RESUMO

ObjectiveTo explore the effect of flipped classroom mode in the clinical probation teaching for undergraduate mental health students, so as to expand the new mode of clinical teaching in psychiatry department. MethodsA total of 85 undergraduate psychiatric students from Xinxiang Medical University in 2016 were selected. All participants were divided into experimental group (n=43) and control group (n=42) according to the random number table method. The flipped classroom clinical teaching mode was adopted to experimental group, and the traditional practice mode for control group. The probation lasted for 12 weeks. Theoretical knowledge of symptomology, psychiatric clinical skills (OSCE), doctor-patient communication ability(SEGUE) and clinical thinking ability were assessed at the end of probation. ResultsAfter the probation, students in experimental group obtained higher scores in theoretical knowledge, clinical skills, doctor-patient communication ability and clinical thinking ability compared with control group. The differences were statistically significant [(33.08±1.72) vs. (32.06±2.33), (51.61±2.12) vs. (48.32±2.86), (18.14±1.98) vs. (14.62±2.15), (91.26±14.13) vs. (82.40±10.89), t=2.307, 6.034, 3.230, 7.846, P<0.05 or 0.01]. ConclusionApplying the flipped classroom mode into the clinical probation teaching for mental health undergraduate students may help to improve students' theoretical knowledge level and clinical operation ability, faciliate doctor-patient communication, and have a positive impact on their clinical thinking ability.

RESUMO

Objective To investigate the effects of olanzapine combined with metacognitive training on social cognition and insight of schizophrenic patients.Methods One hundred twenty patients with schizophrenia were randomly divided into two groups. All patients received four week treatment. The study gr oup received olanzapine combined metacognitive training treatment and the control group only used olanzapine treatment. The positive and negative symptoms scale (PANSS), the Insight and Treatment Attitude Questionnaire (ITAQ), the Beck Cognitive Insight Scale (BCIS) and Faux Pas Recognition Task (FPRT) were assessed at baseline and after four weeks of treatment. Result There was no statistically significant difference in the scores of all battery between two groups before treatment (P>0.05). the scores of total PANSS and subscales, ITAQ, BCIS and FPRT were statistically significant between study and control groups (P<0.05).After 4 weeks of treatment,the total PANSS scores and subscales scores were significantly lower in the study group than in the control group (P<0.05). The ITAQ score, self-reflection factor of BCIS, composite index and FPRT scores were significantly higher in the study group than in the control group (P<0.05). The difference in self-certainty of BCIS difference was not statistically significant(P>0.05).Differences of the remaining scales were statistically significant(P<0.05). Conclusion Olanzapine combined with metacognitive training has a significant effect on the social cognitive dysfunction in schizophrenic patients,improving the patient's insight and enhancing the social function.

Objective To explore the rapid eye movement (REM) sleep characteristics of different episode types of depression and post-stroke depression (PSD) patients and provide support for clinical diagnosis,treatment and prognosis.Methods Patients with single-episode depression (group A,n =22),patients with recurrent depression (≥ 2 episodes,group B,n=26) and patients with PSD (group C,n=19)were included from Henan Provincial Mental Hospital.20 healthy volunteers were assigned to control group.Participants in the 4 groups were performed polysomnographic recordings (PSG) from 9 PM to 6 AM before treatment.The 3 patients groups were performed the second PSG recordings and HAMD score at the end of 6 week SSRIs treatment.The REM indexes of 4 groups were compared and analyzed.Results First PSG detection showed that REM sleep latencies (RL) were shorter in group A and B (A:(65.57±18.29) min,B:(66.32±15.46) min) than that in group C ((79.17±20.18) min) and control group((87.24±16.55)min);REM activity (RA) (A:(99.82±25.71) u,B:(104.70±28.23) u)and REM density (RD) (A:(81.06± 19.35)%,B:(86.61±23.83) %) were increased more significantly in group A and B than those in control group ((79.61 ± 18.40) u;(68.11 ± 17.54) %);REM sleep time (RT) were decreased more signifi-cantly in group C ((51.66±22.26) min) than that in group A((71.43±20.70) min),group B((74.81±17.52) min) and control group ((70.46±16.35) min)(P<0.05).After treatment,REM sleep latency was prolonged in group A ((65.57±18.29) min vs (81.71±21.62) min),and REM activity was decreased in group A((99.82±25.71) u vs (83.58±27.19) u),the difference was statistically significant (P<0.05).REMdensity was decreased,but the difference was not statistically significant (P>0.05).There was no significant difference in the indexes of REM in group C;RT was prolonged ((51.66± 22.26) min vs (68.37 ± 20.16)min) in group C,the difference was statistically significant (P<0.05).Conclusion Most of depression patients with different episode types have REM disinhibition phenomenon.RD increase may be REM characteristic type of depression patient.Sleep disorders of PSD patients are mainly poor sleep process and sleep continuity and have no characteristics of REM sleep disorder of depression ones.

