RESUMO

OBJECTIVE@#To investigate the difference of therapeutic effects on children with thalassemia at different age after hematopoietic stem cell transplantation.@*METHODS@#The clinical data of children with thalassemia treated in our hospital were retrospectively analyzed. The children were divided into 2-5 years old group and 6-12 years old group. The success rate of implantation, transplant-related mortality, GVHD incidence, and other transplant-related complications, as well as thalassemia-free survival (TFS) were compared between the two groups.@*RESULTS@#The incidence of GVHD, hemorrhagic cystitis and severe oral mucositis after transplantation in the 2-5 years old group were significantly lower than those in the 6-12 years old group, while there was no statistically significant difference in the TFS between the two groups.@*CONCLUSION@#Children in the low age (2-5 years old) group show fewer complications and higher quality of life after transplantation, therefore, stem cell transplantation at 2-5 years old is more conducive to rehabilitation of the children with thalassemia.

Methods@#We recruited young patients with nonspecific low back pain for <3 months, who were otherwise healthy. Each patient had EOS images taken in the flexed, erect and extended positions, in random order, as well as magnetic resonance imaging to assess for disk degeneration. Angular and disk height measurements were performed and compared in all three postures using paired t-tests. Changes in disk height relative to the erect posture were caclulated to determine the alignment-specific load-bearing area of each FSU. @*Results@#Eighty-three patients (415 lumbar intervertebral disks) were studied. Significant alignment changes were found between all three postures at L1/2, and only between erect and flexion at the other FSUs. Disk height measurements showed that the neutral axis of the spine, marked by zones where disk heights did not change, varied between postures and was level specific. The load-bearing areas were also found to be more anterior in flexion and more posterior in extension, with the erect spine resembling the extended spine to a greater extent. @*Conclusions@#Load-bearing areas of the lumbar spine are sagittal alignment-specific and level-specific. This may imply that, depending on the surgical realignment strategy, attention should be paid not just to placing an intervertebral cage “as anterior as possible” for generating lordosis, but also on optimizing load-bearing in the lumbar spine.

Objective:To analyze risk factors and prognostic risk factors in aged patients with coronary heart disease (CHD).Methods:Clinical data of 412 aged patients,who were suspected of CHD in our hospital,were retrospec-tively analyzed.According to results of coronary angiography,they were divided into CHD group (n=283 )and non-CHD group (n=129);risk factors and prognostic risk factors for aged CHD patients were analyzed.Results:Compared with non-CHD group,there were significant rise in percentages of >80 years (13.95% vs.28.98%),di-abetes mellitus (DM,37.98% vs.66.78%)and smoking history (32.56% vs.59.72%)in CHD group,levels of to-tal cholesterol [TC,(5.06±1.53)mmol/L vs.(5.41±1.36)mmol/L],triglyceride [TG,(1.53±0.82)mmol/L vs.(2.02±0.95)mmol/L]and low density lipoprotein cholesterol [LDL-C,(2.05±0.86)mmol/L vs.(2.57 ±1.15)mmol/L],and significant reduction in high density lipoprotein cholesterol level [HDL-C, (1.65 ±0.52) mmol/L vs.(0.86±0.38)mmol/L]in CHD group,P <0.05 or <0.01. Multi-factor Logistic regression analysis indicated that age,DM,hypertension history,smoking,high total cholesterol (TC)and hypertriglyceridemia were independent risk factors for CHD (OR= 1.38 ~8.65,P <0.05 or <0.01 );while serum total bilirubin and high density lipoprotein cholesterol were independent protective factors for CHD (OR=0.224,P =0.002);multi-factor Cox model analysis indicated that smoking history,high TC and hypertension were risk factors affecting prognosis of CHD patients (RR= 1.563 ~ 2.315,P < 0.05 all).Conclusion:In clinic,attention should be paid to reduce or eliminaterelated risk factors of aged CHD patients to improve prognosis.

<p><b>OBJECTIVE</b>To observe the changes in spinal cord pathophysiology, motor function and electrophysiology after spinal cord injuries induced by punctures with different needles, and explore a new means for studying spinal neurotoxicity of local anesthetics.</p><p><b>METHODS</b>A total of 144 SD rats were randomly allocated into the sham-operated group (n=36) and 3 spinal cord injury groups (n=36) with the L4-5 segment of the dura mater of the spinal cord punctured using 29G, 25G, and 21G needles. The BBB scores before surgery were recorded, and at 8 h, 24 h, 72 h, 1 week, and 2 weeks after the surgery, the motor evoked potential (MEP), spinal cord pathology and the BBB scores were examined.</p><p><b>RESULTS</b>In the control group, the rats showed normal BBB score, spinal function and microstructure. Spinal cord puncture with 29G needle did not cause obvious pathologies of the spinal cord, whereas puncture with 21G needle resulted in marked changes in the motor function, electrophysiology and histology of the spinal cord, which showed significant improvements at 2 weeks postoperatively.</p><p><b>CONCLUSION</b>Puncture with a 29G needle causes less injuries and minimal functional changes of the spinal cord, which can serve as a new means for studying spinal neurotoxicity of local anesthetics.</p>
