RESUMO

Objective@#To explore the clinical effects of antibiotic bone cement in the treatment of diabetic foot ulcers.@*Methods@#According to the treatment methods, 18 patients with diabetic foot ulcers (11 males and 7 females, aged 53-79 years), who were conformed to the study criteria and admitted to our hospital from January 2016 to January 2017, were enrolled in traditional group; 18 patients with diabetic foot ulcers (11 males and 7 females, aged 55-80 years), who were conformed to the study criteria and admitted to our hospital from February 2017 to February 2018, were enrolled in bone cement group. Wounds of patients in traditional group were treated with vacuum sealing drainage after conventional debridement. Wounds of patients in bone cement group were covered with antibiotic bone cement after conventional debridement. The number of patients with positive bacterial culture in wound exudate in the 2 groups on admission and 3, 6, 9, and 15 days after surgery, the length of hospital stay, the number of operation, and the wound complete healing time were retrospectively recorded. Data were processed with Fisher′s exact probability test and independent sample t test.@*Results@#Compared with (29±10) d and (4.6±1.2) times of patients in traditional group, the length of hospital stay [(9±3) d] of patients was obviously shortened, the number of operation [(1.3±0.6) times] of patients was obviously reduced, the number of patients with positive bacterial culture in wound exudate at each time point post surgery was obviously reduced (t=8.177, 9.896, P<0.05 or P<0.01) in bone cement group. There were no statistically significant differences in the number of patients with positive bacterial culture in wound exudate on admission and wound complete healing time between patients in the 2 groups (t=0.175, P>0.05).@*Conclusions@#The antibiotic bone cement treatment of diabetic foot ulcers can reduce the number of patients with positive bacterial culture in wound exudate and the number of operation, as well as shorten the length of hospital stay.

Objective To investigate the clinical effect of retrograde nasolabial fold flap pedicled with angular artery in the repair of inferior nasal defect .Methods According to the location and size of inferior nasal defect ,the reverse island flap or axial flap pedicled with the inner canthus artery was de-signed to repair the defect at the nasolabial fold of the affected side .Results Eighteen patients were followed up for 6-36 months ,of which 2 cases had partial necrosis at the distal part of the flap ,healed by dressing change ,and the rest of the flaps survived .The flaps were not bulky and the color and tex-ture were similar to those of the surrounding skin .All patients were satisfied with the appearance im -provement .Conclusions The blood supply of the retrograde nasolabial fold flap pedicled with the in-ner canthus artery is reliable ,using flaps from the nasolabial sulcus is surgically convenient ,flexible in design and covert in donor site .It is one of the ideal methods for the repair of inferior nasal defects .It is worthy of clinical application .

RESUMO

Objective To evaluate the efficacy of follicular unit extraction (FUE) in the repair of scarring eyebrow loss.Methods A total of 30 patients with scarring eyebrow loss were treated with FUE technique in the Department of Plastic Surgery of Zhengzhou First People's Hospital,including 18 females and 12 males with 41 eyebrow defects.Fine soft hairs with a length of 5-10 mm were extracted from the back of the ear and temporal region of the patients,and then were split into individual follicular units under a microscope at 6 × magnification.The transplant density ranged from 20 to 30 FU/cm2.Results During the 3-18 months of follow-up after the surgery,eyebrows in the recipient areas grew in a natural direction and showed good appearance.The survival rate of hair grafts was up to 90%,and the patients were satisfied with the results.Conclusion FUE may be an ideal method for the treatment of scarring eyebrow loss with less injury and high survival rate of hair grafts.

Objective@#To observe the efficacy and safety of micro-plasma technology for the treatment of stable scars.@*Methods@#63 cases were divided into 4 groups, including burn scar group (28 cases), traumatic scar group(15 cases), acne scar group(10 cases), and hemangioma scar group (10 cases). Micro-plasma was performed every 2 months, 4 times as one session. Vancouver scar scale(VSS) was used to record the scar score and evaluate its clinical efficacy.@*Results@#All of the scar points in four groups dropped significantly after treatment, showing significant difference between groups(P<0.05). The point decrease rate in acne scar group and burn scar group was significantly higher than that in traumatic scar group and hemangioma scar group (P<0.05). Efficacy evaluation also showed obvious better effect in acne and burn scar than in traumatic scar (P<0.05).@*Conclusions@#The micro-plasma has a good effect on burn, trauma, acne and hemangioma scar, especially for burn and acne scar.

