RESUMO

OBJECTIVE To establish a method for the content determination of 11 components such as protodioscin in Guge fengtong tablets, and to evaluate the comprehensive quality of Guge fengtong tablets by combining with chemometric analysis and entropy weight-technique for order preference by similarity to ideal solution (EW-TOPSIS) method. METHODS HPLC method was adopted. The determination was performed on Agilent Eclipse Plus C18 column with a mobile phase consisted of acetonitrile- 0.2% phosphoric acid solution at the flow rate of 1.0 mL/min by gradient elution. The column temperature was set at 30 ℃ . The detection wavelengths were set at 203 nm (0-28 min, protodioscin, methyl protodioscin, pseudoprotodioscin, dioscin) and 280 nm (28-60 min, catechin, epicatechin, liquiritigenin, medicarpin, 6-gingerol, 8-gingerol, 10-gingerol); the sample size was 10 μL. Using epicatechin as the internal reference, quantitative analysis of multi-components by single marker (QAMS) method was used to determine the contents of protodioscin, methyl protodioscin, pseudoprotodioscin, dioscin, catechin, liquiritigenin, medicarpin, 6-gingerol, 8-gingerol and 10-gingerol, which were compared with the results of the external standard method. SPSS 26.0 software and SIMCA 14.1 software were used for principal component analysis and orthogonal partial least squares-discriminant analysis, with variable importance in projection (VIP) value greater than 1 as the standard, to screen for differential markers that affect the quality; the EW-TOPSIS method was adopted to evaluate the quality of 15 batches of samples comprehensively.RESULTS The contents of protodioscin, methyl protodioscin, pseudoprotodioscin, dioscin, catechin, liquiritigenin, medi-carpin, 6-gingerol, 8-gingerol and 10-gingerol determined by HPLC combined with QAMS were 6.330-10.863, 1.150-2.274, 0.431- 0.740, 2.818-4.823, 0.826-1.510, 0.043-0.094, 0.079-0.231, 0.479-1.020, 0.146-0.288, 0.118-0.318 mg/g, respectively; there were no statistical significances, compared with the external standard method (P>0.05). A total of 15 batches of samples were clustered into 3 groups, with S1-S6, S7-S10, and S11-S15 clustered into one group, respectively. The VIP values of protodioscin, epicatechin, dioscin and 6-gingerol were greater than 1. Euclidean closeness values of the optimal solution (C)i for 15 batches of samples were 0.163 5 to 0.703 7, and Ci values of S11-S15 were all higher than 0.6. CONCLUSIONS The established QAMS method is accurate and simple, and can be used for comprehensive quality evaluation of Guge fengtong tablets, by combining with chemometric analysis and EW-TOPSIS method. Protodioscin, epicatechin, dioscin and 6-gingerol are the differential markers that affect the quality of Guge fengtong tablets. Samples S11-S15 have better quality.

Objective:To explore the causes and preventive strategies of adverse reactions of blood transfusion in patients with hematological diseases.Methods:The clinical data of 150 hematological patients who received 5 184 cases of blood transfusion in Beijing No.6 Hospital from January 2014 to December 2017 were retrospectively analyzed. The blood transfusion adverse reaction reporting forms were collected, and the content of the form included the basic information of the patient, diagnosis, blood transfusion type, blood transfusion time, blood transfusion history, pregnancy history, adverse reaction history of blood transfusion as well as detailed records of clinical symptoms. The blood bank summarized the blood transfusion adverse reaction reporting forms every week.Results:A total of 112 cases of adverse reactions occurred in 5 184 cases of blood transfusion, the incidence rate was 2.16%. Sixty-eight cases (1.32%) had anaphylaxis, mainly caused by platelet transfusion, and 44 cases (0.85%) had fever, mainly caused by transfusion of red blood cells suspension, and no other adverse reactions were found. The incidence rates of adverse reactions of blood transfusion in patients with blood transfusion history and anaphylaxis history were significantly higher than those in patients without blood transfusion history and anaphylaxis history, and the differences were statistically significant [2.34% (102/4 350) vs. 1.20% (10/834), χ2 = 6.899, P = 0.009; 3.06% (98/3 200) vs. 1.42% (14/984), χ2 = 7.767, P = 0.005]; the incidence rate of adverse reactions of blood transfusion in female patients was significantly higher than that in male patients, and the difference was statistically significant [2.82% (60/2 126) vs. 1.70% (52/3 058), χ2 = 8.356, P = 0.004]; there was no significant difference in the incidence rate of adverse reactions of blood transfusion between patients < 18 years old and ≥ 18 years old [1.16% (3/259) vs. 2.21% (109/4 925), χ2 = 1.295, P = 0.255]. Conclusions:The main manifestations of adverse reactions of component blood transfusion are anaphylaxis and non-hemolytic fever. Clinical medical staff must carefully control the blood transfusion indications and select the appropriate blood components to reduce the adverse reactions of blood transfusion and ensure the safety of blood transfusion.

