RESUMO

ObjectiveTo observe the effect of proprioceptive neuromuscular facilitation combined with neuromuscular electrical stimulation on chronic ankle instability (CAI). MethodsFrom April, 2016 to December, 2021, 48 patients with CAI were randomly divided into control group (n = 24) and observation group (n = 24). Both groups accepted routine rehabilitation, and the observation group accepted proprioceptive neuromuscular facilitation combined with neuromuscular electrical stimulation additionally, for eight weeks. They were assessed with Visual Analogue Scale (VAS), peak torque to body weight of ankle dorsiflexors and plantarflexors (AD/W, AP/W), Y Balance Test (YBT) and Foot and Ankle Disability Index (FADI) before and after treatment. ResultsAfter treatment, VAS score, AD/W, AP/W, YBT and FADI improved in the observation group (|t| > 2.208, P < 0.05), while VAS score and AP/W improved in the control group (|t| > 2.156, P < 0.05); and all the VAS score, AD/W, AP/W, YBT and FADI were better in the observation group than in the control group (|t| > 2.067, P < 0.05). ConclusionProprioceptive neuromuscular facilitation combined with neuromuscular electrical stimulation can effectively relieve the pain of patients with CAI, and increase the muscle strength around the ankle, to improve the stability and balance.

RESUMO

Objective:To investigate the clinical effects of tibial transverse bone transport assisted by nose ring drainage (NRD) in the treatment of foot and ankle chronic osteomyelitis.Methods:A retrospective study was conducted to analyze the data of 32 patients with foot and ankle chronic osteomyelitis who had been treated at Department of Orthopedics, Beijing Rehabilitation Hospital from March 2013 to February 2022. The patients were assigned into a study group and a control group. In the control group, there were 12 males and 3 females, aged (39.5±8.8) years. The osteomyelitis was located at the distal tibia in 4 cases, at the talus in 1 case, at the calcaneus in 2 cases, at the midfoot in 4 cases and at the forefoot in 4 cases. According to the Crerny-Mader classification, there were 5 cases of type Ⅰ and 10 cases of type Ⅱ. The control group was treated with Ilizarov transverse tibial bone transport in combination with thorough debridement and anti-infection therapy. In the study group, there were 12 males and 5 females, aged (42.3±13.4) years. The osteomyelitis was located at the distal tibia in 4 cases, at the talus in 1 case, at the calcaneus in 3 cases, at the midfoot in 5 cases and at the forefoot in 4 cases. According to the Crerny-Mader classification, there were 7 cases of type Ⅰ and 10 cases of type Ⅱ. The study group was treated with NRD drainage in addition to the procedures in the control group. The curative effects were evaluated by comparing the cure, recurrence, amputation (toe amputation), antibiotic use time, wound healing time, relevant inflammatory indicators [WBC, neutrophil percentage (NEU), erythrocyte sedimentation rate (ESR), procalcitonin, interleukin-6 (IL-6), C-reactive protein (CRP)], and postoperative functional recovery between the 2 groups.Results:The 2 groups were comparable because there was no statistically significant difference in the general data between them ( P>0.05). The follow-up period was (36.0±9.8) months. There was no significant difference between the 2 groups in the cure or amputation (toe amputation) ( P>0.05), but the recurrence rate in the study group was significantly lower than that in the control group ( P<0.05). The antibiotic use time [(20.7±3.6) d] and wound healing time [(88.3±17.1) d] in the study group were significantly shorter than those [(37.9±6.5) d and (102.2±22.6) d] in the control group ( P<0.05). The ESR, IL-6 and CRP at 1 week after operation in the study group were significantly lower than those in the control group ( P<0.05), but there was no significant difference between the 2 groups in the WBC, NEU or PCT at 1 week after operation ( P>0.05). There was no significant difference either in the inflammatory indicators between the 2 groups at 1 month after operation ( P>0.05). In all patients, the inflammatory indicators like WBC, NEU, ESR, PCT, IL-6 and CRP at 1 week and 1 month after operation were significantly better than those before operation, and the Maryland foot functional score at 12 months after operation was significantly higher than the preoperative one ( P<0.05). There was no significant difference between the 2 groups in the Maryland foot functional score at 12 months after operation ( P>0.05). Conclusion:In the treatment of foot and ankle chronic osteomyelitis, compared with the Ilizarov transverse tibial bone transport, our tibial transverse bone transport assisted by NRD can achieve satisfactory therapeutic effects, shorten antibiotic use time and treatment cycle, and reduce recurrence rate.

