RESUMO

Objective ToanalyzethedistributionanddrugresistancesituationofKlebsiellapneumoniae(KPN)isolatedfromour hospital during 2014 .Methods The drug susceptibility test was carried out by the Siemens fully automatic bacterial identification /drug susceptibility test analyzer .The drug susceptibility test results were judged according to the (CLSI) M100‐S22 document (2012) .Results A total of 291 strains of KPN were detected from 8 486 submitted specimens ,in which 20 strains were multi‐drug resistant Klebsiella pneumoniae(MDR‐KPN)isolates were 213 strains ,48 strains were extended‐spectrum beta‐lactamases producing Klebsiella pneumoniae (ESBLs‐KPN) and 10 strains were carbapenem‐resistant Klebsiella pneumoniae(CR‐KPN) .The KPN detec‐tion rate had statistical difference among different specimens (P= 0 .000) .The constituent ratio of KPN isolation in the sputum specimen was highest(P=0 .000) .The KPN detection rate had statistical difference among different departments (P=0 .000) ,the constituent ratio of KPN isolation in the respiration department was highest (P=0 .000) .The resistance and sensitivity of KPN to different antibacterial drugs had statistical difference(P=0 .000) ,carbapenems had the lowest resistance rate .Conclusion KPN has relatively low resistance rate to commonly used antibacterial drugs ,its multi‐drug resistant strains are in the higher level ,which should arouse clinic and hospital infection management department must to pay attention to and adopt corresponding supervision measures .

Objective To compare the effect of different gastrointestinal motility drugs on capsule endoscopy. Methods Seventy-one patients with suspected small bowel disease were randomly divided into metoclopramide group (24 patients), mosapride group(25 patients) and control group (22 group). The patients in metoclopramide group swallowed capsule endoscopy immediately after intramuscularly injecting 10 mg metoclopramide, the patients in mosapride group swallowed capsule endoscopy 15 min after taking 5 mg mosapride, and the patients in control group did not take any of the gastrointestinal motility drugs. Three groups had the same bowel preparation before checking. The finishing rate of small bowel examinations, stomach and small intestinal transit time, intestinal cleanliness and the detection rates of lesions in three groups were compared. Results The total small bowel examination finishing rate was 94.4%(67/71). The small bowel examination finishing rate in metoclopramide group, mosapride group, and control group was 95.8%(23/24), 96.0%(24/25), and 90.9% (20/22), and there was no significant difference(P>0.05). The stomach transit time in metoclopramide group, mosapride group and control group was(27.5 ± 20.7), (28.1 ± 20.9) and (52.3 ± 33.5) min. The stomach transit time in metoclopramide group and mosapride group had no significant difference (P>0.05), but it was significantly lower than that in control group (P0.05). The image class scores in metoclopramide group, mosapride group and control group was (2.5 ± 0.4), (2.7 ± 0.4) and (1.7 ± 0.3) scores.The scores in metoclopramide group and mosapride group had no significant difference (P>0.05), but they were significantly higher than that in control group (P0.05), but it was significantly higher than that in control group (P<0.05). Conclusions The use of gastrointestinal motility drugs before capsule endoscopy can improve the quality of inspection, and metoclopramide and mosapride shows no significant difference.

RESUMO

Objective To evaluate the influence of platelet-rich plasma (PRP) on autologous fat grafting breast augmentation surgery.Methods A lot of 40 patients were divided randomly into the treatment group and control group, each group of 20 cases.In the treatment group cases PRP was used in addition to autologous fat grafting.Chest circumference and thickness of subcutaneous fat were measured before and 6 month after operation and 12 months later.The results were compared and the influence of PRP was evaluated after operation.Results Both groups of patients were satisfied with the results.Compared with the control group, patients in the treatment group showed the chest circumference and thickness of subcutaneous fat were significantly increased 12 months after operation.Conclusions The efficacy of PRP is significant in enhancing fat graft, reducing fat absorption in 12 mouths after operation.The fat tissue mixed with PRP is a reliable reconstruction option for the breast augmentation.

