RESUMO

Cytomegalovirus hepatitis is a liver disease caused by human cytomegalovirus infection and is one of the most common liver diseases in children and immunocompromised individuals. This disease has no specific clinical manifestations and is easily confused with other types of viral hepatitis, which may lead to delayed treatment or mistreatment. Therefore, the early diagnosis of cytomegalovirus hepatitis is of vital importance, and patients should be given timely and effective treatment with appropriately selected antiviral drugs and course of treatment. This article reviews the recent research advances in the etiology, epidemiology, pathogenesis, clinical manifestations, diagnosis, treatment, and prevention of cytomegalovirus hepatitis.

RESUMO

ObjectiveTo observe the protective effect of Shenlian prescription on acute lung injury induced by particulate matter (PM) exposure in rats and explore the mechanism. MethodFifty male SD rats were randomly divided into the control group, model group, Shenlian low-dose group (4.32 g·kg-1), Shenlian high-dose group (8.64 g·kg-1), and roflumilast group (3.46 mg·kg-1), with 10 in each group. Pre-administration with drugs by gavage was performed for one week. On the 8th and 11th days, the control group was instilled with normal saline in the trachea and the other groups with PM suspension to establish a rat model of acute lung injury induced by PM exposure. After modeling, drugs were given continuously until the end of the experiment. Forty-eight hours after the last exposure, the lung function of rats was detected. Then the rats were sacrificed and the lung morphological changes and pathological changes by hematoxylin-eosin (HE) staining were observed. CD68 expression in lung was detected by immunohistochemistry, and the levels of lung injury markers surfactant protein A (SP-A) and Clara cell protein16 (CC16) in serum were detected by enzyme-linked immunosorbent assay (ELISA). The mRNA expression of interleukin-1α (IL-1α), IL-6, IL-18, and monocyte chemoattractant protein-1 (MCP-1) in lung tissue was measured by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). ResultCompared with those in the control group, the rats in the model group had decreased lung function and obvious structural damage of lung tissue, PM deposition, and infiltration of CD68 positive cells. The expressions of IL-1α, IL-6, IL-18, and MCP-1 in lung tissue were increased (P<0.01). Compared with the model group, Shenlian prescription low and high doses restored the rats' lung function injury(P<0.05,P<0.01), improved lung morphological and pathological structure, and reduced PM deposition. Infiltration of CD68 positive cells in lung was not significantly decreased. The levels of inflammatory factors IL-1α, IL -6, IL-18, and MCP-1 in lung were lowered (P<0.01). ConclusionShenlian prescription could protect the rats' lung injury caused by PM exposure, improve lung morphology, and reduce PM deposition and inflammatory factor expression.

RESUMO

Objective:To investigate the correlations between perfused lung volumes, visual scores (using perfusion SPECT/CT) and right-heart catheter (RHC) hemodynamic parameters in patients with chronic thromboembolic pulmonary hypertension (CTEPH).Methods:A total of 51 consecutive CTEPH patients (17 males, 34 females, age (59±12) years) in the First Affiliated Hospital of Guangzhou Medical University between March 2015 and July 2019 were retrospectively analyzed. All patients underwent lung perfusion SPECT/CT imaging and RHC examinations. Perfused lung volumes were determined using threshold-based (15%-85%) segmentation. Visual semiquantitative scoring in each lung segment was performed using Begic method. RHC hemodynamic parameters including pulmonary artery systolic pressure (PASP), pulmonary arterial diastolic pressure (PADP), mean pulmonary artery pressure (mPAP), pulmonary arteriolar wedge pressure (PAWP), pulmonary vessel resistance (PVR), cardiac output (CO), cardiac index (CI) were recorded. Spearman correlation analysis was used to evaluate the correlations between perfused lung volumes, visual scores and hemodynamic parameters. Results:There were significant correlations between perfused lung volumes (30%-70% threshold) and mPAP ( rs values: from -0.414 to -0.302, all P<0.05). Among them, perfused lung volumes under the threshold of 40% and 45% were moderately correlated with mPAP ( rs values: -0.414, -0.412, both P<0.05). Perfused lung volume (40% threshold) was moderately negatively correlated with PASP, PADP ( rs values: -0.402, -0.440, both P<0.05), and slightly negatively correlated with PVR ( rs=-0.352, P<0.05). Visual scores were slightly positively correlated with the PADP ( rs=0.311, P<0.05), while there was no correlation between visual scores and other RHC hemodynamic parameters ( rs values: from -0.201 to 0.275, all P>0.05). Conclusion:Perfused lung volumes based on threshold-based segmentation in lung perfusion SPECT/CT imaging can accurately reflect hemodynamic status and may provide useful information for severity assessment of CTEPH.

