RESUMO

Objective To study the long term effect and practical value of unilateral cleft lip repair with extended Mohler surgery based on over 10 years experiences.Methods A lot of 320 cases with unilateral cleft lip were repaired with extended Mohler surgery at average age of 5 months and 20 days.65 repaired cases were followed up and long term treatment result was evaluated.26 cases were evaluated in 10.5 to 15 years after surgery.39 cases were evaluated in 6 to 24 months after surgery.And the details of surgery were described.Results Excellent results for most of cleft patients (63 cases, 97%) were achieved, which included unobvious vertical scar symmetric to unaffected philtrum column, symmetric cupid's bow, nostrils and intermedial nasal columellae.Conclusions Extended Mohler surgery is great valuable for unilateral cleft lip repair.Appropriate indications should be stressed.

RESUMO

Objective To explore the method to correct nasal deformity of the bilateral complete cleft lip and to eveluate the benefits of this teachnique.Methods Sixteen patients with the bilateral complete cleft lip were consecutively chosen to correct the nasal deformity of the bilateral complete cleft lip.We performed an open approach to facilitate alar cartilage manipulation while repairing the bilateral complete cleft lip,and elevated the prolabial flap for reconstructing the continuity of orbicularis oris musele artery by anatomizing and protecting the prolabial-columellar artery.Nasal columella length and wideth,nasal wideth,ratio of nasal length to face length,ratio of nasal wideth to the distence of nasolabial angle were analyzed after a minimum of 10 years after surgery.These were compared with those in a normal,age-matched control group.Results Appreance of nose and lip of 16 patients after surgery were satisfying and long-term results were good by following-up for 10 years.Columellar length and ratio of nasal length to face length were not significantly different from that of the control group.Nasal wideth,columella wideth,tip projetion and nasolabial angle were all significantly greater in the cleft group than the normal (P<0.05).Conclusions Prolabial flap manipulation is a safe approach by using anatomizing the columellar-labial artery.This teachnique is effective and yields good long-term results.

Objective To develop a new method for correction of the deformity of the complete bilateral cleft lip with short pre-lip. Methods Two specimens with the complete bilateral cleft lip was chosen to investigate the arterial supply to the pre-lip. The arteries were observed by radiography when the specimen was infused with lead oxide. 16 patients with the complete bilateral cleft lip and shortage in the length of pre-lip were enrolled in this study. Taking advantage of the unique character of arterial supply of complete bilateral cleft lip, those with short pre-lip were corrected. Photos of them were taken when preand post-operation for further measurement. And more than 1 year following-up was conducted to judge if the results were good enough or not. 30 babies were chosen as a control group to confirm the results of surgury objectively. Results Post nasal septal arteries were found to supply the pre-lip of the bilateral complete cleft lip. Appearance of 16 patients was good after surgery or 1 years later. There was no statistical significant difference between the patients and control group in the measurement data but nasal-lip angle. Conclusion It is an effective way to correct deformity of the bilateral cleft lip with short pre-lip by taking advantage of post nasal septal arterie's flap.

Objective To detect the differential expression genes(DEGs)between Barrettg esophagus(BE)and normal esophagus with oligomicroarray,and to explore the target genes related to the development of BE.Methods The total RNAs of matched BE and normal esophagus mucosa from saIne patient were isolated with one step Trizol method.Matched RNAs were qualified with 10g/L agarose gel electrophoresis.After tRNA purification,cRNAs were synthesized and labeled with fluorescence.which were tIlen hybridized with Agilent oligomicroarray containing 30,968 probes.The fluorescence intensity features were detected by Agilent scanner and quantified by software Feature Extraction.Results On average,2 biopsies by disposable jumbo biopsy forceps provided approximately 5μg RNA required for microarray.The total RNA,reverse transcription product and fluorescence labeled cRNA were all of high quality.Among 2-fold DEGs,there were 142 up-regulated genes and 284 down-regulated genes including 15 bel-2 related genes such as bel-2,MCL1,BAX,BIK and BCLAF1 Conclusion Microarray-based studies are feasible in endoscopically obtained tissues.The development of BE is a complicated process involving multi-genes,in which abnormal expression of bel-2 family related genes might be involved,but the exact mechanism needs further research.

