RESUMO

Objective@#To analyze 8 cases of paraquat lung transplantation in the world, and to explore the timing of lung transplantation and the factors affecting prognosis.@*Methods@#An analysis of the clinical data of a paraquat poisoning lung transplant patient completed by The 12th People's Hospital of Guangzhou Medical University and The First People's Hospital affiliated to Guangzhou Medical University in August 2017 and literature review.@*Results@#A 26 years old female patient was admitted to the hospital ingested 20% paraquat solution 20ml. On the 58th day of poisoning, she underwent double lung transplantation under general anesthesia. The operation was successful. Excised lungs show extensive lung fibrosis in both lungs, which was consistent with paraquat poisoning. Used tacrolimus and corticosteroids and mycophenolate antirejection, the patient discharged 46 days after surgery. 7 articles were retrieved through the search tool, and a total of 8 articles included this case were reported. Five patients who underwent lung transplantation within 1 month after poisoning all died, And 3 patients conducted lung transplantation for more than 1 month after poisoning survived; Pathogenic bacteria were isolated from the sputum in 3 of the 8 cases, all containing Pseudomonas, 2 of which died, and our case survived.@*Conclusion@#Appropriate transplantation time window is very important for the prognosis of paraquat poisoning after lung transplantation. Active treatment of the sputum pathogens, improving the donor receptor matching, and exhausting the various means to remove the paraquat from the storage pool which may improve success rate of lung transplantation.

Retrospective analysis of 18 cases of occupational acute 1, 2-dichloroethane (1, 2-DCE) poisoning. The results showed that all patients have the main manifestations such as dizziness, headache, unresponsiveness and other symptoms of nervous system damage; Brain CT showed varying degrees of diffuse white matter lesions. Brain MRI showed extensive involvement of white matter in both cerebral hemispheres. Affected white matter area presented low intensity on T1WI, hyperintensity on T2WI and T2-Flair; Lumbar puncture examination of cerebrospinal fluid (CSF) pressure (262.5±48.39) mm H2O; After treatment, the required time for CSF pressure to restore was (161.56±75.27) days (50-280) days. Summary, Occupational acute 1, 2-DCE poisoning caused by toxic encephalopathy can be manifested as persistent abnormalities in CSF pressure, and the CSF pressure drops slowly during treatment; Early head CT and lumbar puncture examination will be helpful for early detection of intracranial pressure in toxic encephalopathy caused by acute 1, 2-DCE poisoning. Dynamic monitoring of CSF provides guidance for acute 1, 2-DCE poisoning with a long time of treatment and various types of dehydrating agents.

Objective@#This study aims at the review result of Guangzhou occupational health examination from 2012-2016 to analyze the distribution of crowd and to discover the occupational health hazard factors and key Protection object by finding the situation of the goal of occupational hazard factor diseases through review examination. @*Methods@#By using retrospective study method, to choose those people who have accepted occupational health review examination handled by the review examination group of Guangzhou Occupational Prevention and Treatment Hospital from 2012 to 2016. And comprehensively analyze the review result of occupational health examination in 5 years. @*Results@#There are 8618 cases of occupational health review examination handled by Guangzhou Occupational Prevent and Treatment Hospital and with complete data. From the Personnel structure, it refers that they are Predominantly male and their ages are mainly distributed under 40-year-old. Most of them work under the occupational disease inductive factors less than 5 years and most of the factor is contacting with noise. In the recent 5 years, we found out 1906 cases of occupational contraindications through total review, which takes 22.12% of total number of review People, including noise occupational contraindication in 61.59%. And we found 435 cases of suspected occupational disease which takes 5.05% of total number of review People, including suspected occupational noise deafness which takes the property of 60.23% and appears the most common disease. Suspected occupational diseases and occupational taboos are mainly male patients, and the age is mainly in the 30-49 age group, the working age is mainly less than 5 years. @*Conclusion@#Occupational health examination is an important Part in occupational health, occupational health examination review is the most Prior Part in the occupational health examination, which Plays a key role to discover the target disease of the occupational hazard factors in time, Prevent the development of occupational diseases and protect laborer occupational health.

