Purpose@#To study the long-term efficacy and safety of Flexivue Microlens refractive corneal inlays in Koreans for compensation of presbyopia. @*Methods@#Flexivue Microlens inlays were implanted in nine eyes at Samsung Medical Center from October 2015 to February 2018. The follow-up period was 1.95 ± 0.6 years. Pre- and postoperative near/intermediate/distant visual acuity, spherical equivalent, contrast sensitivity, defocus curve, keratometry, and patient satisfaction were evaluated retrospectively. @*Results@#The uncorrected near visual acuity (UNVA) of the operated eye increased significantly to 0.27 ± 0.10 logarithm of the minimal angle resolution (logMAR) after 6 months (p = 0.012). At the last observation, it was 0.32 ± 0.21 logMAR, which improved compared to before surgery, but the difference was not significant (p = 0.127). The binocular UNVA improved significantly to 0.19 ± 0.78 logMAR at 6 months postoperatively (p = 0.017) and to 0.21 ± 0.13 logMAR at the last observation (p = 0.028). There was no difference in the binocular uncorrected distant visual acuity (UDVA) before and after surgery, but the UDVA of the operated eye decreased significantly to 0.38 ± 0.15 logMAR at 6 months postoperatively (p = 0.007) and to 0.32 ± 0.21 logMAR at the last observation (p = 0.012). Satisfaction with near vision improved significantly after surgery; 14.3% of the patients had a score of 4 (good) or higher and 42.9% did not require near-vision glasses. In one case, the inlay was removed after 17 months due to blurred vision. @*Conclusions@#The Flexivue Microlens has a low risk of complications and is a reversible technique. However, the near vision improvement was temporary and distance vision deteriorated. In addition, the Koreans examined had relatively low satisfaction and high spectacle dependence.

Purpose@#To report two cases of treatment using high-frequency radio wave electrosurgery for corneal neovascularization that recurred after medication and laser photocoagulation attempts.Case summary: (Case 1) A 53-year-old man visited our hospital complaining of corneal opacity. The best-corrected visual acuity in the left eye was 0.6. Lipid keratopathy indicated new inferior vessels. There was no significant change in corneal opacity after medication and laser photocoagulation; however, the recurrence of feeder vessels was observed. Therefore, electrocautery was performed via high-frequency radio wave electrosurgery. Visual acuity, measured 1 month later, improved to 0.9. Over the subsequent 4-year observation period, new vessels did not recur and the corneal opacity decreased gradually. (Case 2) A 23-year-old woman visited complaining of left eye pain. She had undergone laser photocoagulation three times for corneal new vessels. Visual acuity in her left eye was 0.4. Recurrence of new vessels at the upper cornea was observed, and electrocautery was performed. After 2 months, corneal opacity decreased without revascularization, and visual acuity improved to 0.5. @*Conclusions@#In patients with corneal neovascularization, electrocautery using high-frequency radio wave electrosurgery is simple and effective and can reduce corneal opacity and improve vision without complications.

Quantum dots (QDs) have received considerable attention due to their potential role in photosensitization during photodynamic therapy. Although QDS are attractive nanomaterials due to their novel and unique physicochemical properties, concerns about their toxicity remain. We suggest a combination strategy, CdSe/ZnS QDs together with curcumin, a natural yellow pigment from turmeric, to reduce QD-induced cytotoxicity. The aim of this study was to explore a potentially effective cancer treatment: co-exposure of HL-60 cells and human normal lymphocytes to CdSe/ZnS QDs and curcumin. Cell viability, apoptosis, reactive oxygen species (ROS) generation, and DNA damage induced by QDs and/or curcumin with or without ultraviolet A (UVA) irradiation were evaluated in both HL-60 cells and normal lymphocytes. In HL-60 cells, cell death, apoptosis, ROS generation, and single/double DNA strand breaks induced by QDs were enhanced by treatment with curcumin and UVA irradiation. The protective effects of curcumin on cell viability, apoptosis, and ROS generation were observed in normal lymphocytes, but not leukemia cells. These results demonstrated that treatment with QD combined with curcumin increased cell death in HL-60 cells, which was mediated by ROS generation. However, curcumin acted as an antioxidant in cultured human normal lymphocytes.

RESUMO

Retrobulbar hemorrhage and permanent visual loss are rare presentations following traumatic asphyxia. In this case, bilateral permanent visual disturbance developed in a woman after chest-crushing trauma without direct trauma to the orbits. A computed tomography scan confirmed bilateral retrobulbar hemorrhages. An ophthalmologic exam revealed bilateral subconjunctival hemorrhages and severe lid edema. Despite high-dose steroid therapy, visual recovery was limited, and optic nerve atrophy developed. Ischemia of the optic nerve associated with retrobulbar hemorrhage may be postulated as one of the causes of permanent visual impairment following traumatic asphyxia.

