Objective: To explore the distribution patterns of cardiometabolic diseases (CMD) in elderly patients with colorectal cancer, and provide a reference for the prevention and treatment of cardiovascular metabolic diseases in these patients. Methods: Clinical data of 3 894 elderly patients with colorectal cancer from January 2008 to March 2018 admitted in the Chinese PLA General Hospital were recruited and the incidence rate of CMD was retrospectively analyzed. The influence factors of elderly patients with colorectal cancer combined with CMD were analyzed by multivariate Logistic regression model. Results: The morbidity rate of CMD in elderly patients with colorectal cancer is 33.4% (1 301/3 894), among them, the morbidity rate of the male was 31.9% (768/2 409), and that of the female was 35.9% (533/1 485). There was not significant difference between these two sex (P=0.074). The morbidity rates of CMD in patients of 65-74 years, 75-84 years and ≥85 years were 30.6% (754/2 462), 37.0% (479/1 294) and 49.3% (68/138), respectively, with significant differences (P<0.001). Multiple Logistic regression analysis revealed that female (OR=1.213, 95%CI: 1.056-1.394), age (75-84 years group: OR=1.344, 95%CI: 1.164-1.552; ≥85 years group: OR=2.345, 95%CI: 1.651-3.331) and body mass index (BMI 18.5-24.9 kg/m(2) group: OR=1.319, 95%CI: 1.065-1.638; ≥25 kg/m(2) group: OR=2.041, 95%CI: 1.627-2.561) were independent risk factors for elderly colorectal cancer patients with CMD. Conclusion: The morbidity rate of CMD in elderly patients with colorectal cancer increases with age and it is urgent to strengthen multidisciplinary cooperation and develop reasonable treatment plans to extend the survival and life quality of these patients.

As a new science and technology, it is more and more extensive used in the medical and health field recent years and shows outstanding ability in the diagnosis and treatment of complex diseases. Pancreatic cancer has the characteristics of high degree of malignancy, high concealment, difficult diagnosis and treatment, poor prognosis and so on. Therefore, clear diagnosis and early treatment are significant to improve patient prognosis. This paper summarizes the development and application of artificial intelligence and radiomics in the field of pancreatic cancer.

RESUMO

In recent years, cases of child abuse that result in injuries and death have occurred from time to time in China, and there may be more undetected child abuse cases. However, many pediatricians and forensic doctors lack professional knowledge and formal training in detecting child abuse, which leads to the missed diagnosis, misdiagnosis and misidentification of many cases of child abuse. This paper reviews a large number of relevant domestic and foreign literatures, combined with practical work experience and China's national conditions, preliminarily summarizes the main points of clinical diagnosis and forensic identification of child abuse cases, in order to provide some help for early detection, accurate identification of child abuse cases and timely and effective treatment and protection for abused children.

Objective:To explore the clinical application of peroneal artery pedicled with lateral calf muscle wear skin flap in repair of foot and ankle soft tissue defect.Methods:From February 2017 to May 2019, the peroneal artery pedicled with lateral calf muscle wear foot and ankle soft tissue defect was used with skin flap in 20 cases, which included car accident injuries in 11 cases, 3 cases of heavy injuries, skin ulcer in 4 cases, and 2 cases of bone scars. All of the affected limb wounds presented with the exposure of deep tissue such as bone or tendons. The flap cut ranged from 5.0 cm×4.0 cm~7.0 cm×5.0 cm. The donor area was closely sutured in 5 cases, and inguinal full thickness skin pack was used with skin grafting in 15 cases.Results:20 cases of postoperative skin flap survived of all, healing of wounds and cuts for phase I. After follow-up of 5~18 months, the flap had good shape and wear-resistant quality, without feeling bloated. The two points of flap were different within 3~5 mm; the functional activity of the ankle joint was good in the affected limb, with satisfactory results.Conclusions:It is a convenient and reliable procedure, with smaller surgical trauma, which is an ideal skin flap for repair of soft tissue defect in foot and ankle.