RESUMO

Objective To study the effects of dexmedetomidine on the swallowing reflex. Methods Sixty adult volunteers (ASAⅠ-Ⅱ) were randomly divided into two groups, dexmedetomidine sedation group and propofol sedation group respectively. The BIS value was maintained between 60-65 in both groups. Swallowing reflex was induced by water injection in the pharynx at different depths of sedation, and the swallowing movements were observed. Meanwhile, the RR, SpO2 and OAA/S scores were recorded. Results The swallowing reflex existed in dexmedetomidine sedation group when BIS values was maintained at 60, which had significant difference compared with that in propofol sedation group. The RR, SpO2, OAA/S score in dexmedetomidine sedation group also had statistical difference compared with that in propofol sedation group during swallowing reflex test. Conclusions Slow infusion of dexmedetomidine to maintain the BIS value above 60 will not affect swallowing reflex, and can obtain satisfactory hemodynamic stability. It′clinical safety is high.

<p><b>OBJECTIVE</b>To investigate the changes in serum amylase (AMY) and lipase (LPS) and their clinical significance in patients with acute paraquat poisoning (APP).</p><p><b>METHODS</b>The clinical data of 62 APP. patients who were admitted to Guangzhou 12th People's Hospital in 2014 were retrospectively analyzed. With clinical death being regarded as the end point of observation, the 62 patients were divided into survival group (n=32) and death group (n=30). The fasting venous blood of the patients on the 1st, 3rd, and 7th day after admission were collected, and the AMY and LPS levels were determined; the obtained data were compared between the two groups and analyzed statistically.</p><p><b>RESULTS</b>The AMY and LPS levels of the survival group on the 1st, 3rd, and 7th day after admission were significantly lower than those of the death group (P<0.05); the AMY and LPS levels of the death group on the 3rd day were significantly higher than those on the 1st day after admission (P<0.05); the survival group demonstrated no significant changes in AMY and LPS levels (P>0.05).</p><p><b>CONCLUSION</b>Changes in AMY and LPS levels in APP patients are reliable indicators for the determination of poisoning severity and prognosis.</p>

Objective To observe and compare the control effect of monitored anesthesia care on stress reaction of operation under local anesthesia.Methods Fourty-five patients who undered ophthalmology and otolaryngology operation and ASA Ⅰ or Ⅱ class,aged 20-55 years were enrolled.They were randomly divided into 3 groups:monitored anesthesia care(M group),general anesthesia (G group),and local anesthesia (L group).All of three groups were performed local nerve block anesthesia.Mean arterial blood pressure (MAP)、heart rate (HR) were recorded at enter room (T1),the strongest operation stimulation (T2),the end of operation (T3).At the same time,the levels of blood glucose,plasma cortisol (Cor) were assayed.The scores of state of anxiety (S-AI) before operation and after operation in two groups were compared.Results The levels of MAP and Cor at T2,T3 and HR at T2 in G group and M group were significantly lower than those at T1:(68.1 ± 8.2),(78.8 ± 12.8) mmHg (1 mmHg =0.133 kPa) vs.(95.7 ± 11.3) mmHg;(80.8 ± 11.7),(86.2 ± 9.0)mmHg vs.(94.7 ± 11.5) mmHg;(207.0 ±71.8),(135.2 ± 60.9) nmol/L vs.(336.7 ± 121.4) nmol/L;(220.8 ± 113.2),(190.4 ± 149.0) nmol/L vs.(347.8 ± 154.6) nmol/L;(68.1 ± 10.6) beats/min vs.(79.9 ± 14.2)beats/min;(70.3 ± 10.1) beats/min vs.(80.6 ± 12.2) beats/min,there were significant differences (P <0.05).The level of blood glucose at T3 was significantly lower than that at T1 in G group [(4.9 ± 0.7) mmol/L vs.(5.5 ± 0.6) mmol/L],there was significant difference (P < 0.05).The levels of MAP,Cor at T2 in G group and M group,at T3 in G group were significantly lower than those at the same point in L group:[(68.1 ± 8.2),(80.8 ± 11.7) mmHg vs.(93.4 ± 12.5) mmHg,(207.0 ± 71.8),(220.8± 113.2) nmol/L vs.(367.1 ± 157.3)nmol/L,(78.8 ± 12.8) mmHg vs.(92.6 ± 15.3) mmHg,(135.2 ± 60.7) nmol/L vs.(311.9 ± 165.6) nmol/L],there were significant differences (P < 0.05).The scores of S-AI at postoperative in G group and L group were significantly lower than that at preoperative:(31.5 ± 6.6) scores vs.(44.3 ± 15.0) scores,(35.2 ± 11.5) scores vs.(49.3 ± 14.2) scores,there were significant differences (P < 0.05).Conclusion Monitored anesthesia care can alleviate stress reaction in operation with local anesthesia,while its effect is similar with general nesthesia.

