RESUMO

Objective To assess the effectiveness of traditional Chinese Medicine (TCM ) syndrome differentiation treatment for diabetic nephropathy in elderly patients. Methods A total of 96 elderly patients with diabetic nephropathy admitted to our hospital from January 2015 to December 2017 were recruited and divided into a regular care group (n= 48)and a TCM group(n= 48).Both groups received conventional treatment ,with TCM syndrome differentiation treatment added to the TCM group. The effectiveness of the treatment modalities was compared. Results In the regular care group ,significant effectiveness was observed in 12 patients ,effectiveness in 24 patients ,and ineffectiveness in 12 patients ,with a total effectiveness rate of 75% .In the TCM group ,significant effectiveness was seen in 18 patients ,effectiveness in 27 ,and ineffectiveness 3 ,with a total effectiveness rate of 93%. The total effectiveness rate of the TCM group was higher than that of the regular care group(χ2 =6.400 ,P=0.011).After treatment ,fasting blood glucose ,2 h postprandial blood glucose , urea nitrogen ,creatinine ,and 24 h urine protein in the TCM group were significantly lower than in the regular care group and than pre-treatment levels(P<0.05). Conclusions TCM differentiation can not only significantly improve the treatment effectiveness in elderly patients with diabetic nephropathy ,but also relieve the clinical symptoms.

RESUMO

Objective To discuss the accuracy of three-dimensional (3D) sonography in diagnosing arcuate uterus and septate uterus and propose a quantitative indicator in distinguishing arcuate uterus and septate uterus. Methods Ninety-six patients with suspected congenital uterine malformation underwent 3D sonography to assess uterine anomalies at Obstetrics and Gynecology Hospital of Fudan University from August 2011 to March 2013.Uterine coronal section was reconstructed and measured in onmi view imaging mode of 3D sonography. Then qualitative diagnosis and categorization was done. The results were compared with diagnosis of hysteroscopy, hysteroscopy combined with laparoscope to determine the sensitivity, specificity, positive predictive values, negative predictive values and total accuracy of 3D sonography in diagnosing arcuate uterus, partial and complete septate uterus respectively. The variable was created in coronal section of uterus constructed through 3D imaging mode to differentiate arcuate uterus from septate uterus. Results There were 33 cases (34.4%,33/96) of arcuate uterus,54 cases (56.2%,54/96) of partial septate uterus and 9 cases (9.4%,9/96) of complete uterus in 96 cases diagnosed by 3D sonography. The accuracy of 3D sonography in diagnosing arcuate uterus and septate uterus was 86.5%.We concluded that 3D sonography had 100% sensitivity,82.9% specificity,60.6% positive predictive value and 100%negative predictive value in the diagnosis of arcuate uterus. The sensitivity of 3D sonography for diagnosis of partial septate uterus was 80.6%,the specificity was 100%,the positive predictive value was 100%,and the negative predictive value was 69.1%.The sensitivity, specificity, positive and negative predictive values of 3D sonography for detecting complete septate uterus were all 100%.The ratio of depth of uterine internal indentation and depth from uterine fundus to the top of intemal indentation was more than 50% for septate uterus, while less than 50% for most of the arcuate uterus. Conclusions The diagnostic efficiency of 3D sonography in specific uterine malformation is different. The new quantitative indicator maybe helpful in improving the accuracy of 3D sonography in diagnosing arcuate uterus and septate uterus.

RESUMO

Objective To investigate the changes of plasma insulin and C peptide level in women with gestational diabetes mellitus(GDM), gestational impaired glucose tolerance(GIGT) and normal pregnant and to find out the patterns of glucose metabolism in pregnancy and the function of islet ? cell in patients with gestational impaired glucose metabolism Methods Thrity one normal pregnant women (group Ⅰ), 36 GIGT (group Ⅱ)and 31 GDM (group Ⅲ) were selected. Insulin and C peptide levels were measured by radioimmunoassay. Results Insulin and C peptide levels of groups Ⅰand Ⅲ increased with the gestational age and decreased at postpartum. At 31 weeks of gestation, insulin and C peptide levels of group Ⅱ and group Ⅲ were significantly higher than that of group I [insulin: (13.95?3.15) mIU/L and (15.10?2.96)mIU/L vs (11.40?3.27) mIU/L; C peptide: (0.60 ?0.21) pmol/ml and (0.80?0.31) pmol/ml vs (0.44?0.26) pmol/ml, P 0.05). The peak value of insulin and C peptide was found at the first hour postprandially in group I, while at the second hour in group Ⅱ and Ⅲ. Conclusions Insulin resistance during pregnancy becomes stronger gradually with pregnant progress. Women with abnormal glucose metabolism during pregnancy would have more insulin resistance which leads to the damage of insulin secretion. These women should have long term followed up.
