RESUMO

Objective: To observe the efficacy of Endobutton combined with the suture in the treatment of Meyers-Mckeever-Zaricnyj type IV tibial eminence fracture, and to explore the characteristics and treatment methods of Meyers-Mckeever-Zaricnyj type IV fracture. Methods: The clinical data of seven patients with Meyers-Mckeever-Zaricnyj type IV tibial eminence fracture were collected; the literature review was performed, and the surgical treatment methods, key points of operation and precautions of the patients were analyzed. Results: After arthroscopic debridement, 3 kinds of main fractures were found: anterior cruciate ligament connected fragment (ACLCF), anterior horn of lateral meniscus connected fragment (ALMCF), and posterior-medial cartilage connected fragment (PMCCF). The PMCCF was first fixed after pre-reduction from back to front and from deep to shallow orders, then the ALMCF and ACLCF were fixed, avoiding the secondary displacement and bone cutting. The follow-up periods of 7 patients were 6-12 months, with an average followed-up period of 7 months. The front drawer test (-), Lachman test (-); the average Lysholm score of patients before operation was less than that in the last follow-up period (P<0. 05). The knee joint stability of patients was good, and the knee joint had no movement disorder and infection. Conclusion: The intra-operative reduction order is crucial during the operation of Meyers-Mckeever-Zaricnyj type IV tibial eminence fracture. Perfect fixation can be achieved by using Endobutton combined with suture.

RESUMO

The aim study was to observe and analyze the effect of glucosamine hydrochloride tablets on patients with cervical spondylosis. This study was conducted on 130 patients diagnosed with cervical spondylosis who were treated in our hospital. The time period was from June 2015 to December 2017 . The subjects were randomly divided into a reference group treated with cervical vertebra exercises and cervical occipital belt traction therapy and the study group was further treated with glucosamine hydrochloride tablets. The treatment efficacy of both groups was observed. Comparison of the overall treatment efficancy of patients showed that compared with the reference group, the study group has more significant advantages, P<0.05; comparison of the overall patient satisfaction rate showed that the study group was also superior to the reference group, P<0.05; In addition, statistical analysis of adverse reactions showed no statistically significant difference, P<0.05. The treatment of glucosamine hydrochloride tablets in patients with cervical spondylosis can achieve ideal results, improve the overall treatment efficiency, and thus, has important application significance.

To observe and analyze the clinical effect of cervical paravertebral nerve block combined with mailuoning and Angelica sinensis is injection in treatment of nerve-root type cervical spondylosis. 120 patients who have been accepted by our hospital for nerve-root cervical spondylosis were selected as research objects. They were randomly divided into a research group and a control group, each containing 60 patients. The control group was applied with glucocorticiod nerve block therapy, while the research group was treated by cervical paravertebral nerve block combined with Mailuoning and Angelica sinesis injection. The total therapeutic response rate of two groups were compared. The total therapeutic response rate of the research group was relatively higher,P<0.05. Through comparing various clinical effect scores between two groups, the research group also had advantages over control group, P<0.05. The treatment satisfaction degree of the research group was also higher than that of the control group, P<0.05. The application of cervical paravertebral nerve block combined with Mailuoning and Angelica sinensis injection can achieve better medical results and achieve higher treatment satisfaction degree

Objective:To observe the process and curative effect of 12 cases of recurrent patellar dislocation treated by double bundle medial patellofemoral ligament (MPFL)reconstruction with anterior half peroneus longus and lateral retinacular release,and to investigate the etiology and treatment method of recurrent patellar dislocation. Methods:A total of 12 patients with recurrent patellar dislocation were enrolled in this study,the MPFL was reconstructed with the anterior half peroneus longus,patellar fixation with suture anchors was completed with 2 parallel 5.0 mm anchors which were spaced 1.0 cm apart at the anatomic insertion site of the native MPFL,the femoral side was secured with a interference screw,and the lateral retinaculum was released at the same time.The Lysholm score,IKDC score,congruence angle,J sign,grind test,and apprehension test of the patients before and after operation were detected.Results:The mean follow-up period was 16.4 months,and the Lysholm score of the patients before operation was lower than the last follow-up (t = 9.03,P < 0.001);the IKDC score of the patients before operation was lower than the last follow-up (t = 9.75,P < 0.001);the congruence angle of the patients before operation was larger than after operation (t = 7.22,P <0.001).All of the patients demonstrated the positive results before operation in J sign,grind test,and apprehension test,and the negative results in J sign, grind test,and apprehension test after operation.No patient appeared pateela fracture and recurrence of patellar dislocation during the follow-up period.Conclusion:The curative effect of reconstruction of the MPFL with anterior half peroneus longus combined with lateral retinacular release is well in the treatment of recurrent patellar dislocation,which is suitable for clinical promotion.

