RESUMO

Objective:The aim of this retrospective study is to evaluate the safety and efficacy of tislelizumab in patients with recurrent/metastatic head and neck squamous cell carcinoma. Methods:Six patients with recurrent/metastatic head and neck squamous cell carcinoma who received tislelizumab monotherapy in our hospital from 2018 to 2020 were retrospectively analyzed. The information of sex, age, TNM stage, efficacy, and adverse reactions were collected. All patients were recruited from the RATIONALE 102 study. The primary end point was the objective response rate, and other end points included progression-free survival and overall survival. We performed tumor immune-related gene sequencing and transcriptome sequencing analysis on the tumor tissues of the patient, and used bioinformatics methods to enrich immune cells and analyze signaling pathways. All analyses were performed using R 4.1. 0 software, SPSS Statistics 24.0 software and GraphPad Prism 8. Results:As of May 31, 2020, the median follow-up time was 26.35 months. The objective response rate with tislelizumab was 50.0%, the median progression-free survival was 6.44 months, and the estimated median survival was 20.07 months. The incidence of grade 3 or higher adverse reactions was 66.7%, including hyponatremia, hypokalemia, hypercalcemia, etc. The expression of macrophage, Treg and neutrophil-related genes are higher in immune-sensitive patients, and the signaling pathways of the intestinal immune network for IgA production, graft versus host disease and autoimmune thyroid disease are significantly activated. Conclusion:Tislelizumab was found to be controllable and tolerable in patients with recurrent/metastatic head and neck squamous cell carcinoma. The response to tislelizumab is related to immune cell infiltration and activation of immune-related signaling pathways.

ObjectiveTo review the drug information and research progress on oral Chinese patent medicines in the treatment of cardiac arrhythmia to identify existing problems and provide references for follow-up research. MethodChinese patent medicines against cardiac arrhythmia were retrieved from the three major drug catalogues,Yaozh.com,and relevant guidelines with arrhythmia as the retrieval term. The instructions for included Chinese patent medicines were retrieved through Yaozh.com and specific information was extracted. The research articles on Chinese patent medicines included were retrieved from the five databases,and the information meeting the inclusion and exclusion criteria was extracted and displayed in the form of text description and graphs after statistical analysis. ResultSixty-five oral Chinese patent medicines were included in this study,with the main functions of activating the blood and resolving stasis. The average daily cost of medicines was 8.17 yuan,and there were 42 medicines with an average daily cost of less than 10 yuan,showing a moderate medicine cost. A total of 351 research articles on Chinese patent medicines were screened out,including 259 randomized controlled trials (RCTs),16 non-RCTs,eight non-controlled trials,62 systematic reviews,two guidelines,and two expert consensuses. Eighteen types of Chinese patent medicines were involved,whose clinical trials had been conducted in 28 provinces,cities,autonomous regions,and municipalities in China. Wenxin granules and Shensong Yangxin capsules were the top medicines under investigation,accounting for 75.21% of all research articles. Among the included studies,the most common comparison design was Chinese patent medicine combined with western medicine vs western medicine (64.25%). The outcome evaluation was mainly based on clinical efficacy,symptom efficacy,arrhythmia efficacy,adverse reactions,and heart rate changes. ConclusionThe number of clinical studies of oral Chinese patent medicines against cardiac arrhythmia varies greatly,but traditional Chinese medicine (TCM) syndrome differentiation thinking is less considered in practical application. Due to unstandardized clinical research and low-quality literature,further advancement is required in the future.

In recent years, researchers have found that magnesium ion homeostasis imbalance is common in tumor cells, and the deficiency and supplement of magnesium ion can affect the occurrence and development of tumor. As an abundant divalent cation in cells, magnesium ion plays an important role in maintaining genetic stability, metabolism, cell growth and proliferation, signal transduction and other physiological processes. Magnesium ion homeostasis is regulated by a variety of transporters, and the research reports on the changes of magnesium ion transporters expression leading to the imbalance of magnesium ion homeostasis which affects the occurrence, development and treatment prognosis of tumors are increasing year by year. In this article, magnesium ion and its related transport proteins include magnesium ion transient receptor potential melastatin (TRPM) protein, magnesium transporter (MagT) protein, cyclin and cystathionine beta-synthase (CBS) domain divalent metal cation transport mediator (CNNM) protein and solute carrier (SLC) protein and other related reports in tumors are summarized, with the purpose of providing ideas for exploring whether magnesium ion and its transporters can become new targets for tumor diagnosis, treatment and prognosis.

