RESUMO

As the guiding theory for the diagnosis and treatment of encephalopathy in traditional Chinese medicine (TCM), The marrow sea theory has important theoretical connotation and clinical value. This paper summarized the clinical research literature on the differentiation and treatment of common encephalopathy based on the marrow sea theory published in recent years, analyzed the treatment method and effects from eight aspects in terms of stroke, dizziness, insomnia, headache, constraint syndrome, dementia, tremor syndrome, and atrophy syndrome, and discussed the possible mechanism based on the relevant basic research. It is believed that marrow sea depletion is the common pathogenesis of encephalopathy in TCM. Guided by the method of supplementing essence and boosting marrow, the corresponding formulas and medicinals are recommended in accordance with differentiated syndromes, which can effectively improve the symptoms of the disease, delay the progression, increase the daily life ability of the patients, and improve the quality of life. Based on the marrow sea theory, the method of supplementing essence and boosting marrow, rectifying healthy qi and dispelling pathogen can be used to highlight the advantages of TCM and provide ideas for the diagnosis and treatment of encephalopathy in TCM.

Objective:To explore the effects of kangaroo nursing after cesarean section on the paternal role adaptation and subjective well-being of first-time fathers.Methods:In this randomized controlled trial, a total of 160 first-time fathers of newborns delivered by cesarean section from May 2021 to May 2022 in the First Affiliated Hospital of Wenzhou Medical University were continuously selected as research objects and randomly divided into experimental group (80 cases) and control group (80 cases) by random number table method. In the control group, the nursing staff carried out routine obstetric care after the birth of the newborns, the newborns were swaddled to keep warm and transferred to the ward, the fathers took care of them by the side; and the fathers were given postnatal routine education and guidance once a day, including newborn bathing, touching and umbilical buttock care, maternal breast care, breastfeeding, diet, medication and discharge precautions etc. The experimental group were given kangaroo nursing on care basis of control group: the father took a comfortable position that half leaned on the recliner with his shirt unbuttoned, the naked newborn was placed on the father′s chest at an angle of approximately 60 degree on the head up and feet down for maximum skin to skin contact with the help of researchers. The newborn′s back was covered with a blanket and wrapped in the father′s clothes, soft music was playing throughout the whole process, meanwhile, researchers encouraged interactions through verbal communications and touching between the father and the newborn. After newborns returned to the ward, they were given kangaroo nursing immediately by the fathers for the first time, and then twice a day in the same way, lasting for 30-40 min every time, and a total of 6 times of kangaroo nursing were completed during in first 3 days after birth. The average hospitalization period of both groups was 5 days. The father-role adjustment scale and subjective well-being scale were asseessed in the fathers, the independent sample t test was used to compare differences of father-role adjustment level and subjective well-being at the time of discharge between the two groups, the Pearson correlation analysis was used to evaluate the correlation between the two variables. Results:The total score of paternal role adaptation, and the three dimensions scores including role identity, parent-child attachment establishment and nursing behavior implementation in the experimental group were all significantly higher than those in the control group (83.91±7.90 vs 78.79±9.44, 28.94±2.64 vs 27.85±3.36, 28.39±3.15 vs 26.46±3.52, 26.59±4.26 vs 24.48±3.81) ( t=3.725, 2.276, 3.647, 3.309, all P<0.05). The total score of subjective well-being, and the three dimensions scores including health worry, energy, melancholy or pleasant mood in the experimental were all significantly higher than those in the control group (88.34±6.38 vs 83.43±7.26, 7.70±1.77 vs 6.95±1.46, 20.03±3.15 vs 18.76±2.94, 19.41±2.01 vs 17.99±2.68) ( t=4.546, 2.928, 2.625, 3.805, all P<0.05), and were all significantly higher than those before the intervention (74.83±5.84, 6.11±2.22, 16.98±2.68, 15.18±2.13) ( t=13.069, 5.207, 6.261, 13.332, all P<0.05). There was a positive correlation between fathers′ subjective well-being and paternal role adaptation ( r=0.614, P<0.001). Conclusions:The father-centered kangaroo nursing after cesarean section can improve postpartum paternal role adaptation and subjective well-being of first-time fathers. There is a positive correlation between the subjective well-being and the paternal role adaptation level.

