RESUMO

Objective: To analyze the difference of application methods and effects of local flap in small and medium-sized defects of different aesthetic subunits of nose, in order to provide reference for clinical work. Methods: A retrospective analysis was made on 59 patients with external nasal masses and scars who underwent surgical treatment in the Department of Aesthetic Plastic Surgery of the Affiliated Hospital of Qingdao University from July 1, 2021 to January 30, 2022, including 27 females and 32 males, aged 15 to 69 years. Using Likert scale, the repair methods and effects of local flap for nasal soft tissue defects were evaluated and summarized from three aspects of texture, flatness and scar concealment. GraphPad Prism 5.0 software was used for data statistics and analysis. Results: The use of skin flaps to repair small and medium-sized areas of the nose could achieve satisfactory results. For patients with different subunits, in terms of skin flatness and scar concealment degree in the operation area, patients' satisfaction with the dorsal and lateral nasal areas was higher than that of the alar and tip areas, respectively (F=6.40, P=0.001; F=10.57, P<0.001). For patients with different skin flap repair methods, the satisfaction of patients with Z-plasty and Dufourmentel skin flap was higher than that of other skin flap repair methods (F=4.38, P=0.002), and the satisfaction of patients with Dufourmentel skin flap was the highest in the degree of scar concealment (F=2.57, P=0.038). Conclusions: In the small and medium-sized defects of the nose, the use of multiple local flaps can achieve good cosmetic effects and functional recovery. The operator should select the appropriate flap repair method according to the characteristics of different aesthetic subunits of the nose.

RESUMO

A retrospective analysis of a case of death from sudden convulsions caused by oral high-dose diquat was conducted, and the mechanism and treatment of central damage caused by diquat were investigated to lay the foundation for increasing the success rate of treatment of high-dose diquat poisoning. At the same time, at the same time, our clinical treatment experience has also been accumulated.

RESUMO

Objective To evaluate the efficacy and safety of oral magnesium sulfate solution in split doses as bowel preparation in elderly patients undergoing colonoscopy. Methods A total of 368 elderly patients undergoing colonoscopy were enrolled at PLA General Hospital. The patients were randomly divided into magnesium sulfate solution orally in split doses group (group A,n=178) and single dose group (group B,n=190).Parameters including general information, defecation frequency,Boston bowel preparation score (BBPS),detection rate of lesions and adverse reactions. Results The frequency of defecations in group A was (7.6± 1.4),more than that in group B (6.6± 1.5) with statistical significance (P<0.05).The duration of bowel preparation in group A was (128.6±25.3) min, shorter than that of group B (165.4±29.7) min (P<0.05).The BBPS in group A was (8.09±0.67),better than that of group B (7.34±0.58) (P<0.05).The detection rates of intestinal polyps and micropolyps (diameter<0.5 cm) in group A were 73/178 (41.0%) and 51/178 (28.7%) respectively, compared with 58/190 (30.5%) and 37/190 (19.5%) in group B (both P<0.05).In group A,8 patients reported adverse reactions as abdominal distension and discomfort. One patient had ST-T abnormality of electrocardiogram (ECG).No nausea or vomiting occurred, yet 2 cases needed enema for inadequate bowel preparation.Twenty-one cases in group B reported adverse events including 7 with nausea and vomiting. There were 13 patients treated with enema.Abnormal ECG was found in 4 patients in group B.The satisfaction rate of group A was 97.8%,higher than that of group B (91.6%) (P<0.05).Conclusions The effect of bowel preparation of elderly patients with magnesium sulfate solution in split dose has a better tolerance, good cleaning effect and low incidence of adverse reactions. It is an ideal choice for the elderly to prepare colonoscopy.

RESUMO

Objective:To study the chemical constituents in 95%ethanol extract of the whole plant of Camptosorus sibiricus and determine its antioxidant activity. Method:Compounds were isolated by a combination of various chromato-graphic techniques, including column chromatography over silica gel and Sephadex LH-20 and reversed-phase HPLC. Their structures were identified on the basis of physicochemical properties and spectral data reported in the literature. Result:Eleven compounds were identified as trans-p-coumaric acid (1),trans-p-coumaric acid 4-O-β-D-glucoside (2),cis-p-coumaric acid 4-O-β-D-glucoside (3),(E)-ferulic acid-4-O-β-D-glucoside (4),caffeic acid methyl ester (5),ferulic acid methyl ester (6),syringic acid (7),syringic acid-4-O-β-D-glucopyranoside (8),protocatechualdehyde (9),vanillain (10) and syringaldehyde (11),respectively. Conclusion:Compounds 3-11 are isolated from the genus Camptosorus for the first time. In the in vitro SOD-like activity assays, compounds 7,9-11 show an antioxidant activity with half maximal inhibitory concentration(IC50)values of 16.70,11.70,12.23 and 13.52 μmol·L-1,respectively.

RESUMO

Objective:To investigate the mastery and application of medical ethics basic knowledge in geriatric ward medical staffs, and the way which they receive relevant education. Methods:From November 2016 to January 2017,240 questionnaires about medical ethics were distributed among medical staffs in clinical department in the Chinese PLA General Hospital, among them,30 were medical groups and 210 were nursing groups;and a total of 233 valid questionnaires were collected. The nurses were divided into geriatric ward nursing group and non - geriat-ric ward nursing group, according to whether they work in the geriatric wards or not. Results:Only 2. 14% and 7. 72% participants fully and correctly answered the core concepts and basic principles of medical ethics,respec-tively. The rate of informed consent in medical staffs in geriatric group was significantly higher than that of non -geriatric group (92. 66% vs 82. 11% ,P = 0. 02);the patient' s privacy protection in geriatric nursing group was significantly higher than that of non - geriatric nursing group (92. 66% vs 31. 57% ,P < 0. 001). 28. 75% of the respondents received medical ethics continuing education curriculum, and the proportion of correctly receiving ex-pectable death in these medical staffs (71. 40% ) was significantly higher than that of other education pathway groups (36. 17% ). Conclusion:Medical staffs lack more knowledge on medical ethics. Clinical work in geriatric medicine has higher demand for medical ethics knowledge, and continuing medical education can help to improve the cognitive level of medical ethics in medical staffs.
