RESUMO

Objective: To investigate the clinicopathological features, treatment and prognosis of patients with RET fusion positive non-small cell lung cancer (NSCLC). Methods: A total of 1 089 NSCLCs were retrieved at Affiliated Hospital of Jiangnan University from August 2018 to April 2020. In all cases, multiple gene fusion detection kits (fluorescent PCR method) were used to detect the gene status of RET, EGFR, ALK, ROS1, KRAS, BRAF and HER2; and immunohistochemical method was used to detect the expression of PD-L1 and mismatch repair related proteins. The correlation between RET-fusion and patients' age, gender, smoking history, tumor stage, grade, pathologic type, and PD-L1, mismatch repair related protein expression was analyzed. Results: There were 22 cases (2.02%) detected with RET fusion-positive in 1 089 NSCLC patients, in which 11 males and 11 females; and the median age was 63.5 years. There were 20 adenocarcinomas, including 11 acinar predominant adenocarcinoma (APA), five solid predominant adenocarcinoma (SPA) and four lepidic predominant adenocarcinoma (LPA); There were one case each of squamous cell carcinoma (non-keratinizing type) and sarcomatoid carcinoma (pleomorphic carcinoma). There were 6 and 16 patients with RET fusion-positive who were in stage Ⅰ-Ⅱ and Ⅲ-Ⅳ respectively, and 16 cases with lymph node metastasis, 11 cases with distant metastasis. Among RET fusion-positive cases, one was detected with HER2 co-mutation. The tumor proportion score of PD-L1≥1% in patients with RET fusion positive lung cancer was 54.5% (12/22). Defects in mismatch repair protein expression were not found in patients with RET fusion positive NSCLC. Four patients with RET fusions positive (two cases of APA and two cases of SPA) received pratinib-targeted therapy, and two showed benefits from this targeted therapy. Conclusions: The histological subtypes of RET fusions positive NSCLC are more likely to be APA or SPA. RET fusion-positive NSCLC patients are associated with advanced clinical stage, lymph node metastases, and they may benefit from targeted therapy with RET-specific inhibitors.

<p><b>BACKGROUND</b>Road traffic injuries (RTIs) are a worldwide issue associated with increasing development and motorization. However, statistical studies do not include any analyses of Beijing's geriatric population. Using data from the Beijing Emergency Medical Center, we present the main characteristics of traffic injuries involving the elderly in Beijing. We also provide objective information for those concerned with the safety of traffic systems and the prevention of traffic injuries.</p><p><b>METHODS</b>In a longitudinal, retrospective study, data were collected on 1706 victims aged 65 years and older who sustained traffic injuries in Beijing between 2004 and 2010. Personal information, time of injury event, emergency care response time, road user type, striking vehicle type, injury site, and severity of injury were analyzed using χ(2) tests and Logistic regression analysis.</p><p><b>RESULTS</b>The annual rate of traffic injuries was 21.80 per 100 000 elderly people in Beijing, and the morbidity rate decreased from 2004 to 2010 (P < 0.001). The mean age was (72.92 ± 5.67) years, and 911 (53.40%) of the victims were male. The majority of victims sustained head and lower limb injuries and were classified as being of medium severity. Traffic collisions occurred most frequently in the daytime excluding rush hours; these collisions included being hit by a car (85.64%) and pedestrian victim injuries (79.19%). Our statistical analysis found three factors for injury severity: abdominal injuries (P < 0.001), number of injury sites (P = 0.027), and head injuries (P = 0.034). The decline in traffic injuries is due to a decrease in victims aged 65-74 years and pedestrians; the severity of RTIs also decreased.</p><p><b>CONCLUSIONS</b>This study highlights the declining trend in traffic injuries among older adults in Beijing. However, traffic injuries remain a serious public health problem for the elderly and effective measures are required to reduce their incidence.</p>

Objective To explore the applicability of pubertal development scale in estimating sexual maturation among urban Chinese adolescents.Methods A cross-sectional survey was conducted among 2562 students of 4th to 12th grade in Shanghai,selected by stratified cluster sampling method.Self-reported pubertal maturation level was obtained by Pubertal Development Scale (PDS).Meanwhile,visual depiction of Tanner stages of breast,genital and pubic hair development were rated by a trained rater of the same gender,Results In 2562 students,the correlation between self-reported PDS and assessment from the raters were substantial,both in boys and girls (Pearson' s correlation coefficients:0.822 and 0.814,respectively).The overall agreement of pubertal development stages based on PDS and the rater' s assessment were 35.24% and 51.42% for boys and girls,respectively,with Kendall' s correlation coefficients as 0.713 and 0.685,respectively.Adolescents who were older,had relatively higher level of pubertal stages or adolescents from the richer families,tended to underestimate their actual level of pubertal maturation while those male subjects and overweight or obese subjects were even more likely to be so.Conclusion Using the self-reported PDS,we could conveniently obtain the necessary information regarding sexual maturation among the urban adolescents.It seemed especially useful in the related epidemiological survey when physical examination was precluded by cost,privacy and other concerns.

<p><b>OBJECTIVE</b>To investigate the change of early serum TNF-alpha and IL-6 levels in acute cerebral infarction and its significances.</p><p><b>METHODS</b>Serum TNF-alpha and IL-6 levels in 30 health subjects and 35 patients with acute cerebral infarction (ACI) within 6 hours of onset were measured by enzyme linked immunosorbent assay (ELISA); neurological deficits scores (NDS) in all cases were determined, and Spearman test was used for correlation.</p><p><b>RESULTS</b>The serum levels of TNF-alpha and IL-6 in ACI group were markedly higher than those in health subjects and there was a positive correlation of TNF-alpha and IL-6 levels with 6 h NDS (rs=0.89 and 0.93, P<0.001) and with NDS progression (rs=0.90 and 0.91, P<0.001). Early serum TNF-alpha and IL-6 levels in progressive cerebral infarction (PCI) group were evidently higher than those in stable cerebral infarction (SCI)[(49.56+/-12.12) pg/L compared with (24.30+/-7.4) ng/L and (39.76+/-7.88) ng/L compared with (20.78+/-6.28) ng/L, respectively, P<0.01)].</p><p><b>CONCLUSION</b>The early serum levels of TNF-alpha and IL-6 in ACI markedly increase and are closely correlated with disease severity; which may be of value in PCI risk evaluation.</p>

<p><b>OBJECTIVE</b>To discover the current condition of the floating population's quality of life (QOL) in a community of Shanghai and find out the possible influencing factors.</p><p><b>METHODS</b>480 people identified as 'floating', were chosen by systematic sampling method, whose QOL was evaluated by WHOQOL-BREF (Chinese Version). A multiple linear regression model was used to analyze the relationship between the factors and the scores of the four domains.</p><p><b>RESULTS</b>The physical and psychological scores of the floating population were 14.09 +/- 2.09 and 11.28 +/- 2.18 which were lower than the average levels of the Chinese population which were 15.8 +/- 2.9 and 14.3 +/- 2.5. Women showed higher score in the social relationship domain than men (P = 0.017). With the increase of age, the scores of physical and psychological domain were descending (P(Phys.) = 0.030, P(Phych.) = 0.023). The score in every domain was rising when the appetite was better or the family conflicts had a lighter impact on life (P < 0.01).</p><p><b>CONCLUSION</b>The QOL of the young females was better than other groups. Considering the whole QOL of the floating population, their physical and psychological demands should be highly concerned.</p>
