RESUMO

Objective:To investigate the application value of white noise therapy on the alleviation of procedural pain of colostomy newborns.Methods:By a prospective, randomized and controlled trial, a total of 88 colostomy newborns in Hunan Children′s Hospital from January 2018 to January 2020 divided into experimental group (44 cases) and control group (44 cases) according to the random number table method. The control group received routine nursing; based on thesis, the experimental group played white noise intervention therapy on the basis of routine nursing. The intervention effect was assessed byNeonatal Infant Acute Pain Assessment Scale (NIAPAS), the first crying time and the duration of first crying, the first painful face and the duration of first painful face as well as heart rate and blood oxygen saturation.Results:The first crying time and the duration of first crying, the first painful face and the duration of first painful face were (28.05 ± 7.39) s, (46.18 ± 13.29) s, (32.89 ± 6.79) s, (52.75 ± 10.71) s in the experimental group, significantly shorter than in the control group (35.79 ± 5.81) s, (35.79 ± 5.81) s, (38.64 ± 10.53) s, (59.79 ± 13.52) s, the difference was statistically significant ( t values were 2.71-5.47, all P<0.05). During and after the procedure, the scores of NIAPAS were (6.32 ± 1.62) points, (4.18 ± 1.06) points in the experimental group, significantly lower than that in the control group (7.43 ± 1.78) points, (4.79 ± 1.34) points ( t=3.06, 2.38, both P<0.05); the heart rate were (152.82 ± 13.25) times/min and (147.84 ± 12.37) times/min in the experimental group, significantly lower than in the control group (166.11 ± 13.79) times/min and (155.77 ± 12.84) times/min ( t=4.61, 2.95, both P<0.05); the blood oxygen saturation were 0.979 8 ± 0.009 5 and 0.980 9 ± 0.012 4 in the experimental group, significantly higher than in the control group 0.969 1 ± 0.014 9, 0.972 3 ± 0.017 8, the difference was statistically significant ( t=4.01, 2.65, both P<0.05). Conclusions:White noise therapy can effectively alleviate procedural pain and stabilizing vital signs of colostomy newborns.

RESUMO

The outbreak of COVID-19 posed a huge threat to human health and social stability. With the rapid spread of the virus around the world, the drug development and related research of novel coronavirus (SARS-CoV-2) have become an urgent issue in the medical field. COVID-19 fails into the category of epidemics in the theory of TCM. LHQW capsule has repeatedly played an important role in many major epidemics. Previous studies have shown that LHQW capsule can inhibit the biological activity of varied viruses including MERS-CoV and SARS-CoV. The paper summarizes the relevant research data and achievements of LHQW capsule in the past few years, reviews the chemical constituents, clinical efficacy and pharmacological effects of LHQW capsule, and provides scientific basis for the anti-virus mechanism of LHQW capsule and clinical treatment of COVID-19.

Objective@#To investigate the contamination, antimicrobial resistance and virulence genes of S. aureus from toilets of primary schools in Guangzhou.@*Methods@#The surface samples of toilets were collected from eight primary schools in Guangzhou from May to July 2016. The standard microbiological assays, disk diffusion methods and PCR technique were used for the isolation and identification, antimicrobial resistance and virulence genes of S. aureus .@*Results@#The contamination rate of S. aureus and MRSA was 6.25% and 3.13%, respectively. There was significant difference in the contamination rate of S. aureus among different sampling sites ( χ 2=15.15, P <0.01) and the highest contamination rate was on the ground (15.00%).The most predominant antibiotic for S. aureus was penicillin (100.00%) and the proportions of resistant to teicoplanin, erythromycin,rifampicin, clindamycin and linezolid were more than 75.00%.The multidrug resistant rate of S. aureus was 85.00%.The detection rate of virulence genes of S. aureus was sea (50.00%), tst (30.00%), etb (15.00%), eta (10.00%), seb (10.00%) and pvl (5.00%), respectively.@*Conclusion@#The contamination rate of S. aureus from toilets of primary schools in Guangzhou is in a lower level among similar researches. However, the contamination of MRSA is serious, which accounts for half of S. aureus . In addition, S. aureus isolates show high multi-drug resistant rate and high detection rate of virulence genes.

