RESUMO

Objective:To analyze the current problems in the management of stem cell clinical research in China, explore how to improve the construction ability of stem cell research and management, and promote the well development of stem cell clinical research.Methods:By searching the relevant laws and policies of stem cell clinical research in China, identifying problems that need to be solved at the regulatory level, institutional level and individual research personnel, proposing countermeasures and suggestions on strengthening the construction of stem cell clinical research management in medical institutions.Results:There are uncertainties and high risks in the clinical application of stem cells, which is full of opportunities and challenges in the process of promoting the clinical research and application transformation of stem cells. There are many problems to be solved, such as the lack of detailed management standards, the imperfect internal system at the level of specific management institutions, and the lack of regulations and ethical concepts at the researcher level.Conclusions:Stem cell research is one of the promising new frontiers of science and technology. The management of stem cell clinical research needs the joint efforts of government departments and institutional management departments to discuss and establish the management system of stem cell clinical research. At the same time, it also needs the active engagement of stakeholder, such as researchers, stem cell enterprises and human subjects to work together in promoting the well development of stem cell clinical research.

Objective To observe the changes of serum heart-type fatty acid-binding protein (h-FABP) and brain natriuretic peptide (BNP) for assessing the clinical values in children with chronic heart failure (CHF).Methods A total of 50 patients with CHF,including 22 endocardial fibroelastosis (EFE) cases and 28 dilated cardiomyopathy (DCM) cases,were enrolled.Thirty healthy children were enrolled as controls.The concentrations of serum h-FABP and BNP were measured by enzyme-linked immunosorbent assay (ELISA),and the left ventricular end-systolic diameter (LVESD),left ventricular end-diastolic diameter (LVEDD),left ventricular ejection fraction (LVEF),left ventricular fractional shortening (LVFS),and cardiac index (CI) were measured by echocardiography.Results The concentrations of serum h-FABP and BNP in patients with CHF were significantly higher than those in control group (22.68 ±4.18 ng/mL vs.6.31 ± 1.72 ng/mL;565.52±179.64 pg/mLvs.31.22±9.83 pg/mL,all,P < 0.01),and h-FABP and BNP were positively correlated with the degree of heart failure (all,P <0.01),and they were both higher in group EFE and DCM than those in control group (all,P<0.01),but there was no statistically significant difference between the two groups (P > 0.05).The h-FABP concentration in CHF patients was positively correlated with BNP ((r =0.782,P <0.01).The h-FABP concentration in CHF patients was positively correlated with LVESD,LVEDD (r =0.626,O.662,respectively;all,P<0.01),and negatively correlated with LVEF,LVFS,and CI (r =-0.652,-0.636,-0.709,respectively;all,P < 0.01).The BNP concentration was also positively correlated with LVESD,LVEDD (r =0.733,0.689,respectively;all,P <0.01),and negatively correlated with LVEF,LVFS,and CI (r=-0.745,-0.612,-0.791,respectively;all,P<0.01).Conclusions Serum concentrations of h-FABP and BNP can be used as biomarkers to evaluate the severity of chronic heart failure.

