RESUMO

Bedside ultrasound plays an important role in the evaluation of critically ill patients. In order to standardize the application of bedside ultrasound, Chinese Research Hospital Association of Critical Care Medicine and Nursing Research Group of Chinese Research Hospital Association of Critical Care Medicine organized the experts in related fields in China to analyze, discuss and summarize the following contents: ① bedside ultrasound assessment of lungs; ② bedside ultrasound -guided nutrition tube placement; ③ bedside ultrasound assessment of gastric residual volume; ④ bedside ultrasound -guided endovascular catheterization. Finally, the Evidence-based nursing expert consensus on adult bedside ultrasound was formulated.

Objective:To investigate the effects of health management based on the theory of protection motivation on fatigue status, neurological function recovery and life ability of stroke patients, and evaluate its clinical effects.Methods:A total of 120 stroke patients admitted to the First Affiliated Hospital of Zhengzhou University from January 2018 to January 2019 were selected as subjects. Randomized digital table method was used to divided them into observation group and control group, 60 cases in each group; the control group underwent routine nursing and follow-up of neurology, and the observation group was given health management based on protection motivation theory on the basis of the control group. The Fatigue Severity Scale (FSS) was used to assess the patient's fatigue, the European Stroke Scale (ESS) was used to evaluate the patient's neurological function, the modified Barthel index was used to assess the patient's viability. The fatigue, neurological recovery, and changes in living ability were compared between the two groups before and after the nursing intervention.Results:The Scores of FSS, MBI and ESS of the observation group were 45.34±8.84, 54.3±4.69 and 45.24±4.18 before intervention and 32.48±5.80, 75.50±4.93, 63.12±3.32 after intervention. The Scores of FSS, MBI and ESS of the control group was 44.97±8.47, 53.47±4.20, 43.48±5.67 before intervention and 39.59±7.43, 63.81±3.25, 55.32±3.48 after intervention. The difference after intervention between the two groups was statistically significant ( t values were 15.335, 12.562, 5.843, P<0.01). The difference in the observation group before and after intervention was statistically significant ( t values were 9.422, 24.133, 25.945, P<0.01). The differences in the control group before and after intervention was statistically significant ( t values were 3.699, 15.082, 13.786, P<0.01). Conclusions:Health management based on the theory of protection motivation is more effective than routine nursing to improve the fatigue, neurological function and life ability of stroke patients.

Objective To explore the consistency between the Chinese Version of Stressors in Nursing Students (SINS-CHI) and the Student Nurse Stress Index Scale (SNSI-CHI) in assessing the pressure of undergraduate nursing students.Methods A total of 960 nursing undergraduates from three universities in Henan province were randomly selected.SINS-CHI and SNSI-CHI were applied to all subjects and SPSS 21.0 was used to analyze the validity and reliability.Results The total score of SINS-CHI was (112.73± 26.07).The Cronbach's α was 0.926,the Guttman's partial reliability coefficient was 0.893,the test-retest reliability was 0.820,the content validity index (CVI) was 0.890,the sensitivity was 70.0%,and the specificity was 62.3%.The Yoden index was 0.323 and the area under the ROC curve was 0.710.The total score of SNSI-CHI was (58.71± 14.18).The Cronbach's α was 0.893,Guttman split-half reliability coefficient was 0.829,test-retest reliability was 0.966,CVI was 0.954,sensitivity was 71.4%,specificity was 63.6%.The Yoden index was 0.350 and the area under the ROC curve was 0.714.Conclusion The consistency of SINS-CHI and SNSI-CHI is good.

Objective To investigate the risk factors for stroke in population with family history in rural areas of Zhengzhou. Methods A cluster random sampling method was used to collect 386 residents with family history of stroke from 5 villages of Zhengzhou. The participants were divided into stroke group and non-stroke group according to diagnosis. IBM SPSS STATISTICS 21.0 and SPSS Modeler 14.1 software were used to analyze the data.Results Hypertension, dyslipidemia, atrial fibrillation or arrhythmia, smoking and drinking in stroke group were significantly higher than those of the non-stroke group (P < 0.05). Logistic regression analysis indicated that hypertension (OR=35.705, 95% CI9.045~140.950, P =0.000) and dyslipidemia (OR=2.669, 95% CI 1.077~6.614, P = 0.034) were linked with stroke. Artificial neural network (ANN) model indicated the independent variables in predicting stroke were hypertension, dyslipidemia, smoking, drinking and atrial fibrillation or arrhythmia according significance, respectively. The accuracy of the model was 82.0%. Conclusion Among population with family history of stroke, those who had hypertension, dyslipidemia, atrial fibrillation or arrhythmia, smoking and drinking were more susceptible to develop this condition.