With the development of modern imaging technology and improvement of physical examination consciousness of people, the incidence of pancreatic cystic neoplasm (PCN) has increased significantly. The overall prognosis of PCN is good, and great progress in surgical techniques and multidisciplinary teamwork has been made in pancreatic surgery. Although many guidelines have been released at home and abroad, there are controversies in the differential diagnosis, operative indications, surgical procedure, and the follow-up strategies toward patients with PCN. There are also significant differences in the level of diagnosis and treatment of medical centers, which brings certain challenges to the standardized diagnosis and treatment of PCN. Based on the latest clinical guidelines and new developments in clinical researches, the authors summarize the controversies of different guidelines in the diagnosis, treatment and follow-up of PCN in order to further improve the standardization of the diagnosis and treatment.

Pancreatectomy is difficult and has various complications. Pancreatic fistula is one of the most serious postoperative complications after pancreatectomy. It will corrode the surrounding tissues of pancreas, which will cause abdominal infection, if the drainage of pancreatic fistula is not complete. Pancreatic fistula combined with abdominal infection may lead to systemic inflammatory response syndrome. Patients at the severe stage often die of multiple organ dysfunction and septic shock. Up to now, the treatment of pancreatic fistula with abdominal infection has not been well determined. This paper reviewed the relevant studies of pancreatic fistula combined with abdominal infection, analyzed the diagnostic criteria and common pathogenic bacteria of pancreatic fistula with abdominal infection, discussed the indications and systematic treatment plan of the surgical intervention, in order to provide direction for the treatment of pancreatic fistula combined with abdominal infection.

BACKGROUND@#The patients with advanced lung adenocarcinoma should select targeted drugs based on the type of tumor epidermal growth factor receptor (EGFR) gene mutation. However, it is difficult to collect tumor tissue of advanced lung adenocarcinoma, and some experts agree that peripheral blood can be used as a substitute for tumor tissue as a test specimen. This paper aimed to investigate the clinical value of ddPCR and super-amplification refractory mutation system (ARMS) in detecting EGFR gene mutation in peripheral blood of patients with advanced lung adenocarcinoma.@*METHODS@#A total of 119 patients diagnosed in Beijing Chest Hospital Affiliated to Capital Medical University from February 2016 to February 2019 were collected, and the sensitivity and specificity of plasma ctDNA EGFR gene mutation detected by ddPCR and super-arms were compared. Some patients with positive EGFR gene mutations received oral treatment with first-line EGFR tyrosine kinase inhibitors (EGFR-TKI). The patients were divided into subgroups according to the test results. In group 1, both ddPCR and super-arms showed positive EGFR gene mutation results, with 21 cases. In group 2, ddPCR and super-arms detection of EGFR gene mutation were all negative, with 16 cases. In group 3, the ddPCR test was positive and the super-arms test was negative, with 5 cases. In group 4, the ddPCR test result was negative while the super-arms test result was positive. Since the number of patients in group 4 was 0, no statistics were included. Objective response rate (ORR) and disease control rate (DCR) were used to evaluate the short-term outcome, and progression-free survival (PFS) was compared with survival analysis to evaluate the long-term outcome.@*RESULTS@#EGFR mutations were detected in 58 (48.7%) of 119 patients with advanced lung adenocarcinoma. The coincidence rate between ddPCR and EGFR gene mutation in tumor tissues was 82.4% (Kappa=0.647, P0.05). Survival analysis showed that the PFS of the three groups was compared. The difference was not statistically significant (χ²=2.221, P=0.329).@*CONCLUSIONS@#ddPCR, as a high sensitivity and specificity liquid gene detection method, can be used as a reliable method to detect the mutation of plasma ctDNA EGFR gene in patients with advanced lung adenocarcinoma. The results of plasma genetic testing can also be used as the basis for predicting the efficacy of EGFR-TKIs in patients.

