RESUMO

OBJECTIVE@#To study mechanism of improvement of stress concentration on patellofemoral joint by stiletto needle releasing lateral patellar retinaculum guided by the theory of Jinshugu() and based on the finite element model of knee joint. and to elucidate the biomechanical mechanism of stiletto needle releasing changing patellar trajectory and reducing patellofemoral joint pressure.@*METHODS@#CT data of knee joint from a normal male (aged 29, heighted 171 cm, weighted 58 kg) was selected. Starting with construction of three-dimensional model of knee joint by using finite element software, the finite element model of knee joint with complete tendonand bone structures were established through several steps, such as geometric reconstruction, reverse engineering, meshing, material assignment and loading analysis. The loading condition was set as 500 N load on knee joint, and the average tensile stress of quadriceps femoris tendon was about 200 N. To simulate the release of lateral patellar retinaculum by stiletto needle at 30 and 90 position of knee flexion in finite element model separately, and to compare the improvement of stress concentration of patellofemoral joint by stiletto needle intervention under different knee flexion conditions.@*RESULTS@#The peak stress of patellofemoral joint and tibiofemoral joint decreased after stiletto needle releasing of patellofemoral lateral retinaculum compared with before intervention, which was(1) knee flexion at 30 degrees:patellar cartilage decreased by 0.498 MPa (decreased 9.06%), femoral trochlea decreased by 0.886 MPa(decreased 16.27%);(2) knee flexion at 90 degrees:patellar cartilage decreased by 0.558 MPa (decreased 8.6%), femoral trochlea decreasedby 0.607 MPa (decreased 9.94%).@*CONCLUSION@#Releasing lateral patellofemoral retinaculum with stiletto needle could effectively alleviate the stress concentration of patellofemoral joint and reduce local stress peak value, which it is helpful to improve patellar trajectory and make stress distribution more uniform.

RESUMO

OBJECTIVE@#To construct a dynamic knee joint finite element model based on CT image data and verify the validity of the model. To provide a simulation model and basic data for biomechanical research of the knee joint by further finite element analysis.@*METHODS@#The CT data of a healthy male knee joint was selected. With the help of Mimics 19.0 and Hypermesh 12.0 software, a high simulation finite element model of knee joint was established following steps, including geometric reconstruction, reverse engineering, meshing and material characterization. The dynamic knee flexion model was generated by determining the boundary conditions and torque loading, and the validity of themodel was confirmed. The biomechanical changes of the tibiofemoral and patellofemoral joints under different knee flexion angles were analyzed by applying the loads (500 N) to the finite element model during knee flexion.@*RESULTS@#A finite element model of knee joint was established based on CT images and anatomical characteristics. The model included three-dimensional elements such as bone, ligament, cartilage, meniscus and patellar retinaculum. The different finite element models of knee flexion states were produced by applying different torques after establishing boundary conditions. According to equivalent conditions (knee flexion 30 degrees, quadriceps tendon under 200 N stretch), the peak stress value of patella was 2.209 MPa and the average Mises stress was 1.132 MPa; the peak stress value of femoral trochlear was 1.405 MPa and the average Mises stress was 0.936 MPa. The validity of the model was proved by the difference between the model and previous studies of 1% to 13.5%. Dynamic model loading showed that the Mises stressof tibiofemoral joint decreased with the increase of knee flexion angle, while the Mises stress of patellofemoral joint was positively correlated with knee flexion angle. The Mises stress of cartilage stress planes at different knee flexion angles was significantly different(<0.05).@*CONCLUSION@#The finite element model established in this study is more comprehensive and can effectively simulate the biomechanical characteristics of dynamic knee joint, which provides support for further simulation mechanics researches of the knee joint.