BACKGROUND:It has been an urgent problem of how to promote cartilage repair of the knee and shorten the total course through a tissue engineering approach. Fortunately, microfracture plus stem cel transplantation may open up a new path for this issue. OBJECTIVE:To investigate the clinic feasibility of arthroscopic microfracture technique plus stem cel transplantation for repair of articular cartilage injury of the knee. METHODS:From October 2010 to March 2012, a total of 16 patients with articular cartilage injury of the knee were enrol ed, including 12 males and 4 females, with the average age of 38.6 years (16-52). Al cases of cartilage injury were confirmed by arthroscopy. Autologous bone marrow was extracted from patients at 2 weeks before treatment to isolate, culture and amplify bone marrow mesenchymal stem cel s in vitro. The cel culture solution of 3-5 mL (about 107 cel s) was harvested. The articular cavity was clean by arthroscopy and microfracture technique was performed at the area of cartilage injury that was then covered with hemostatic gauze through a minimal y invasive incision and the prepared bone marrow mesenchymal stem cel s were injected. The knee was bandaged with the elastic bandage after aspirating the joint cavity effusion by vacuum suction. Functional exercises were performed early by CPM.RESULTS AND CONCLUSION:After fol ow-up of 4-18 months, there were 13 cases of excel ent, 2 cases of valid and 1 case of ineffective. According to Lysholm knee scores, the average scores were improved from 42 points (33-67 points) to 89 points (75-99 points) at 4 weeks after treatment. The function was satisfied and al patients were fol owed up without recurrence or worse. Under the arthroscopy, the combination of microfracture technique and autologous bone marrow mesenchymal stem cel transplantation is proved to be effective for articular cartilage injury of the knee and it can notably improve the clinic symptoms and recover the function of the knee.