Objective@#To evaluate the current status of the primary and secondary schools’ teaching environment in China and associated factors,and to provide the support for making differentiated interventional stategies.@*Methods@#Data regarding to information of the primary and secondary schools was obtained by random inspection from the National Health and Family Planning Commission in 2018. Chi-square test was used to compare differences of qualified rates between groups. Spearman correlation analyses was applied to examine the correlations among population density, per capita GDP and qualified rates.@*Results@#Among the total of 37 531 schools, the overall qualified rate of teaching environmental sanitation of primary and secondary schools in 2018 was 59.1%. The qualifying rate in urban areas (60.3%) was higher than that of rural areas (59.4%) and towns (56.7%). Primary schools (59.8%) had higher qualifying rates than junior high schools (58.2%) and senior high schools (51.8%). In terms of each evaluation item, the qualifying rates of school furniture equipment including desks and chairs were the lowest, while the qualifying rates of dormitory ventilation ranked highest. Large variations among provinces were observed for the qualifying rates for school furniture equipment, as well as blackboard illumination. Those provinces with higher per capita GDP and higher population density had higher the qualifying rates for school furniture equipment.@*Conclusion@#The overall qualifying rate of Chinese primary and secondary schools’ teaching environment is low. Among all the evaluation measures, qualifying rates of dormitory ventilation are relatively good, while school furniture equipment needs to be improved. Notably, strengthened and targeted management should be applied to improve schools’ teaching environment in new era.

RESUMO

This paper reviews relevant literatures at home and abroad. The cases of child sexual abuse are systematically discussed from the aspects of epidemiology and forensic examination, in order to develop and improve the inspection and appraisal thinking and ability of domestic forensic workers in such cases. The paper also briefly introduces the current situation and existing problems of dealing with child sexual abuse cases for case-handling units at base level in China, in order to provide help to the formulation, improvement and development of relevant standards and norms for child protection in China.

Pancreaticoduodenectomy is cumbersome and difficult to operate,with a long operative time and high risk of postoperative complications,thus it is one of the most complicated operations among general surgery.With the popularization and progress of minimally invasive techniques,minimally invasive pancreaticoduodenectomy (MIPD) has obtained a well developing.It has been confirmed that MIPD is noninferior or even superior to the traditional open pancreaticoduodenectomy in term of the feasibility,safety and effects of radical cure.However,the relevant conclusions are mostly from single-center retrospective studies,without high-quality evidence support.The authors has reviewed the recent research progress of MIPD in the indications and contraindications,safety,feasibility and tumor curative effect,and illustrated the current status and prospects of MIPD with clinical experience and related literature,contributing to the standardization of MIPD in China.

Lipedema is secondary to local fat deposition, a disease characterized by the symmetric thickening of lower limbs, mostly occurs in women, especially in adolescence and pregnancy. In its early stage, it could be easily confused with lymphedema. Extensive literature review on primary fat edema in recent years, as well as a summary of the clinical symptoms and signs and diagnosis and treatment of lipedema were conducted, so as to provide a useful reference for clinicians.

