RESUMO

A bronchogenic cyst is an uncommon benign congenital anomaly of the primitive ventral foregut. Although bronchogenic cysts in the midline of anterior neck have been reported, they are known to occur very rarely. Moreover, it has never been reported to be located submentally in adults. Clinically, it is usually asymptomatic and thus incidentally diagnosed. Radiologically, it can not be differentiated from other cystic lesions. Therefore, it is pathologically confirmed only when there are bronchial tissues such as pseudostratified ciliated columnar epithelium, smooth muscle, mucous gland or cartilage. Since this cyst has potential for complication, a complete excision is mandatory. We recently experienced a case of bronchogenic cyst presenting as a submental mass in a 26 year-old man. In this article, we reviewed the etiology, the diagnosis, and the treatment of this case with a review of the literatures.

BACKGROUND AND OBJECTIVES: Recently, there have been many reports of antrochoanal polyp (ACP), but the classification of ACP according to the extension of ACP is not yet available. Little is known about what effects the extention of ACP has on the nasal structure. So, in this article, we classified ACP according to the extension of ACP (Type I-extend to nasal cavity, Type II-to nasopharynx, Type III-to oropharynx). SUBJECTS AND METHOD: There were 29 patients who were diagnosed as ACP at the Daejin Medical Center (DMC) recently. There were 4 cases of type I, 20 cases of type II, and 5 cases of type III ACPs. We assessed the clinical, radiological, image-related, and surgical aspects according to the type. The clinical symptom, average symptom duration and surgical aspect were examined using the medical and surgical record. Radiological and image-related feature were analyzed by using CT, navigation software. RESULTS: Nasal obstruction was the most common symptom and the average symptom duration of each type was 18.5 months for type I, 31.1 months for type II, and 32.3 months for type III, repectively. The average fontanelle size of affected maxillary sinus (MS) was larger than that of non-affected side. The volume of affected MS was smaller than that of non-affected side. There was no difference between non-affected and affected MS in the 3-D image at each ACP type. CONCLUSION: We classified ACPs based on the extension of ACP. According to the ACP extension, CT findings were changed but 3-D image and volume of MS were not affected.

RESUMO

A 61-year old female presented with intermittent abdominal pain. Further examination showed an pelvic mass, partially obstructing the sigmoid colon. A search of the past medical records revealed that a primary low-grade ESS of the uterus had been treated with total abdominal hysterectomy and bilateral salpingo-oophorectomy 10 years earlier. Surgical resection was performed under suspicion of recurrent ESS and subsequent pathologic examination of the tumor mass specimen established the recurrent LGSS and free resection margin. Then she has been treated with megestrol acetate and doing well for about 1 year since the surgical resection of the pelvic mass.

RESUMO

Noncommunicating uterine horns are rare, occasionally presenting with functional endometrial cavities, Surgical removal of the noncommunicating horn is commonly performed to prevent endometriosis and rupture of a pregnancy in these patient. We report a case of rudimentary horn with pelvic and appendicular endometriosis treated by laparoscopic resection of those lesions.

RESUMO

To evaluate the efficacy of pelviscopic surgery in gynecologic patient, we have reviewed 1196 patients treated pelviscopically from June 1992 to November 1996. The results of this study were summarized as follows; 1. The common indication were ectopic pregnancy(44.6%), ovarian cyst(14.0%), and myoma uteri(10.5%). 2. The major types of surgery were salpingectomy(24.8%), cystectomy(12.6%), salpingostomy(9.9%), adhesiolysis(6.5%),and myomectomy(5.8%). 3. The age of the patients ranged from 15 to 79 years and the mean age was 30.8 years. 4. the mean parity of the patients was 1.24. 5. The mean duration of hospitalization was 3.2 days. 6. The most common complication was hemorrhage at the trocar site, but it was controlled without difficulty.