Objective To evaluate the reliability and validity of the Chinese Version of the Tampa Scale of Kinesiophobia (TSK-11) in patients receiving total knee arthroplasty. Methods The Chinese ver-sion of TSK-11 was translated from the original one,and adjusted for cultural adaptation. The reliability and validity of Chinese TSK-11 were tested among 254 eligible patients receiving total knee arthroplasty from a tertiary hospital in Henan province by convenience sampling. Results Three common factors which ex-plained 65. 177% of the total variance was extracted after the exploratory factor analysis,and each item had high factor loading quantity (>0. 4). The scale CVI was 0. 83~1. 00,the S-CVI/UA and S-CVI/Ave were 0. 91 and 0. 94,respectively. The Cronbach's α coefficient of the scale was 0. 883,and the test-retest reliabil-ity was 0. 798. The item-to-total correlations ranged from 0. 424~0. 757 (P<0. 05). Conclusion The Chi-nese version of TSK-11 has been proved to be reliable and valid with fewer understandable items,and short completion time. It can be used as a valuable tool for evaluating kinesiophobia in patients following total knee arthroplasty.

Objective To test the reliability and validity of the Chinese Version of the Student Nurse Stress Index Scale ( SNSI-CHI ) among Chinese nursing students. Methods A random sampling method was used to collect 1100 nursing students who were from two medical universities of Henan Province, China,and SPSS 17. 0 and AMOS 17. 0 software were used. Results The average total score of SNSI-CHI was 58. 46±13. 90. The Cronbach's α was 0. 886,the test-retest intra-class correlation coefficient( ICC) of SNSI-CHI was 0. 996 (95%CI:0. 992-1. 000,P<0. 01). The item-to-total correlations ranged from 0. 351 to 0. 664 ( all P<0. 01) . The content validity index( CVI) was 0. 954. The result of exploratory factor analysis ( EFA) was that three factors together explained 75. 013% of the total variances,and the confirmatory factory analysis( CFA) also indicated a good fit (χ2/df=1. 347,GFI=0. 956,AGFI=0. 945,RMR=0. 032,RMSEA= 0. 025, NFI=0. 974, IFI=0. 993, TLI=0. 992, CFI=0. 993 ) . Criterion-related validity was between 0. 330 and 0. 903 ( all P<0. 01) . Conclusion The SNSI-CHI is proved to be reliable and valid in China, and it can be used to measure the stress of Chinese nursing students.

Objective To test the agreement between two new self-care-related instruments,the Chinese version of the Appraisal of Self-care Agency Scale-Revised (ASAS-R-CHI) and the Chinese version of the Self-care Ability Scale for the Elderly (SASE-CHI) among older stroke patients by Bland-Altman method.Methods A random sampling was used to conduct this study in Neurology Department in a tertiary referral hospital in Henan Province,during November 2016 to October 2017.The sample consisted of 500 older stroke patients.Data were collected by a demographic questionnaire,SASE-CHI and ASAS-R-CHI.SPSS 21.0 was used to analyze the data.Results The mean total scores of ASAS-R-CHI and SASE-CHI were 54.98±5.06 and 61.92±7.34,respectively.Partial correlation coefficients between the total score of ASAS-R-CHI and SASE-CHI and their factors were from 0.217 to 0.953,showed significantly correlation (P<0.01).The difference analysis and ratio analysis of Bland-Altman showed a good consistency of the two scales,with 5.0% and 4.5% of the points outside the 95%CI boundaries,separately.Conclusion There is a good agreement between the two instruments among older stroke patients.Each scale has its own characteristics,therefore the users should choose the most appropriate scale according to the specific situation.