Objective: To investigate the protective effects of tanshinone ⅡA (tan ⅡA) against memory deficits induced by streptozotocin (STZ) in a model of sporadic Alzheimer's disease. Methods: Kunming mice were randomly divided into five groups (n=10 in each group): control group, STZ group, and tan ⅡA low, medium and high groups. Mice in the STZ and three tan ⅡA groups were given intracerebroventrically injection of STZ (3 mg/kg bilaterally, on day 1 and day 3). Tan ⅡA (20, 40, and 80 mg/kg every day) was administered intragastrically for 28 days starting from the first dose of STZ. Learning and memory were evaluated with a Morris water maze (MWM) test. Nissl method was used to observe neurons damage in the cortex and the hippocampus, and Western blot method to analyze endoplasmic reticulum stress markers and apoptosis signaling proteins. Results: Daily treatment with tan ⅡA showed a dose-dependent improvement in STZ-induced memory deficits. Nissl staining results confirmed the protective effects of tan ⅡA on cortical and hippocampal neurons damage induced by STZ. In addition, tan ⅡA markedly prevented STZ-induced abnormal expression of glucose regulated protein 78 (GRP78), initiation factor 2α (eIF2α), activating transcription factor 6 (ATF6), as well as suppressed the activation of C/EBP homologous protein (CHOP) and c-Jun N-terminal kinase (JNK) pathways. Moreover, tan ⅡA induced an up-regulation of the Bcl-2/Bax ratio and down-regulation of cleaved caspase-3 protein activity. Conclusion: Tan ⅡA prevents STZ-induced memory deficits, which may be attributed to attenuating endoplasmic reticulum stress and neuronal apoptosis.

Objective@#To investigate the distribution and drug resistance situation of staphylococcus aureus (SA) and methicillin-resistant staphylococcus aureus (MRSA) from the classroom environments in primary schools of Guangzhou.@*Methods@#The air and the surfaces of door handles, desks, chairs, light switches and floor were sampled in the classrooms of 8 primary schools selected through stratified clustering method in Guangzhou from May to June, 2016. SA and MRSA were isolated and identified, and drug sensitivity tests were conducted.@*Results@#A total of 760 samples were collected, the detection rate of SA and MRSA were 8.8% and 4.2%, respectively. There were statistically significant differences in the detection rate of staphylococcus aureus among different sampling sites(P<0.01).Detection of SA and MRSA on the floor,light’s witches and surface of deskes was both above 6.0%. The multiple drug resistance rate of MRSA was up to 100.0%, and the main resistance mode was Penicillin-Erythromycin-Rifampin-Tetracycline-Teicolanin.@*Conclusion@#MRSA can be detected in air, door handles, desk surface, chair surface, light switch and floor of primary schools. Relevant administration departments should pay attention to the environments health of Guangzhou primary schools.

Objective:To explore the technical advantages of MR cholangiopancreatography (MRCP) with single breath holding high parallel acquisition factor 3-D variable flip angle fast spin echo (3D-SPACE) sequence.Methods:From November 2018 to March 2019, 75 patients who underwent MRCP examination in our hospital were prospectively enrolled, with single breath holding high parallel acquisition factor 3D-SPACE sequence and free breathing navigation gated 3D-SPACE sequence. Three experienced radiologists scored the overall image quality, artifacts, CBD visibility, left and right hepatic ducts, right anterior and posterior branches, second and third branches, main pancreatic duct and gallbladder duct with four scales. Paired t test was used for statistical analysis. Results:The scanning time of single breath holding method (18 s) was significantly shorter than that of free breathing diaphragm navigation method[264(226,313)s], and the difference between the two methods was statistically significant ( Z=-7.520, P<0.001). The SNR, CR and CNR (8.31±4.23, 0.92±0.30, 11.46±5.77) of single breath holding method were lower than those of free breathing diaphragm navigation method (11.23±5.70, 0.93±0.38, 15.06±7.37), and the differences between the two methods were also statistically significant ( t=4.378, 3.429, 4.063, P<0.05). The overall image quality, artifact, the CBD, left and right hepatic duct, right anterior and posterior branchs, the second and third branches, main pancreatic duct and cystic duct of single breath holding method were higher than those of free breathing diaphragm navigation method, and the differences between the two methods were statistically significant ( P<0.001). Conclusions:Compared with the free breathing diaphragm navigation gated 3D-SPACE MRCP imaging method, the single breath holding high parallel acquisition factor 3D-SPACE MRCP imaging method has less artifacts and examination time, but higher visibility to pancreaticobiliary tree and work efficiency, which is worthy of further promotion.