ObjectiveTo explore the effect of trunk control training during unstable sitting on knee pain and function in patients with patellofemoral pain syndrome. MethodsFrom January, 2019 to December, 2021, 41 patients with patellofemoral pain syndrome in Beijing Rehabilitation Hospital were randomly divided into control group (n = 20) and experiment group (n = 21). Both groups accepted routine rehabilitation, and the experiment group accepted trunk control training during unstable sitting in addition, for four weeks. They were assessed with Visual Analogue Scale for pain (VAS) and Anterior Knee Pain Scale (AKPS), and measured stability indexes with Balancer before and after treatment. ResultsAll the VAS score, AKPS score, and the overall, anterior-posterior and left-right stability indexes improved in both groups after treatment (|t| > 12.089, P < 0.001); and improved more in the experiment group than in the control group (|t| > 5.864, P < 0.001). ConclusionTrunk control training during unstable sitting may improve knee pain and function, and motor control.

ObjectiveTo observe the effect of neuromuscular training on femoroacetabular impingement. MethodsFrom January, 2017 to November, 2021, 27 patients with femoroacetabular impingement in Beijing Rehabilitation Hospital were randomly divided into control group (n = 13) and observation group (n = 14). The control group accepted routine rehabilitation training, and the observation group accpeted neuromuscular training in addition. They were assessed with Visual Analogue Scale (VAS) for pain, peak torque (PT) of hip flexion and extension, Y-balance test (YBT) and simplified International Hip Outcome Tool (iHOT-12) before and after treatment. ResultsThe VAS score, PT, YBT score and iHOT-12 score improved in the observation group after treatment (|t| > 3.628, P < 0.01), while the VAS score and PT improved in the control group (|t| > 3.409, P < 0.01). After treatment, the VAS score, PT, YBT score and iHOT-12 score were better in the observation group than in the control group (|t| > 2.067, P < 0.05). ConclusionNeuromuscular training can relieve the pain of patients with femoroacetabular impingement, and improve the muscle strength and function of hip joint.

Femtosecond laser has been widely used in corneal refractive surgery.The refractive state, corneal tissue structure and biomechanical properties are changed through cutting the corneal tissue.Femtosecond laser-assisted in situ keratomileusis(FS-LASIK)and small incision lenticule extraction(SMILE)are the most commonly used femtosecond laser corneal refractive surgery.In this study, the structural basis of corneal biomechanical properties, the commonly used methods for corneal biomechanics measuring, biomechanical effect of corneal flap thickness, side-cut angles, residual stromal bed thickness, flap hinge position for F-LASIK and the size of corneal incision, optical zone diameter, cap thickness, tissue cutting thickness for SMILE on corneal biomechanics are reviewed.

Objective:To study the pupillary centroid shift of myopia and characteristics of pupil diameter change from scotopic to photopic condition.Methods:A case series study was carried out, 140 eyes of 70 myopia patients from September to November 2016 in Tianjin Eye Hospital were enrolled.The pupillary centroid shift and pupil diameter parameters were measured by visual quality analyzer from scotopic (0.017 lx) to photopic (10.400 lx) condition.This study followed the Declaration of Helsinki.Results:Under the scotopic and photopic conditions, the pupil diameter was positively correlated between the bilateral eyes (scotopic: rs=0.85, P<0.001; photopic: r=0.85, P<0.001), and the pupil diameter variation from scotopic to photopic condition was positively correlated between the bilateral eyes ( r=0.75, P<0.001). The pupil diameter in scotopic and photopic conditions, and the change of pupil diameter in the right eyes were significantly higher than those in the left eyes (all at P<0.05). The pupillary centroid shift was within 0.2 mm in the left eyes of 94.2% (66/70) subjects and in the right eyes of 97.1%(68/70) subjects.The pupillary centroid shift of all subjects was within 0.3 mm.From scotopic to photopic condition, the pupil centroid was mainly shift to the nasal superior direction.There was no significant correlation between pupil diameter and age or gender.There was no significant correlation between pupillary centroid shift and age, diopter or pupil diameter. Conclusions:The binocular pupillary centroid shift are symmetrical from scotopic to photopic condition in myopic eyes, and the pupil centroid mainly shift to nasal superior direction.