OBJECTIVE: To study the mechanism of tumor necrosis factor( TNF)-α and its receptor( TNFR) signal transduction pathways in regulating cell apoptosis of alveolar macrophage( AM) in coal workers' pneumoconiosis( CWP).METHODS: Twenty-four coal workers with pneumoconiosis at stage Ⅰ were selected as CWP group and four observation subjects exposed to coal were chosen as observation group by using simple random sampling method. The bronchoalveolar lavage fluids of whole-lung lavage of two groups were collected. AMs were separated and purified. Then they were divided into 6 groups: a control group,a superoxide dismutase( SOD) group,a TNF/TNFR group,an anti-TNF-α antibody group,a Caspase-8 suppression group and a nuclear factor-κB( NF-κB) suppression group. The AMs of 6 groups with corresponding treatment were cultivated. After 24 hours,the cells were harvested and proteins extracted. The relative expression of TNF-α,TNFR1,TNFR2,Caspase-8,Caspase-3,NF-κB P50 and NF-κB P65 protein was detected by Western blotting. RESULTS: The protein relative expression of TNF-α,TNFR2,Caspase-8,Caspase-3,NF-κB P50 and NF-κB P65 in CWP group was significantly higher than those in the observation group( P < 0. 05). The protein relative expression of TNF-α,TNFR1,Caspase-8,Caspase-3 and NF-κB P50 in the TNF/TNFR group and the anti-TNF-αantibody group was lower than that of the control group( P < 0. 05). The above indexes in the anti-TNF-α antibody group were lower than that of the NF-κB suppression group( P < 0. 05). The protein relative expression of TNFR1,Caspase-8and Caspase-3 in the TNF/TNFR group was higher than that of the SOD group and the Caspase-8 suppression group( P <0. 05). The protein relative expression of TNFR1,Caspase-8 and NF-κB P50 in the TNF/TNFR group was lower than that of the NF-κB suppression group( P < 0. 05). Among the CWP patients,the relative expression of TNFR2 and NF-κB P65 in the TNF/TNFR group was lower than that of the control group( P < 0. 05),and higher than that of the SOD group( P <0. 05). CONCLUSION: AM apoptosis mediated by TNF-α/TNFR/NF-κB signal transduction pathway plays an important role in the occurrence and development of CWP. The TNF-α/TNFR/NF-κB signal transduction pathways inhibited or blocked at different stages can affect the expression of proteins related to AM apoptosis.

Objective To explore the recovery time of hepatic function in infants with jaundice type of human cytomega-lovirus (HCMV) hepatitis and its influencing factors. Methods The clinical data of 73 infants with jaundice type of HCMV hepa-titis admitted to hospital from February 2005 to October 2012 were retrospectively analyzed. The effects on hepatic function of nine factors including age, sex, liver size, total bilirubin (TBil), direct bilirubin (DBil), alanine aminotransferase (ALT), aspar-tate aminotransferase (AST), total bile acid (TBA) and the loads of HCMV DNA were assessed by Kaplan Meier method, and further analyzed by Cox proportional hazards regression model. Results The results of Cox proportional hazards regression showed that age≤3 month (RR=0.27, 95%CI:0.10-0.70), DBil≤90μmol/L (RR=0.16, 95%CI:0.08-0.32), male (RR=0.49, 95%CI:0.26-0.94) and enlarged liver size<3 cm (RR=0.50, 95%CI:0.27-0.93) were independent factors that shorten the time for TBil back to be normal. Furthermore, AST≤120 U/L (RR=0.16, 95%CI:0.08-0.33) and enlarged liver size<3 cm (RR=0.28, 95%CI:0.15-0.49) were independent factors that shorten the time for AST back to be normal. The time for TBil back to be normal was (2.23 ± 1.54) months, which was significantly shorter than that [(3.63 ± 1.93) months] of AST (t=10.37, P<0.001). Conclusions Jaundice type HCMV hepatitis had good outcome and varied in disease course. The recovery of hepatic function was significantly adversely affected by the degree of cholestasis and hepatomegaly as well as AST level, and early treatment was conducive to the recovery.

Objective To study the influencing factors of retinal vessel caliber in type 2 diabetes mellitus and supply the elementary data for clinic. Methods Two hundred diabetic patients were involved in this study, and the vessel caliber was measured by fundus oculi color photo and computer-assisted imaging program. Biochemical indicator and blood pressure was examined and other document was achieved by questionaire. Results In type 2 diabetes mellitus, the retinal vein caliber had negative correlation with age and positive correlation with fasting blood glucose (FBG), glycosylated hemoglobin A1c (HbA1c) and the duration of diabetes mellitus(P < 0.01 ). The retinal arteriole caliber had negative correlation with age, blood pressure and the duration of diabetes mellitus (P < 0.01 or < 0.05 ). Conclusion The influencing factors of retinal vessel caliber in type 2 diabetes mellitus are age,FBG,HbA1c,blood pressure and the duration of diabetes mellitus.