Objective:To explore the effect of oral motor intervention on oral feeding ability and growth of neonates with intestinal atresia.Methods:A total of 80 intestinal atresia neonates in the Hunan Children`s Hospital from January 2017 to January 2019 were admitted to the present study. Neonates were randomly assigned to oral motor group and control group according to the random number table. The control group received routine nursing, while the oral motor group carried out oral motor intervention for 14 consecutive days. Both groups were followed up for six months. The oral feeding ability and growth index such as body weight, body length and head circumference were compared between two groups.Results:The complete oral feeding rate transfer, complete oral feeding rate transfer, time to complete oral feeding were (10.48±2.96) ml/min, (90.02±8.36) %, (15.06±2.99) days in the oral motor group, those index were (8.18±2.44) ml/min, (72.58±9.46) %, (18.08±4.98) days in the control group, the differences were statistically significant ( t values were 3.633, 8.316, 3.106, P<0.01). After 7, 14 days of intervention, the non-nutritive sucking scores were (52.24±8.89) points, (69.81±12.94) points in the oral motor group, significantly higher than (48.08±6.72) points, (63.09±11.73) points in the control group, the differences were statistically significant ( t values were 2.265, 2.327, P<0.05). After 3, 6 months of intervention, the levels of body weight were (6 234.21±560.25) g, (7 630.19±782.25) g, significantly higher than (5 916.89±462.40) g, (7 293.65±979.98) g in the control group, the differences were statistically significant ( t values were 2.648, 2.148, P<0.05). After 6 months of intervention, the levels of head circumference were (43.81±5.59) cm in the oral motor group, significantly higher than (40.85±3.73) cm in the control group, the difference was statistically significant ( t value was 2.635, P<0.05). Conclusion:Oral motor intervention can promote oral feeding ability and improve growth performance of neonates with intestinal atresia.