OBJECTIVE@#To evaluate the effects of membrane induced by antibiotic-loaded bone cement in skin grafting for tendon exposed wound healing.@*METHODS@#A total of 10 traumatic patients with tendon exposed wound were admitted to our department between February 2016 and December 2018, including 6 males and 4 females, with a mean age of 34.6 years old (ranged, 19 to 43 years old), and treatment duration ranged from 2 to 6 months. There were 7 cases of traffic accidents, 3 cases of mechanical belt injuries, including 8 cases of lower leg and foot wounds and 2 cases of hand back wounds. These tendons exposed wound were covered by antibiotic-loaded bone cement at the earlier stageto induce the formation of the biomembrane, and then skin grafting were performed on the induced membrane. The survival, appearance, texture, sensation of the skin grafting and healing condition of the wounds were studied.@*RESULTS@#Among the 10 patients, skin graft survived well in 8 patients. Partial skin graft necrosis occurred in 2 patients and cured by dressing.@*CONCLUSION@#Using antibiotic bone cement to seal the wound to form induction membrane followed by skin grafting can effectively repair the tendon exposed wound, which has the characteristics of simple operation and less trauma.

Objective To investigate the efficacy of unicompartmental knee arthroplasty(UKA) on treating elderly patients with knee osteoarthritis in comparison with total knee arthroplasty(TKA),and discuss the indications of the procedure.Methods The clinical data of elderly patients(≥75 years) undergoing UKA in our institute from October 2011 to May 2015 were retrospectively reviewed.Patients treated with UKA were in UKA group.Patients who had no significant difference in the age,gender,BMI (body mass index) and HSS (hospital for special surgery) compared with UKA group and received TKA were selected as TKA group.The outcomes were assessed by HSS and satisfaction score(range,0 ~ 10).Results A total of 29 cases (41 knees) of UKA group were followed up for (30.3 ± 13.6) months (13 ~49 months),29 cases(39 knees) of TKA group.The postoperative HSS(P =0.00),surgery time(P =0.00),perioperative hemorrhage(P =0.00),hospital stay(P =0.04) and satisfaction(P =0.03) of UKA group were all better than those of TKA group.The pain location of the knee (P =0.86) and patellofemoral degeneration (P =0.24) did not affect the postoperative outcomes,11 knees presented with varus > 15° with an average angle of 18.4 ° (15.7 ° ~ 20.1 °),4 knees were with flexion contracture > 15 °,averaging at 18.8 ° (17.7 ° ~ 20.1 °),range of motion measured < 100° in 6 knees with an average range of 79.0° (66.8° ~ 92.1 °).The aforementioned abnormalities were effectively corrected under anesthesia,the range of motion inproved significantly,but there was no significant difference in postoperative HSS among these patients (P =0.85).One case of UKA group had wound dehiscence and superficial infection,recovery occurred after debridement and suturing.Conclusion UKA is effective for elderly patients with satisfactory outcomes,which is superior to TKA with respect to trauma control and preservation of the limb function.Medial patellofemoral degeneration poses no negative effect on postoperative outcomes,medium knee varus (≤20°) and flexion contracture(≤20°) that are corrective under anesthesia among elderly patients might not be considered as contraindications for UKA.

<p><b>OBJECTIVE</b>To compare the effects on joint dysfunction after meniscal suture surgery between rehabilitation training combined with modified-acupuncture and simple rehabilitation training.</p><p><b>METHODS</b>Seventy-one patients with meniscal suture surgery were randomized into an observation group (=36) and a control group (=35). Patients in the observation group received modified-acupuncture combined with rehabilitation training. Acupuncture for 8 weeks were at Zutonggu (BL 65), Shugu (BL 66), Neiting (ST 44), Xiangu (ST 43), Xiaxi (GB 43), Zulinqi (GB 41), Dadu (SP 2), Taibai (SP 3), Xingjian (LR 2), and Taichong (LR 3), once a day for continuous 6 days with 1 day for rest. Patients in the control group received simple rehabilitation training for continuous 8 weeks. The training included quadriceps femoris, range of knee joint motion and motion and limb walking on the affected side. The effect score for meniscus injury after treatment from Japanese Orthopaedics Association (JOA) and visual analogue scale (VAS) score were recorded before and after treatment. The effects were compared in the two groups.</p><p><b>RESULTS</b>After treatment, the VAS and JOA scores were improved in the two groups (all<0.05), with better results in the observation group (both<0.05). The effective rate was 91.7% (33/36) in the observation group, which was better than 80.0% (28/35) in the control group (<0.05).</p><p><b>CONCLUSION</b>Rehabilitation training combined with-acupuncture achieve better effect than simple rehabilitation training for joint dysfunction after meniscal suture surgery.</p>