Objective To observe the signs of retinal vessel diameter among different clinical characteristic groups such as sex,age,BMI,the duration of DM. Methods200 diabetic patients were selected.Using face to face questionnaire survey forms filled in the questionnaire in order to collect the general situation of all subjects selected.The diameters of vessels were measured using a computer-assisted imaging program. ResultsIn type 2 diabetes mellitus,there was no significant difference in the retinal arteriolar and veular diameters between the groups with different sex.In type 2 diabetes mellitus,as age increased,both the retinal arteriolar diameters and the retinal veular diameters declined.In type 2 diabetes mellitus,there was no significant difference in the retinal arteriolar and veular diameters between the groups with different BMI.In type 2 diabetes mellitus,as the duration of DM increased,the retinal veular diameters dilated and the retinal arteriolar diameters declined.After storied in accordance with age,we can draw the sameConclusion . ConclusionIn type 2 diabetes mellitus,as the duration of DM increased,the retinal veular diameters dilated and the retinal arteriolar diameters declined.

Objective To study the serum levels of brain natriuretic peptide (BNP) and the correlation with the heart function in children with Kawasaki disease(KD), and to explore its clinical value for diagnosis of KD. Method A total of 43 children aged from 5 months to 8 years with mean age of (2.3 + 0.6) years with KD admitted from February 2007 to April 2009 were enrolled into this study as KD group, and patients with myocarditis, myocardiopathy, congenital heart disease and other primary heart disease were ruled out. Another 30 healthy children were taken as control health group. There were no significant differences in age and gender between two groups (P >0.05) .The serum levels of BNP were measured both in acute and recovery stages of KD by using ELISA. The serum levels of BNP in healthy children were measured randomly once. The left ventricular ejection fraction (LVEF), left ventricular shorten fraction ( LVSF), cardiac index (CI) and left ventricular inflow velocity through the mitral annulus (including E-velocity and A-velocity) were measured by using two-dimensional echocardiography in acute and recovery stages of KD. Data were analyzed with t -test and the linear regression analysis test. Results The serum level of BNP in acute stage was (517.26 + 213.40) ng/mL and was significantly higher than that in recovery stage (91.56 + 47.97) ng/mL, and higher than that in control group (91.56 + 47.97) ng/mL (P < 0.01). The levels of LVEF, LVSF and CI in the acute stage were significantly lower than those in the recoverystage ( P < 0.0%), but there was no significant difference in E/A between acute stage and recovery stage (P > 0.05). The BNP level had negative correlation with the levels of LVEF, LVSF and CI(r = -0.63, -0.52, and - 0.53, respectively, P < 0.05), but had no significant correlation with E/A (r = - 0.18, P > 0.05). Conclusions The serum levels of BNP increase significantly in the KD patients, and have negative correlation with the levels of LVEF, LVSF and CI. The detection of serum levels of BNP has an important significance for diagnosis of KD.

