RESUMO

We present a patient with scrub typhus complicated with a splenic infarction. A 40-year-old man visited the emergency medical center complaining of fever for the previous week. He had no past medical history, but reported engaging in outdoor activities. Examination revealed a maculopapular rash on his trunk and an eschar on his epigastrium. Abdominal computed tomography was performed to examine the cause of the tenderness on the left upper quadrant of his abdomen, which revealed a splenic infarct. The patient was diagnosed with scrub typhus based on the results of blood polymerase chain reaction testing, and genetic sequencing confirmed the presence of Orientia tsutsugamushi Boryong. His symptoms improved following doxycycline treatment.

Four Gram-positive cocci were isolated from the cerebrospinal fluid or blood of four different patients, but they could not be identified by an automated conventional identification system, so they were identified using cellular fatty acid (CFA) composition analysis and 16S rRNA gene sequencing analysis. Of these, two strains (SMC-A2662 and SMC-A5889), which were previously supposed to be Rothia dentocariosa according to the API Coryne system, were identified as Rothia aeria by the 16S rRNA gene analysis. SMC-A608, which was unidentified by both the VITEK2 and API Coryne systems, was identified as Rothia mucilaginosa. The one remaining SMC-2244T was distinguished from the other Rothia species by its biochemical profile, its CFA composition and its 16S rRNA gene sequence. Phylogenetic analysis showed that it was closely related to Rothia nasimurium but the 16S rRNA gene sequence dissimilarity of 1.8% was enough to differentiate it from R. nasimurium. Based on both the phenotypic and phylogenetic evidence, we propose a new species name for this bacterium, Rothia arfidiae sp. nov. The results of this study show that several Rothia species were isolated from human and we have identified them using 16S rRNA gene sequences.

RESUMO

PURPOSE: We evaluated the safety and efficacy of intrarectal administration of anesthetic agent for transrectal prostate biopsy. MATERIALS AND METHODS: For a double-blind prospective trial, 100 men undergoing transrectal prostate biopsy were randomized into three groups using an established list. In the control group (n=36), 40mL of normal saline was administered transrectally 10 min before the procedure. The lidocaine group (n=31) and the benzocaine group (n=33) were give 40 mL of 1% lidocaine and 0.3% benzocaine, respectively. Patients were asked to score the severity of the pain expected before and after the biopsy procedure using a self-administered verbal rating scale. Pain was assessed using a 10-point linear visual analog scale. RESULTS: The median pain score expected before the transrectal prostate biopsy was 6.7+/-0.3, 7.1+/-0.2, and 6.7+/-0.3 in the control, lidocaine, and benzocaine group, respectively. The median pain score during the biopsy was 6.1+/-0.3, 5.8+/-0.3 (P<0.05), and 5.9+/-0.4 in the control, lidocaine, and benzocaine group, respectively. Ninety seven percent of the control group, 100% of the lidocaine group, and 91% of the benzocaine group complained of severe and bothersome pain, with a score of 5 or greater during the biopsy. No adverse events were noted. CONCLUSIONS: Intrarectal administration of anesthetic agent is a simple, safe, and efficacious method of providing satisfactory anesthesia in patients undergoing transrectal prostate biopsy. We recommend 1% lidocaine solution in the patients to perform transrectal prostatic biopsy.

RESUMO

A clinical observation was made on the renal tumors of the in-patients in the department of Urology, Chonnam National University Hospital during the periods from March, 1968 to Feb., 1971 and the following results were obtained. 1. During the period, of 350 cases (275 male and 75 females) hospitalized, 13 cases had tumors of kidneys giving a rate of 3.71%. Among the 13 cases, 9 cases were male and 4 cases were female with ratio of 2.3 to 1. 2. Age distribution ranged from 2 year old to 65 year old with average of 30 year old of 13 cases, 5 cases (39%) were seen under the age of 10 years, and 4 cases (31%) were seen over the age of 50 years. 3. Tumors involved right side in 8 cases, left side in 5 cases. 4. The most common complaints on admission were palpable mass (61%), pain (31%) and hematuria (31%) in order. 5. Excretory urogram was taken in 12 cases. In 6 cases, the diseased kidney was not visualized. Remaining 6 cases gave compression deformities compatible with renal tumor. Retrograde pyelogram was performed in 6 cases. 6. 12 patients underwent nephrectomy (trans-abdominal approach was in 6 cases, extraperitoneal approach in 6 cases. 7. Renal cell cancer was observed in 5 cases, Wilms' tumor in 3 cases, neuroblastoma in 2 cases, adenofibroma of pelvis in 1 case, and transitional cell cancer of pelvis in 1 case. 8. Mean weight of removed tumors was 704 gm.

RESUMO

A case endometriosis of the bladder in s 40-year old woman is presented. Urinary ferquency, tenesmus and intermittent hematuria had been complained, associated with menstruation over one year period. Tumor-like mass of the bladder recognized incidentally during hysterectomy on suspicious uterine myoma. Segmental resection of this mass with involved bladder resulted in cure and this lesion was confirmed histologically as endometriosis of the bladder.