Objective To investigate the relationship between the single nucleotide polymorphisms(SNPs) of disrupted in schizophrenia 1 (DISC1) gene and autism in Chinese Han children.Methods Genome-wide SNP genotyping was performed by using Illumina HumanHap CNV370-Duo Chip in 278 autistic trios,157 autistic individuals and 435 healthy controls.Genotype data of SNPs within DISC1 gene were selected.The association between these SNPs loci and autism was analyzed through case-control association analysis and family-based transmission disequilibrium test.Results Fifty-two SNPs were involved for further analysis.1.Case-control association analysis showed that 6 SNPs (rs4658939,rs2793093,rs10495309,rs2492367,rs1 1122362,rs1073179) and 7 haplotypes (AGAAAG constructed with rs823163-rs823161-rs4658933-rs1417585-rs10864693-rs6541281,GGG and AAA constructed with rs4658939-rs2793093-rs10495309,GG and AG constructed with rs2492367-rs12046794,GAA constructed with rs9432040-rs2356606-rs1 1122362 and GG constructed with rs9431714-rs1073179) had significant differences between autistic patients and controls(x2 =4.704,4.915,5.568,8.087,4.043,5.183,5.369,5.295,4.440,5.304,7.615,4.018,4.811,P =0.030,0.027,0.018,0.005,0.044,0.023,0.021,0.021,0.035,0.021,0.006,0.045,0.028).2.In the transmission disequilibrium test analysis,2 SNPs (rs4658945,rs11122362) and 2 haplotypes (AG constructed with rs10495310-rs4658945,GAA constructed with rs9432040-rs2356606-rs11122362) showed significant transmission disequilibrium(x2=4.445,5.400,3.973,5.126,P =0.035,0.020,0.046,0.024).Conclusions The polymorphism of rs11122362 and GAA haplotype constructed with rs9432040-rs2356606-rs11122362 are associated with autism,and DISC1 gene is a susceptibility gene for autism in Chinese Han children.

<p><b>BACKGROUND</b>Chitosan (CS) scaffolds combined with osteogenically induced bone marrow mesenchymal stem cells (BMSCs) have been proved to be promising substitutes for repairing bone defects. Nevertheless, the bone-forming and scaffold-biodegrading processes are seldom studied. This study aimed to determine the osteogenic ability of CS/osteo-induced BMSC composites by observing the bone-forming process and explore the relationship between bone formation and scaffold biodegradation.</p><p><b>METHODS</b>The CS/osteo-induced BMSC composites (CS+cells group) and the CS scaffolds (CS group) were, respectively, implanted into SD rat thigh muscles. At 2, 4, 6, 8, and 12 weeks postoperatively, the rat femurs were scanned by CT, and the CT values of the implants were measured and comparatively analyzed. Subsequently, the implants were harvested and stained with hematoxylin and eosin and Masson trichrome, and the percentages of bone area, scaffold area, and collagen area were calculated and compared between the two groups.</p><p><b>RESULTS</b>The imaging results showed that the densities of implants of the two groups gradually increased along with time, but the CT values of implants in the CS+cells group were much higher than in the CS group at the same time point (P < 0.05). The histological results showed that the de novo bone and collagen formed in the pores of the scaffolds and gradually increased since 2 weeks postoperation in both groups, and the scaffold gradually degraded along with the boneforming process. However, the comparative analysis results showed that the CS+cells group gained more de novo bone and collagen formation and had less scaffold than the CS group at the same time point (P < 0.05).</p><p><b>CONCLUSION</b>The CS/osteo-induced BMSC composites are excellent bone tissue engineering substitutes, and the scaffold biodegradation is accordant with the bone formation.</p>

ObjectiveTo study the clinical effect of the proximal femoral anatomical locking plate in the treatment of ipsilateral femoral shaft and neck fractures.Methods A retrospective study was done on 10 patients with ipsilateral femoral shaft and neck fractures treated with proximal femoral anatomical locking plate in our hospital from February 2009 to February 2011.After treatment,the outcome was assessed regularly by fracture union as was seen on serial radiographs and clinical function was estimated by Friedman and Vyman System.ResultsAll patients were followed up for 6-24 months ( average 16 months).All the fractures were healed within 2.5-4 months (average 3 months) in the femoral shaft and within 4-9 months (average 6 months) in the neck,respectively.According to Friedman and Vyman System,the overall clinical result was good in eight patients and fair in two,with excellent rate of 80% (8/10).No osteonecrosis of the femoral head and fixation failure were observed during the followup. Conclusion Proximal femoral anatomical locking plate internal fixation is an effective treatment method for ipsilateral femoral shaft and neck fractures.

Objeetive To study the therapeutic effect of the H2 antagonist fomotidine on preventing weight gain olanzapine-induced.Methods Forty first-episode CCMD-3 schizophrenia patients were randomly allocated to received either famotidine or placebo in addition to olanzapine for 8 weeks.Their body weights and heights were measured before treatment and 2 weeks,4 weeks,8 weeks after treatment to calculate the body mass index and the percentage of patients gained more than 7% of initial bpdy weight after 8 weeks.SAPS and SANS were selected to evaluate the therapeutic effect.Results While the weight and the body mass index of patients in two groups after 2 weeks,4 weeks,8 weeks post-treatment increased significantly compared with those of pre-treatment(P<0.05).The weight of patients in two groups gained 3.7kg at the edd of 8 weeks,and the BMI increased 2.5 or 2.6 kg/cm2,reapectively.However 8 weeks later,the difference between two groups wan not significant(P>0.05).The difference between two groups on the percentage of patients gained more than 7% of initial body weight at the end of 8 weeks Wan not significant(P>0.05).The sCOres of SAPS and SANS at the end of 8 weeks decreased significantly compared with those of the baseline(P<0.05),but the difference between two groups was not significant(P>0.05).Conclusion Allocated to the H2 antagonist fomotidine could not reduce weight gain olanzapine-induced.