Objective To establish and evaluate the animal model induced by inhalation injury of airborne fine particulate matter (PM2?5). Methods We manufactured equipment for rats aerosol inhalation with PM2?5. The effects of several facters such as concentrations(100 ± 10 μg/m3、150 ± 10 μg/m3、200 ± 10 μg/m3 )、time(1w、2w、4w、8w、12w)、method (non?exposed intratracheal instillation method and aerosol inhalation) and animals (Wistar rats, BN rats and guinea pigs) were investigated to establish the model. The respiratory rate, forced vital capacity ratio of forced expiratory volume ( FEV1/FVC) and arterial partial pressure of oxygen ( PO2 ) were measured, the pathological changes of bronchial and lung tissues under light microscope were observed. The success animal model was builded as the pneumonia was observed from the pathological changes of lung tissue. Results The Wistar rats exposed to PM2?5 aerosol inhalation for 8 weeks, we can see that the weight growth rate of rat decreased, WBC count and mononuclear cells count increased, the macrophages ratio decreased in BALF, the respiratory rate of lung increased while arterial PO2 and FEV1/FVC decreased, inflammation and pulmonary fibrosis changes were observed by bronch and pulmonary pathology, inflammatory changes with a dose?response relationship were observed. Exposed to PM2?5 aerosol inhalation for different time(1 w、2 w、4 w、8 w、12 w)with same dose, the score issue lesions of lung and bronchus in Wistar rats increased and the 8w group is obvious. The Wistar rats exposed to PM2?5 with different method ( aerosol inhalation and non?exposed intratracheal instillation method) for 8w, the aerosol inhalation worked as effectively as perfusion while mortality rate of aerosol inhalation is lower. Different animals ( Wistar rats, BN rats and guinea pigs) exposed to PM2?5 aerosol inhalation for 8w, the same results were observed with three method respectively while mortality rate of Wistar rats lower. Conclusions The optional conditions that the Wistar rats were continuously inhaled for 8w PM2?5 with a dose of 150 ± 10 μg/m3 were established. The animal model could be used on a national scale, especially in Fujian province. The results would be useful for the development of the research of the prevention and countermeasures of PM2?5 pollution.

OBJECTIVE@#To establish logistic regression model for prostate cancer and provide basis for prostate biopsy.
@*METHODS@#A total of 117 cases of prostate biopsy were retrospectively analyzed in chronological sequence. All cases were assigned into a model group (n=78) and a validation group (n=39). Logistic regression model was established and its value was estimated by receiver operating characteristic (ROC) curve. 
@*RESULTS@#Digital rectal examination(DRE), transrectal ultrasound(TRUS), MRI, prostate-specific antigen density (PSAD), and free PSA/total PSA (fPSA/tPSA) were the influential factors for prostate biopsy (P<0.01). The established logistic regression model for prostate cancer by regression coefficient was: logit P=-2.362+2.561×DRE+1.747×TRUS+2.901×MRI+1.126×PSAD-
2.569×fPSA/tPSA and area under curve was 0.907. When the cutoff aimed at 0.12, the sensitivity and specificity were 81.80% and 89.30%, respectively.
@*CONCLUSION@#Logistic regression model for prostate cancer can provide sufficient basis for prostate biopsy. Prostate biopsy should be performed when P value is more than 0.12.