<p><b>OBJECTIVE</b>To observe the clinical characteristics and regular patterns of subacute 1, 2-dichloroethane poisoning patients for providing evidences to it's diagnosis, treatment and prognosis.</p><p><b>METHODS</b>51 cases of subacute 1, 2-dichloroethane poisoning analyzed. They were divided into 3 groups according to their main clinical manifestation: group A mainly with intracranial hypertension (n = 25), group B with limbs tremor (n = 18), group C with mental and behavior disorder (n = 8). All cases' clinical symptoms, cranial computer tomography, cerebrospinal pressure (Group A) were observed, the durations of the onset, deterioration, improvement, recovery and whole course of the disease were compared between groups and in each group.</p><p><b>RESULTS</b>In all of 51 cases, only the differences between the deterioration duration of cranial CT and symptom was significantly (t = 2.555, P<0.05), which indicate the deterioration of symptom was earlier than radiological change. The symptom deterioration of group C was the fastest than group A and group B (P<0.00). As to the change of symptom duration, group B's improvement, recovery and whole course was the longest comparing with group A and group C (P<0.05). As to the change of cranial CT duration, group B's recovery duration was the shortest and group A's recovery duration was the longest (P<0.01); group B's whole course was also the shortest and group A's whole course was the longest (P<0.05). The clinical course of symptoms, cranial computer tomography, cerebrospinal pressure (Group A) was compared in each group, in group A, the duration of improvement and whole course of the cranial CT and cerebrospinal pressure change was longer than that of the symptom change (P<0.01), this indicated that group A has longer asymptomatic intracranial hypertension and their cranial radiography recover slowly. In group B, their symptoms (3.94 ± 4.31 days) deteriorated is earlier than cranial CT changes (P<0.05), the recovery (92.39 ± 55.04 days) and whole course of symptom was longer than cranial CT change (all P<0.01). In group C, symptom deterioration was earlier than CT deterioration (P< 0.05).</p><p><b>CONCLUSION</b>The clinical characteristic of subacute 1, 2- dichloroethane poisoning is central nervous system damage, it differs according to the different stage of course, the regions and severity of pathology lesions.</p>

RESUMO

Conducting polymers have stable long-chain structure and good electrical conductivity. They have been used in various types of biosensors because of their excellent characteristics of the immobilization and electrical signal transmission. In recent years, researchers mainly study on improving its micro-nano structures and its signal conductivity to enhance its effect on the enzyme immobilization and signal conductive properties. This paper reviews firstly the application of conducting polymer on enzyme-immobilized glucose biosensor and the new technologies and methods in this field. This paper also points out the future application of conducting polymers in enzyme immobilization and biosensor preparation areas.

ObjectiveTo determine the depth of sedation induced by target-controlled infusion (TCI) of propofol required to inhibit the swallowing reflex.MethodsThirty-five ASA Ⅰ or Ⅱ healthy volunteers,aged 20-60 yr,with body mass index < 30 kg/m2,were enrolled in the study.Sedation was performed with TCI of propofol.The initial target plasma concentration (Cp) of propofol was set at 2.4 μg/ml and the Cp increased or decreased by 0.2 μg/ml every 5 min until the BIS value reached the predetermined level.The initial Cp of propofol was increased or decreased by 0.3 μg/ml in the next subject according to the BIS value in the previous one.The up-and-down sequential experiment was pedormed.The BIS value was set at 65 in the first subject and the ratio of the BIS value between the two consecutive subjects was 1.1.The BIS value was increased or decreased depending on the occurrence of the swallowing reflex in the previous subject.The half-effective BIS value (BIS50) and 95%confidence interval (CI) of propofol inhibiting the swallowing reflex were calculated by up-and-down sequential method.ResultsThe BIS50 of propofol required to inhibit the swallowing reflex was 64.4 (95% CI 59.3-69.8).ConclusionThe BIS50 and 95% CI of propofol administered by TCI and required to inhibit the swallowing reflex is 64,4 (59.3-69.8).

RESUMO

Quantum dots, as a new kind of biological fluorescence material, have properties superior to the traditional organic dyes. In the recent decade, researches about the application of quantum dots in biomedicine have made great progress. Tumor markers have vital importance in the diagnosis and treatment of carcinoma; however, they are not widely used for lack of specificity and sensitivity. Researches about applications of quantum dots in the detection of tumor markers are centralized in three aspects, namely the detection of serum/plasma tumor markers, the detection of tissue and cell tumor markers (such as molecular imaging of cell and tissue), and the detection of tumor in vivo animals. A combination of quantum dots and flow cytometry technique for developing a new technique to detect multiple kinds of tumor markers at the same time with greater sensitivity, specificity, rapidity and convenience may have the potential for clinical use of quantum dots.