Objective@#To investigate the changes of anterior chamber profiles after small incision lenticule extraction(SMILE) and analyze the possible influencing factors.@*Methods@#A case observation study was designed.Thirty-one patients(59 eyes) with myopia and myopic astigmatism who underwent SMILE surgery at the Refractive Surgery Center of Tianjin Eye Hospital from April to December 2015 were enrolled and examined preoperatively and at 1 month, 3 and 6 months postoperatively.The anterior chamber profiles, including anterior chamber depth (ACD), anterior chamber volume (ACV) and anterior chamber angle (ACA) were measured using the Pentacam.The correlations between parameters were analyzed and the differences of the parameters at different time points were compared.This study followed the Declaration of Helsinki.This study protocol was approved by Ethic Committee of Tianjin Eye Hospital (No.TJYYLL-2015-20). Written informed consent was obtained from each subject before the operation.@*Results@#Compared with pre-operation, the central corneal thickness (CCT) was significantly decreased at 1 month, 3 and 6 months postoperatively (all at P<0.05). Three months after surgery, the CCT was significantly higher than that at one month after surgery (P<0.001), and no significant change of CCT was found between 3 months and 6 months after surgery (P>0.05). The posterior corneal elevation (PCE) at different time points before and after surgery was significantly different (F=19.249, P<0.001), compared with the preoperation, the PCE at 1 month, 3 and 6 months postoperatively moved slightly backward (all at P<0.05). The preoperative ACD was (3.33±0.25)mm, the preoperative ACV was (217.46±31.55)mm3, and the ACD was (3.31±0.25), (3.30±0.25) and (3.30±0.25)mm, the ACV was (214.71±33.09), (211.14±33.65) and (210.08±32.19)mm3, respectively at 1 month, 3 and 6 months postoperatively, with significant differences at different time points before and after surgery (F=12.477, 12.205; both at P<0.001), compared with the preoperation, the ACD at 1 month, 3 and 6 months postoperatively and ACV at 3 months and 6 months postoperatively were obviously decreased, with a significant difference between them (all at P<0.05). There was a significant positive correlation between ACD and ACV at different time points before and after surgery (r=0.870, 0.893, 0.886, 0.884; all at P<0.001). The preoperative ACD was negatively correlated with CCT (r=-0.286, P<0.05).@*Conclusions@#The ACD is decreased postoperatively, it is basically stable at (3.30±0.25)mm, which satisfies the requirements of the ACD required for implantable collamer lens.And the stable state of ACD is also important for keratoconus detection.

Objective To investigate the correlations between corneal biomechanical properties and corneal high-order aberrations (HOAs) in myopic eyes.Methods In this retrospective study,263 right eyes with myopia or myopic astigmatism were enrolled from January to October 2015 in Tianjin Eye Hospital.The values of first applanation time (A1T),first applanation length (A1L),first applanation velocity (A1V),second applanation time (A2T),second applanation length (A2L),second applanation velocity (A2V),highest concavity time (HCT),highest concavity deformation amplitude (HCDA),highest concavity peak distance (HCPD),highest concavity radius (HCR) and non-contact intraocular pressure (IOPnct) were measured using the corneal visualization Scheimpflug technology (Corvis ST).The central corneal thickness (CCT) and wavefront aberrations of the anterior,posterior surface,and the total cornea were measured with Pentacam.Pearson correlation and stepwise multivariate linear regression analysis were applied to analyze the correlations between biomechanical properties and corneal HOAs.This study follows the Helsinki declaration.The experiment was approved by the Ethic Committee of Tianjin Eye Hospital.Informed consent was signed from each patient.Results There was a correlation between biomechanical properties and corneal HOAs.Negative correlations were found between A1L and the spherical aberration,HOAs RMS of anterior surface,total RMS and HOAs RMS of the posterior surface and HOAs RMS of the total cornea (P<0.05).HCRs were negatively correlated with total RMS of the total cornea and the anterior surface (P<0.05).The A2V was correlated with various aberrations.Multiple stepwise linear regression model analysis showed that,the correlation between the aberrations of anterior surface and biomechanical parameters,and the correlations between the total corneal aberration and biomechanical parameters were weak,the correlations between the aberrations of posterior surface and biomechanical parameters were strong.Conclusions There is a correlation between the biomechanical properties and corneal HOAs.The changes of the corneal biomechanical properties are mainly associated with the posterior corneal aberrations,which may contribute to the early diagnosis of subclinical keratoconus or dilated corneal disease.