OBJECTIVE@#To explore the mechanisms for urinary system disorders before and after ketamine withdrawal in rats and to evaluate the recovery degree of the urinary system damage after ketamine withdrawal.@*METHODS@#Fifteen male healthy Sprague-Dawley rats were randomly divided into 3 groups: A control group, an experimental group, and a withdrawal group. The rats in the control group were given normal saline. The rats in the experimental group were given ketamine 30 mg/(kg.day) for 30 days. The rats in the withdrawal group were treated as the experimental group except for drug withdrawal for 2 weeks. In the experimental period, we randomly selected 1 rat of kidney, ureter, and bladder from each group to perform HE staining. The bladder tissues in each group were used to detect mRNA expression by quantitative real-time polymerase chain reaction (qRT-PCR).@*RESULTS@#1) The behavior of ketamine-injected rats was obviously changed, but the weight of ketamine-induced rats was not changed. 2) As compared with the control group, the experimental and withdrawal groups showed infiltration of mononuclear inflammatory cells in the kidney tissues, the thinner epithelium of bladder and infiltration of submucosal mononuclear inflammatory cells under the optical microscope. 3) As compared with the control group, the expression of H1R mRNA was increased in the experimental group (P<0.05). As compared with the experimental group, H1R mRNA expression was significantly decreased in the withdrawal group (P<0.05).@*CONCLUSION@#Ketamine abuse could induce behavior changes in rats. The infiltration of mononuclear inflammatory cells in kidney and bladder, the thinner bladder epithelial layer, and the increased H1R gene mRNA expression in bladder might be an important pathogenesis of KAUD. Ketamine withdrawal may effectively reverse the pathogenic process of KAUD.

Objective: To determine the therapeutic effect of regular urethral dilatation on patients with postoperative urethral stricture. Methods: A total of 142 patients underwent urethral stricture. The unimproved patients after the surgery were divided into a reoperation group and a regular urethral dilatation group. All the patients were followed up for 1-3 months, and the curative effect was compared. Results: Of the 142 patients, 42 had no improvement and 27 of them had reoperation, and symptoms in 21 were improved. Another 15 patients received regular urethral dilatation, and 5 improved. hTere was signiifcant difference between the 2 groups (P<0.01). Conclusion: Regular urethral dilatation has some effect on postoperative patients, but no obvious effect on patients with dissatisifed operation for urethral stricture or restenosis.

Objective:To determine the effect of ketamine on the apoptosis of human uroepithelial cells (SV-HUC-1) and the pathogenesis of ketamine-associated cystitis. Methods:SV-HUC-1 cells were cultured under various concentrations of ketamine and different time. Flow cytometry was used to analyze the rate of cell apoptosis. hTe protein levels of Bax, Bcl-2, pro-caspase-3, and cleaved caspase-3 were detected by Western blot. Results:Compared with the control group, the apoptotic rate of ketamine cultured SV-HUC-1 cells increased. hTe expression of Bax increased, Bcl-2 expression decreased, and Bax/Bcl-2 in the ketamine cultured SV-HUC-1 cells was signiifcantly higher. hTe protein level of pro-caspase-3 was signiifcantly lower, and that of cleaved caspase-3 was signiifcantly higher than that in the control group (P<0.05), positively correlated with the dose of ketamine and time of culture (P<0.05). Conclusion:Ketamine can induce the apoptosis of SV-HUC-1 cells in a dose and time dependent manner.

BACKGROUND: Electrical stimulation at different intensity, frequency and time on the human body may produce a variety of pathophysiological reactions. OBJECTIVE: To observe the effects of surface electric-impulse stimulation on heart rhythm and heart rate in mice. METHODS: Thirty Kunming mice were randomly divided into three groups, each group contained 10 mice. Electrical stimulation at different voltage, time and frequency was respectively applied to the three groups. The stimulus power was supplied by BL-420F Data Acquisition & Analysis System. The II lead electrocardiogram was recorded. The systemic reactions and local body changes of mice were observed.

