RESUMO

BACKGROUND: The canonical Wnt/β-catenin signaling pathway is a fundamental regulatory system involved in various biological events. ICG-001 selectively blocks the interaction of β-catenin with its transcriptional co-activator cyclic AMP response element-binding protein (CBP). Recent studies have provided convincing evidence of the inhibitory effects of ICG-001 on Wnt-driven disease models, such as organ fibrosis, cancer, acute lymphoblastic leukemia, and asthma. However, the effects of ICG-001 in atopic dermatitis (AD) have not been investigated. OBJECTIVE: To investigate whether β-catenin/CBP-dependent signaling was contributed in the pathogenesis of AD and ICG-001 could be a therapeutic agent for AD. METHODS: We examined the effects of ICG-001 in an AD-like murine model generated by repeated topical application of the hapten, oxazolone (Ox). ICG-001 or vehicle alone was injected intraperitoneally every day during the development of AD-like dermatitis arising from once-daily Ox treatment. RESULTS: Ox-induced AD-like dermatitis characterized by increases in transepidermal water loss, epidermal thickness, dermal thickness accompanied by increased myofibroblast and mast cell counts, and serum levels of thymic stromal lymphopoietin and thymus and activation-regulated chemokine, and decreases in stratum corneum hydration, were virtually normalized by the treatment with ICG-001. Elevated serum levels of periostin tended to be downregulated, without statistical significance. CONCLUSION: These results suggest that β-catenin/CBP-dependent signaling might be involved in the pathogenesis of AD and could be a therapeutic target.

<b>Background</b>: The need for palliative care is increasing, and psychological care for cancer patients has been recognized to have an important role in palliative care. <b>Purpose</b>: To determine the medical knowledge and skills (especially in psychiatry) required in palliative care and to propose psychiatric training for palliative care education. <b>Subjects/Methods</b>: Using a questionnaire, palliative care doctors (working less than 3years) and nurses were asked their opinions on the necessity of palliative care and requested to self-evaluate their knowledge and skills in palliative care. <b>Results</b>: The survey revealed that many doctors and nurses felt the need for an "Interdisciplinary team approach" "Good communication with patient and family", an "Understanding of total pain" and "Listening". Palliative care doctors had low self-evaluations of their psychiatric knowledge and skills with regard to palliative care, but they recognized the necessity for greater knowledge and skills in this area. <b>Conclusions</b>: In a palliative care setting, it is important to establish training programs for consultation-liaison psychiatry that provide opportunities for learning about psychiatric practices, including diagnostic evaluation, psychotherapeutic and pharmacologic treatment, as well as skills for communicating with cancer patients, families and staff. RESUMO

Thirteen patients with primary hepatocellular carcinoma less than 5 cm in size were seen in our clinic during the past four years. Clinical data of these patients were evaluated to clarify the clinical significance of minute hepatocellular carcinoma. Eleven of these patients were male, and average age was 58 years old. Of these patients, ten (77%) were complicated by liver cirrhosis, and only one revealed positive HBs antigen in serum. The serum alpha-fetoprotein level showed more than 400 ng/ml in five patients (38%). And we thinked that ultrasonographic examinations were most effective to diagnose minute hepatocellular carcinoma in various diagnostic imaging methods.<BR>Most of patients exhibited a decreased functional reserve in the liver, but seven patients underwent hepatic resection. After operation, one patient died of acute hepatic insufficiency on the 8th day, and one died of the recurrence of tumor on the llth month. Other five patients have been alive now.<BR>It is suggested that the survival time of patients with minute hepatocellular carcinoma is fairly influenced by the condition of accompanied liver cirrhosis