Objective To explore the visinin-like protein-1 (VILIP-1) levels in the blood in the Alzheimer disease (AD) and mild cognitive impairment (MCI) due to AD patients,and also to explore its feasibility as a biomarker for the early diagnosis of AD and for the detection of progression of AD.Methods Fifty-eighty participants were included:20 cases for AD group,19 cases for MCI due to AD group,19 cases for normal control group.The level of VILIP-1 was tested by the ELISA method.Results The level of VILIP-1 in AD group was significantly higher than that in normal control group and MCI due to AD group:(9.0 ± 2.9) ng/L vs.(3.3 ± 1.7) and (6.5 ± 3.1) ng/L,and that in MCI due to AD group was significantly higher than that in normal group,there were statistical differences (P < 0.01).The MMSE score in AD group was significantly lower than that in normal group and MCI due to AD group:(15 ± 3) scores vs.(27 ± 2) and (23 ± 2) scores,and that in MCI due to AD group was significantly lower than that in normal group,there were statistical differences (P < 0.01).The level of VILIP-1 was negatively correlated with MMSE score (r =0.463,P < 0.01),but positively correlated with age (r =0.417,P =0.01).Conclusions With the progression of the disease,the cognitive impairment of the AD patient is decreasing.VILIP-1 increased in the blood of the patients of AD and MCI due to AD.It means that the blood VILIP-1 could be a new and potential biomarker for the early diagnosis of AD,and it may be clinical useful for the early diagnosis and effective detection of AD to some extent.

Objective To explore the correlation of data obtained through three dimensional reconstruction and measurement of mandibular angle related angles and bigonial breadth.Methods Sixty cases of adult female head spiral CT data were involved in this study.By the three dimensional reconstruction,mandibular angle related angles and bigonial breadth were measured.Through SPSS13.0 software their correlation was analyzed,including among mandibular angle related angles and these with bigonial breadth.Results The related data of the mandibular angle were measured on the image of three dimensional reconstruction; the mandibular angle was (124.28-±-4.12)°,the mandibular elevated angle was (25.52±2.22)°,the valgus angle of mandibular angle was (9.35±7.85)°,the mandiblular included angle was (77.32 ± 2.34)°,and the tangent angle of mandible was (105.53 ± 1.79)°.The correlation analysis of the related angles of mandibular angle and bigonial breadth showed that there was a significant correlation between the angles.Conclusions Three dimensional reconstruction and measurement of mandibular angle related angles and their correlation analysis can provide theoretical foundation and basis for the preoperative design and effect evaluation of mandibular angle plastic surgery.

RESUMO

Objective To study Barrett's esophageal consistency with its histological findings,which were found with magnifying chromoendo6copy,and to advance the endoscopic diagnosis of Barrett's esophagus. Methods The patients with BE were diagnosed by endoscopy and histology, and their histological findings and endoscopic appearances of a total of 67 patients with BE were observed. Results Endoscopic appearances type of BE showed island (28 cases) ,tongue(7 cases) ,and circum ference(32 cases). Three types of BE under magnifying chromoendoscopy,and in magnifying endoscope group,type Ⅲ which were found with magnifying endoscope were verified intestinalisation epithelium pat ho-type, compared with type I and type Ⅱ,the discrepancy of them had statistical significance(P<0.01). Conclusion Magnifying chromoendoscopy had a relatively better consistency in the diagnosis of Barrett's esophagus with histological findings.