BACKGROUND:The treatment of autologous pericardium transplantation has been widely applied in clinics, mainly involving cardiovascular repair and reconstruction, the treatment of ocular surface disease. The study addressing protection effects of autologous pericardium transplantation on the heart with ischemia injury is rarely reported. The investigations on the safety and protection effects of autologous pericardial transplantation on the heart with ischemia injury are of important significance. OBJECTIVE:To explore effect of autologous pericardial transplantation on cardiac electrical activity and the protective effects on myocardial ischemia. METHODS:Rongchang pork pigs and Sprague-Dawley rats were randomly divided into three groups:autologous pericardium transplantation, myocardial ischemia, and myocardial ischemia+autologous pericardium transplantation. The model of myocardial ischemia was established by ligation of the left anterior descending coronary artery in the groups of myocardial ischemia and myocardial ischemia+autologous pericardium transplantation. The model of transplantation was established by autologous pericardium transplant with flap in the groups of autologous pericardium transplantation and myocardial ischemia+autologous pericardium transplantation. RESULTS AND CONCLUSION:Porcine electrocardiogram monitoring results showed that, superventricular premature beat was frequently observed in each group of pigs;the ventricular premature beat was occasional observed in autologous pericardium transplantation group, ventricular tachycardia and ventricular fibril ation did not appear. Compared with myocardial ischemia group, the ventricular premature beat decreased and the heart function was improved in myocardial ischemia+autologous pericardium transplantation group (P<0.05). Rat electrocardiogram monitoring results showed that, the ventricular fibril ation did not appear in autologous pericardium transplantation group, the lethal ventricular fibril ation did not appear in myocardial ischemia and myocardial ischemia+autologous pericardium transplantation groups. Compared with myocardial ischemia group, the heart function was improved, the apoptosis index decreased, the expressions of Bcl-2 protein increased, the expressions of Caspase-3 protein decreased in myocardial ischemia+autologous pericardium transplantation group (P<0.05). The autologous pericardium transplantation with flap cannot induce malignant ventricular arrhythmia and is relatively safe;the ventricular premature beat is reduced, the cardiac function is improved, which is possibly related to the inhibition of apoptosis in myocardial ischemic area.

BACKGROUND:Mesenchymal stem cels can differentiate into cardiomyocytesin vitro induced by different methods, such as chemical drug induction, autologous transplantationin vivo, andin vitro simulation of cardiac-like microenvironment, but these inducible methods show low induction rate, complex operation, and toxic side effects. OBJECTIVE:To investigate the role of H9C2 cellculture medium in the differentiation of mesenchymal stem cels into cardiomyocyte-like cels. METHODS: Mesenchymal stem cels were obtained by the whole bone marrow adherent culture and H9C2 cellculture medium was prepared as a culture medium. Then mesenchymal stem cels were co-cultured with H9C2 cellculture medium for 1, 3, 5, 7 days. H9C2 cels cultured in 10% Dulbecco’s modified Eagle’s medium/Ham’s nutrient mixture F-12 served as positive control groups, while mesenchymal stem cels cultured in 10%Dulbecco’s modified Eagle’s medium/Ham’s nutrient mixture F-12 as negative control group. Immunofluorescence and western blot assay were used to detect expression of myocardial celljunction protein (desmin) and troponin T. Real-time quantitative PCR was applied to detect mRNA expression of myocardial celltrait genes, α-cardiac myosin heavy chain and β-myosin heavy chain. RESULTS AND CONCLUSION:After co-culture of H9C2 cellculture medium and mesenchymal stem cels for 7 days, the troponin T positive cels were up to (16±7)%, which was significantly higher than that of non-induced mesenchymal stem cels. Compared with the negative control group, the expression of troponin T protein and desmin after induction were significantly increased (P < 0.05) by western-blot detection; real-time PCR showed that the mRNA expression ofα-cardiac myosin heavy chain and β-myosin heavy chain in differentiated cels were both up-regulated (P < 0.05). These findings suggest that H9C2 cellculture medium may induce the differentiation of mesenchymal stem cels into cardiomyocyte-like cels.

