RESUMO

Objective To obtain the weight coefficient of indicators for evaluating the service capacity of maternal and child health care institution at county level. Methods Expert scoring method and CRITIC method were used to calculate the subjective weight coefficient and objective weight coefficient respectively, then comprehensive weight coefficient of each indicator was calculated. Results A total of 25 indicators were selected, among which the minimum comprehensive weight coefficient was 0.033 4 and the maximum was 0.050 4.The 5 lowest weight coefficient indicators were the proportion of financial subsidies accounted for general income (X12),number of equipment valued above 10,000 Yuan (X7),number of available beds (X6),annual person -time number of hospitalization (X22)and annual person -time number of outpatient and emergency treatment (X21).The 5 highest weight coefficient indicators were the outpatient expenditure each time (X25),mortality rate of pregnant and lying -in women (X18),proportion of medical expenditure accounted for total expenditure (X13),indicators of the average hospital stay each time (X9)and diagnosis coincidence rate of hospital admission and discharge (X19).Conclusion This weight coefficient of indicators focuses more on the public health service and the comprehensive weight coefficient calculated by combining subjective weighting method and objective weighting method is more reasonable.

RESUMO

Objective To investigate the knowledge and opinions of the staff in community health service centers. Methods Staff in Ganjiang,Jiefang and Nanwai community health service centers of Zhanggong District,Ganzhou City,Jiangxi Province were interviewed with questionnaire based on meeting by chance by trained interviewers about their opinions on 10 aspects of community health services. The data were evaluated by score method and factors influencing the score were studied with logistic regression analysis. Results Generally, the staff's evaluation for community health services was high, satisfying with colleagues' cooperation, professional training and service capacities in 95.96% ,90.91% and 88.88% of them, respectively. But their satisfaction with personal income and staff' s income was poor(24.24% and 43.43%).Staff for logistics were not so satisfied with their personal income.Staff for logistics and senior staff were not so active in their professional training. Conclusions The staff in community health service centers could better understand community health services with satisfaction.Incentive mechanism for the staff in community health services should be introduced with optimized management system to increase staff's income and sustain its stable development.

RESUMO

OBJECTIVETo study the effects of rare earth exposure on human telomerase and apoptosis of human peripheral mononuclear cells (PBMNs).

METHODSRare earth mine lot in Xunwu county, the biggest ion absorptive rare earth mine lot of China, was selected as the study site. Another village of Xunwu county, with comparable geological structure and social environment was selected as the control site. Thirty healthy adults were randomly selected from the study site as exposure group and another 30 healthy adults randomly selected from the control site as control group. The blood content of 15 rare earth elements, including La, Ce, Pr, Nd, Sm, Eu, Gd, Tb, Dy, Ho, Er, Tm, Yb, Lu and Y, were determined by inductive coupled plasma-source mass spectrometry (ICP-MS). The total contents of rare earth elements in the blood were calculated. The TRAP and FCM assays were carried out to analyse the telomerase and apoptosis of human PBMNCs respectively.

RESULTSIn the exposure group, the concentration of La, Ce, Dy and Y were significantly higher (P<0.001), and Pr, Nd, Sm, Gd and Yb were higher than those in the control group (P<0.05). The total content of rare earth in the blood of exposure group showed significant difference compared with control group (P<0.001). Telomerase activity in PBMNs of the exposure group was higher than that in the control group (P<0.05); there were 11 adults in the exposure group (30 adults) and 5 adults in control group (30 adults) showed positive telomerase activity. The average age of the exposure group was (38.69 +/- 8.02) years-old, while the control group was (40.45 +/- 9.02) years-old (P >0.05). It was found that there was a significant relationship between telomerase activity and the total content of rare earth elements (P <0.01). 3. The proportion of apoptosis was not different between the two groups (P >0.05), but the cells in the S-phase and G2-M phase were increased (P <0.01) in the exposed group.

CONCLUSIONThe telomerase activity of PBMNs in the rare earth elements exposed group was higher than that of the control group, and there is no effect on apoptotic rate of PBMNs, but may promote the diploid DNA replication, and increase the percentage of G2/M and S phase cells.

RESUMO

Objective To study the effects of environmental exposure to rare earth elements REEs on intelligence of children. Methods Intelligence quotient IQ were examined with Drawing a Man Test in 464 children aged 7-10 years living in RE ore containing area and the control area in Xunwu country Jiangxi province China. Fifteen kinds of REEs in 112 blood samples 69 samples from RE ore area and 43 samples from the control area  were detected by inductively coupled plasma source mass spectrometry ICP-MS. Results All 15 kinds of REEs were detected in each sample the content of blood REEs of the children in the rare earth area 2.18?1.08 ng/g was 1.73 times of that of the children in the control area 1.26?0.35 ng/g the difference was significant P

RESUMO

Objective To explore the effects of lower concentrations of rare earth elements in environment on human health. Methods The representative well water samples, soil samples and vegetable samples were selected near the rare earth ore and non_rare earth ore. The contents of La and other 15 kinds of rare earth elements were determined by ICP_MS. Results The total contents of rare earth elements were 9.18 ?g/L for well water samples, 869.73 ?g/g for soil samples, 6 370.35 ng/g (dry weight) for vegetable samples respectively near the countryside of rare earth elements ore, which were all higher than those in non_rare earth elements. Conclusion Human health might be affected by rare earth elements in drinking water and foods in rare earth ore.
