RESUMO

PURPOSE: Hepatocellular carcinoma (HCC) is a rarely occurring disease in the pediatric population. We report our center's experience of management of HCC in children and adolescents. METHODS: From 1996 to 2012, 16 patients aged 18 or younger were diagnosed with HCC at our center. The medical records of these 16 patients were retrospectively reviewed. RESULTS: There were 9 boys and 7 girls. Median age at diagnosis of HCC was 14.5 years. All patient had pathologically confirmed diagnosis of HCC. Three patients had distant metastasis at the time of HCC diagnosis. Eight patients were surgically managed, including 4 liver resections, 3 liver transplantations, and 1 intraoperative radiofrequency ablation. The remaining 8 patients received systemic chemotherapy. Overall, 6 patients are alive at median 63.6 months after diagnosis of HCC. All survivors were surgically managed patients. CONCLUSION: HCC is a rare disease occurring in childhood. Patients with systemic disease have poor outcome. Liver transplantation may be a good option for treatment of pediatric HCC.

RESUMO

Myotonic dystrophy is the most common manifestation of mytonic disease. It is a systemic disease and its most serious complications after general anesthesia result from respiratory failure. We report a case of prolonged respiratory failure after general anesthesia for cesarean section in a presumed myotonic dystrophy patient. A 36-year-old parturient underwent emergency cesarean section under general anesthesia for premature rupture of the amniotic membrane with fetal distress. At the end of the operation, the recovery of respiratory muscle from paralysis was insufficient to maintain spontaneous ventilation despite the administration of reversel agents. Her TOF count was 3/4 and she was reintubated in the PACU. The patient was transferred to the ICU for proper respiratory care and her respiration fully recovered after 21 hours of conservative supportive care.

RESUMO

We report a young man with congenital bilateral perisylvian syndrome (CBPS), who showed eating reflex seizures (ERS). His ERS were related to the taste of foods. The subtracted ictal SPECT co-registered to MRI (SISCOM) revealed the hyperperfusion in the insula as well as the subcortical nuclei and brainstem. The ERS in CBPS may be partially related with the taste function of the insula and SISCOM may be helpful for the localization of the epileptogenic foci in ERS.

RESUMO

Epileptic seizure is a frequent sign of mitochondrial encephalopathies with lactic acidosis and stroke-like episodes (MELAS). We report a 21-year-old woman with MELAS who had alternating hemispheric seizures. Her brain MRI showed migratory short lasting lesions. Serial EEG recordings revealed epileptiform discharges, which occurred first on the right frontotemporal region but then switched to the left frontotemporal region concordant to her clinical and MRI manifestations. Migratory short lasting lesions may be helpful features for differentiating MELAS syndrome from other epileptic syndromes.

RESUMO

Modified two layer vasovasostomy with optical loupe magnification was performed in 66 patients from Jan. l984 to Dec. 1990 in Seoul Eulji General Hospital. 44 patients could be followed up, viable sperms more than 10 x 10(6)/ml appeared in 37(84.1 %) and pregnancy occurred in 24(54.5 %). Patients presenting for vasovasostomy less than 5 years after vasectomy had a 88.9 % (24/27) patency rate and a pregnancy rate of 66.7% ( 18/27). Beyond 5 years, a 76.5% (13/17) patency rate and a pregnancy rate of 35.3% (6/17). In conclusion, we have found that there is little difference in the patency and pregnancy rates resulting from microscopic vasovasostomy and vasovasostomy with optical loupe magnification, however, modified two-layer vasovasostomy with optical loupe magnification was simple, easy, convenient time-saving and cost-effective comparing with microscopic vasovasostomy.

RESUMO

We experienced a case of malignant lymphoma with the simultaneous involvement of right testis left adrenal gland and retroperitoneal lymph nodes and treated with radical orchiectomy followed by systemic chemotherapy, who died with widespread malignant lymphoma 9 months later.

Recently we experienced a case of mucinous prostatic adenocarcinoma treated with radical prostatectomy. The needle biopsy of indurated both lateral lobes showed prostatic adenocarcinoma. But the radical prostatectomy specimen showing a prostatic adenocarcinoma of which 90% was mucinour component.

RESUMO

The results of intraprostatic injection of fosfomycin were compared to those of oral minocycline therapy in patients with nonbacterial prostatitis. Nonbacterial prostatitis did not recur in 26 out of 33 patients with fosfomycin injected into the prostate and in 16 out of 48 patients treated with oral minocycline. Non-recurrence rate within each group was compared as 78.8 per cent to 33.3 per cent, respectively, demonstrating that intraprostatic fosfomycin treatment was more successful than oral minocycline therapy. The pain and discomfort experienced by the patients during direct injection into the prostate was minimal. Hematuria was always practically present after the injection. Intraprostatic fosfomycin injection is believed to be new alternative method in the treatment of nonbacterial prostatitis.
