RESUMO

Legal clinic education originating in the United States, aims at educating students on legal practical skills, and plays an positive role in cultivating students' ability of integrating theory with practice and training their various practical vocational skills. The present Chinese training mode of health law specialty does not perform very well in training students' practical ability, while legal clinic education can better make up for some deficiencies. Therefore, this paper learns from the experience of American universities in their legal clinic education, such as the particular division of specialties, the development of outside-school clinics, taking pre-training seriously, and the unique evaluation mechanism. Correspondingly, we explore to construct a set of reasonable legal clinic education system in health law specialty, including the orientation of legal clinics, the double training system inside and outside school, the construction of teaching staff and the establishment of evaluation mechanism.

RESUMO

Stroke is a devastating cerebrovascular event that occurs as a result of cerebral vascular occlusion (ischemic stroke) or burst/bleeding (hemorrhagic stroke) leading to cerebral blood flow disruption, causing physical disability and multiple functional impairment. The incidence of ischemic stroke is higher than hemorrhagic stroke, accounting for more than half of the total incidence of stroke. The main cause of the disease is caused by atherosclerosis, various arteritis, trauma and other plaques that cause cerebral vascular disease, which may be caused by a variety of cardiovascular diseases such as atrial fibrillation. Blood, caused by blocking blood vessels in the brain. Due to the complexity of the causes of ischemic stroke, the pathogenesis and development mechanism are complex and diverse, such as energy failure, excitotoxicity, neuroinflammation, apoptosis and oxidative stress. Traditional Chinese medicine and compound have the characteristics of multi-component, multi-target, multi-effect, and can be applied to the treatment of different stages of stroke occurrence and development. Therefore, an in-depth understanding of the pathological process and related mechanisms of ischemic stroke, and the development of targeted drugs, is important to prevent the occurrence of ischemic stroke and control its development. In this paper, the timeline of the occurrence and development of ischemic stroke is the main line. The pathological process and related mechanisms are systematically reviewed. The most widely used traditional Chinese medicine and compound in clinical and basic research are summarized, in order to study the pathological study of ischemic stroke. Clinical treatment provides a theoretical basis and data reference.

RESUMO

To analyze the medication regularity of traditional Chinese medicine(TCM) prescriptions for gastropyretic excessiveness diabetes recorded in Chinese Medicine Prescriptions Dictionary. A total of 103 eligible prescriptions were input into the system platform, and the Apriori algorithm was used to analyze their medication regularity. The 103 prescriptions for gastropyretic excessiveness diabetes were selected from the system, and 29 herb medicines were found with frequency of usage more than 8. Totally 33 commonly used herbal pairs(support degree≥10), twenty-three 3-herb core combinations(support degree≥8, confidence values≥0.5), and twenty-one 4-herb core combinations(confidence values≥0.5) were discovered after the medication regularity analysis by Apriori algorithm. The herbal medicine combinations with the highest correlation degree were discovered after the association rule analysis on the 103 prescriptions(support degree≥10, confidence values≥0.5). The four properties, five tastes, channel distributions and frequency of dose of the 103 prescriptions were also obtained after the corresponding analysis. According to the analysis and summary of the above data, the combination of Trichosanthis Radix, Anemarrhenae Rhizoma, Coptidis Rhizoma and Ophiopogonis Radix could reflect the medication regularity of TCM prescriptions for gastropyretic excessiveness diabetes to a certain degree, which is of great significance in guiding value in clinic.

RESUMO

Moot Court practice teaching is an important part of the teaching of medical and health law. Based on several years of university students' participating in moot court competition, and combined with the personal experience of the instructors and the participating students, we summed up our inexperience of medical law moot court practice teaching, and put forward feasible suggestions to professional Moot Court simulation practice teaching reform of Health Law in order to effectively improve the medical and health professional moot court practice teaching, thus implementing the law to strengthen the students practical skills trainingteaching objectives.

