RESUMO

In the process of tumor growth, with the proliferation and expansion of cancer cells, the reconstruction of extracellular matrix (ECM) of cancer tissues, the restriction of surrounding tissues and the flow of cancer tissue interstitial fluid, the special stress environment is formed in the tumor tissues. Significant differences are found in the mechanical environment and mechanical characteristics of different regions of tumor tissues, that is, mechanical heterogeneity. The reseach shows that the mechanical properties of tumor tissue invasion frontier areas are more significant and complex. In particular, the epithelial-mesenchymal transition (EMT) of tumor cells also prefers to concentrate on this area. The mechanical stress generated by the invasion front can induce EMT of tumor cells through TWIST1, TGF-β, WNT and other force signal transduction pathways, and promote tumor cell invasion. From the perspective of tumor biomechanics, this review focuses on the relationship between mechanical heterogeneity of tumor cells and EMT, so as to provide the theoretical basis for mechanoenvironment-targeted therapy of tumors.

RESUMO

Objective To investigate the effect of target -oriented fluid therapy on the tissue perfusion of patients with hemorrhagic shock during ectopic pregnancy .Methods From December 2015 to June 2017 ,80 patients with ectopic pregnancy hemorrhagic shock in Laiyang Central Hospital of Yantai were enrolled in this study .They were divided into two groups according to the random number method ,with 40 cases in each group .The control group was given routine fluid supplementation .The observation group was given target -oriented fluid therapy .The average arterial pressure,central venous pressure and urine output in two groups were compared .The vasoactive drug use and use time were analyzed in the two groups .Results The mean arterial pressure in the observation group was (115.6 ±10.3)mmHg,which was higher than that in the control group [(86.9 ±5.6)mmHg](t=15.482,P<0.05).The central venous pressure in the observation group was (7.1 ±0.5)cmH2O,which was higher than that in the control group [(5.9 ±0.2)cmH2O](t =14.093,P<0.05).The urine output in the observation group was (256.8 ±15.9)mL,which was more than (53.1 ±5.0)mL in the control group(t=77.294,P<0.05).The use rate of vasoactive drug in the observation group was 7.5%,which was lower than 37.5%in the control group(t=8.674, P<0.05).The duration of vasoactive drug use in the observation group was (35.0 ±0.1)min,which was shorter than (49.6 ±1.8)min in the control group(t=13.707,P<0.05).Conclusion Target-oriented fluid therapy for ectopic pregnancy hemorrhagic patients is more effective for fluid resuscitation and less usage of vasoactive drugs .

Objective To investigate the association between global registry of acute coronary events (GRACE) score,N-terminal pro brain natriuretic peptide (NT-proBNP) and heart function in patients with acute coronary syndromes(ACS).Methods One hundred and seventy-four ACS patients and 60 healthy persons were enrolled our study.Of ACS patients,96 cases were with unstable angina pectoris(UP) and 78 cases with acute myocardial infarction (AMI).The level of NT-proBNP,left ventricular ejection fraction (LVEF) and GRACE score were measured.Results The NT-pro BPN of patients with UP or AMI were 87.00(38.25,245.75) ng/L and 686.00 (169.00,1 852.75) ng/L,and GRACE score were (87.51 ± 17.69) and (134.74 ± 23.56),which were higher than that in the control group (31.50 (13.00,54.75) ng/L,(81.10 ± 15.75)).LVEF in patients with AMI (57.72 ± 9.04) %,lower than that of the control group ((64.45 ± 6.31) %),and there were statistically significant difference among the three groups (Z =44.111,P < 0.001 ; F =172.366,18.176,P <0.01).There was positive correlation between serum NT-pro BNP levels and GRACE score(r =0.451,P < 0.001),and negative correlation was seen between serum NT-pro BNP and LVEF (r =-0.294,P < 0.001)Conclusion NT-pro BNP level increases and LVEP decreases in patients with ACS and they are related to GRACE score as well as associated with the degree of risk in patients with ACS.

RESUMO

Objective To explore the degree of cooperation and the effect of hemodynamics in preschool children who received preoperative use of dexmedetomidine.Methods One hundred preschool children who scheduled for lower abdominal operation were randomly divided into two groups,each 50patients.The treatment group was given bolus infusion dexmedetomidine 0.5 μ g/kg (20 ml),the control group was given bolus infusion 0.9% sodium chloride 20 ml.They were completed in 20 min.The cooperation was assessed by induction compliance checklist (ICC) score,and the level of mean arterial pressure (MAP),heart rate(HR),pulse oxygen saturation(SpO2),end-tidal pressure of carbon dioxide(PnCO2) at monitoring ward before bolus infusion (T1),end of bolus infusion (T2),before induction (T3),after induction (T4),incised skin (T5) and end of operation (T6) were compared.Results The scores ofICC in treatment group was significantly lower than that in control group [(2.73 ± 0.12) scores vs.(6.17 ± 0.91) scores] (P < 0.05).In treatment group,48 patients ICC score was 0,2 patients ICC score >5 scores,and in control group,there was 48,2 patients,there was significant difference(P < 0.05).The level of MAP,HR at T1,T2,T4between two groups had no significant difference (P > 0.05).The level of MAP,HR at T3,T5,T6 in treatment group were significantly lower than those in control group(P < 0.05).In control group,the level of MAP,HR at T3,T5,T6 were significantly higher than those at T1 (P < 0.05).In treatment group,the level of MAP,HR at T2-T6 had no significant difference compared with T1 (P > 0.05).The level of SpO2,PnCO2 between two groups and interclass had no significant difference (P >0.05).Conclusion Bolus infusion dexmedetomidine before operation can improve the induction of anesthesia and operation smoothly,and improve the patients early rehabilitation.