RESUMO

Glioma is the most common lethal tumor of the human brain. The median survival of patients with primary World Health Organization grade IV glioma is only 14.6 months. The World Health Organization classification of tumors of the central nervous system categorized gliomas into lower-grade gliomas and glioblastomas. Unlike primary glioblastoma that usually develop de novo in the elderly, secondary glioblastoma enriched with an isocitrate dehydrogenase mutant typically progresses from lower-grade glioma within 5-10 years from the time of diagnosis. Based on various evolutional trajectories brought on by clonal and subclonal alterations, the evolution patterns of glioma vary according to different theories. Some important features distinguish the normal brain from other tissues, e.g., the composition of the microenvironment around the tumor cells, the presence of the blood-brain barrier, and others. The underlying mechanism of glioma recurrence and evolution patterns of glioma are different from those of other types of cancer. Several studies correlated tumor recurrence with tumor heterogeneity and the immune microenvironment. However, the detailed reasons for the progression and recurrence of glioma remain controversial. In this review, we introduce the different mechanisms involved in glioma progression, including tumor heterogeneity, the tumor microenvironment and drug resistance, and their pre-clinical implements in clinical trials. This review aimed to provide new insights into further clinical strategies for the treatment of patients with recurrent and secondary glioma.

Objective:To study the dose response of two-dimensional ionization chamber matrix detectors under IMRT fields irradiation at different gantry angles and different directions.Methods:After the dose calibration of the medical accelerator and the two-dimensional ionization chamber matrix detector respectively, the solid water phantom was used to cover the upper part of the two-dimensional ionization chamber matrix detector. Adjusting the thickness of the solid water, the effective distances between the effective measuring center of the two-dimensional ionization chamber matrix detector and the incident point of the central axis of the radiation on the surface of the phantom was always controlled at 5 cm. Four locations with 0、45、90 and 135 degrees of the long axis of the treatment bed were selected, and each location was irradiated by IMRT fields at different frame angle of 60°, 45°, 30°, 15°, 0°, -15°, -30°, -45° and -60°, respectively.Results:The dose deviation of the medical accelerator was less than 0.18% in the range of different rack angles. The directional response deviation of the two-dimensional ionization chamber matrix detector was less than 0.63%, and the measurement deviation of each probe of the two-dimensional ionization chamber matrix detector to its center probe was less than 0.50%.Conclusions:The two-dimensional ionization chamber matrix detector has good directional response and it is a useful tool for developing intensity-modulated radiotherapy technology. The method proposed in this study can provide a basic basis for the relevant medical accelerator metrology departments to formulate corresponding quality control test specifications.

Objective To investigate the antidepressant effects of Chaihu Shugan Powder(CSP) on depressive rats induced by reserpine and its influences on the kynurenine (KYN),indoleamine 2,3-dioxyge-nase(IDO),interleukin-6(IL-6) and tumor necrosis factor-α( TNF-α). Methods Forty rats with similar behavior results were divide into 4 groups randomly,including Control group(Con),Model group(Res),Flu- oxetine group(Res+Flu) and Chaihu Shugan Powder group(CSP). The depressive rat model was established by intraperitoneal injection reserpine. The rats in Res+Flu group were administered with fluoxetine by intrap-eritoneal and rats in Res+CSP group were administered with CSP by intraperitoneal. After 14 days,the be-havior of rats was measured and then the rats were executed and sampled. The content of tryptophan and kynurenine in raphe nuclei tissue were detected. The mRNA expression level of IDO,IL-6,TNF-α in raphe nuclei tissue were detected. Results ( 1) Compared with Con group (( 81. 81 ± 36. 13) s, ( 83. 51 ± 5. 34)%), the swimming immobility time((150. 50±31. 45)s) in Res group increased(t=68. 7, P<0. 05) and the sucrose perference (59. 73±11. 30)%) in Res group decreased(t=23. 8,P<0. 05). Compared with Res group, the swimming immobility time in Res+Flu group((114. 90± 14. 29) s) and Res+CSP group ((111. 7±11. 34)s) decreased(t=35. 6,35. 8,both P<0. 05). Compared with Res group, the sucrose pref-erence in Res+Flu group((78. 21±10. 07)%) increased(t=18. 3, P<0. 05). Compared with Res+Flu group,the level of IDO mRNA and in-flammatory factors' mRNA in Res+CSP group were significantly decreased(t=18. 3,20. 8,31. 5,all P<0. 05). Conclusion Chaihu Shugan Powder has antidepressant effect,and the mechanism is related with de-creasing the inflammatory factors,inhibiting IDO activation and decreasing the IDO mRNA.