Objective Currently, there are few articles about kinesiophobia in patients with total knee arthroplasty (TKA) in China.This study aims to investigate the incidence of kinesiophobia and its influencing factors in TKA patient, and provide evidence for the intervention of kinesiophobia.Methods A total of 298 TKA patients from our hospital were investigated by general information questionnaire, Tampa Scale of Kinesiophobia (TSK), Knee Self-Efficacy Scales, Simplified Coping Style Questionnaire and Social Support Rating Scale.Single-factor analysis and logistic regression analysis were conducted to explore influencing factors.Results The score of TSK was 38.50±13.52, and 31.88% of TKA patients reported kinesiophobia.Logistic regression analysis showed that duration of pain (OR=5.546, 95%CI: 2.143-14.353), education level (OR=0.145, 95%CI: 0.067-0.314), self efficacy(OR=0.606, 95%CI: 0.470-0.780), positive response (OR=0.784, 95%CI: 0.683-0.900), objective support (OR=0.807, 95%CI: 0.691-0.943) and utilization of social support (OR=0.507, 95%CI: 0.461-0.705) were factors influencing kinesiophobia in TKA patients.Conclusion Attention should be paid to the kinesiophobia in TKA patients, especially those influencing factors including duration of pain, education level, and objective support.Health care providers should encourage early stage rehabilitation exercise to improve the postoperative knee function of TKA patients.

Objective To investigate the level and influence factors of sleep quality in caregivers to neurological intensive care unit (NICU) survivors. Methods The Pittsburgh Sleep Quality Index (PQSI) and the sleep quality influence factor scale were administered on 167 family caregivers to NICU survivors. Results The score of PQSI was 8.81 ± 3.25,and 55.68%(98/167) of caregivers reported poor sleep quality. Logistic regression analysis showed that medical insurance(OR=2.025, P<0.01), hospital stay (OR==2.235, P<0.01), whether caregivers in service (OR=0.583, P<0.05), resisdence(OR=2.341, P<0.01), and tracheal tubes/ventricular drainage (OR=5.857, P<0.01) were influencing factors of sleep quality. Conclusions It proved that our intervention should focus on family caregivers to NICU survivors social and psychological characteristics may help to enhance their sleep quality and quality of life.

BACKGROUND:Early rehabilitation after total knee arthroplasty (TKA) can help the rapid functional recovery of knee,but some patients are fear of moving and refuse to do exercising because of kinesiophobia,thereby delaying functional recovery.Foreign study has confirmed that patients with cognitive behavioral therapy (CBT) can effectively alleviate kinesiophobia,but no related studies have been reported in China.OBJECTIVE:To explore the effectiveness of CBT on patients with kinesiophobia after TKA.METHODS:108 patients with kinesiophobia after TKA were recruited from Department of Orthopedics,the First Affiliated Hospital of Zhengzhou University,China between March 2014 and January 2015,and were then equivalently randomized into experimental and control groups using random number table.Both groups were subjected to conventional treatment,while the experimental group received extra CBT.The scores of Tampa Scale of Kinesiophobia and American Knee Society were evaluated immediately,1,3,and 6 months after hospital discharge;the hospitalization time and total hospitalization expenditure were compared between two groups immediately after discharge.RESULTS AND CONCLUSION:The scores of Tampa Scale of Kinesiophobia in the experimental group were significantly lower than those in the control group at different time points postoperatively (P < 0.05).The scores of American Knee Society in the experimental group were significantly higher than those in the control group at 3 and 6 months after discharge and kept on rising (P < 0.01),but showed no significant difference at discharge and 1 month after discharge (P > 0.05).(2) The hospitalization time and total hospitalization expenditure in the experimental group were significantly lower than those in the control group (P < 0.01).(3) To conclude,CBT cannot only shorten the hospitalization time and total hospitalization expenditure,but also relieve kinesiophobia,which is conductive for functional recovery of the knee.

RESUMO

The birth and development of oocyte donation technology have brought hope for women with poor ovarian reserve and repeated failure for in vitro fertilization, as well as for those with chromosomal abnormalities, premature ovarian failure, or at perimenopausal or menopausal stages. It has not only preserved their reproductive right, but also stabilized their families and increased social harmony. However, this technology does not only involve infertile couples themselves, but also social and ethical issues concerning their families and the society. This paper has reviewed and discussed the hot issues concerning oocyte donation, e.g., source of eggs, compensation for donors, prerequisites for recipients and donors, privacy of donors, and made suggestions for further improvement for the administration of oocyte donation.