<p><b>OBJECTIVE</b>To compare the clinical efficacy on the motor impairment of knee joint after tracture operation between the combined therapeutic method of relaxing needling manipulation and exercise therapy and the simple exercise therapy.</p><p><b>METHODS</b>Sixty-four patients after the operation for the fracture of femoral shaft were randomized into a relaxing needling combined with exercise therapy group (group A) and an exercise therapy group (group B), 32 cases in each one. In the group A, the relaxing needling manipulation was applied to the local painful area of knee or the stiff soft tissues. Additionally, the exercise therapy was used in combination. In the group B, the exercise therapy was applied simply. Hospital for special surgery (HSS) pain score, the range of movement (ROM) of knee joint and Lysholm score were compared before and 60 days after treatment in the patients of the two groups. The efficacy was compared between the two groups.</p><p><b>RESULTS</b>After treatment, HSS pain score, ROM and Lysholm score were all improved in the two groups, presenting the significant differences as compared with those before treatment (all P<0. 05). The results in the group A were better than those in the group B (all P<0. 05). The total effective rate was 96. 9% (31/32) in the group A, which was better than 75. 0% (24/32) in the group B (P<0. 05).</p><p><b>CONCLUSION</b>The combined therapeutic method of relaxing needling manipulation and exercise therapy achieves the significant efficacy on the motor impairment of knee joint after the operation for the fracture of femoral shaft, superior to the simple exercise therapy.</p>

<p><b>BACKGROUND</b>Recently, 1,5-dicaffeoylquinic acid (1,5-DQA), a caffeoylquinic acid derivative isolated from Aster scaber, was found to have neuroprotective effects. However, the protective mechanisms of 1,5-DQA have not yet been clearly identified. The purpose of this study was to explore the protective mechanisms of 1,5-DQA on neuronal culture.</p><p><b>METHODS</b>We investigated the neuroprotective effects of 1,5-DQA against amyloid β(1-42) (Aβ(42))-induced neurotoxicity in primary neuronal culture. To evaluate the neuroprotective effects of 1,5-DQA, primary cultured cortical neurons from neonate rats were pretreated with 1,5-DQA for 2 hours and then treated with 40 µmol/L Aβ(42) for 6 hours. Cell counting kit-8, Hoechst staining and Western blotting were used for detecting the protective mechanism. Comparisons between two groups were evaluated by independent t test, and multiple comparisons were analyzed by one-way analysis of variance (ANOVA).</p><p><b>RESULTS</b>1,5-DQA treated neurons showed increased neuronal cell viability against Aβ(42) toxicity in a concentration-dependent manner, both phosphoinositide 3-kinase (PI3K)/Akt and extracellular regulated protein kinase 1/2 (Erk1/2) were activated by 1,5-DQA with stimulating their upstream tyrosine kinase A (Trk A). However, the neuroprotective effects of 1,5-DQA were blocked by LY294002, a PI3K inhibitor, but not by PD98059, an inhibitor of mitogen-activated protein kinase kinase. Furthermore, 1,5-DQA's anti-apoptotic potential was related to the enhanced inactivating phosphorylation of glycogen synthase kinase 3β (GSK3β) and the modulation of expression of apoptosis-related protein Bcl-2/Bax.</p><p><b>CONCLUSION</b>These results suggest that 1,5-DQA prevents Aβ(42)-induced neurotoxicity through the activation of PI3K/Akt followed by the stimulation of Trk A, then the inhibition of GSK3β as well as the modulation of Bcl-2/Bax.</p>

