RESUMO

OBJECTIVE@#To evaluate the efficacy and safety of idarubicin combined with high-dose cytarabine as a post-remission therapy for elderly patients with acute myeloid leukemia (AML).@*METHODS@#From November 2017 to June 2021, 24 AML patients aged ≥60 years who were in complete remission for the first time were enrolled in consolidation chemotherapy with idarubicin (10 mg/m2 intravenously once for day 1) combined with high-dose cytarabine (1.5 g/m2 intravenously over 3 hours every 12 hours for day 1-3), and the efficacy and safety were observed.@*RESULTS@#Among the 24 patients, there were 12 males and 12 females, the median age was 65 (60-78) years old, and the median follow-up time was 23.3 (2-42.7) months. By the end of the follow-up, 15 patients relapsed and 11 patients died. The median disease-free survival (DFS) was 9 months and there were 3 cases of 2-year DFS. The median overall survival (OS) was 16.2 months, and there were 4 cases of 2-year OS. In terms of safety, 6 patients had grade 1-2 non-hematological adverse reactions, 12 patients had grade 3-4 hematological adverse reactions, and a total of 6 patients developed infection after consolidation chemotherapy. Multivariate analysis showed that two induction cycles and high-risk cytogenetic abnormalities were the adverse factors of DFS and OS in elderly patients with AML in this study.@*CONCLUSION@#For AML patients ≥60 years old in first complete remission, idarubicin combined with high-dose cytarabine as post-remission therapy has a better safety, but compared with other regimens does not improve the prognosis of elderly patients, which needs further exploration.

RESUMO

OBJECTIVE@#Melittin, a cell-penetrating peptide, improves the efficiency of many non-viral gene delivery vectors, yet its application in viral vectors has not been well studied. The non-pathogenic recombinant adeno-associated virus (rAAV) vector is an ideal in vivo gene delivery vector. However, its full potential will only be achieved after improvement of its transduction efficiency. To improve the transduction efficiency of rAAV2 vectors, we attempted to develop a melittin-based rAAV2 vector delivery strategy.@*METHODS@#The melittin peptide was inserted into the rAAV2 capsid either in the loop VIII of all viral proteins (VPs) or at the N terminus of VP2. Various rAAV2-gfp or -fluc vectors were subjected to quantitative real-time polymerase chain reaction and Western blot assays to determine their titers and integrity of capsid proteins, respectively. Alternatively, the vectors based on wild-type capsid were pre-incubated with melittin, followed by transduction of cultured cells or tail vein administration of the mixture to C57BL/6 and BALB/c nude mice. In vivo bioluminescence imaging was performed to evaluate the transgene expression.@*RESULTS@#rAAV2 vectors with melittin peptide inserted in the loop VIII of VPs had low transduction efficiency, probably due to dramatically reduced ability to bind to the target cells. Fusing the melittin peptide at the N-terminus of VP2 produced vectors without the VP2 subunit. Interestingly, among the commonly used rAAV vectors, pre-incubation of rAAV2 and rAAV6 vectors with melittin significantly enhanced their transduction efficiency in HEK293 and Huh7 cells in vitro. Melittin also had the ability to increase the rAAV2-mediated transgene expression in mouse liver in vivo. Mechanistically, melittin did not change the vector-receptor interaction. Moreover, cell counting kit-8 assays of cultured cells and serum transaminase levels indicated melittin had little cytotoxicity.@*CONCLUSION@#Pre-incubation with melittin, but not insertion of melittin into the rAAV2 capsid, significantly enhanced rAAV2-mediated transgene expression. Although further in vivo evaluations are required, this research not only expands the pharmacological potential of melittin, but also provides a new strategy to improve gene therapy mediated by rAAV vectors.

RESUMO

There exists a complex relationship between liver and thyroid hormones. Liver plays an important role in the activation, inactivation, transportation, and metabolism of thyroid hormones. At the same time, thyroid hormones also affect hepatocytes activity and liver metabolism, such as lipid and bilirubin metabolism. Importantly, thyroid hormone levels often change abnormally in patients with liver cirrhosis. Therefore, studying the change of thyroid hormone levels in patients with liver cirrhosis has a certain clinical value for assessing the severity, prognosis, diagnosis and treatment. This paper reviews the research progress on the relationship between liver cirrhosis and thyroid hormone.

