RESUMO

PURPOSE: This study was designed to identify mother and infant related factors that influence child-care stress among the mother of newborn infants. METHODS: Data were collected through self-administered questionnaires and descriptive statistics, correlation and multiple regression were used to analyze the data. The data survey was conducted with 957 conveniently selected mothers of infants when they visited a public health center in Seoul to have their children immunized. RESULTS: The average item score for the Childcare Stress Inventory was 38.03, for the Edinburgh Postnatal Depression Scale, 9.31 and for the Degree of Bother Inventory, 23.42. The CSI was positively correlated to EPDS (r=.44, p<.001) and DBI (r=.40, p<.001). Also these two variables explained 30.0% of CSI in infants' mothers. CONCLUSION: These findings are expected to expand the understanding about postpartum mothers' child-care stress and can contribute to the development of comprehensive interventions based on community health nursing.

RESUMO

The effects of Allium vegetables on blood glucose levels and lipid metabolism in streptozotocin (STZ) induced diabetic rats were investigated. Diabetes mellitus was induced by STZ injection (45 mg/kg b.w.) into the tail vein. Sprague-Dawley rats weighing 220+/-10 g were randomly assigned to 7 groups: normal, STZ-control and five Allium groups (Allium cepa, Allium fistulosum, Allium sativum, Allium tuberosum and Allium victorialis) . Normal and STZ-control groups were fed an AIN-93 diet and five Allium groups were fed a modified diet containing 10% Allium powder each for 4 weeks. Body weight, diet intake, food efficiency ratio (FER) and organ weights were monitored. Activities of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were observed. Plasma levels of glucose, free fatty acid, triglyceride and HDL-cholesterol were analyzed. Levels of glycogen, cholesterol and triglyceride in liver were determined. Levels of malondialdehyde (MDA) in liver, lung, kidney, and pancreas were assayed. The hepatic contents of chromium (Cr) , iron (Fe) , zinc (Zn) and manganese (Mn) were measured. The Allium sativum group had weight gain and suppressed a hypertrophy of the kidney significantly. The activity of ALT was significantly lowered in the diabetic groups except Allium sativum group compared to STZ-control group. The Allium sativum and Allium tuberosum groups showed the hypoglycemic effects at 4 weeks. There were no significant differences between the control and all the other diabetic groups in the plasma levels of cholesterol, HDL-cholesterol, triglycerides and free fatty acids. Most of the Allium groups except Allium fistulosum were observed significantly lowered level of MDA in the lung compared to STZ-control group. The diabetic rats fed the Allium cepa and Allium sativum have shown significantly lowered hepatic Zn contents. The results suggested that the intake of the Allium vegetables may be effective in the antihyperglycemia by lowering blood glucose levels.

PURPOSE: This study was designed to investigate the relationships of the types of nurse staffing, the competency of nursing performance and job satisfaction by analyzing the types of nurse staffing in operating room. METHOD: The subjects were 472 nurses who work in operating room. The instruments used in this study were three tools: competency of nursing performance, job satisfaction and type of nurse staffing. RESULT: Type 4 was most in the type of nurse staffing of the subjects. According to the type of nurse staffing, the competency of nursing performance was significantly different. There was significant difference in the competency of nursing performance according to age, marital status, bachelor, employ, position, and career, and job satisfaction according to age, position, and career. In opinion about the ideal type of nurse staffing, type 2 was most regardless of career. CONCLUSION: Type 4 which has lower competency of nursing performance was adopted in many hospitals now but, type 2 which has highest competency of nursing performance was selected by many nurses as the best one. So, it should be considered the type of nurse staffing of in operating room and change it.

BACKGROUND: To evaluate the efficacy and safety of paclitaxel and cisplatin against advanced non-small cell lung cancer (NSCLC) as a second-line chemotherapy. SUBJECTS AND METHODS: Twenty-five patients were enrolled. The patients received 200 mg/m2 paclitaxel as a 3-hour intravenous infusion and 60 mg/m2 cisplatin as 30-minute intravenous infusion with vigorous hydration on day 1 every 28 days. The response was assessed every 2 cycles. RESULTS: All 25 patients were assessed for their response and toxicity. Partial responses were observed in 5 patients. The overall response rate was 20% (95% confidence interval, 4%~36%) and the median response duration was 4.5 (range, 2-11) months. The median time to progression was 3.3 (range, 0-14) months. The median overall survival of all patients was 7.4 (range, 1.3-39) months. The hematologic toxicities were minor and easily controlled. CONCLUSION: The combination chemotherapy of paclitaxel and cisplatin as a second-line treatment has a moderate efficacy with an acceptable toxicity in patients with advanced NSCLC.

BACKGROUND: For the rapid sequence induction of general anesthesia, succinylcholne, a depolarizing muscle relaxant, has been used. But succinylcholine has many side effects, so various efforts using nondepolarizing muscle relaxants have been tried. We have tried and observed the effect of a curonium-lidocaine mixture administration. METHODS: 120 patients who were scheduled for elective surgery were randomly assigned in 6 groups (Group I, II, III, IL, IIL, IIIL). For groups I, II and III vecuronium was administered only 0.10 mg/kg, 0.12 mg/kg, and 0.15 mg/kg, respectively. For each member of groups IL, IIL and IIIL, lidocaine 1.5 mg/kg was added to the dose of vecuronium of groupI, II and III. The vecuronium or vecuronium- lidocaine mixture was injected for 15 seconds and then thiopental sodium was injected for 15 seconds. 90 seconds after the administration of the vecuronium or vecuronium-lidocaine mixture, every patient was intubated. Intubation condition scores, TOF responses of the adductor pollicis of the thumb, arrhythmia, heart rates, systolic blood pressure, diastolic blood pressure and mean arterial blood pressure were checked and compared. RESULTS: Intubation condition scores were better in groups IL, IIL and IIIL in comparison with groups I, II and III. The TOF responce of the adductor pollicis of the thumb showed a statistically signifiant difference between the lidocaine groups and the nonlidiocaine groups. The time elapsed before the disappearance of TOF was less in the lidocaine groups. Changes of systolic pressure, diastolic pressure, mean arterial blood pressure and heart rate were no difference in the lidocaine groups and the nonlidocaine groups. CONCULUSIONS: The results suggest that administration of a vecuronium-lidocaine mixture administraion improves the intubation condition score during a rapid sequence induction of general anesthesia and shortens the time of the disapprearance of the TOF response.

RESUMO

Large thyroid tumors present distortion of the airway, endocrine disturbance and metastatic effect. It may compromise airway directly or indirectly through dysfunction of the recurrent laryngeal nerve. The management of abnormal airway structure and the potential for both difficult intubation and surgical access to the airway are based on the principle of burning no bridges. Thyroidectomy can be performed under regional anesthesia using cervical epidural anesthesia in which maintenance of the airway and monitoring of recurrent laryngeal nerve function are possible. We describe a patient in whom a large thyroid mass with marked tracheal deviation was treated successfully by cervical epidural anesthesia and spontaneous respiration.

RESUMO

Blunt tracheobronchial injuries are rare, and usually lethal without aggressive and appropriate management. Recently, the incidence of tracheobronchial injuries has been increased with the increase of traffic accident and mechanization. We report a case of complete tracheal transection combined with a longitudinal rupture of the membranous portion of the trachea extending to the right main bronchus following blunt chest trauma, and review the anesthetic management.