Objective To investigate the effects of acute reserpine-induced pain and depression co-morbidity model on behavior and related inflammatory cytokines in rats. Methods Twelve male 8-week-old SD rats were randomly divided into control group and model group,6 in each group. Rats in the model group were intraperitoneally injected with reserpine(1 mg/kg/d)for 3 days to establish the model. Rats in the con-trol group were injected with the same amount of distilled water. The pain threshold of rats was measured by Von Frey Hairs and Hot Plate Analgesia Test. The depression behavior of rats was evaluated by open-field test,Forced Swimming Test and Sucrose Consumption Test. Serum IL-1β, IL-6, IL-18 and TNF-α content were detected by ELISA. Results Compared with the control group(1d (15. 00±0. 00) g;3d ( 13. 20±4. 03)g),the PWTs of the model group (1d (6. 20±0. 45) g;3d (4. 20±1. 64) g) decreased significantly(t=44. 00,4. 63,both P<0. 05). Conclusion A large number of intraperitoneal injections of acute reserpine can cause hyperalgesia,depressive behavior and serum inflamma-tory factors in rats,effectively simulating the symptoms of pain and depression,and can be used as a model of pain and depression comorbidity for biological mechanisms and treatment research.

Chaihu Shugan Powder has the function of soothing liver and promoting qi flow, activating blood and relieving pain. In clinic, it is mainly used for the syndrome of liver qi stagnation, with wide application. This article reviewed the literature about effects of Chaihu Shugan Powder on neurobiochemistry, endocrine regulation, immunity and antioxidant pharmacological effects and mechanism, with a purpose to provide references for clinical medication and new medicine research.

Objective To observe the effects of granulocyte colony stimulating factor (G-CSF) on electrophysiological properties of post-infarct ventricles.Methods Sixty-seven survival Wistar rats were divided into 4 groups:Sham group,Control group,MI early G-CSF group (E-G) and MI delay G-CSF group (D-G) after ligation of the left coronary artery as myocardial infarction model.Monophasic action potential(MAP) was recorded by absorption electrode in ex vivo perfused rat hearts.Effective refractive period(ERP),sinus cardiac length (SCL),action potential amplitude (APA),maximal depolariged (Vmax),ventricular fibrillation threshold(VFF) and ventricular fibrillation duration(VFD) were measured.Results The electrophysiological parameters (SCL,VFT,VFD,APA,ERP/MAP90,dispersion of ERP and MAP90) of the E-G group were improved significantly (all P < 0.05) at day 7 post MI.Improvement in SCL,dispersion of ERP and MAP 90 were found in the D-G group as well at day 7 post MI (all P < 0.05).Substained improvement in electrophysiological parameters were found in the E-G group at 3 months after MI (P <0.05).Besides SCL,APA,Vmax and dispersion of MAP90,all other parameters in the D-G group were similar to that of the control group with no statistical significance and even had a tendency of deterioration in ERP and MAP90 3 months after MI.Conclusion G-CSF intervention could improve electrophysiological properties of ischemic ventricles.Early G-CSF intervention showed better outcomes compared to delay G-CSF intervention on electrical remodeling ischemia myocardiumwhich may have effect on reducing the development of ventricular arrhythmia.

Objective To investigate the risk factors of hyperuricemia and type 2 diabetes mellitus(T2DM ) with vascular com‐plication .Methods 97 individuals undergoing the physical examination were included into the control group ,103 patients with pure T2DM and 78 patients with T2DM complicating hyperuricemia were selected .The fasting plasma glucose(FPG) ,glycosylated he‐moglobin(HbA1c) ,total cholesterol(TC) ,triglycerides(TG) ,high density lipoprotein cholesterol (HDL‐C) ,low density lipoprotein cholesterol (LDL‐C) ,apolipoprotein A1(ApoA1) ,apolipoprotein B (ApoB) ,serum creatinine (Cr) and uric acid (UA ) were com‐pared among 3 groups ,meanwhile the first morning urine was collected for detecting urine microalbumin (UMA) ,and the body mass index(BMI) was detected .Then the correlation between hyperuricemia with the diabetes related complications was investigated .Re‐sults The levels of FPG ,TG ,Cr ,UMA and HbA1c in the pure T2DM group were higher than those in the control group(P < 0 . 05) ;BMI ,FPG ,TG ,TC ,LDL‐L ,UMA ,Cr and HbA1c in the T2DM complicating hyperuricemia group were higher than those in the control group (P < 0 .05) ;BMI ,TG ,Cr ,UMA and HbA1c in the T2DM complicating hyperuricemia group were higher than those in the pure T2DM group(P < 0 .05) .The incidence rates of diabetic nephropathy (DN ) ,limb arterial plaques in the T2DM complicating hyperuricemia group were significantly higher than those in the T 2DM group ,the differences were statistically signifi‐cant (P< 0 .05) .Conclusion The patients with T2DM complicating hyperuricemia are easier to develop dyslipidemia ,and their inci‐dence rates of DN and lower limb arterial plaque are also significantly higher than those in patients with pure T 2DM .