RESUMO

Early diagnosis of cancer can significantly improve the survival rate of cancer patients, especially in patients with hepatocellular carcinoma (HCC). Machine learning is an effective tool in cancer classification. How to select high⁃classification accuracy feature subsets with low dimension in complex and high⁃dimensional cancer datasets is a difficult problem in cancer classification. In this paper, we propose a novel feature selection method, SC⁃BPSO: a two⁃stage feature selection method implemented by combining the Spearman correlation coefficient, chi⁃square independent test⁃based filter method, and binary particle swarm optimal (BPSO) based wrapper method. It has been applied to the cancer classification of high⁃dimensional data to classify normal samples and HCC samples. The dataset in this paper is obtained from 130 liver tissue microRNA sequence data (64 hepatocellular carcinoma, 66 normal liver tissue) from National Center for Bioinformatics (NCBI) and European Bioinformatics Institute (EBI). First, the liver tissue microRNA sequence data was preprocessed to extract the three types of features of microRNA expression, editing level and post⁃editing expression. Then, the parameters of the SC⁃BPSO algorithm in the liver cancer classification were adjusted to select a subset of key features. Finally, classifiers were used to establish classification models, predict the results, and compare the classification results with the feature subset selected by the information gain filter, the information gain ratio filter and the BPSO wrapper feature selection algorithm using the same classifier. Using the feature subset selected by the SC⁃BPSO algorithm, the classification accuracy is up to 98. 4%. The experimental results showed that compared with the other three feature selection algorithms, the SC⁃ BPSO algorithm can effectively find feature subsets with relatively small size and higher accuracy. This may have important implications for cancer classification with a small number of samples and high⁃ dimension features.

RESUMO

Colorectal cancer (CRC) has a high incidence and mortality rate worldwide. Its lesions are associated to gene mutation, epigenetic changes and activation of related signaling pathways. microRNAs (miRNAs) are a class of non-coding RNAs of 20-24 nt in length that can regulate the expression of target mRNAs and control various cellular mechanisms. As a novel marker for the treatment and prognosis of colorectal cancer, miRNAs are closely related to the occurrence and progression of colorectal cancer. In this review, we summarize the miRNAs dysregulated in CRC tissues, analyze the relationship between specific miRNAs and CRC proliferation, metastasis, apoptosis, and chemotherapy, and present the clinical applications of miRNAs in CRC treatment and prognosis.

RESUMO

Prostate cancer (PCa) is an epithelial malignancy that occurs in the prostate tissue and is the second most common cancer in men after lung cancer, affecting millions of men worldwide. MicroRNA (miRNA) is a non-coding small RNA of length 20-22nt that regulates gene expression post-transcriptionally. miRNAs are involved in the regulation of almost all important biological life processes such as cell cycle progression, cell proliferation, cell apoptosis, and cell migration. Recently, more and more studies have shown that miRNAs are involved in the occurrence of various human tumors including PCa. This review summarizes the current research progress of PCA-related miRNAs, and analyzes the role of malregulated miRNAs in the pathogenesis, diagnosis and treatment of PCa. In addition, the role of miRNA in the diagnosis and treatment of castration-resistant prostate cancer (CRPC) is emphasized.

RESUMO

Breast cancer is the most common malignant tumor in women all over the world. It is a heterogeneous disease, and the exact cause of which is still unclear. However, breast cancer is composed of different subtypes with great differences in clinical characteristics, genetic background, and molecular markers. MicroRNAs (miRNAs) are small non-coding RNAs that regulate genes involved in various stages of key cellular processes after transcription. Persistent dysregulation of miRNAs plays a critical role in the malignant transformation of breast cancer cells, acting as carcinogens or tumor suppressors depending on their cellular environment. As a promising biomarker, dysregulation of miRNA expression seems to open up new opportunities for the diagnosis and prognosis of various cancers. Many studies have revealed the exact functions of miRNAs in different cancer types, however, the results have been inconsistent and further research is needed to determine the underlying mechanisms. In this review, miRNA is associated with the genetic and molecular background of breast cancer, the dual role of miRNA in breast can is described and summarized.

