RESUMO

Objective To evaluate the clinical effect of deep acupuncture at Lianquan (CV23) combined with swallowing rehabilitation training in the treatment of dysphagia after stroke.Methods A total of 84 patients with dysphagia after stroke who met the inclusion criteria were divided into 2 groups, 42 in each. The control group received swallowing rehabilitation training, and the observation group was treated with acupuncture therapy based on deep acupuncture atLianquan (CV23) combined with swallowing rehabilitation training. The VFSS score and clinical efficacy were compared between the 2 groups before and after treatment.Results The cure rate of observation group was 69.1% (29/42), the total effective rate 85.7% (36/42), while the control group were 42.86% (18/42) and 66.67% (28/42). The comparison of cure rate and total efficiency rate between the two group had statistically significant differences (χ2=5.845, 4.200,P=0.016, 0.040). After treatment, the VFSS score of the observation group was significantly higher than that of the control group (7.43 ± 2.27vs.4.31 ± 1.53;t=5.433, P<0.05). The recovery time of the 29 cured patients in the observation group was (12.0 ± 3.0) d, while the recovery time of the 18 cured patients in the control group was (17.8 ± 2.2) d. There was statistically significant difference between the 2 groups (t=7.094,P=0.001).Conclusions Deep acupuncture at Lianquan (CV23) combined with swallowing rehabilitation training can improve the swallowing function of patients with dysphagia after stroke, shorten the recovery time of swallowing function and improve the clinical efficacy.

RESUMO

<p><b>OBJECTIVE</b>To explore the application of enhanced CT in the differential diagnosis and treatment of peritonsillar abscess (PTA) and intratonsillar abscess (ITA).</p><p><b>METHODS</b>Thirty-eight in-patients with clinically suspected PTA from June 2011 to June 2013 were included in this study. All these patients underwent enhanced CT scan for the throat region. According to CT results, the location of abscess was determined, and the thickness of the posterior wall of abscess as well as its distance with the internal carotid artery was calculated.Incision and drainage were then guided with this information.</p><p><b>RESULTS</b>Twenty-six of the 38 patients (68.4%) met the diagnosis of PTA, demonstrating a hypodense collection with rim enhancement in the peritonsillar space, including 4 cases with multilocular abscess. Ten cases (26.3%) should actually be diagnosed as ITA, with a abscess collection located in the palatine tonsil tissue. Two cases(5.3%) were diagnosed as peritonsillar cellulitis (PTC), showing diffuse isodense lesion around the peritonsillar space. The 26 cases of PTA and 10 cases of ITA patients were all cured using incision and drainage under the precise guidance of CT, while the 2 cases of PTC only treated with medicine. The mean distance between the posterior wall of abscess and the carotid artery (X(-) ± s) were (0.76 ± 0.34) cm and (0.90 ± 0.37) cm for the two entities respectively, with no significant difference (P > 0.05).</p><p><b>CONCLUSIONS</b>Enhanced CT scan can clearly demonstrate the characters of PTA and ITA, and make identification. Moreover, it is helpful for the determination of therapy, improving the success rate of drainage and reducing the potential risk of large artery injury.</p>

RESUMO

Objective To establish a three-dimensional (3D) finite element model of a swine mandible, simulate the dynamic procedure of bullet damaging the swine mandible and explore a finite ele-ment analysis method on maxiilofacial gunshot wound. Methods The digital imaging and communica-tions in medicine (DICOM) data obtained from CT scanning of a swine mandible were remerged into a 3D finite element model of the original specimen through Mimics and ANSA software, then a simulation of 3D finite element model penetrated by a 7.62 mm bullet was carried out through LS-DYNA software. The simulation data were compared with those from animal experiment in laboratory to test the feasibility of 3D finite element model and the simulation method. Results A 3D finite element model of a swine mandi-ble was established, with highly identical geometric size with the specimen. In the meantime, the dynam-ic process of a 7.62 mm bullet damage to the model was successfully simulated. Data from the simulation and those from animal experiment showed a high level of consistency. Conclusion 3D finite element method is prosperous in application in basic research on maxillofacial gunshot wound.

RESUMO

Objective:To evaluate fixation effect of intermaxillary fixation screw.Methods:63 cases with mandibular fractures were treated with open reduction,and then internal fixation were managed with intermaxillary fixation using dental arch splint or intermaxillary fixation screw randomly.The operation time,postoperation periodontal index,mouth open degree,occluding relation and fracture healing were compared.Results:There was no significant difference upon occluding relation,fracture healing and mouth open degree under two treatment methods.Compared with dental arch splint fixation method,intermaxillary fixation screw had shorter operation time and better periodontal index.Conclusion:Intermaxillary fixation screw is a good intermaxillary fixation method because of short operation time.Furthermore,it can provide enough traction retention strength and easy to maintain oral hygiene.

RESUMO

Objective:To evaluate the possibility of prevention of Frey's syndrome by reservation and reconstruction of parotid fascia in parotidectomy.Methods:Parotidectomy with parotid fascia reservation and reconstruction was conducted in 31 cases. 12 months-12 years after operation the patients were examined by Minor test.Result:Positive reaction of Minor test was observed in 3 out of the 31 cases(9.67%).Conclusion:A parotidectomy with reservation and reconstruction of parotid fascia can efficiently prevent Frey's syndrome.

RESUMO

Objective To evaluate whether it can prevent Frey's syndrome by reserving parotid fascia or reconstructing parotid fascia with sternocleidomastoid muscle and remaining of parotid fascia in parotidectomy. Methods Fifty-two patients undergoing parotidectomy with maintained and reconstructed parotid fascia were followed up. A test of chewing vitamin C slice guidance and iodine-amylum pilot was conducted for observing the incidence of postoperative Frey's syndrome. Results Flush with perspires was found only in 5 out of the 52 patients (9.61%), which was lower than the incidence after traditional parotidectomy. Conclusion The method of reserving and reconstructing parotid fascia with sternocleidomastoid muscle and remaining of parotid fascia in parotidectomy can efficiently prevent Frey's syndrome, and can also repair the hollow abnormity of the face.