RESUMO

Objective:To explore the value of improved sequential organ failure assessment (ISOFA) in predicting the prognosis of Klebsiella pneumoniae sepsis. Methods:The clinical data of 379 patients with Klebsiella pneumoniae sepsis admitted to the First Hospital of Jilin University from January 1, 2018 to June 30, 2019 were retrospectively analyzed. They were divided into survival group and death group according to the 28-day prognosis, and the age, gender, C-reactive protein, procalcitonin, grade of bacterial resistance, and the occurrence of septic shock of the two groups were compared.Statistical analysis was conducted by using independent sample t test, Mann-Whitney U test and chi-square test. Risk factors for the prognosis of the disease was analyzed by logistic regression analysis. The correlation between ISOFA and other scoring system including Charlson′s weighted index of comorbidities (WIC), national early warning system (NEWS), acute physiology and chronic health evaluation Ⅱ (APACHEⅡ), simplified acute physiology scoreⅡ (SAPSⅡ), mortality in emergency department sepsis score (MEDS), sequential organ failure assessment (SOFA), acute gastrointestinal injury (AGI) was determined by Spearman correlation coefficient. The area under the receiver operating characteristic curve (AUROC) was used to compare the evaluation value of each scoring system for the prognosis of Klebsiella pneumoniae sepsis. The clinical significance of ISOFA risk stratification was evaluated by Kaplan-Meier survival curve. Results:Among the 379 patients with Klebsiella pneumoniae sepsis, 278 were in the survival group and 101 were in the death group. The differences of age, gender, the occurrence of septic shock, grade of bacterial resistance, C-reactive protein, procalcitonin, WIC, NEWS, APACHEⅡ, SAPSⅡ, MEDS, SOFA, AGI and ISOFA score between the two groups were all statistically significant ( t=-3.218; χ2=6.781, 24.374 and 27.208, respectively; Z=-3.689, -5.022, -4.396, -4.697, -7.438, -6.348, -6.358, -8.676, -6.680 and -11.658, respectively; all P<0.01). The calculation method of ISOFA was obtained by single factor analysis: ISOFA=SOFA+ 1.5×AGI.Multivariate logistic regression suggested that ISOFA, gender, SOFA, procalcitonin, C-reactive protein, and bacterial resistance were independent risk factors for the prognosis of the disease. The Spearman correlation coefficients of ISOFA and WIC, NEWS, APACHEⅡ, SAPSⅡ, MEDS and SOFA were 0.327, 0.371, 0.614, 0.564, 0.578 and 0.847, respectively. The AUROC and its 95% confidence interval for WIC, NEWS, MEDS, SAPSⅡ, APACHEⅡ, SOFA alone, SOFA plus AGI, and ISOFA to predict the prognosis of Klebsiella pneumoniae sepsis were 0.646 (0.584-0.708), 0.657 (0.597-0.716), 0.712 (0.654-0.771), 0.713 (0.653-0.773), 0.749 (0.693-0.806), 0.788 (0.737-0.838), 0.872 (0.826-0.917) and 0.891 (0.845-0.937), respectively. The results showed that ISOFA had the best predictive effect. The Kaplan-Meier survival curve suggested that there were statistical differences in survival rate among ISOFA low-risk level, medium-risk level, and high-risk level (all P<0.01). Conclusion:ISOFA has important clinical significance in predicting the prognosis of Klebsiella pneumoniae sepsis.

RESUMO

Type Ⅰ, Ⅱ, Ⅲ interferons have anti-tumor potential, but the mechanism of action and clinical effect are different. Signaling pathways associated with interferon-treated tumors mainly include JAK-STAT, PI3K/AKT/mTOR, MAPK, NF-κB, c-Abl, etc. Type Ⅰ interferon has the widest clinical application and the most definite curative effect at present. Because of its serious adverse reaction and possible role in the process of cancer induction, type Ⅱ interferon has a great controversy on whether it can be used in anti-tumor therapy. The biological function of type Ⅲ interferon has good characteristics of type Ⅰ and Ⅱ interferon, but its anti-tumor medicinal value needs to be further evaluated.

RESUMO

Objective To investigate the impact of transplantation of human amniotic mesenchymal stem cells(hAMSCs)on the histopathological change in paraquat-induced pulmonary fibrosis in rats. Methods Forty-six female SD rats were randomly divided into the sham surgery group and the hAMSCs transplant group. Pulmonary fibrosis model was induced by 2% of paraquat intragastric administration(100 mg/kg/rat). hAMSCs were injected through caudal vein(2 × 106 cells/mL/rat). The histopathological changes were observed through microscopy after HE and the immunohistochemical staining. Results General conditions in rats received hAMSCs transplantation were better than those of the model rats. More large area and white fibrosis nidus were observed in bilateral lung of model rats, with less dispersal spot or nidus. The construction of lung tissue was disordered in the model rats. The thickness of alveolar wall was found increased. There were large area interstitial hyperplasia and a large number of inflammatory cells infiltrations. The construction of lung tissue was apparently improved. A majority of alveolar wall was monolayer cell. There were only less and small area with interstitial hyperplasia. Inflammatory cell infiltration was significantly decreased. The anti-human nucleus specific antibody positive hAMSCs were observed planted and survived in lung interstitial tissue. And few hAMSCs were observed planted in alveolar wall. Conclusion The transplanted hAMSCs can be planted and survived in lung tissue ,and may play a therapeutic role in araquat-induced pulmonary fibrosis.

<p><b>OBJECTIVE</b>To observe the effect of oxysophoridine (OSR) on the EEG and its power spectrum of reticulum formation in mesencephalon of anaesthetized rat.</p><p><b>METHOD</b>Utilizing the technique of brain stereotactic apparatus, electrodes were implanted into reticulum formation of mesencephalon. Monopolar lead and computerized FFT technique were employed to record and analyse the index of EEG, power spectrum and frequency distribution in order to study the effect of oxysophoridine on the bioelectricity change of mesencephalon reticulum formation in rats.</p><p><b>RESULT</b>After administrating(icy) with oxysophoridine at the dose of 2.5,5, 10 mg/rat, the EEG of mesencephalon reticulum formation mainly characterized with low amplitude and slow waves accompanied by spindle-formed sleeping waves with a significant decrease of total power of EEG (P < 0.05) while the ratio of theta, alpha waves increased in total frequency of rats (P < 0.05).</p><p><b>CONCLUSION</b>Oxysophoridine possesses central inhibitory effects and its inhibitory mechanism may associate with the reduction of bioelectricity in mesencephalon reticulum formation. Mesencephalon reticulum formation may serve as one part of the structure serving as the circuit conducting the central inhibitory effect of oxysophoridine.