RESUMO

OBJECTIVE@#Plant-derived cytotoxic transgene expression, such as trichosanthin (tcs), regulated by recombinant adeno-associated virus (rAAV) vector is a promising cancer gene therapy. However, the cytotoxic transgene can hamper the vector production in the rAAV producer cell line, human embryonic kidney (HEK293) cells. Here, we explored microRNA-122 (miR122) and its target sequence to limit the expression of the cytotoxic gene in the rAAV producer cells.@*METHODS@#A miR122 target (122T) sequence was incorporated into the 3' untranslated region of the tcs cDNA sequence. The firefly luciferase (fluc) transgene was used as an appropriate control. Cell line HEK293-mir122 was generated by the lentiviral vector-mediated genome integration of the mir122 gene in parental HEK293 cells. The effects of miR122 overexpression on cell growth, transgene expression, and rAAV production were determined.@*RESULTS@#The presence of 122T sequence significantly reduced transgene expression in the miR122-enriched Huh7 cell line (in vitro), fresh human hepatocytes (ex vivo), and mouse liver (in vivo). Also, the normal liver physiology was unaffected by delivery of 122T sequence by rAAV vectors. Compared with the parental cells, the miR122-overexpressing HEK293-mir122 cell line showed similar cell growth rate and expression of transgene without 122T, as well as the ability to produce liver-targeting rAAV vectors. Fascinatingly, the yield of rAAV vectors carrying the tcs-122T gene was increased by 77.7-fold in HEK293-mir122 cells. Moreover, the tcs-122T-containing rAAV vectors significantly reduced the proliferation of hepatocellular carcinoma cells without affecting the normal liver cells.@*CONCLUSION@#HEK293-mir122 cells along with the 122T sequence provide a potential tool to attenuate the cytotoxic transgene expression, such as tcs, during rAAV vector production.

RESUMO

In this study, a composite cell model for evaluation of idiosyncratic drug-induced liver injury (IDILI) was established in vitro from the perspective of immune inflammation. And this model was used to evaluate the risk of IDILI for 2,3,5,4'-tetrahydroxy-cis-stilbene-2-O-β-glucoside (Cis-SG) and 2,3,5,4'-tetrahydroxy-trans-stilbene-2-O-β-glucoside (Trans-SG). To determine the low, medium, and high dosage of Cis-SG and Trans-SG, CellTiter-Glo® 3D Cell Viability Assay was used to detect the effects of Cis-SG and Trans-SG on cell viability of HepG2 cells in three dimensional (3D) culture, and MTT assay was used to detect the effects of Cis-SG and Trans-SG on cell viability of THP-1 derived macrophages. THP-1 derived macrophages were incubated by Cis-SG and Trans-SG directly or supernatants from HepG2 cells incubated with them. Enzyme linked immunosorbent assay (ELISA) was used to detect the levels of interleukin-1β (IL-1β) in the supernatants of the THP-1 derived macrophages. Western blot and reverse transcription-polymerase chain reaction (RT-PCR) were used to determine the expression of apoptosis-associated speck-like protein (ASC), Nod-like receptor protein 3 (NLRP3), cysteinyl aspartate specific proteinase-1 (caspase-1), and IL-1β in THP-1 derived macrophages. The results showed that there was no effect on the secretion of IL-1β in THP-1 derived macrophages incubated by Cis-SG and Trans-SG directly. However, the secretion of IL-1β, the protein and mRNA expression of ASC, NLRP3, caspase-1, and IL-1β significantly increased in THP-1 derived macrophages incubated by supernatants from HepG2 cells incubated with 1, 5, and 25 μmol·L-1 Cis-SG or 25 μmol·L-1 Trans-SG. In summary, the composite cell model for evaluation of IDILI established in vitro has been successfully applied in testing Cis-SG and Trans-SG. This composite cell model is helpful to evaluate and screen drugs with IDILI risk in vitro preliminarily, which provides methods for predicting and solving the idiosyncratic liver toxicity of drugs.

