology of the vital organs were investigated.Results2 weeks after the establishment of the model,the affected rats had a 36% higher blood pressure compared with the control group.The acute hypertension rats showed significant pathologic changed in the brain,kidney and heart,while there was no obvious difference in the heart rate,electrocardiaogram,and blood pressure between the administrated groups.After treatment with JNQ,the contraction force and the oxygen extraction of the myocardiocytes were significantly reduced and the increasing velocity of the left ventricular pressure was decreased.The brain,kidney and heart showed improvement in pathology analysis to different levels.ConclusionAlthough it has no obvious effects on the heart rate and blood pressure in acute hypertension rats,JNQ can decrease the oxygen extraction in myocardiocytes and can obviously alleviate brains,kidneys and hearts pathologic changes.

AIM:The objective of this study was to observe the effects of AP-1 element on endothelin-1(EDN1)gene transcription in homocysteine(Hcy)-induced human umbilical vein endothelial cells(HUVECs).METHODS:The redox level in Hcy-induced HUVECs was determined by using the fluorescent product DCF.The influences of Hcy on expressions of EDN1 mRNA and protein of c-jun/AP-1 in HUVECs were measured by the methods of RT-PCR and Western blotting,respectively.The EDN1 secretion level of HUVECs induced by Hcy was determined by enzymatic immunoassay.The transient transfection assay was performed and luciferase activity of transcriptional factor AP-1 reporter gene induced by Hcy was detected.RESULTS:The Hcy significantly increased EDN1 secretion,EDN1 mRNA expression and oxidative stress in HUVECs.Hcy remarkably induced the expression of pGL3-EDN1-AP-1(115/+135)luciferase reporter gene plasmid.However,basic expression of mutation of pGL3-EDN1-AP-1(-115/+135)luciferase reporter gene plasmid was downregulated markedly in HUVECs induced by Hcy.CONCLUSION:Redox-active and release of ROS are changed by Hcy.Activated AP-1 element plays an important role in EDN1gene transcription in HUVECs induced by Hcy.

AIM: To investigate the effect of 17? estradiol (17?E 2)on atherosclerosis and hemorrheology in ovariectomized (OVX) rabbits. METHODS: Thirty four female rabbits were divided into 4 groups, group A: normal control; group B: sham+CHO; group C: OVX+CHO; group D: OVX+CHO+17? E 2. Cholesterol(CHO) was fed to rabbits for 12 weeks, before and after feeding CHO, the serum lipid (TC, TG, HDL-C, LDL-C, ApoA1, ApoB) and area of aortic atherosclerotic plaques were measured. Blood viscosity, plasma viscosity (?p), aggregation index of RBC (AIRC) and fibrinogen were also determined, respectively. RESULTS: ① The ratio of area of aortic atherosclerotic plaque to total area of aortic intima was 0.02?0.003 (in group A), 0.42?0.15 (group B), 0.67?0.23 (group C), and 0.12?0 11 (group D), respectively. In group D, the ratio of aortic atherosclerotic plaque was markedly decreased compared with group C ( P