Objective To study the relationship between cancer-related fatigue (CRF) and clinicopathological features and quality of life in patients with advanced gastric cancer. Methods The clinical and pathological data of 182 patients with advanced gastric cancer were collected, The CRF level and quality of life were assessed by the brief fatigue inventory (BFI), European Organization for Research on Treatment of Cancer (EORTC) life quality scale QLQ-C30 (V3.0). The correlations between CRF and clinicopathological features and quality of life were analyzed. Results One hundred twenty-two patients with advanced gastric cancer were associated with different degrees of fatigue. The CEF BFI score had correlation with the age(F=6.637,P=0.001),PS score(F=15.559,P=0.000),the World Health Organization(WHO) pathologic type (F=7.352, P=0.000). The CRF index of the 50-59 age group was higher than that of other age groups; the CRF BFI scores of PS score 1 and 2 were higher than that of 0 group; CRF BFI score of gastric signet ring cell carcinoma was significantly higher than that of gastric squamous cell carcinoma. The Pearson correlation analysis showed that the CRF BFI score was positively correlated with mood (r=0.423, P=0.008). There were significant differences in body function and mood (F=-16.254, F=-13.256, both P<0.05)and fatigue,pain and loss of appetite among 3 CRF groups(F=12.075,F=14.361,F=13.837, all P<0.05). There was a significant difference in the mean CRF BFI score between the three groups before and after chemotherapy(tSD=6.981,tPR=6.903,tPD=7.437,all P<0.05). Compared with pre-chemotherapy, the mean CRF BFI score of SD and PR group decreased after 3 months follow-up, score of PD group increased, the differences were statistically significant(t=2.371,t=6.912,t=18.237,all P<0.05).Conclusions The incidence of CRF in patients with advanced gastric cancer is very high,the degree of CRF has correlation with age, PS score and pathological type. Relieve symptoms such as fatigue, pain and loss of appetite and control adverse reactions could help to reduce the degree of CRF.

Objective@#To test the expression of miR-1178 in pancreatic cancer and study its clinicopathological significance and mechanism.@*Methods@#The expression of miR-1178 in 87 paired paraffin pancreatic ductal adenocarcinoma specimens and adjacent non- cancerous pancreatic tissue diagnosed by Pathology Department of Peking Union Medical College Hospital was detected by hybridization in situ. The relationship between the expression of miR-1178 and clinicopathological characters was analyzed.miR-1178 mimics and inhibitor were used to further detect the close relationship among miR-1178 and cancer invasion. Establish a nude mice subcutaneously transplanted tumor model, 4 weeks after vaccination for tumor volume and weight measurement.Student t-test, rank sum test, and χ2 test was used respectively to compare the data between two groups. Cox regression was adopted to improve the single factor and multiple factors analysis.@*Results@#The results of hybridization in situ showed the expression of miR-1178 was increased in 72 cases with pancreatic cancer compared to that in paired normal pancreatic tissues (50/72 vs. 11/72, χ2=43.26, P<0.05). miR-1178 expression was positively associated with tumor lymph node stage (χ2=4.189, P=0.041). Univariate and multivariate analysis revealed that miR-1178 was an independent adverse prognostic indicator for patients with pancreatic cancer (HR=2.364, 95%CI: 1.114-5.019, P=0.025). Transwell assay indicated the over-expression of miR-1178 increased the number of AsPC-1 cells that penetrated the ECM-coated membrane (177.0±19.8 vs. 119.7±15.9)(χ2=8.21, P<0.05). For the in vivo experiment, overexpression of miR-1178 significantly promoted tumor growth, compared with control group (tumor volume: (5 122.4±760.2)mm3 vs. (1 976.8±601.8)mm3, t=2.413, P<0.05; tumor weight: (1.55±0.21)g vs. (0.67±0.17)g, t=2.960, P<0.05). Over-expression of miR-1178 down-regulated the expression of Stub1 and elevated the expression of FAK/MMP-9 signal pathway(P<0.05).@*Conclusions@#MiR-1178 is overexpressed in pancreatic cancer, and is effective for predicting patients′ prognosis. MiR-1178 regulate Stub1/FAK/MMP-9 signal pathway and promote the invasion of AsPC-1 cells.

