RESUMO

Gouty arthritis (GA) is a metabolic disease caused by disorders of purine metabolism and/or abnormal excretion of uric acid in the body. Its pathogenesis is mostly related to dietary structure as well as excessive intake of protein, sugar and fat, and the clinical manifestations are joint redness, swelling, heat and pain, which seriously affect the daily life of patients. Therefore, it is urgent to carry out research on anti-GA drugs. Western drugs for the treatment of GA, such as colchicine, can relieve pain in the short term, but with obvious side effects in long-term treatment. Traditional Chinese medicine has definite efficacy and high safety in the treatment of GA and is more acceptable to patients than western medicine. Modern medical research has concluded that inflammatory factors, oxidative stress, apoptosis and intestinal dysbacteriosis are closely related to the pathogenesis of GA. In-depth research has found that single traditional Chinese medicine and its compounds can regulate Toll-like receptors/myeloid differentiation factor 88 (TLRs/MyD88) signaling pathway, NLR family pyrin domain containing 3 (NLRP3) inflammasome, nuclear transcription factor-κB (NF-κB) and other inflammatory signaling pathways, and further intervene in the downstream cytokines such as interleukin-1β (IL-1β), interleukin-2 (IL-2), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), peroxisome proliferator-activated receptor γ (PPARγ), nuclear factor of kappa light polypeptide gene enhancer in B-cells inhibitor-α(IκB-α) and aspartate-specific cysteine protease 1 (Caspase-1) to reduce inflammatory factors and increase anti-inflammatory factors, thereby exerting the anti-GA role. Therefore, this paper summarized and elaborated the experiments of inflammatory response mediated by traditional Chinese medicines and their compounds via regulating inflammatory signaling pathways in recent years, which provides new ideas and theoretical basis for finding more related anti-inflammatory traditional Chinese medicines for the treatment of GA.

RESUMO

The pneumonia caused by the novel coronavirus (2019-nCoV), which began in December 2019, has become the most serious public health problem, threatening people's health and life. This threat is posing a severe challenge on the diagnosis and treatment of 2019-nCoV infection, the prevention and control of hospital cross infection of medical staff. It is suggested that in addition to strengthening the organization and leadership of the abovementioned work, establishing and improving the prevention and control mechanism deserve greater attention. Furthermore, special attention should be given to the safety of the medical staff, strengthening their infection monitoring and outbreak management. Medical staff in different work areas and positions should be placed under careful protection, cleaning and disinfection measures. The protection during specimen collection, transportation and medical waste management should also be prioritized. This paper also put forward management suggestions for the outpatient department, isolation ward and other key departments. These measures are proposed to provide a guidance for the prevention and control of 2019-nCoV nosocomial infection in the pediatric outpatient and ward.

The epidemic of COVID-19 has become the topmost public health threat worldwide. The authors suggested that in addition to strengthening the organization and leadership of the abovementioned work, greater attention be paid to establishing and improving the prevention and control mechanism.Furthermore, special efforts should be given to the safety of the medical workers, by strengthening their infection monitoring and outbreak management. Medical workers in different work areas and positions should be placed under careful protection with due cleaning and disinfection measures. The protection during specimen collection, transportation and medical waste management should also be prioritized. Such special issues in the management of pediatric patients, as allocation of nurses, the screening and management of caregivers, the problem of breastfeeding, and the disinfection of children′s toys and diapers were discussed. This paper also put forward management suggestions for the outpatient department, isolation ward and other key departments, which aim to guide the prevention and control of COVID-19 nosocomial infection in the pediatric outpatient and ward.

RESUMO

Objective To explore non-phacoemulsification small incision extracapsular cataract extraction,the capsular bag IOL implantation and trabeculectomy(hereinafter referred to as plainer combined surgery)treatment of glaucoma and cataract surgical method and clinical efficacy.Methods Operated to 45 patients with 47 white combined glaucoma and cataract combined surgery,in-traoperative visual acuity,intraocular pressure,bleb,and postoperative complications,followed postoperative up for 3 months to 24 months.Results The preoperative and postoperative visual acuity and intraocular pressure difference was statistically significant (P 0.60°12 eyes,accounting for 25.53%;IOP control in the normal range by 39,accounting for 83.00%;8 postoperative IOP at 26 mm Hg or so.40 eyes(85.00%)to form a flat diffuse functional filtration.Where type blebⅠ 18,type blebⅡ 32.Complications:transient corneal edema 12,early shallow anterior chamber 4,hyphema 3,holding an intraocu-lar lens.No serious intraoperative and postoperative complications.Conclusion The surgical treatment of glaucoma and cataracts can control intraocular pressure effectively and restore useful vision with fewer complications,simple,safe,less investment,low cost,suitable by using in primary hospital.