RESUMO

Objective:To observe the clinical effect of helmet therapy in cranial remodeling of infants with deformational plagiocephaly. Methods:A total of 114 infants with plagiocephaly received helmet therapy at the age of 4 to 12 months over the period from 2016 to 2018 and the results were analyzed retrospectively. Seventy-four were male and forty female. The average ages were (7.49±1.91) and (7.54±2.10) months, respectively. A Spectra scanner was used to quantify each head′s radial symmetry index (RSI), cranial vault asymmetry (CVA) and cranial vault asymmetry index (CVAI) before and after 100 days of treatment, and those data were related with age (4-6 months, 7-9 months and 10-12 months), gender and severity of deformity (levels III-V).Results:The average RSI, CVA and CVAI improved significantly for all of the infants studied. The average RSI, CVA and CVAI improved the most for those 4-6 months old. Those with level-IV deformity showed significantly better improvement than those with level III or V deformity. Conclusions:Helmet therapy is an effective way to improve deformities due to plagiocephaly among infants. The best age for treatment is 4-6 months. Patients with less malformation are more easily corrected.

RESUMO

Clinical data of 4 cases of infants with hemophilia complicated with hematorrhachis diagnosed in Guangzhou Women and Children′s Medical Center from January 2017 to December 2018 was retrospectively analyzed. The clinical manifestations of 4 children was atypical, only showing irritability and crying, poor spirit and weak head in the early stage, and torticollis and limb weakness in the late stage may easily contribute to a misdiagnosis. The spinal magnetic resonance imaging is an important diagnosis of hematorrhachis. The most common site of disease was cervicothoracic and thoracolumbar spinal epidural hemorrhage.Of the 4 patients in this article, 1 patient was treated with surgery combined with infusion factor Ⅷ replacement therapy.Three patients were treated with infusion factor Ⅷ replacement therapy alone to obtain clinical rehabilitation. The prognosis of early diagnosis was good, the time from onset to diagnosis in 3 cases was within 1 week, after treatment all had rehabilitation; 1 case within 2 weeks of diagnosis, partial rehabilitation. It is suggested that hemophilia complicated with hematorrhachis in infants is hidden, and it can only be irritable and crying in the early stage, and neurological symptoms appear in the late stage.Improving diagnostic awareness, early diagnosis and early infusion factor replacement therapy are critical for prognosis.

RESUMO

Objective To investigate the effects from head angle and locking times of locking screws on mechanical properties of the screw-plate system, so as to provide a theoretical basis for doctors to select appropriate locking screws and master the locking times in clinical surgical procedure. Methods Locking screws with different head angles were selected to match with locking compression plates in the case of different locking times, and cantilever bending method was used to compare the differences of mechanical properties in each group. Results There were significant differences in failure load and bending stiffness between locking compression plate and locking screw with different head angles (P0-05). The failure modes of locking screw with different head angles and different locking times were different, and the failure mode of locking screw had a positive correlation with its head angle and locking times. Conclusions The differences in mechanical properties between locking screw with different head angles and locking compression plate cannot be neglected. It is suggested that doctors should choose locking screw and locking compression plate with the best locking degree in clinical procedure, and choose the right torque wrench to lock in multiple times if necessary, thus to prevent screw loosening from affecting recovery of the patients.

RESUMO

Aim To study and identify the antibacterial activity of three strains of Streptomyces (K57M, K51M, K131M) collected from the roots of mangrove rhizosphere in Maowei Sea, Guangxi. Methods Acti-nobacteria were isolated on four media and purified by ISP2 medium. The 16S rRNA gene sequences of three actinobacteria strains were obtained by PCR amplification and sequencing. Sequences of 16S rRNA gene obtained by PCR amplification were submitted to gene database for BLAST search. Phylogenetic tree was constructed by adjacency method, and homology analysis was carried out to study the diversity and novelty of actinobacteria. Three actinobacteria strains underwent primary screening using Oxford cup method, and the antimicrobial activity of the sample of El-3, AI-3 and Ml-3 received secondary screening using disk diffusion method. Results Sequence alignment analysis of 16S rRNA gene revealed that strain K57M might be a potential new actinobacteria. The results of screening for antimicrobial activity showed that three strains of Streptomyces, K57M, K51M and K131M, had good antimicrobial activities against different drug-sensitive strains. Conclusions Streptomyces from rhizosphere in the roots of mangrove forests in Guangxi can produce strong antibacterial activities, which provides great possibilities to explore natural active antibiotics.