Objective@#To investigate the antimicrobial resistance, macrolide-resistance genes, virulence genes and emm types in Streptococcus pyogenes isolates.@*Methods@#A total of 247 oropharyngeal swab specimens were collected from pediatric outpatients (aged 2-11 years) who were clinically diagnosed as scarlet fever in Children′s Hospital of Fudan University from January to December, 2018. These specimens were timely sent to the Microbiology Laboratory for isolation and identification of Streptococcus pyogenes strains were isolate after culturing and identified with bacitracin susceptibility test. Moreover, the diameter of bacitracin inhibition zone was measured by vernier caliper. Their susceptibility to seven antibiotics, including erythromycin, clarithromycin, clindamycin, ampicillin, ceftriaxone, norfloxacin and chloramphenicol, were measured using KB method. Macrolide-resistance genes (mefA, ermA, ermB and Tn916 transposon) and virulence genes (speA, speB, speC, speG, speH, speI, speJ and speK) were detected by PCR. Amplification and sequencing of emm gene were conducted according to the protocol in the website of Centre for Disease Control and Prevention (CDC).@*Results@#A total of 86 strains of Streptococcus pyogenes were isolated from the 247 specimens. Their resistance rates to erythromycin, clarithromycin and clindamycin were 89.5%, 95.3% and 96.5%, respectively. However, these isolates showed high susceptibility to ampicillin (100.0%), ceftriaxone (100.0%), norfloxacin (90.7%) and chloramphenicol (95.3%). The positive rates of mefA, ermA, ermB and Tn916 genes were 20.9%, 24.4%, 98.8% and 97.7%. There was significant difference in the mefA-carrying rates between patients with scarlet fever and angina. The positive rates of virulence genes of speA, speB, speC, speG, speH, speI, speJ, speK, speL and speM were 8.1%, 100.0%, 95.3%, 100.0%, 80.2%, 90.7%, 10.5%, 100.0%, 5.8% and 5.8%. Seven emm types were identified and the predominant types were emm12.0 (75.6%), emm12.19 (9.3%) and emm1.0 (8.1%). The diameter of bacitracin inhibition zone was smaller in isolates of emm1.0 type than in emm12.0 type strains. The profile of virulence genes varied in the strains of different emm types. All types of strains carried speB, speG and speK genes. The isolates of emm1.0 type carried speA virulence gene, while speB, speC, speG, speH, speI and speK genes were more often identified in emm12.0 type isolates.@*Conclusions@#This study showed that emm types were associated with the profile of virulence genes and the diameter of bacitracin inhibition zone. It was recommended that the diameter of bacitracin inhibition zone should be measured in bacitracin inhibition susceptibility test apart from only observing the formation of inhibition zone.

Salmonella enterica Serovar Indiana is a common serotype of Salmonella isolated from food especially from poultry meat. Recently it demonstrated a raising tendency of infection cases and isolate numbers with high antimicrobial resistant rate against many common antimicrobials, including quinolones and cephalosporin which were regarded as the first line drug for the treatment of Salmonella infections, and this kind of Salmonella serotype was always carrying complex resistance mechanisms and also a variety of mobile elements, all of these features made the very clinical infections caused by Salmonella hard to treat and brought great difficulties and risks. Here, we review the prevalence of Samonella Indiana on national and international view, and we also anticipate the research progress on antimicrobial drug classes, multi drug resistance, co-resistance and resistance mechanism. We discuss the resistant genotypes, phenotypes, mechanism and transmission of Salmonella Indiana strains isolated from different origins. By introducing the resistance of Salmonella Indiana, we want to attract people's attention to this bacteria and its hazard, and offer some idea to evaluate and treat infections in clinical.

Objective@#To obtain the serotype diversity and antimicrobial resistance of Salmonella isolates recovered from retail chicken carcasses for sale in six regions of China.@*Methods@#From August 2010 to March 2012, each month 20 retail chicken carcasses including freshly slaughtered, chilled and frozen samples were collected from supermarkets and farmer's markets in 7 monitoring sites in Beijing, Jilin province, Inner Mongolia Autonomous, Shanxi province, Jiangsu province and Guangdong province, respectively. Samples were routinely collected for 12 months for each site. 1 680 chicken carcasses were collected in total and 2 629 Salmonella strains were isolated by PCR and biochemical method. Luminex xMAP method and classical slide agglutination method were carried out to determine isolates' serotypes. Minimal inhibitory concentrations (MICs) of 10 classes of antimicrobials including 14 agents were determined using broth micro-dilution method. Mocular methods were used to determine antimicrobial resistance genes of CIP-CTX-CT co-resistant isolates.@*Results@#In all, 2 629 Salmonella isolates, there were 17 seorgroups and 58 serotypes, B and D1 were the dominant serogroups with rates of 34.7% (n=913) and 31.0% (n=815), Enteritidis (30.8%, n=810), Indiana (17.6%, n=463), Infantis (10.6%, n=278) were the top three serovars. We found 224 CIP-CTX co-resistant S. Indiana containing 3 colistin resistant strains, one of them carrying mcr-1 gene and being ESBLs positive, which demonstrated a nine multi drug resistance against 11 antimicrobials tested.@*Conclusion@#These data began to describe the complicated serovar diversity and heavy antimicrobial resistance of Salmonella isolates recovered from retail chicken carcasses in six regions of China. The findings highlight the emergence of ciprofloxacin and cefotaxime co-resistant S. Indiana and also a mcr-1 positive S. Indiana with heavy multi drug resistance.