<p><b>OBJECTIVE</b>To compare the clinical effect on motor impairment after elbow operation between electroacupuncture combined with rehabilitation and rehabilitation training.</p><p><b>METHODS</b>Fifty-three cases were randomized into an electroacupuncture combined with rehabilitation group (group A, 30 cases) and a rehabilitation group (group B, 23 cases). In group A, electroacupuncture was applied on the stiff region of the elbow and Sanjian (LI 3), Shousanli (LI 10), Hegu (LI 4), Yanglingquan (GB 34), Yinlingquan (SP 9) as adjunctive acupoints. Additionally, the passive movement was followed in the elbow joint after needles removed. In group B, the constant passive movement exercise was applied. The therapeutic effect was observed after 1.5 months of treatment. Mayo Elbow Performance Score (Mayo) and range of motion (ROM) were analyzed statistically after treatment in two groups.</p><p><b>RESULTS</b>Mayo score and elbow ROM were all improved significantly after treatment in two groups (all P < 0.01). Compared with group B after treatment, the total Mayo score, joint motion range and ability as well as elbow ROM were all improved much apparently in group A (all P < 0.01).</p><p><b>CONCLUSION</b>Compared with rehabilitation therapy, electroacupuncture in combination of rehabilitation training can achieve much significant effect on the motion function recovery of elbow joint after fracture operation.</p>

<p><b>OBJECTIVE</b>To investigate the clinical characteristics and the prevalence of mitochondrial gene A3243G mutation in diabetic pedigrees.</p><p><b>METHODS</b>Nineteen suspected mitochondrial DNA diabetic family members from three families were recruited. The gene fragment was amplified by PCR, and mutation was detected by direct sequencing.</p><p><b>RESULTS</b>In three pedigrees, the three probands and their mothers were found carrying the most common nt3243A>G mutation. Most of diabetic patients in these families were deaf and diabetes was developed at early age, characterized by impaired beta cell function and low body mass index (BMI).</p><p><b>CONCLUSION</b>The mitochondrial gene A3243G mutation may cause diabetes mellitus and deaf.</p>

<p><b>OBJECTIVE</b>To observe clinical therapeutic effects of electroacupuncture, radiation of infrared rays plus passive exercise on movement disorders of shoulder joint after operation of fracture of surgical neck of humerous.</p><p><b>METHODS</b>Sixty cases were randomly divided into an acupuncture plus exercise group (n = 32) and an exercise therapy group (n = 28). The acupuncture plus exercise group were treated with electroacupuncture and infrared ray radiation at rigid soft tissue on the affected side, with adjuvant points, Xuehai (SP 10), Sanyinjiao (SP 6) and Yang-lingquan (GB 34) selected for acupuncture treatment and after the needles were withdrawn passive exercise of the shoulder joints were made. The exercise therapy group were treated by active exercise after passive exercise of shoulder joint. Therapeutic effects were observed after treatment for one month, and shoulder pain scores VAS and scores of activity of shoulder joint before and after treatment were observed.</p><p><b>RESULTS</b>The total effective rate of 84.4% in the acupuncture plus exercise group was better than 64.3% in the exercise group. Before and after treatment, the VAS scores were 5.8 +/- 0.5 and 3.1 +/- 0.3 in the acupuncture plus exercise group, and 5.7 +/- 0.5, 4.3 +/- 0.4 in the exercise therapy group, respectively, with significant decreases after treatment in the two groups (both P < 0.001), the acupuncture plus exercise group being more significantly decreased than the exercise therapy group (P < 0.001). Before and after treatment, the total scores of activity of shoulder joint were 103.38 +/- 11.10 and 193.61 +/- 10.21 in the acupuncture plus exercise group, and 103.01 +/- 10.01 and 133.45 +/- 9.81 in the exercise therapy group, respectively, with significant increases after treatment in the two groups (both P < 0. 01), the acupuncture plus exercise group being better than the exercise therapy group (P < 0.001).</p><p><b>CONCLUSION</b>Electroacupuncture, radiation of infrared rays plus passive exercise has obvious therapeutic effect on movement disorders of shoulder joint after operation of fracture of surgical neck of humerous.</p>