Objective To investigate the incidence of enteric pathogens causing acute gastroenteritis (AGE) among children to measure the incidence of coinfections,and to compare the clinical characteristics of those infected with one versus multiple agents.Methods A retrospective study was conducted from January 2014 to December 2014.All patients between 1 month and 14 years of age admitted to the Pediatric department with a diagnosis of AGE were eligible for enrollment.Two stool samples for each patient were tested for gastrointestinal pathogens.We summarized the clinical severity of episodes,describing the duration of diarrhea,duration and frequency of vomiting,fever.All patients underwent medical evaluation with estimation of dehydration.Results One or more etiological agents were detected in 3595 out of 4728 patients(76.0%),while we did not detect any etiological agent in 1133 (24.0%).Rotavirus was detected in 1889 (40.0%),adenovirus in 412 (8.7 %),norovirus in 309 (6.5 %),verotoxigenic Escherichia coli (VTEC) in 274 (5.8 %),Salmonella spp.in 276(5.8%),Klebsiella pneumoniae in 123 (2.6%),Shigella spp.in 78 (1.6%),Staphylococcus aureus in 70 (1.5%),C.perfringens in 126(2.7%).In 1370 children out of 4728(29.0%),we found evidence of coinfection.with rotavirus and norovirus was the most common 150 (3.2%),rotavirus and C.perfringens was also common 127(2.7%).Children with coinfection had a more severe clinical presentation.The difference has statistical significance.Conclusion Rotavirus is still the most common pathogen in children with acute diarrhea,followed by NV,adenovirus,Salmonella spp.and VTEC.Rotavirus with norovirus infection was the most common.VTEC combined with three kinds of virus infection had the highest incidence.Children with multiple viral infections were more severe than those of single virus infection in the duration of vomiting and dehydration.There was no significant difference in the duration of fever and diarrhea and the frequency of diarrhea.Children infected by viruses and bacteria had a more severe clinical presentation such as fever,vomiting and diarrhea lasting for a long time,more serious diarrhea and dehydration than those with single bacteria and single virus infection.The difference has no significant difference in degree and duration of diarrhea.

Purpose To explore the effects of progesterone receptor isoforms A and B ( PR-A,PR-B) on carcinogenesis and progres-sion of ovarian serous cystadenocarcinoma ( OSC) . Methods The expressions of PR-A and PR-B in 52 cases of OSC, 22 cases of o-varian borderline serous cystadenoma ( OBSC) , 22 cases of umbrella of normal fallopian ( UNF) were detected by immunohistochmical Elivision technique. Results The expression of PR-A in OSCs, OBSCs and UNFs were 94. 5%, 94. 5%, and 68. 38%, respective-ly, with there were statistical significance among three groups (P<0. 05). The expression of PR-B in OSCs, OBSCs and UNFs were 100%, 77. 27%, and 40. 38%, respectively, with there were statistical significance among three groups (P<0. 05). The difference of PR-A/PR-B ratio in OSCs, OBSCs and UNFs was statistical significance ( P<0. 05 ) . The expressions of PR-A and PR-B in OSCs were lower, there were statistical significance between the clinical stageⅠ+Ⅱ andⅢ+Ⅳ (P<0. 05), and histological gradeⅠandⅡ. The difference of PR-A/PR-B ratio between the histological gradeⅠandⅡin OSC was statistical significance. There was statistical significance of PR-B between OSCs with lymph metastasis and without lymph metastasis (P<0. 05). Expression of PR-A and PR-B in OSC was positive correlation (P<0. 05). Conclusion With the carcinogenesis and progression of OSC, the expressions of PR-A and PR-B gradually declined, and the downregulation of PR-B is more obvious, may be an important biological sign of malignant transfor-mation in ovarian tissue. The increasing ratio of PR-A to PR-B in OSC may indicate poor differentiation. The relatively higher expres-sion of PR-B may inhibit the lymph metastasis in OSC.