PURPOSE: To determine the efficacy of frontalis sling operation with silicone rods in patients with compromised corneal protective mechanisms. METHODS: The authors retrospectively studied 6 consecutive patients (7 eyelids) with severe blepharoptosis with poor ocular motility who had undergone frontalis sling operations using silicone rods. RESULTS: Preoperative diagnoses included third nerve palsy in 4 patients, double elevator palsy in 1 patient, and suspicious oculopharyngeal muscular dystrophy in 1 patient. With a mean follow up of 27.7 months, a good final lid height was achieved in all 7 eyelids. Mild exposure keratopathy occurred postoperatively in 3 patients. During the follow-up period, no other significant complications, such as extrusion of the sling or infection, occurred. CONCLUSIONS: Silicone rods are effective and safe materials for use in frontalis suspension in treating blepharoptosis in patients with inadequate or absent Bell's phenomenon, resulting in poor eye protective mechanisms associated with an increased incidence of corneal exposure.

PURPOSE: To evaluate the effectiveness and safety of treating orbital lesions with gamma knife radiosurgery (GKS). METHODS: Between April 2004 and January 2006, ten patients who had orbital tumors or vascular lesions and who underwent GKS were included in this retrospective study. RESULTS: Ten patients with orbital lesions were treated with GKS. The group of orbital lesions consisted of 5 meningiomas, 2 schwannomas, 1 cavernous hemangioma, 1 arteriovenous fistula, and 1 adenoidcystic carcinoma of the lacrimal gland. The most common symptom was proptosis. The tumors were located at the orbital apex in eight patients, and five of these patients were treated with fractionated stereotactic radiosurgery. The mean cumulative marginal dose was 17.0 Gy (12-20 Gy), and the mean cumulative maximal dose was 30.8 Gy (16.2-40.4 Gy). The follow-up period ranged from 3 to 25 months (mean 13.9 months). During the follow-up period, magnetic resonance imaging revealed a decrease of tumor volume in 3 patients with symptomatic improvement. In two patients, tumor volume increased. No radiation-induced optic neuropathy, retinopathy, or cataract was observed in any of the 10 patients during the follow-up period. CONCLUSIONS: Gamma knife radiosurgery is an effective and relatively safe treatment when orbital lesions have a high risk of neurosurgical deficits with surgery, when they recur after incomplete resection, or when complete removal of tumor is impossible.

PURPOSE: To determine the effect of central corneal thickness (CCT) on the measurement of intraocular pressure (IOP) and diagnosis of glaucoma on the resultant reclassification of patients as having primary open-angle glaucoma (POAG), normal tension glaucoma (NTG), or ocular hypertension (OHT). METHODS: Intraocular pressure (Goldmann applanation tonometry) and visual field test (Humphrey 24-2) were measured in 22 eyes of 23 patients with OHT, 20 eyes of 20 patients with POAG, 31 eyes of 31 patients with NTG, 27 eyes of 27 control subjects. CCT was measured and used to obtain a corrected value for the IOP and to reclassify the type of glaucoma. RESULTS: There was no significant difference in CCT between controls and patients with POAG, but the CCT in the group with NTG was significantly lower than that in the control group (p=0.012), and the CCT in the group with OHT was significantly higher than in controls (p=0.002). Correcting IOP for corneal thickness, 22.58% of patients with NTG could be reclassified as having POAG, and 56.52% of the patients with OHT as normal. CONCLUSIONS: Underestimation of the IOP in patients with POAG who have thin cornea may lead to a misdiagnosis of NTG, while overestimation of the IOP in normal subjects who have thick cornea may lead to a misdiagnosis of OHT. We suggest that CCT must be considered in diagnosis of glaucoma.

RESUMO

PURPOSE: To evaluate the postoperative refractive error after vitrectomy and gas tamponade in macular hole patients. METHODS: Thirty patients with full-thickness macular hole were enrolled in this study. Thirty eyes had vitrectomy, internal limiting membrane peeling, and gas tamponade. Preoperative and postoperative refractive errors were compared. Five eyes also had cataract extraction and implantation of an IOL and actual and expected refractive errors were compared. RESULTS: The refraction after vitrectomy and gas tamponade shifted toward myopia by 0.71 diopter compared with preoperative refraction. The actual refractive errors in cataract surgery group shifted toward myopia by 0.65 diopter. CONCLUSIONS: . Use of a gas tamponade in macular hole patients is thought to have induced the myopic change by pressing the crystalline lens forward.

OBJECTIVES: Antikeratin antibody is an autoantibody to the cornified epithelium of rat esophagus and known to be a specific marker in Caucasian patients with rheumatoid arthritis. This study was performed to evaluate the prevalence and clinical correlation of the antibody in Korean patients with rheumatoid arthritis. METHOD: We measured antikeratin antibodies by indirect immunofluorescence method and evaluated functional class, anatomical stage, grip strength, Ritchie articular index, tender joint count, swollen joint count, rheumatoid factor and antinuclear antibody in 88 patients with rheumatoid arthritis, 20 patients with systemic lupus erythematosus and 43 healthy controls. RESULTS: The sevsitivity and specificity of the antikeratin antibody test was 48% and 98% respectively in Korean patients with rheumatoid arthritis. Intensity of antikeratin antibody was correlated with duration of morning stiffness(r=0.22, p=0.01) and rheumatoid factor titer(r=0.23, p=0.04). Antikeratin antibody was positive in 8(31%) out of 26 patients without rheumatoid factor. CONCLUSION: Antikeratin antibody is a specific marker in Korean patients with rheumatoid arthritis and correlated with duration of morning stiffness and rheumatoid factor titer.