Objective To develop a novel endoscopic classification system to determine the invasion depth of early esophageal cancer.Methods The esophageal lesion was endoscopicaUy stained with Lugol's iodine first,then methylene blue.According to the growth pattern,height and cup depth under endoscope,the lesions were classified into 5 types,including surface diffusion growth,intra-lumen growth,intra-wall growth,bi-direction growth and mix growth types.The lesions were then removed by endoscopic mucosa reection or surgery,the precise invasion depth of the lesion was determined pathologically and the results were compared with the endoscopy classification.Results The data of 44 cases of esophageal mucosal cancer and 34 cases of esophageal sub-mueosal cancer were included.With the criteria of mucosal cancer as surface diffusion growth,intra-lumen growth <5mm,bi-direetion growth <2mm and intra-wall growth <0.5 mm,the diagnostic specificity was 89.1%(41/46) and sensitivity was 93.2%(41/44).With the criteria of submucosal cancer as intra-lumen growth≥5 mm,hi-direction growth≥2mm,intra-wall growth≥0.5 mm and mix growth type,the diagnostic specificity was 90.6%(29/32) and sensitivity was 85.3%(29/34).The overall diagnostic accuracy in differentiating esophageal mucosal cancer from esophageal submueosal cancer by endoscopic classification was 89.7%(70/78).Conclusion This endoscopic classification system is effective in differentiating esophageal mucosal cancer from submucosal ones.

RESUMO

Objective To investigate the galectin-3 expression in squamous carcinioma of esophagus and analyze its relationship with clinicopathological characteristics.Methods S-P immunohistochemistry was used to detect the expression of galectin-3 in 80 paraffin-embedded specimens of esophageal carcinoma and 22 specimens of normal tissues adjacent to the benigh lesions of esophagus.RT-PCR was performed to detect galectin-3 mRNA expression in 36 pairs of fresh squamous carcinoma tissues and corresponding adjacent normal mucosa.Results Out of 80 specimens of squamous carcinoma of esophagus,62 specimens had positive expression of galectin-3,with the positive rate of 77.5%(62/80)that was significantly higher than 27.27% in normal esophageal tissues(6/22,P

RESUMO

Objective To investigate the phonological characteristics of patients with functional speech problems. Methods Ninety patients with functional speech problems were classified by speech analysis. All types of speech problems were analyzed. Results The functional speech problems can be categorized as unaspiration, palatalization, lateralization, fronting of tongue, backing of tongue, plosion, omission of consonants, glottal stop, affrication and backing of tongue and nasalization. Backing of tongue and nasalization was only related to the consonant l, and the unaspiration was often related to such consonants as p, t, k, q, c, ch. Conclusion The functional speech problems were related to consonants. There was regularity with the involvement of the consonants in different types of functional speech problems.

RESUMO

OBJECTIVE: To evaluate the feasibility of transplanting composite mandibular allografts to repair large mandibular defects. METHODS: Three composite mandibular trans-plantation models were established. The first model consisted of hemimandible with the attached teeth, muscle and skin,and oral mucosa. The second model was transplanted in the same way with the first one excluding oral mucosa and some teeth, and third one excluding the oral mucosa and all dental crowns. Fourteen transplanting operations were performed in canines. Cyclosporine A and methylprednisone were given for immunosuppression. RESULTS: The composite mandibular organs had an effective and closed return circuit. Transplantation of vascularized allograft of mandibular compound organs was feasible. Two longest time survivors of 67 d and 76 d were in the third model group. Cyclosporine A was successful in suppressing rejection of transplanted composite allograft and prolonging survival time of transplantation models. CONCLUSIONS: The composite mandibular allografts were available with large block of living composite tissue, and helpful in restoration of appearance and function for severe mandibular defects.