Objective:To determine the diagnosis, treatment, and pathogenesis of ketamine-associated cystitis. Methods:Clinical data from 3 patients with ketamine-associated cystitis were analyzed retrospectively and discussed in light of relevant literature. Results:In the 3 cases, 2 presented severe lower urinary tract symptoms, including frequency, urgency, dysuria, urge incontinence, and painful haematuria. Urinalysis and urine culture were negative. Imaging examination demonstrated thickening of the bladder wall and a small capacity. Inflammatory changes in the bladder mucosa were observed by cystoscopy and biopsies. After cessation of ketamine use, with the addition of steroids or hydrodistension, the symptoms in the 3 patients improved. The symptoms recurred in 2 patients, as 1 was exposed to ketamine again and 1 had severe bladder contraction after for 3-4 month follow-up. Conclusion:Ketamine-associated cystitis is a new urinary system inlfammatory damage. Its etiology and treatment methods are not clear. early abstinence from ketamine use and early treatment are crucial for patients with ketamine-associated cystitis to avoid irreversible damage.

BACKGROUND: The copper-superoxide dismutase (Cu-SOD) can not play an effective role in free radical-scavenging in the process of hypoxia or reaeration because of blood-brain barrier, whether the treating effect can be enhanced by chitosan nanoparticles is uncertain. OBJECTIVE: To observe the ability of chitosan nanoparticles loaded the Cu-SOD plasmid on free radical-scavenging in ischemia-reperfusion brain tissue. DESIGN: An observation experiment. MATERIALS: The Cu-SOD plasmid was provided by the Center Laboratory of Tangdu Hospital, Forth Military Medical University of Chinese PLA. A total of 36 rats were prepared ischemia-reperfusion models, and randomly divided into the control, physiological saline (PS) plus Cu-SOD plasmid and chitosan nanoparticles loaded the Cu-SOD plasmid groups. METHODS: The chitosan nanoparticles were prepared by cross-linking method. The Cu-SOD plasmid was absorbed to chitosan nanoparticles by electrostaficforces, mixed the 50.0 ?L nanoparticles with 10.0, 25.0, 50.0, 75.0, 100.0 ?L SOD plasmid respectively. The electrophoretic analysis was used to examine the combination condition of the chitosan nanoparticles with superoxide dismutase plasmid, and the embedding rate was calculated with ultraviolet spectrophotometer. Chitosan nanoparticles, SOD and Cu-SOD plasmid were infused into the rat after brain ischemia-reperfusion. Further more, to detect the content of malondialdehyde and superoxide dismutase in brain tissues. MAIN OUTCOME MEASURES: The combination condition of the chitosan nanoparticles with superoxide dismutase plasmid, and the activity of malondialdehyde (MDA) and SOD were measured. RESULTS: Chitosan nanoparticles and superoxide dismutase in volume ratio of 50: 75 could harvest the largest embedding rate with 100%, which had good protective effect on DNA. Objective To evaluate the efficacy of calicecotomy combined with trans-renal parenchyma pneumatic lithotripsy for complicated staghorn renal calculi.Methods The severe hydrocalycosis was incised using electrocautery,then pneumatic lithotripsy was performed and the broken stones were taken out.For those patients with stenotic entrance to renal calyces without hydrocalycosis,we stabbed into the stones with the lithotriptic pole(1 mm in diameter) through renal parenchyma and took the broken stones out of the entrance.Results The renal pedicle were not blocked in 19 cases.The operation time was 90-150 minutes,with a mean of 120 minutes.There was no blood transfusion with the blood loss ranging form 100 to 250 ml.The procedures were successful in 17 cases without residual stones after operation;intraoperative residual sand-like calculi were found in 1 case and removed by irrigation and drainage through nephrostomy tube;intraoperative missing calyceal calculi occurred in 1 case and were cleared by extracorporeal shock wave lithotripsy(ESWL).A follow-up for 10-60 months(mean,18 months)in 15 patients showed recurrence in 2 ones,and the stones were removed by ESWL.Conclusions Calicecotomy combined with trans-renal parenchyma pneumatic lithotripsy for complicated staghorn renal calculi has the advantages of less blood loss and definite efficacy.