Objective To investigate the relationship between heart rate variability and type 2 diabetes mellitus(T2DM)in Kirgiz and Han,to understand the differences between ethnic groups,at the same time to provide effective method for type 2 diabetic autonomic neuropathy.Methods 24 -h Holter recording was performed in 1 46 individuals.75 cases of T2DMconfirmed by glycolated hemoglobin(HbA1 c)were selected as observation group, Kirgiz 40 people,Han 35 people.While 71 subjects without T2DM were chosen as control group,Kirgiz 34 people, Han 37 people.The HRV indices,including standard deviation (SD)for the time between normal -to -normal complexes in the entire 24 h electrocardiographic recording(SDNN),the average of SD for the time between normal -to -normal intervals for each 5 min period(SDNN5),the percentage of adjacent RR intervals 50 ms apart(PNN50), normal RR intervals divided by RR intervals derived from the triangle index(TRIA),SD of the average normal -to -normal intervals for each 5 min period (SDANN)and root -mean square of differences of successive RR intervals (RMSSD)were compared between these groups.Results There were no significant differences in age,gender, education level and smoking between Kirgiz and Han patients.The differences were statistically significant in SDNN, SDNN5,PNN50,SDANN,TRIA(t =3.392,P =0.001 ;t =3.652,P =0.001 ;t =3.826,P =0.001 ;t =3.235,P =0.002;t =2.378,P =0.02)between patients with or without T2DMin Kirgiz population.In contrast,the mean square root of SDNN,SDNN5,PNN50,TRIA,RMSSD(t =3.498,P =0.001 ;t =3.41 4,P =0.001 ;t =2.1 05,P =0.039;t =2.1 02,P =0.039;t =3.41 3,P =0.001 )were statistically significant in Han population between T2DM patients and nondiabetic patients.There was no deference between Han and Kirgiz patients without T2DM in HRV,whereas the TRIA(t =2.887,P =0.005 )had significant difference in T2DM patients between Han and Kirgiz population. Conclusion The HRV is significantly reduced in the T2DMpatients,especially the Kirgiz.The HRV analysis using 24h Holter is a noninvasive technology to detect the T2DM.

Objective To evaluate the value of three-dimensional ultrasound volume contrast imaging (VCI) and tomo graphy ultrasound imaging (TUI) techniques in observing fetal spinal conus medullaris (CM) position and lumbar enlargement of spinal cord morphologic changes,for assessment of tethered cord (TC).Methods Totally 17 abnormal fetuses of spinal diseases combined with TC (abnormal group) were examined by three dimensional ultrasound VCI and TUI techniques.The position of CM was recorded,and the transverse and anteroposterior diameters of lumbar enlargement of spinal cord were measured and compared with 300 cases of normal fetuses (normal group).Results As the growth of the gestational age (CA),CM terminal position increased.All the ends of CM located at L3 or L3 above level in normal.The transverse and anteroposterior diameters of lumbar enlargement in normal group showed good linear relationship with GA.Transverse diameter (mm) =0.677+0.147 ×GA (R2 =0.836,P<0.05),anteroposterior diameter (mm)-0.994+ 0.152× GA (R2=0.894,P<0.05).Compared with the corresponding GA fetuses in normal group,the anteroposterior diameter of lumbar enlargement decreased in abnormal group (P=0.002),while no statistical difference of the transverse diameter was found between the two groups (P=0.082).Conclusion Position of CM and lumbar enlargement measure ment can provide valuable reference information for clinical prenatal diagnosis of fetal spinal TC.

Objective To evaluate the effect of schistosomiasis control and prevention in Yangxin County from 2004 to 2015,so as to provide the evidence for improving the work of schistosomiasis transmission interrupted and elimination in the fu-ture. Methods According to the endemic types and endemic regularity of schistosomiasis in Yangxin County,the comprehen-sive control strategies were adopted,and the programs related to sanitation,water conservancy,forestry,and agriculture were implemented continuously. The schistosomiasis control effects in this county from 2004 to 2015 were analyzed and compared. Re-sults After the implementation of the schistosomiasis comprehensive control strategies in Yangxin County ,the calculated num-ber of patients reduced from 22240 in 2004 to 1471 in 2015,the infection rate of residents reduced from 8.57%in 2004 to 0.16%in 2015,the number of patients with acute schistosome infection reduced from 64 in 2004 to 0,and no cases of acute schistosomiasis found since 2009. The infection rate of cattle decreased from 8.87%in 2004 to 0. The area with Oncomelania hu-pensis snails and the area of susceptible zone reduced from 3446.21 hm2 and 1111.59 hm2 in 2004 to 2285.75 hm2 and 41.28 hm2 in 2015 respectively,and the schistosome-infection rate of snails reduced from 0.76%in 2004 to 0. Conclusions Since the comprehensive control strategy implemented from 2004,the endemic situation of schistosomiasis in Yangxin County has de-creased significantly. However,the harness force of the Fu River as well as the control of infection source of livestock still should be strengthened to consolidate the control achievement.