RESUMO

Objective To promote upper extremity function rehabilitation in patients with breast cancer postoperatively.Methods The application of upper extremity function rehabilitation was designed,and was applied in patients with breast cancer after surgery who were selected from June 2015 to September,and we established management and evaluation scheme.Result The application improved patients' functional exercise compliance and upper extremity dysfunction(P<0.001).In the short term,upper extremities edema incidence was 2.17%,which was lower than the national average.The nurses thought that it improved the work efficiency.Conclusion The application is an effective tool to improve postoperative upper extremity function rehabilitation in patients with breast cancer.

Objective To translate the English version of the Exercise Self-Efficacy Scale (ESES) into Chinese and test the reliability and validity of the Chinese version of ESES in patients after stroke. Methods Totally 214 patients after stroke were recruited and were investigated by the Chinese version of ESES. Results The internal consistency coefficient of the Chinese version of ESES was 0.879. The split half reliability coefficient was 0.858(P<0.01). The test-retest reliability was 0.657. Item-total correlation coefficient ranged between 0.627 and 0.779 (P<0.01). Factor analysis got one factor, which explained 59.745%of the total variance, and the factor loading of each item was above 0.4. Using General Perceived Self-Efficacy Scale (GSES) as an external criterion, the Pearson correlation coefficient of ESES with GSES was 0.496. Conclusions The Chinese version of ESES has been proved to be reliable and valid. It can be used as a valid tool for the measurement of exercise self-efficacy in patients after stroke.

Objective The research aimed to translate the English version of the Caregiver Preparedness Scale (CPS) into Chinese and to test the reliability and validity of the Chinese version of CPS in caregivers of stroke survivors. Methods Totally 176 stroke survivors and their caregivers were recruited and were investigated by the Chinese version of CPS. Results The internal consistency coefficient of the Chinese version of CPS was 0.925;the test-retest reliability was 0.807 (P0.4. Conclusions The Chinese version of CPS has been proved to be reliable and valid. It can be used as a valid tool for the measurement of caregiver preparedness in caregivers of stroke survivors.

Objective To understand the status of stroke patients' perceived control and healthrelated quality of life,and to analyze the effect of perceived control on stroke patients' depression and health-related quality of life.Methods By convenience sampling,264 stroke patients were chosen to complete the questionnaire survey,Perceived Control Questionnaire,Hamilton Depression Scale and ShortForm Health Survey Scale-2.Results The score of patient's perceived control and health-related quality of life were (18.21±2.74),(46.62±11.56) points respectively.The perceived control had a negative correlation with the depression (r=-0.32,P < 0.05),had a weak positive correlation with the body health (r=0.05,P <0.05),and had a positive correlation with the mental health of health-related quality of life (r=0.29,P <0.05),depression was negatively correlated with health-related quality of life of the body health,mental health (r=-0.26,-0.28,P < 0.05).Conclusions Medical personal should improve the level of stroke patients' perceived control through various methods,to reduce depression and improve health related quality of life.

Objective Self-management was adopted to intervene patients with breast cancer during chemotherapy in order to know about its effects on breast cancer patients′medical compliance behavior. Methods Samples were 154 breast cancer patients undergoing chemotherapy after surgery in breast surgery of the first affiliated hospital of Zhengzhou university. They accepted the docetaxel and cyclophosphamide (TC) program of chemotherapy. Patients were divided into two groups by random number table, one group as control group(80 patients) and the other group as experimental group (74 patients). Different nursing care were conducted during the two groups, the control group patients given routine nursing care, the experimental group patients given routine nursing care and self-management. The medical behavior compliance and upper extremities edema incidence for the two groups of patient were surveyed after the intervention and then evaluate the effect of the intervention. SPSS (version 17.0) was used to carry out statistical analysis. Results The total score of medical compliance behavior, the experimental group was (18.36±1.30) points, the control group was (16.70±1.57) points, the experimental group was higher than the control group, and there was statistically significant difference between two groups(t=7.093, P0.05) in scores of chemotherapy compliance on time.The influence of self-management on the incidence of upper extremities edema was as follow: The upper extremities edema incidence in control patients was 23.68% (18/76) which was higher than 11.11% (8/72) in the experimental group (χ2=4.036, P < 0.05). Conclusions Given self- management intervention for breast cancer patients with postoperative chemotherapy, the medical behavior compliance was increased, and the incidence of upper extremities edema was decreased.