Objective To investigate the effectiveness analysis of fine nursing care on quality of nursing of critically ill patients with stroke. Methods The control group who came from the neurology department of our hospital between February and April in 2015 were given the routine care. The test group who came from the neurology department of our hospital between May and July in 2015 were given the fine nursing care. The quality of nursing and satisfaction of the relatives of patients were compared. Results In the test group, unqualified rates of nursing quality was 8.8%(81/924) and satisfaction of the relatives of patient was 90.9% (229/252); In the control group, unqualified rates of nursing quality was 22.5%(183/814) and satisfaction of the relatives of patient was 82.0%(182/222).There were significant differences between two teams (χ2=63.191, 8.096, P<0.01 or 0.05). Conclusions Fine nursing care can significantly improve the quality of nursing, improve the nurse-patient relationship and improve satisfaction of the relatives of patients.

Objective To investigate the immunization effect of influenza A/H1N1 vaccine in health care workers (HCW) in Inner Mongolia Greater Khingan Mountains area. Methods Five hundred and five HCW who received A/H1N1 influenza vaccination (immunized group) and 129 staffs who didn't receive the vaccination (unimmunized group) were randomly sampled for semiquantitative testing of serum H1N1 antibody (IgG) levels by enzyme-linked immunosorbent assay (ELISA).Results were analyzed and stratified by age, sex, occupation and the time interval between the time of vaccination and serum sample collection. The antibody positive rates of the two groups were compared by x2test. Results There were 401 (79. 4%) HCW whose H1N1 antibody were positive and 50 (9.9%) whose antibody were weak positive among 505 immunized HCW. While among 129 unimmunized HCW, there were 59 (45.7%) whose antibody were positive and 15 (11.6%) whose antibody were weak positive. The seroconversion rates of specific antibody were not significantly different among the different age groups after receiving A/H1N1 influenza vaccine (P> 0.05).However, there were statistical differences of the seroconversion rates among different sex groups (men 95.7% vs women 87.4% in immunized group, x2=6.40, P<0.05; and men 73.3% vs women 52.5% in unimmunized group, x2 =4.07, P<0.05) and different occupation groups (doctor 86.0% vs nurse 94.5% in immunized group, x2 = 9. 16, P<0.01; and doctor 43. 8% vs nurse 75.0% in unimmunized group, x2=12.61, P<0.01 ). The seroconversion rate was 81.5% after 80 to 89 days of vaccination, which was significantly lower than those after 30 to 39, 50 to 59 days and 60 to 69 days of vaccination, which was 100.0%, 94.7% and 93.6%, respectively (x2 =3.96, P <0.05; x2=7.15, P <0. 01; x2 = 9. 98, P<0. 01). Conclusions A/H1N1 influenza vaccination can induce effective immune response in HCW in Greater Khingan Mountains area of Inner Mongolia. However,the level of specific antibody significantly reduces after 80 to 89 days of vaccination.

Objective To construct the eukaryotic expression vector encoding IT15 gene and detect its expression in SH-SY5Y cells.Methods The exon1 of IT15 gene was amplified from genomic DNA isolated from a healthy adult and a Huntington disease patient lymphoblastoid cell lines by PCR.PCR products were subcloned into eukaryotic expression vector pEGFP-C1.The SH-SY5Y cells were transiently transfected with the recombinant plasmids and observed under fluorescence microscope.Western blot was used to detect the expression of the fusion protein.Results PCR products had approximately 170 and 310 bp specific segments.The recombinant eukaryotie expression plasmids were confirmed by restriction endonuclease analysis and sequencing.The N-terminal fragment of normal huntingtin primarily localized in the cytoplasim of SH-SY5Y cells,while the N-terminal fragment of mutant huntingtin aggregated both perinuclearly and intranuelearly.Conclusions The recombinant eukaryotic expression vectors encoding wild type and mutant IT15 gene fragments are useful for studying pathogenesis of Huntington disease.The Nterminal fragment of mutant huntingtin aggregats both perinuclearly and intranuclearly,which could be important for Huntington disease pathophysiology.