The famous veteran doctors of traditional Chinese medicine ( TCM ) are models to combine TCM the-ories , previous practitioners' clinical experiences with current medical practice for the difficult to solve clinical problems . The selection of appropriate inheritance study method is especially important in the learning of fa-mous veteran doctors of TCM . This article discussed the inheritance method of academic experiences of famous veteran doctor of TCM professor Huang Yongsheng from four aspects, which were the reading of classic books, clinical practice, following medical masters, and summarization of clinical cases. It was hoped that during the inheritance process , efforts can be made to academic characteristics of TCM itself in accordance with its own law in order to create more and better clinical experience inheritance method.

Objective To investigate the therapeutic effects of retinal angiomatosis in differentclinical stages.To discuss the indication of vitrectomy for retinal hemangioblastoma. Methods TheclinicaI data of 22 cases(33 eyes)were retrospectively analyzed.The retinal hemangiomas were dividedinto 5 stages according to their degrees of development from simple angioma without vessel dilation tofeeder vessel dilation and intra-retinal exudates.local retinal detachment,massive retinal detachment andcomplication occurrence in proper order.The methods of treatment were laser photocoagulation,trans-scleral cryotherapy and vitrectomy. 13 eyes were treated with laser photocoagulation, 5 eyes withcryotherapy combined with laser and 11 eyes with vitrectomy.Tumor resection and silicone oil tamponadewas performed in 3 eyes during vitrectomy.The patients were followed up for 46 months on average.Visual acuity(VA),the condition of the hemangioma and retina was compared pre~and post-operationrespectively. Results In a11 13 eyes treated with laser phot.coagulation the hemangiomas regressed andthe retina remained attached.VA improved in 2 eyes,and remained unchanged in 11 eyes.Cryotherapycombined with laser photocoagulation was performed on 5 eyes. In this group,4 eyes'hemangiomasregressed and no new hemangiomas occurred,proliferative vitreous retinopathy and vitreous hemorrhagewas observed in 1 eye which vitrectomy was performed later.VA improved in 2 eyes,unchanged in 2 eyesand decreased in 1 eye.In the 11 eyes treated with vitreoretinal surgery,new hemangiomas was found in 1eye,exudative retinal detachment was caused by hemangiomas in 2 eyes,proliferative vitreous retinopathywas observed in 2 eyes.and the retina remained attached in 8 eyes.VA improved in 3 eyes,unimproved in3 eyes,and decreased in 5 eyes.In the 3 eyes with surgical resection of retinal hemangioma during vitrectomy,2 eyes'retina remained attached,1 eye had exudative retinal detachment and no new hemangiomas occurred.VA improved in 2 eyes and decreased in 1 eye. Conclusions Laser photocoagulation or combined with cryotherapy is effective in treating the hemangiomas in early stage.Vitreetomy is advisable for late stage of retinal angiomatosis,especially with vitreous hemorrhage,epiretinal membrane,proliferation and large scale of retinal detachment.Surgical resection of isolated large retinal hemangioblastoma may be useful for selected patients.

Objective To investigate the effects of Hep-A and He p-B on vascular endothelial growth factor (VEGF)-induced breakdown of blood-r etinal barrier. Methods The mice were subcutaneously injected vehicle, Hep-A or Hep-B 10 mg/kg twice a day for 5 days. Then, 1 ?l of 10 -6 mol/L VEGF were intravitreous injected. After 6 hours, 13.7?10 4 Bq /g 3H-mannital were injected intraperitoneally. The mice were sacrificed and the retinas, lungs, kidneys were removed and examined for radioactivity. The result were analyzed using SPSS software to calculate and compare retina/lung and et ina/kidney leakage ratio among groups of different treatment.