Objectives To investigate the changes in the masseter muscle following osteotomy of the prominent mandibular angle using real-time three-dimensional (3D) ultrasonography, and to supply guidance for resection of the mandibular angle. Methods Real-time 3D ultrasonography was applied preand post-operatively (over a 6-month follow-up period) to 10 patients who underwent curved osteotomy with the following objectives: (1) to reconstruct the morphological changes of the masseter under different conditions; (2) to assess masseter muscle volume changes, and (3) to obtain the dynamic morphological changes of masseter during mouth opening and closing. Results The reconstructed 3D images revealed that longitudinal diameters of masseter muscle decreased and angle regions changed to be arc-shaped with significant thinning 6 months after operation. The mean volume of masseter muscle was (18. 222 ±3. 028) cm36 months post-operatively, compared with the pre-operation mass of (25. 480 ± 7. 113) cm3,the statistical difference was significant (P<0.01). Transverse and longitudinal changes of the thickest masseter muscle section 6 months post-operatively were of no statistic difference (P>0. 01) compared with pre-operation status during mouth opening and closing motions. Conclusion A certain extent of atrophy occurs primarily in the angle region of masseter muscle after mandibular angle ostectomy. However, these changes do not significantly impair masseter muscle function. Real-time 3D ultrasonography offers a novel, safe, and convenient technique for masseter muscle reconstruction and observation of masseter muscle movement.

Objective To investigate the changes of the constitution and its ratio of collagen fiber in the process of masseter reattachment following different osteotomies of the prominent mandibular angle so as to offer guidance for the resection of mandibular angle. Methods Sixteen adult goats were randomized into four groups. In group A we performed unilateral curved osteotomy of the mandibular angle. In group B unilateral curved ostectomy was performed with partial masseter resection. In group C unilateral angle splitting ostectomy, while in group D unilateral dissection of the masseter muscle was conducted. The constitution and its ratio of collagen fiber in the interface were observed at 1-month, 2-month, 3-month, and 6-month after operation. Results On the changes of collagen fiber in the process of muscular reattachment, at 1-month post-operation, the constitution of collagen fiber (types Ⅰ and Ⅲ) in groups A and B were significantly different from that of control group (P<0.05). However, both groups C and D had no statistic difference from control group (P>0.05). At 2-month, 3-month and 6-month post-operation, those of all experimental groups had no statistic difference from control group. And with time, the percentage of collagen fiber type Ⅰ increased and type Ⅲ decreased gradually. Conclusion The recovery sequences of masseter muscle reattachment in this study are firstly group C, secondly group A and finally group B. It suggests that the recoveries of mastication and other oral activities are different. Group B turns out to be with a slow muscle reattachment. Thus, we recommend treating different kinds of mandibular hypertrophy with different ostectomies.

RESUMO

Objective To explore the effect of botulinum toxin type A(BTXA)in the treatment of early hypertrophic scars(HTS).Methods BTXA was injected into and around the eady HTS,and then the modal and histological changes of the scars as well as the clinical reaction were observed in the patient.BTXA was also injected into muscle around the incision and effect on the cicatrization observed.Results Injection of BTXA could obviously alleviate ache and pruritus of eady HTS and could impel the atrophy and inteneration of eady HTS.Changes were found in paraffin-embedded tissue section by the hemetoxylin and eosin(HE)staining.Injection of BTXA into muscle around the cut could can reduce occurrence of HTS.Conclusion BTXA can help prevent the early HTS to a certain extent.The mechanism underlying this effect may be related to the reducing the tension around scars and proliferative activity,interfering with the signal transduction of small nerves,affecting the proliferation and apoptosis of fibroblasts and subsequently decreasing the collagen synthesis.

