RESUMO

Phlebosclerotic colitis is a rare disease of intestinal ischemia and differentiating it from the typical ischemic colitis. It is caused by venous obstruction due to colonic and mesenteric venous calcification. We report a 36-year-old woman presenting with intermittent abdominal pain. Initial radiologic findings showed multiple tortuous thread-like calcifications in the region of the right side of the colon and transverse colon on plain abdominal radiographs and computed tomography images. In the colonoscopy, edematous dark-bluish colonic mucosa, sclerotic colon wall, and multiple ulcers without clear boundaries were observed from the ascending colon to the transverse colon. In the sigmoid colon only showed the edematous dark-bluish colonic mucosa, sclerotic colon wall. On the basis of these findings, we diagnosed the patient as having phlebosclerotic colitis. We report a rare case of phlebosclerotic colitis in healthy young woman.

RESUMO

Hepatic portal venous gas (HPVG) is a rare radiographic finding associated with severe intra-abdominal disease and fatal outcome. Most cases of HPVG are historically related to mesenteric ischemia accompanied by bowel necrosis. The current spread of computed tomography scan promotes not only the early detection of related severe diseases but also the identification of other causes of HPVG. It has been reported in many non-fatal conditions, such as inflammatory bowel disease, intra-abdominal abscess, bowel obstruction, paralytic ileus, endoscopic retrograde cholangiopancreatography and endoscopic sphincterotomy, and gastric dilatation. Among these, paralytic ileus is a very rare condition, with no case yet reported in South Korea. Reported herein is a case of HPVG in paralytic ileus, which was treated well internally and was promptly resolved.

Hematoma of gastric wall is very rare, and occasionally associated with coagulopathy, trauma, peptic ulcer disease, and therapeutic endoscopy. Ischemic gastric necrosis is also rare because of the abundant anastomotic supply to the stomach, and it is usually associated with surgery and disruption of the major vessels. Endoscopic submucosal injection of hypertonic saline-epinephrine (HS-E) is a safe, cost-effective, and widely used therapy for hemostasis but it may cause tissue necrosis and perforation. We describe a case of gastric wall hematoma with oozing bleeding after endoscopic gastric mucosa biopsy in 71-year old woman with chronic renal failure and angina pectoris undergoing anti-platelet medication. We injected a small dose of HS-E (7ml) for controlling oozing bleeding. Two days later, endoscopy showed huge ulcer with necrotic tissue at the site of previously hematoma. Therefore we should pay particular attention for hematoma and mucosal necrosis when performing endoscopic procedure in a patients with high bleeding and atherosclerotic risk.

RESUMO

Malarial infection is one of the most important tropical diseases, but also increasing in the temperate regions. Severe malaria with organ dysfunction is commonly associated with Plasmodium falciparum, but rarely with Plasmodium vivax. Malarial hepatitis is also unusual in P. falciparum and very rare in P. vivax. Only 3 cases of malarial hepatitis caused by P. vivax have been reported in the world. Because the presence of hepatitis in malaria indicates a more severe illness with higher incidence of other complications and poor prognosis, malarial patients should be meticulously monitored for hepatic dysfunction with or without jaundice. We report here a case of malarial hepatitis caused by P. vivax that was presented by fever, general ache, nausea, fatigue, and significant elevation of aminotransferase and bilirubin.

RESUMO

Behcet's disease is a chronic disease characterized by recurrent systemic vasculitis. The pathogenesis remains unclear. This disease involves various organs of the body and it can have a variety of symptoms with an unpredictable course. The diagnosis of Behcet's disease is made in patients with oral and genital ulcers and uveitis. In addition, gastrointestinal symptoms are frequently present; however, intestinal ulcers are infrequent. The most common extra-oral sites of gastrointestinal involvement are ileocecal and colon lesions. The lesions are typically resistant to medical treatment and frequently recur after surgical treatment. Here we present two cases treated with the monoclonal antibody anti-tumor necrosis factor (TNF) and regression of all symptoms.

BACKGROUND: Despite its high prevalence and importance in terms of one's quality of life, male sexual dysfunction has been ignored. This study is intended to evaluate the validity of a single-item question concerning sexual satisfaction as a screening tool for assessing patients' erectile dysfunction in primary care. METHODS: The male patients, who visited the department of family medicine from May to July of 2002, were required to fill out a questionnaire in order to assess their sexual satisfaction. Sexual dysfunction was examined through the erectile function (IIEF-5) questionnaire. The questionnaire on sexual satisfaction was evaluated to determine the sensitivity, specificity, positive predictive value, and negative predictive value for erectile dysfunction in 299 subjects. RESULTS: The prevalence of erectile dysfunction was 41.5% in patients aged over 40. The ratio of sexual unsatisfaction was 35.8%, and the sensitivity and specificity of this response were 0.609 and 0.799, respectively. The prevalence of erectile dysfunction demonstrated a significant correlation with increasing age (P<0.001), but was not influenced by body mass index, smoking, alcohol consumption, and exercise. Diabetes mellitus was a risk factor of erectile dysfunction, but patients' personal medical history such as hypertension, hyperlipidemia, and prostatic hypertrophy were not related to sexual dysfunction. CONCLUSION: The single-item question concerning overall sexual satisfaction may be a useful screening tool for assessing erectile dysfunction.

RESUMO

Agenesis of the right lobe of the liver is an extremely rare anomaly of the liver, and few cases are reported in the literature. Most of the patients with this anomaly are accompanied by additional anormalies such as retrohepatic or suprahepatic gallbladder and other biliary tract diseases, including cholelithiasis, carcinoma of the gallbladder and portal hypertension. The diagnosis of this rare anatomical variant was established by ultrasonography and computed tomography. The radiological findings, clinical presentation, and differential diagnosis are reviewed.

RESUMO

Anisakis is an accidental parasitic infection caused by the larvae of marine mammals. An individual becomes infected with a larval form of the nematode Anisakis by consuming raw or undercooked fish containing the parasite. Generally, the clincal symptoms are characterized by severe cramping abdomianl pain, nausea, vomiting, and an epigastric full sensation. A case was recently experienced by a patient who suffered acute esophageal anisakiasis, and had a history of eating raw sea fish. His symptoms included substernal chest pain and odynophagia. Early endoscopic examination revealed a whitish linear worm in the mid-esophagus, which was removed by biopsy forceps.

BACKGROUND: In order to investigate the effects of intensive training on cardiorespiratory response to maximal exercise in soccer players, graded excercise stress tests were carried out before and after the training, and the results were compared. METHODS: Fourteen male soccer players of a university team with the mean age and career of 19.9 and 7.8 years, respectively, underwent intensive training for approximately 20 weeks, 6 days a week and 4 hours a day, which included running of 2 km daily during the last 8-9 weeks. The symptom-limited maximal exercise test was done by Bruce protocol. RESULTS: After the training. HR max showed no significant changes but Vo2 max, O2 pulse and AT were increased approximately 13%, 12% and 10%, respectively. At maximal exercise, the V(E)/M2 was also increased 18%, but VE/MVV was significantly decreased owing to a marked increase in MVV. However, the rest of various ventilatory parameters showed no significant changes. CONCLUSION: These data indicate that the intensive training of about 20 weeks duration can further improve the level of cardiorespiratory fitness in the soccer players who have been regularly trained and are already highly fit.