Objective This paper analyzes the spectrum of ultrasonic echo and its change with depth when using dual unipolar pulse excitation.Methods According to the theory of matched filter,a group of aynmic filter was designed to ensure the near-field image resolution while maintaining the sensitivity of far-field image.Results This filter was accomplished in FPGA chip.Conclusion Higher image quality Can obtained applying this filter in the au-digital ultrasonic imaging system compared with those using constant filter.

As a novel functional material with great application values, magnetic nanobead has been in fast development in recent years. Magnetic nanobead is provided with characteristics of beth polymeric and mag-netic particles, so it can be detected easily fi'om the medium and can be easily located since they can move di-rectionally when motivated by external electric field. The magnetic nanobead is widely used in biomedical engi-neering research due to its unique physical and chemical properties which simplify the complex experiments op-erations and shorten the testing period.

Stochastic resonance (SR) is given to a phenomenon that is manifest in nonlinear systems whereby generally feeble input information can be amplified and optimized by the assistance of noise. First, the basic concepts and the characteristic quantities of SR are introduced in this paper. Second, SR in biological system and its applications are reviewed in detail. At last, a summary is presented and the future researches on SR are prospected.

Radio frequency identification sensor network, which is a product of integrating radio frequency identification (RFID) with wireless sensor network (WSN), is introduced in this paper. The principle of radio frequency identification sensor is analyzed, and the importance of the antenna is emphasized. Then three kinds of common antennae, namely coil antenna, dipole antenna and microstrip antenna, are discussed. Subsequently, according to requirement, we have designed a microstrip antenna in a wireless temperature-monitoring and controlling system. The measurement of factual effect showed the requirement was fulfilled.

Radio frequency identification sensor network, a technique integrating radio frequency identification (RFID) with wireless sensor network (WSN), is introduced. The basic structure of RFID sensor network is analyzed at first. Then components of RFID sensor and relevant supporting techniques including thin film battery, quartz crystal microbalance (QCM) and surface acoustic wave (SAW) are discussed. Research status and foregrounds of both domestic and international researches are introduced as well as applications in different fields.

Objective To study the relation between expressions of transforming growth factor ?1 (TGF-?1), transforming growth factor receptor type Ⅰ (T?RⅠ) and cell proliferation, cell cycle in gallbladder carcinomas, to disclose the mechanism of TGF-?1 and T?RⅠin the gallbladder carcinogenesis,and to evaluate their values in the prognosis of gallbladder carcinomas. Methods Thirty five gallbladder carcinomas age (57.94? 4.61) years, 14 male cases and 21 female cases comprised 32 adenocarcinomas, 2 adenosquamous carcinoma and 1 squamous cell carcinomas. Formalin fixed, paraffin embedded sections from gallbladder carcinomas were immunostained with TGF-?1, T?RⅠ, PCNA, cyclin E antibodies by immunochemical assays. Gallbladder adenoma and chronic cholecystitis were collected as non-malignant controls. Patients of gallbladder carcinomas were followed up. Results Positive immunostaining rate of TGF-?1 was 57.14% in gallbladder carcinomas, which was significantly higher than that in gallbladder adenomas and chronic cholecystitis (P

Objectives To investigate and evaluate the effect of Nutrison-5Fu on immune function of perioperative patients with progressive gastrointestinal cancer,and simultaneously we also recommended the methods of application of Nutrison-5Fu.Methods Seventy eight cases were equally divided at random into three groups,26 cases in each group.⑴Nutrison-5Fu group:the patients were treated with Nutrison-5Fu.⑵Chemotherapy group:the patients only received 5-Fu.⑶Control group:there were not any chemotherapy and immunetherapy during the perioperation.The values of CD 3,CD 4,CD 8 and NKCC were determined in all cases.Results Values of CD 3,CD 4,CD/CD 8 ratio and NKCC in most patients were distinctly low,but the condition rapid improved by giving them Nutrison-5Fu.On the contrary,the values declined further after chemotherapy alone.Conclusion The treatment of Nutrison-5Fu not only can repress the development of the tumor,but also can improve the dystrophic status and immune function of the patients with cancer.