Objective To evaluate the feasibility and efficacy of twin grasper combined with endoloops on closing acute stomach perforation.Methods Two experimental porcines of stomach perforation underwent endoscopic closure by twin grasper combined with endoloops.Survival conditions and closure of perforating were observed.Results Mild infection and abdominal adhesions were observed in one week after operation.The nylon rope fell off and an ulcer-like lesion was presented in the closed position.The methylene blue leak test was negative.Histologic examination revealed chronic inflammatory cells infiltration,granulation,fibroplasias,and regenerative mucous membrane crawling toward the center of perforation.Conclusion The technique of twin grasper combined with nylon ropes in closing the acute stomach perforation is feasible and effective.

<p><b>OBJECTIVE</b>To elucidate the epidemic condition and molecular subtyping of ciprofloxacin and cefotaxime co-resistant Salmonella Indiana (S. Indiana) isolated from retail chicken carcasses in six provinces of China.</p><p><b>METHODS</b>A total of 2 647 Salmonella strains isolated from retail chicken carcasses collected from six provinces of China were subjected to antimicrobial susceptibility testing. All Salmonella isolates co-resistant to ciprofloxacin and cefotaxime were further characterized by serotyping, extended-spectrum beta-lactamases (ESBLs) producing strains screening and pulsed field gel electrophoresis (PFGE) typing.</p><p><b>RESULTS</b>Among 2 629 Salmonella isolates tested, 227 (8.52%) isolates were co-resistant to ciprofloxacin and ceftazidime/cefotaxime (Beijing: 11.67% (99/874), Jilin: 8.20% (60/726), Guangdong: 1.39% (7/502), Jiangsu: 15.61% (42/260), Shaanxi: 8.56% (16/186), Inner Mongolia: 0 (0/81)), and 224 of them were identified as S. Indiana. 213 (95.10%) isolates of S. Indiana were ESBLs producing strains. All ciprofloxacin and cefotaxime co-resistant S. Indiana isolates developed a multi-drug resistant profile and 17.86% (40/224) of them were resistant to all antibiotics tested except carbapenems, and 50.89% (114/224) of them resistant to 9 antibiotics, additionally, 25.45% (57/224) of them showed multi-drug resistance to 8 antibiotics. All ciprofloxacin and cefotaxime co-resistant S. Indiana isolates were divided into 32 PFGE clusters and 150 PFGE patterns. Strains of S. Indiana from same or different sampling site and time seemed to either share the same PFGE patterns or be differential to each other in different regions.</p><p><b>CONCLUSION</b>The results indicated that chicken carcasses collected from parts of China were heavily contaminated by ciprofloxacin and cefotaxime co-resistant S. Indiana and could serve as an important reservoir of ciprofloxacin and cefotaxime co-resistant Salmonella. Molecular subtyping results indicated that cross contamination or common pollution source might be in these strains.</p>

Objective to observe the efficacy and application value of PFNA and Gamma in the treatment of intertrochanteric fractures in elderly. Methods 100 patients with intertrochanteric fractures were divided into PFNA group and Gamma group, who received PFNA treatment and Gamma treatment respectively. And the operation status of patients at different ages,postoperative recovery,and complications were observed. Results The operation time,intraoperatve blood soss of PFNA group were lower than those of Gamma group(P<0. 05). There was no significant difference in fracture healing time. The Harris score of the over 75s in PFNA group was higher than that in Gamma group(P<0. 05),and the complications were less than Gamma group(P<0. 05). Conclusion PFNA was suitable for the patients at differ-ent ages with the advantages of more rigid fixation,good anti-rotation,fewer trauma and fewer complications.