BACKGROUND: The clinical manifestation of senile depression is not typical. The patients with senile depression seldom visit in general hospital, which is hard for clinical doctors to identify the disease at early time.OBJECTIVE: To research the traits of Coping style and social support in the recovered senile depressive patients.DESIGN: Case-control observation.SETTING: Department of Psychology in North China Coal Medical College.PARTICIPANTS: Ninety-six senile depressive patients at convalescent stage were selected for depressive group in clinic service of Beijing Anding Hospital from November 2004 to October 2005. Inclusion criteria: ①Aged≥60 years old. ② Coded with depression with DSM-Ⅳ. ③ Being the period of recovery, geriatric depression scale(GDS)≤10. ④Informed consent was obtained from each participant. Exclusion criteria: ① Depression resulted from other factors for instance substance abuse and schizophrenia,etc. ②Serious and instable body disease such as cerebrovascular disease,liver and kidney disease, falling sickness, disease of internal secretion. Totally 104 questionnaires were sent out, and 96 were qualified. 37 were male and 59 were female in the depressive group, aged 60-87 years old,with average of (68±6)years. The course of disease was 0.3-11 years, with mean of 2 years. The elderly from ordinary community of Beijing were invited for control group. Inclusion criteria: ①Aged ≥ 60 years old; ② No mental disease. ③ Informed consent was obtained from each participant.Exclusion criteria: The same to the depressive group. There are 100 participants in the control group, 44 were male and 56 were female, aged 60 to 84 years old, with average (67±5) years old. There were no significant differences between depressive group and control group in gender, age, education, living conditions and marriage status.METHODS: 96 senile depressive patients at convalescent stage and 100healthy aged were investigated with self-made questionnaire, Geriatric Depression Scale (GDS), Social Support Rating Scale (SSRS) and Simplified Coping Style Questionnaire (SCSQ).MAIN OUTCOME MEASURES: The scores of the self-made questionnaire, GDS, SSRS and SCSQ answered by participants in each group.RESULTS: All the 196 participants entered the result analysis. There was not disqualified questionnaire. There were significantly lower scores of positive coping style and higher scores of negative coping style in the depressive group than the control group(P < 0.05 ). There was lower score of social support, subjective social support and use of social support in the depressive group than the control group (P < 0.05). The scores of objective social support was not significantly different in each group. There was remarkable positive correlation between active coping style and score of social support, subjective social support and use of social support in recovered senile depressive group (P < 0.01). Passive coping style was negatively correlated with score of social support, subjective social support and use of social support(P < 0.01 ).CONCLUSION: Subjective social support and use of social support and negative coping style are the major risk factor in the onset of senile depression.

<p><b>OBJECTIVE</b>To evaluate the immunogenicity, safety, and dosage of a new inactivated hepatitis A vaccine administered to young adults.</p><p><b>METHODS</b>One hundred and four normal adult volunteers, seronegative for hepatitis A virus and hepatitis B surface antigen, were randomly assigned to one of three groups. The high-dose group received a primary dose of 1000 units of the new vaccine, the low-dose group received a primary dose of 500 units of the same vaccine, and the Havrix group received a primary dose of 1440 enzyme-linked immunosorbent assay units of Havrix, a licensed inactivated hepatitis A vaccine. All groups received a booster dose of the same vaccine 6 months after the primary dose. Local and systemic adverse reactions, seroconversion rates, and geometric mean titers of hepatitis A virus antibodies were measured in all three groups.</p><p><b>RESULTS</b>Local and systemic reaction types and rates were similar in all three groups after primary and booster doses, although local reactions were more frequent in the Havrix group following the primary dose. No serious adverse reactions occurred. One month after the primary dose, the seroconversion rate was 87.5% in the high-dose group, 70.0% in the low-dose group, and 50.0% in the Havrix group (P = 0.001, versus the high-dose group). At month 6 (before administration of the booster dose), seroconversion rates were 96.9% in the high-dose group, 65.0% in the low-dose group (P = 0.0029), and 68.8% in the Havrix group (P = 0.007). All subjects in all groups seroconverted by one month after receipt of the booster dose. Geometric mean titers were similar in all three groups at month 1, but were higher in the high-dose group (264 mIU/ml) than those in the Havrix group (135 mIU/ml) at month 6 (P = 0.0013). One month after the booster dose, geometric mean titers in the high-dose group (2747 mIU/ml) were higher than those in the low-dose group (1657 mIU/ml) (P = 0.0223) or in the Havrix group (1316 mIU/ml) (P = 0.01).</p><p><b>CONCLUSIONS</b>This new inactivated hepatitis A vaccine is immunogenic and safe; two doses of either 500 or 1000 units can induce hepatitis A virus antibodies well above the protection level.</p>