Objective The research aimed to translate the English version of the Breastfeeding Self-efficacy Scale Short Form (BSES-SF) into Chinese and to test the reliability and validity of the Chinese version of BSES-SF. Methods Totally 308 mothers were recruited and were investigated by the Chinese version of BSES-SF. Results The internal consistency Alpha coefficient of the Chinese version of BSES-SF was 0.927, P<0.01, the half reliability coefficient was 0.853, P<0.01;the test-retest was 0.811, P<0.01. Factor analysis got one factor, which explained 52.498% of the total variance. Conclusions The Chinese version of BSES-SF has been proved to be reliable and valid. It can be used as a valid tool for the measurement of the breastfeeding self-efficacy among Chinese mothers.

Objective To investigate the effect of griping exercise on wrist palmar swelling after percutaneous coronary intervention. Methods A total of 90 patients of the First Affiliated Hospital of Zhengzhou University with planned percutaneous coronary intervention via radial artery were enrolled and divided into the control group, griping exercise group 1 and griping exercise group 2 with 30 cases in every group according to the sequence of admission. Preoperative education and postoperative care were performed with a conventional approach in the control group. Griping exercise in group 1: Fisting exercise with electronic grips in 15 times per min, lasted 1 min, 2 hours per day for 3 days. Griping exercise in group 2: Fisting exercise with electronic grips in 25 times per min, lasted 2 min, 2 hours per day for 3 days. The degree of swelling and pain in patients′wrist palmar 6 hours after operation (sheath tube removal) were observed. Results The pain score in wrist and palm in griping exercise group 2 was 0.83±0.39, and 4.20±1.98 in the control group, the difference was significant (t=3.912, P < 0.05), and also lower than that of the griping exercise group 1 (1.92±0.78, t=3.087, P < 0.05). The swelling value of finger circumference in griping exercise group 2 was (0.13±0.05) cm, and in the control group was (0.39±0.13) cm, the difference was significant (t=3.013, P < 0.05), also lower than (0.23±0.11) cm in the griping exercise group 1 (t=2.612, P < 0.05). Conclusions Fisting exercise with electronic grips in 25 times per min, lasted 2 min, 2 hours per day for 3 days preoperation could reduce the swelling and pain in patients′wrist and palm on the operation side.

Objective To explore the effect of the combination of video feedback and telephone intervention on postdischarge rehabilitation in patients receiving artificial total hip replacement.Methods A total of 60 patients receiving artificial total hip replacement were divided into intervention group and control group with 30 cases each by random digits table method.The combination of video feedback and telephone intervention was conducted in intervention group,and conventional discharge guidance and telephone follow-up on a regular basis in control group.In intervention group,the patients or their family member were asked to record the video of the exercises and daily living when patients were at home according instructions and researchers assessed the condition of patient's joint and muscle function and provided individual education and instructions to the patients on the basis of the feedback video through telephone.Harris hip function rating scale and Functional Independence Measure (FIM) were investigated at discharge and 1,3,6 months after discharge.Results There were no significant differences in Harris hip function rating scale and FIM score at discharge between 2 groups,P>0.05.Harris hip function rating scale score were (72.70± 11.65),(83.11±5.12),(94.83±3.08) scores at 1,3,6 months after discharge in intervention group,and (63.20±14.96),(77.00±11.62),(87.50±7.94) scores in control group,there were significant differences,P<0.05 or <0.01.FIM score were (96.30±4.63),(108.76±3.28),(125.79±1.19) scores at 1,3,6 months after discharge in intervention group,and (78.23±5.09),(86.43±6.30),(96.54±5.35) scores in control group,there were significant differences,P<0.01.Conclusion The combination of video feedback and telephone intervention could help to promote the postdischarge hip function for patients receiving artificial total hip replacement.