BACKGROUND:During artificial knee replacement for al kinds of severe knee disease or conservative treatment for end-stage knee disease, Deluxe-PS knee prosthesis and PFC Sigma RP knee prosthesis have their advantages. OBJECTIVE: To compare and analyze the matching of the Deluxe-PS and PFC Sigma RP knee prostheses with bone cross section. METHODS:According to the use of the knee prosthesis, 87 gonarthritis patients undergoing bilateral knee replacement were divided into two groups. 46 cases in the test group received Deluxe-PS knee prosthesis replacement. 41 cases in the control group received PFC Sigma RP knee prosthesis replacement. Operation time and intraoperative blood loss were compared between the two groups. American Knee Society score, knee score of American Special Surgical Hospital and knee joint range of motion were used to evaluate knee joint function before replacement, 3 months after replacement and during final folow-up. Anteroposterior X-ray films were obtained after replacement to measure valgus angle and flexion angle of femoral prosthesis. RESULTS AND CONCLUSION: (1) Operation time and intraoperative blood loss were significantly better in the test group than in the control group (P 0.05). Above indexes were significantly improved in both groups at 3 months after replacement and during final follow-up (P 0.05). (4) These findings indicate that effect of Deluxe-PS type artificial knee prosthesis was identical to that of PFC Sigma RP knee prosthesis in knee osteoarthritis patients receiving bilateral knee joint replacement. Deluxe-PS type artificial knee prosthesis can perfectly realize high-accuracy combination of the prosthesis and the bone cross section, and enhance the fusion suitability of the prosthesis and the bone surface. Deluxe-PS type artificial knee prosthesis has the advantage of operation. This advantage is possibly because Deluxe-PS type artificial knee prosthesis is more in line with the anatomical structure of the knee joint of Chinese.

This investigation was to assess the prevalence and risk factors of hypertension in 35 to 70 years old adults living in Xicheng District of Beijing. Cluster sampling was conducted in 2230 adults, and only 1958 individuals agreed to enter into the study. Written informed consents were obtained before questionnaire and physical examination. A total of 1889 data were used for analysis. The prevalence rate of hypertension was 47.8% (n=902). The age-standardized prevalence rate was 40. 1%. Hypertension was seen in 368 men (53.0%) and 534 women (44.7%) (P=0.0005). The rates of disease awareness, treatment, and control were 47.5%, 38.5%, and 24.1%, respectively. The prevalence of hypertension increased with age (P<0.0001). Linear trend was found between body mass index (BMI) and systolic blood pressure (P<0.0001). In multiple logistic model analysis, the hypertension risk for men was 1.33 times that of women (95% CI, 1.08-1.64), for participants with obesity was 2.81 times that of those with normal weight (95%CI, 2.00-3.97). Hypertension has become one of the most concerned public health problems in the district. Middle-aged people should pay attention to changed blood pressure, especial for overweight men. Health promotion should be needed for hypertension prevention. More attention should be paid to substantial increase in hypertension.

Objective To investigate the expression of lymphocyte function-associated antigen-1 (LFA-1)on peripheral blood mononuclear cells(PBMCs)from patients with SLE and its relation to the occurrence and development of SLE.Methods PBMCS were obtained from 30 patients with SLE and 24 normal human controls.Atier labeled with anti-CD3-FITC and anti-CD11 a-PE,PBMCS were detected for the expression of CD11a by flow cytometric analysis.Results The positivity rate of CD11a was significantly higher in patients with SLE and those with active SLE than in the normal controls (73.74%±7.89%and 77.11%±7.46%vs 68.21%±4.58%.both P<0.05).Furthermore,increased positivity rate of CD11a was observed in patients with active SLE compared with those with stable SLE (77.11%±7.46%vs 69.33%±6.27%,P<0.05).but there was no significant difieFence between the latter group and normal controls(P>0.05).In addition.the expression level of CD11a was positively correlated with SLE disease activity index (r=0.64,P<0.0 1)in patients.Conclusion The overexpression of LFA-1 may take part in the occurrence of SLE and correlate with the activity of SLE.

Objective To optimize the medium for rhizomatous induction of Belamcanda chinensis in vitro. Methods By plant tissue culture technology, the effects of various carbon source, NAA, and active carbon at different concentration on the rhizomatous formation of B. chinensis in vitro were studied. Results MS+6-BA 2.0 mg/L+NAA 0.5 mg/L+6% white sugar was the optimal medium for the rhiz-omatous formation of B. chinensis in vitro. Active carbon should not be added to the medium. The germination rate of rhizomatous in vitro was 61.03% on the MS + BA 2.0 mg/L+3% white sugar. Conclusion Sugar concentration is the main factor of the influence on the rhizomatous formation of B. chinensis in vitro.