Traditional Chinese Medicine (TCM) has its own unique features in the treatment of post-stroke depression (PSD). The kidney is the congenital foundation and is closely related to the brain. Kidney deficiency runs through the entire course of stroke. Liver regulates the normal operation of qi in the human body, which is closely related to depression syndrome. Kidney deficiency and liver depression affect each other. The treatment of PSD with the Bushen Shugan (tonifying the kidney and soothing the liver) method has achieved good efficacy in clinic. The method of tonifying kidney and soothing liver can not only reflect the holistic view of TCM and the association of viscera, but also coordinate the relationship between body and spirit. In the future, the development direction of PSD in TCM research should be to further strengthen the concept of co-regulation of body and spirit and integration of brain and viscera.

ObjectiveTo discuss the thought of treatment of orifices in the Chinese herbal classics in the past dynasties based on the correspondence between drugs and symptoms to guide the clinical treatment based on syndrome differentiation. MethodAll the literature data of Chinese herbal classics were retrieved from the database of the Chinese Medical Dictionary, involving 76 works of Chinese herbal classics and covering representative works from the Han dynasty to the Ming and Qing dynasties. The information on Chines herbal drugs for the treatment of orifices was collected and sorted out. According to Chinese Materia Medica (11th Edition) and Pharmacopoeia of the People's Republic of China (2020 Edition), the nature, flavor, and meridian tropism of the selected Chinese herbal drugs were statistically analyzed. The pathogenesis elements in the treatment of orifices were classified and counted, and the contents of syndrome differentiation and treatment in various Chinese herbal classics were extracted. ResultIn 76 Chinese herbal classics in the past dynasties, 93 Chinese herbal drugs for the treatment of orifices were selected. The nature of drugs was mainly warm, followed by cold and mild. The flavor was mainly pungent, followed by bitter and sweet. In terms of meridian tropism, drugs mainly acted on the lung meridian, followed by stomach, heart, liver, spleen, and kidney meridians. The pathogenesis elements of orifices could be divided into six categories, i.e., wind invasion, turbid obstruction and Qi stagnation, water and dampness stagnation, blood stasis and collaterals blockage, heat and toxin damage, deficiency of vital Qi and cold coagulation. ConclusionOrifices are mainly treated with drugs effective in dispelling wind and pathogenic factors, resolving turbidity and removing stagnation, inducing diuresis and eliminating dampness, promoting blood circulation and dredging collaterals, clearing heat and purging fire, tonifying deficiency and dispelling cold, which are used in combination. Eliminating pathogenic factors and dredging, tonifying deficiency and purging excess are the main characteristics of treatment of orifices based on syndrome differentiation, which is in line with the physiological dysfunction state of orifices in losing the function, evil Qi blockage and healthy Qi deficiency.

Meige syndrome is one of the rare Diseases of neurology. It is a part of the group of segmental cranial dystonia, which affects more than two cranial muscle groups. Especially, blepharospasm is associated with another cranial dystonia. Currently, the etiology and pathogenesis of this disorder are not well-understood. Based on the theory of collaterals and the relationship between collateral disease and five zang organs,we try to classify the disease into the category of collateral disease, and hold that the mechasnism of disease is not only linked to brain, but also the five zang organs. The disorder of qi and blood leads to the "deficiency of qi and blood", "phlegm-damp" and "stagnant blood" in collateral. These pathological changes cause the deficiency of both qi and blood in brain marrow and tendon.We think that "unblocking and regulating the collterals"is the general program. Adjusting the five zang organs to heel the brain is the key point of the treatment. What's more, we create Tongmai Heluo Decoction to regulate qi and blood of the collateral. This decoction is composed of Huangqi Jianzhong Decoction with some herbal medicine for tonifying qi and activating blood. By adjusting the middle energizer, the qi and the qi movement of five zang organs are well regulated. We use this decoction effectively on refractory Meige syndrome.