<p><b>OBJECTIVE</b>To compare differences between Crowe IV developmental dysplasia of the hip (DDH) with secondary acetabulum and Crowe IV DDH without secondary acetabulum,and determine whether it is necessary to divide Crowe IV DDH into two subtypes.</p><p><b>METHODS</b>From June 2007 to May 2015,145 hips of 112 Crowe N patients who underwent total hip arthroplasty (THA) using S-ROM stem were divided into two groups: secondary acetabulum formaton group (group A) and no secondary acetabulum formaton group (group B). In group A,there were 12 females, 96 males,with an average age of (39.38 ± 11.19) years old. In group B, there were 2 females, 35 males, with an average age of (38.19 ± 10.92) years old. All the patients were evaluated by using Harris Hip Score. Radiographic evaluations were made preoperatively and during follow up. The differences between two groups were compared on dislocation height, canal flare index (CFI), subtrochanteric shortening osteotomy (SSTO) usage, pre- and post-operation Harris scores, complications.</p><p><b>RESULTS</b>The dislocation height for group A was (4.74 ± 1.57) cm, while the dislocation height for group B was (3.12 ± 1.15) cm. Significantly difference was detected between two groups. The CFI for group A was 2.69 ± 0.68, while the CFI for group B was 3.42 ± 0.79, and the significantly difference was detected between two groups. Harris scores were totally improved from 58.18 ± 15.67 preoperatively to 91.20 ± 3.79 post-operatively and the difference was significant. Pre-operative Harris scores was 58.1 ± 15.3 in group A, 58.3 ± 16.9 in group B. Post-operative Harris scores was 91.0 ± 4.1 in group A, 91.0 ± 5.1 in group B. No significant difference was found on Harris scores between A and B preoperatively and post-operatively. Complications of 4 cases peri-prosthesis fracture, 4 cases dislocation and 4 cases nerve injury occur in group A; While only one case dislocation and one case nerve injury occur in group B. No statistical significance was detected.</p><p><b>CONCLUSION</b>Crowe IV DDH with secondary acetabulum is significantly different from Crowe IV DDH without secondary acetabulum on dislocation height and femoral morphology, which causes the different selections of surgical techniques (SSTO usage or not). These important differences in fundamental parameters indicate the necessity to further divide Crowe IV DDH into IVA and IVB two subtypes.</p>

Pancreatic cancer is a one of the most malignant digestive cancer.Because the lack of effective methods for early diagnosis,most patients have been ineligible for surgical resection when diagnosed.Kallikrein family is a group of serine proteases,because of its ability to decompose the extracellular matrix proteins,it may be closely related to the invasion and metastasis of various cancers.And some members of kallikrein family may become cancer diagnostic biomarkers.This paper reviews all the recent articles about kallikrein family study in pancreatic cancer.

RESUMO

Pancreatic cancer is a highly malignant tumor,the incidence is increasing year by year globally.Due to concealed pathogenesis of which and rapid progress,only 25% of the patients could receive operation.Adjuvant therapy has become the important methods of improving the prognosis of patients with pancreatic cancer,including chemotherapy and radiotherapy.In recent years,some prospective clinical trials have promoted the development of the adjuvant therapy.New controversy and consensus appear in the treatment of pancreatic cancer.This article reviewed the progress of chemotherapy and radiotherapy for pancreatic cancer based on adjuvant therapy and neo-adjuvant therapy for unresectable metastatic pancreatic cancer and locally advanced pancreatic cancer.