Objective: To investigate the relationship between long-, short-term systolic blood pressure variability (SBPV) and renal damage in elder population. Methods: Our research was conducted in the 3rd physical examination of healthy population from Kailuan group by cohort study. Cluster sampling was used by 25% ratio in subjects≥60 years of age to monitor their 24-hour ambulatory blood pressure and finally, 2464/3064 participants with inclusion criteria were recruited. SBPV indexes as standard deviation of systolic blood pressure (SSD), variability independent of the mean (VIM), maximum-minimum difference (MMD) and average real variability (ARV) were examined; renal damage indexes as estimated glomerular filtration rate (eGFR) and microalbuminuria (ALBU) were detected. Relationships between different long-term, short-term SBPV indexes and eGFR, ALBU were studied by multi-liner regression analysis. Results:①The mean age of 2464 participants was (67.41 ± 6.05) years including 1667 (67.7%) male and 797 (32.3%) female.②Multi-liner regression analysis indicated that different long-term SBPV indexes were not related to eGFR and ALBU; 24h SBPV in all 4 indexes and day-time SSD, MMD, ARV were negatively related to eGFR; 24h ARV and day-time MMD, ARV were positively related to ALBU; night-time SBPV indexes were not related to eGFR and ALBU.Conclusion: Different short-term SBPV indexes were, at certain point related to eGFR and ALBU

Objective To explore the effect of Chaihu Shugan Powder (CSP) on the content of 5-hydroxytryptamine (5-HT) in the hippocampus and the expression of tryptophan hydroxylase 2(TPH2) in median raphe nuclei in depression rats.Methods SD rats were randomly divided into normal group(n=12),model group (n=12),fluoxetine group (n=12),low-dose and high-dose CSP group (n =12,respectively).Depression model was made by reserpine intraperitoneal injection.During the experiment,the weight and the open-field scores were calculated;the content of 5-HT was detected by ELISA.The expression of TPH2 in median raphe nuclei was detected by Western blot.Results Compared with the weight ((225.02±5.23) g),the open-field scores ((12.6± 5.1)score) and content of 5-HT ((1.09±0.27) ng/ml) in the model group,high-dose CSP showed significantly improve the depressive rats in weight,open field score and content of 5-HT ((238.78±5.16) g,(15.6±7.8) score and (1.80±0.58) ng/ml,respectively;P<0.05 or P<0.01).The expression of TPH2 (0.66±0.21) in median raphe nuclei in the high-dose CSP group was apparently increased compared with that in the model group(0.16±0.04) (P <0.01).Conclusion CSP have the effects of anti-depression,which could be related with the increase of the 5-HT content in the hippocampus and the expression of TPH2 in median raphe nuclei.

Objective To observe the relationship between expression changes of γH2AX and the radiosensitivity of lung cancer cells in vitro. Methods The radiosensitivity of lung cancer cell lines A549 and SBC-3 was measured by clone forming assay. The DSBs damage of lung cancer cell lines A549 and SBC-3 was determined by Western blot assay. Re-sults The clone forming rates of lung cancer cell lines A549 and SBC-3 were gradually decreased with the increased radia-tion dose.γH2AX expression was related to the cell radiosensitivity 1 hour and 6 hours after radiated. Conclusion The phosphorylated histoneγH2AX is a powerful tool to monitor DNA DSBs and to predict the radiosensitivity in lung cancer ra-diotherapy.

Objective To explore the relationship between plasma cholesterol ester transfer protein(CETP) and TaqIB gene polymorphism and type 2 diabetes mellitus(DM).Methods The genotypes of the first intron of CETP TaqIB gene in 102 patients with type 2 DM and 103 healthy controls were analyzed by polymerase chain reaction-restricted fragment length polymorphism(PCR-RFLP),and plasma CETP were measured by enzyme-linked immunosorbent assay(ELISA).The association of gene polymorphism with plasma lipids,lipoproteins and apolipoproteins was also analyzed.Results No significant differences in both of the alleles frequencies and genotypes in type 2 DM were noted in comparison with controls.There were no relationships between CETP TaqIB gene polymorphism and gender,family history,smoking and body mass index(BMI).No significant differences of plasma lipid levels were observed in healthy controls with different genotypes.However,significant differences in level of HDL-C or apoAI among different CETP TaqIB genotype groups were observed,and significant correlation between low high-density lipoproteinemia and B1 allele in type 2 DM was observed.Conclusion CETP TaqIB gene polymorphisms associated with lipid metabolism,which may be an important genetic factor for abnormal lipid metabolism in type 2 DM.