Objective To study the changes of biochemistry indexes in blood serum and bone mineral density (BMD) of secondary osteoporosis after spinal cord injury (SCI) in rats. Methods 120 male Wistar rats were divided randomly into 12 groups (n=10). The rats in control groups were only removed larmina with dura intact, without spinal cord injury at the level of the tenth thoracic vertebrae, the rats in experimental groups were also removed larmina, then paraplegic model was made by the method of Allen's (60gcm). The changes of biochemistry indexes in blood serum and BMD were observed. Results Compared with control groups, the concentrations of alkaline phophatase (ALP) were decreased at the 1st and the 3rd week in experimental groups (P0.05). BACKGROUND: Osteoporosis is one of the complications of spinal cord injury, but its mechanism is unclear, different scholars have different points about whether all bones above and below the level of trauma are affected after spinal cord injury (SCI).OBJECTIVE: To observe the ultrastructure of bone tissue of secondary osteoporosis after spinal cord injury and the change of serum biochemical indexes, to analyze the suffered condition and injured degree of the bones above and below the level of the trauma.DESIGN: Randomized controlled experimentSETTING: Laboratory of Animals, and Department of Laboratory Medicine of the Second Clinical Hospital of Jilin University; Transmission Electron Microscope Center, Norman Bethune Division of Medical Sciences of Jilin UniversityMATERIALS: This experiment was conducted at the Laboratory of Animals, Second Clinical Hospital of Jilin University from May 2002 to May 2003. Totally 110 male Wistar rats, aged 4 to 5 month-old, with the body mass of (300 -320)g were involved.METHODS: Ten rats were randomly chosen from 110 male Wistar rats and were set as 0 week blank control group, the ther rats were randomly divided into experimental group and control group, there were 10 rats in week 1, 2, 3, 7 and 11 control group and experimental group separately.1 mL/L of pentobarbital natrium was intraperitoneal injected to perform anesthesia, in the control group, lamina with dura intact of rats were removed only, without spinal cord injury at the level of tenth thoracic vertebrae. In the experimental group, lamina of rats was also removed, the spinal cord injury model of the rats were made by the method of Allen's.4 mL venous blood was collected from the animals at the end of postoperative week 0, week 1, week 2, week 3, week 7 and week 11, then the animals were put to death, perform blood centrifuging .We observe the change of serum concentration of calcium, phosphorus and alkaline phophatase (ALP) with 7170A HITACHI auto-biochemistry analyzer. Right side humerus, tibia in week 0 blank control group, week 7 experimental group and week 11 experimental group were chosen, then 25 mL/L ethylene dinitrilotetraacetic acid was used for decalcium for 1 month for preparingthe sections. We observe the change of ultrastructure of osteocytes at the sites of tibial plateau and the surgical neck of the humerus after stained by uranyl acetate and lead citrate under transmission electron microscope.biochemistry detection: the level of phosphorus at week 2 of the experiment was significantly lower in the control group than experiment group [(1.54±0.21),(2.76±0.16)mmol/L, (P < 0.01)]; the level of calcium at week 3 in the experiment group was significantly higher than in the control group [(2.52±0.06),(2.35±0.12)mmol/L, (P < 0.01)];the level of alkaline phophatase at week 7 in the experiment group was significantly higher than in the control group [(155.86±20.42), (129.25±7.30)Nμ/mg,operative week 7, the tibia sample showed that the osteocyte separated from the osseous lacuna, the nucleus showed anomaly, fluffed materials appeared. Mitochondrion swelled, rough endoplasmic hollowed; At week 11, osteocyte separated from the osseous lacuna. At postoperative 7 week,the humerus sample showed that osteocyte separated from osseous lacuna,some nucleus pycnosis and rough endoplasmic hollowed; At postoperative week 11, the above changes still were seen, but the degree of cellular swelling and hollowing were lighten.CONCLUSION: At the early stage of spinal cord injury, the activity of osteoclast is increased and the activity of osteoblast is decreased, which leads to the increase of bone absorption, the bone formation is largely weakened. The bones above and below the level of trauma are both affected after spinal cord injury, but different extents of osteoporosis can be seen in different bones .The change of ultrastructure of osteocytes is remarkable when osteoporosis has happened.

Objective To identify the application of puncture biopsy under ultrasound guidance in diagnosis of bone tumor.Methods The results of ultrasonic puncture biopsy were comparatively analysed with that of imaging diagnosis and postoperative-pathological diagnosis in 17 patients with bone tumors.Results The giant cell tumor of bone were diagnosed by puncture biopsy in 8 cases,by imaging in 12 cases and by pathology,postoperation in 9 cases,the corresponding rate was 66.7% and 88.9% respectively.Bone cyst in 2 cases,fibrous dysplasia in 2 cases,metastasis from lung carcinoma in one case,osteosarcoma in 2 cases were confirmed by both pathology and puncture biopsy.The aneurysmal bone cyst in one case was diagnosed by imaging,but no blood-like fluid was harvested in puncture,one case of metastasis in proximal femur,one case of fibrous dysplasia and one case of osteosarcoma were diagnosed by imaging,while one case,2 cases and one case diagnosed by puncture respectively.One case of bone cyst was confirmed by imaging,puncture biopsy and pathology.Conclusion The puncture biopsy under ultrasound guidance is of high diagnostic rate in bone tumors,it is a good choice for the diagnosis of bone tumor.