<p><b>BACKGROUND</b>In cardiology, it is controversial whether different therapy strategies influence prognosis after acute coronary syndrome. We examined and compared the long-term outcomes of invasive and conservative strategies in patients with non-ST-segment elevation myocardial infarction (NSTEMI) and characterized the patients selected for an invasive approach.</p><p><b>METHODS</b>A total of 976 patients with acute NSTEMI were collected from December 2006 to October 2012 in the First Affiliated Hospital of Dalian Medical University Hospital. They are divided into conservative strategy (586 patients) and invasive strategy (390 patients) group. Unified follow-up questionnaire was performed by telephone contact (cut-off date was November, 2013). The long-term clinical events were analyzed and related to the different treatment strategies.</p><p><b>RESULTS</b>The median follow-up time was 29 months. Mortality was 28.7% (n = 168) in the conservative group and 2.1% (n = 8) in the invasive management at long-term clinical follow-up. The secondary endpoint (the composite endpoint) was 59.0% (n = 346) in the conservative group and 30.3% (n = 118) in the invasive management. Multivariate analysis showed that patients in the conservative group had higher all-cause mortality rates than those who had the invasive management (adjusted risk ratio [RR] = 7.795; 95% confidence interval [CI]: 3.796-16.006, P < 0.001), and the similar result was also seen in the secondary endpoint (adjusted RR = 2.102; 95% CI: 1.694-2.610, P < 0.001). In the subgroup analysis according to each Thrombolysis in Myocardial Infarction risk score (TRS), log-rank analysis showed lower mortality and secondary endpoint rates in the invasive group with the intermediate and high-risk patients (TRS 3-7).</p><p><b>CONCLUSIONS</b>An invasive strategy could improve long-term outcomes for NSTEMI patients, especially for intermediate and high-risk ones (TRS 3-7).</p>

BACKGROUND: The melanocortin 4 receptor (MC4R) is involved in the regulation of homeostatic energy balance by the hypothalamus. Recent reports showed that MC4R can also control the motivation for food in association with a brain reward system, such as dopamine. We investigated the expression levels of MC4R and the dopamine D2 receptor (D2R), which is known to be related to food rewards, in both the hypothalamus and brain regions involved in food rewards. METHODS: We examined the expression levels of D2R and MC4R by dual immunofluorescence histochemistry in hypothalamic regions and in the bed nucleus of the stria terminalis (BNST), the central amygdala, and the ventral tegmental area of transgenic mice expressing enhanced green fluorescent protein under the control of the D2R gene. RESULTS: In the hypothalamic area, significant coexpression of MC4R and D2R was observed in the arcuate nucleus. We observed a significant coexpression of D2R and MC4R in the BNST, which has been suggested to be an important site for food reward. CONCLUSION: We suggest that MC4R and D2R function in the hypothalamus for control of energy homeostasis and that within the brain regions related with rewards, such as the BNST, the melanocortin system works synergistically with dopamine for the integration of food motivation in the control of feeding behaviors.

BACKGROUND:Polylactic acid(PLA)surface is hydrophobic and there are no nataral recognition sites,so its application is limited.Many different strategies have been studied such as composition and chemical Drafting,to produce hydrophilic groups.However,these traditional methods are always involved in complex process and many organic reagents,resulting in decrease of biocompatibility.OBJECTIVE:To conduct the surfacr modification of PL.A and improve its hydrophilicity with low temperature plasma treatment.DESIGN:Controlled observation.SETTING:Department of Materials Science and Engineering in Jinan University.MATERIAIS:The experiment was processed from October 2004 to October 2005 at Guangzhou Research Center of Artificial Organs and Materials Engineering(Ministry of Education).The mainly used materials were:Poly-D,L-lactic acid(Mr 2.9×104,Shandong Medical Instrument Institute),N-vinyl-pyrrolidone(NVP,Acros Company,purified before use).METHODS:The materials were treated in the plasma reactor under different conditions(power=150 W,t=3,2,1 minutes;power=30 W,t=10,8,5,3,1 minutes)to choose the optimal condition,and then PLA were grafted with NVP by gas phase polymerization and liquid polymerization,respectively.MAIN OUTCOME MEASURES:THe surface morphology of the films was observed with scanning electron microscopy and atomic force microscopy.The hydrophilicity of native and treated material surfaces was measured using a water contact angle meter.Surface composition was detected with Fourier transform infrared spectrometer.Mass changesmeasurement was applied to characterize the grafting efficiency.RESULTS:Scanning electron microscopy and atomic force microscopy showed that the plasma medified materials possessed coarse surface with pores and notches.Water contact angles decreased obviously from 78°to 50°after grafting,Fourier transform infrared spectrometer showed a new absorption peak at 1 637.19 cm-1,which corresponded to the carbonyl stretch vibration absorption of amide group in poly-N-vinyl-pyrrolidone.After the plasma treatmerlt at low temperature.the mass change ratio of PLA film material was-0.6.CONCLUSION:The plasma treatment and polymerization can improve the hydrophilicity and cause coarsr surface of PLA material,which will be helpful for the cell attachment.