Objective To investigate the changes of the masseter muscle following osteotomy of the prominent mandibular angle and to provide guidance for the resection of mandibular angle. Methods Ten goats were equally divided into two groups. In group A we performed unilateral curved osteotomy of the mandibular angle, and in group B we performed unilateral dissection of the masseter muscle. The cross section area (CSA) and the sarcomere length of masseter muscle were measured beore and after operation. Results (1) Cross section area (CSA) of masseter muscle fiber in curved ostectomy group decreased at 1,2, 3 and 6 months after operation in different extent. Comparing with the control group, the difference was statistically significant (P<0.01). CSA of masseter muscle fiber in dissection group decreased 1 month postoperatively, which had significantly statistic difference with control group (P<0.01). But, they had no significant difference with control group at 2, 3, and 6 months after operation (P>0.01). (2) Sarcomere length of masseter muscle in curved ostectomy group decreased in 1 week, 1 and 2 months after operation, which had significantly statistic difference with control group (P<0.01). At 3 months after operation, sarcomere length recovered to normal. In dissection group, sarcomere length decreased in 1 week and 1 month after operation, which had significantly statistic difference with control group (P<0.01). At 2 month after operation, it recovered to normal. Conclusion Certain extent of atrophy does happen to masseter muscle after mandibular angle ostectomy. Meanwhile, these changes do not significantly impair the function of masseter muscle. According to this, we suggest a simple mandibular angle ostectomy without partial resection of masseter muscle in case of mild to morderate mandibular angle hypertrophy. Doing so, we can not only achieve the cosmetic effect but also reduce the implications.

Objective To investigate the methods of rectifying the excessive width of fold line and conglutination of the upper eyelid following blepharoplasty. Methods The width of double eyelid was redesigned, ranging from 6 mm to 8 mm. Then the scar of upper eyelid was excised about 1 mm to 2 mm, and the orbital septum fat flap, orbicularis oculi flap, infraorbicularis oculi fat flap and injection of lipochondria were utilized to fill the depression of upper eyelid. Results In our series, there were 20 eyelids of 16 cases receiving rectification. Orbital septum fat flap was used to correct 2 eye-lids, orbicularis oculi flap 4 eyelids, infraorbicularis oculi fat flap 10 eyelids and injection of lipochon-dria 4 cases. Among 12 eases of following-up, 10 of them were satisfied with the postoperative effects. The results were acceptable in other 2 cases. Conclusion It is a good approach to correct the excessive width of fold line and conglutination of the upper eyelid with local flaps, including the orbital septum fat flap, orbicularis oculi flap, infraorbicularis oculi fat flap and injection of lipochondria.

RESUMO

Objiective To provide the applied anatomy for the eyebrow lifting plasty. Methods The clinical surgical anatomy of the eyebrow region were studied bilaterally in 15 embalmed cadaveric heads and 9 fresh adult cadaveric heads. The cross sections of 2 embalmed cadaveric heads were observed. Results ⑴ A lot of fibrous tissue origined from the dermis of eyebrow attached to the SMAS. The eyebrow was moved by those fibrous tissue when the frontalis contracted. ⑵ The eyebrow was moved on the loosen layer under the SMAS. Conclusions ⑴ The loosen layer under the SMAS is the anatomic basis of eyebrow movement. ⑵ The basic methods of eyebrow lifting are the skin excision or suspension and restoration of the eyebrow.

Objective To explore the mechanism of protective effect of liposome carried prostaglandin E 1 (PGE 1) on rat skin flap ischemia / reperfusin(I/R). RESUMO

Objective To determine the mechanism underlying the mandibular angle enlargement and to provide a morphological basis for correcting the enlarged mandibular angle. Methods By axial computed tomography, it is possible to measure the width and length of the masseter and the medial pterygoid, the gonial angle, and the vertical height of the posterior mandible. The date were initially analysed by using a simple correctionials and indpendent sample T test. Results The distance between two Go points and gonial angle were significantly related with the thickness of masseter muscle, but the thickness of the medial pterygoid was on the contrary. There was significant differences between the group of prominence and the normal control in the gonial angle, the distance between two Go points and thickness of masseter . But there was no difference between the the two groups in the thickness of medial pterygoid. Conclusion The thickness of the masseter muscle is an important factor that affects the lower facial morphology. The hypertrophy of masserter muscle may cause the mandibular angle enlargement.