<p><b>OBJECTIVE</b>To study the prevalence rate of rheumatoid arthritis in Zhuang nationality population of Nanning, Guangxi.</p><p><b>METHODS</b>A total of 7407 people with Zhuang nationality, aged 16 and over were surveyed. Questionnaire was administered to each subject under survey, while X-ray and serum rheumatoid factor were carried out to all those who gave positive answers. Physical examinations were done to the suspicious cases by experts on rheumatism.</p><p><b>RESULTS</b>were compared with those in 6826 people of Han ethnicity living in the same district.</p><p><b>RESULTS</b>The prevalence rates of rheumatoid arthritis in Zhuang nationality population was 0.27% when comparing to the Han population which was 0.28% (P > 0.05). Rates of awareness on rheumatoid arthritis in Zhuang and Han population were 5.0% and 10.5% (P > 0.05). After the diagnosis of rheumatoid arthritis was made and among patients who had received the treatment, the rates were 0% vs. 5.25%.</p><p><b>CONCLUSION</b>The prevalence rate of rheumatoid arthritis in Zhuang nationality population of Nanning, Guangxi was not significantly different than that in Han ethnic group. However, the rates on awareness and the treatment of rheumatoid arthritis were still under satisfaction.</p>

Novel solid lipid nanoparticle (SLN) system is prepared with Compritol ATO 888 and tricaprylic glyceride. DSC, XRD, SAXS and NMR are employed to study the novel carrier property and microstructure. When the peak melting point decreased from 70.8 degrees C to 61.4 degrees C, the enthalpy sharply decreased. It could be concluded that the regular crystal lattices in the novel carriers are broken out for the oil joined in them. Melting behavior is occurred at -17.7 degrees C while novel SLN is composed of oil and solid lipid mixture from the DSC measurement. Most alpha phase and least beta' phase are in the nano carrier system whether drug loading or not from the XRD investigation. There is only 0.1 nm change of long space among the novel SLN made of mixture and the lipid matrix and traditional SLN; therefore, it is impossible of the oil molecular insert into the solid glyceride structure. Since the different melting behavior (DSC measurements) and molecular move state (NMR investigations), two lipid matrix are still in two state of liquid and solid lipid in the novel SLN carrier. Presume the microstructure of the novel SLN prepared by our experiment would be that liquid oil has formed superfine nano accommodation encapsulated with solid lipid, but the whole particle is still in nano size range.

RESUMO

BSA liposomes were prepared with approximately 100 nm mean particle size under rather gentle experiment conditions, and two-colorimetric coomassie brilliant blue protein was employed to measure the free drug in the entrapped efficiency (EE%) determination of BSA liposomes. Gel filtration was used to measure the EE%, and several Sephadex gels were examined by the separation of liposomes and free drug. To determine the free drug, three methods were compared on two-colorimetric UV spectrophotography, Bradford and two-colorimetric coomassie brilliant blue, separately. Two-colorimetric coomassie brilliant blue process increased the accuracy and improved the sensitivity of the assay about 20-fold comparing with the Bradford method. Two-colorimetric coomassie brilliant blue assay appeared to be more sensitive and showed broader dynamic range to measure the free BSA in the EE% determination of BSA liposome.

<p><b>OBJECTIVE</b>To probe into the effect of acupuncture in imaging diagnosis on changing pyloric antrum and duodenal bulb metamorphosis, and provide reliable evidence for confirming pathological characters.</p><p><b>METHODS</b>Among 30 cases needing image examination, 18 cases had metamorphosis of duodenal bulb and 12 cases had metamorphosis of pyloric antrum. Acupuncture was given at Zusanli (ST 36) with uniform reinforcing-reducing method, and with moderate-strong stimulation. The needle was retained for 2 min. After pressing point Zhongwan (CV 12), changes of image were investigated.</p><p><b>RESULTS</b>After acupuncture, the metamorphosis alleviated in 26 cases, with an effective rate of 86.7%.</p><p><b>CONCLUSION</b>This method is simple, convenient and objective with no adverse effects.</p>

Objective To observe the influence of rotenone on the distribution of alpha-synuclein (ASN) in rat model of Parkinson's disease (PD). Methods Wistar rats were randomly divided into two groups and received 2 mg/kg rotenone (s.c.) or sunflower oil (as control group) for about 4 weeks. The hippocampus, substantia nigra and striatum of brain were observed. Hematoxylin and eosin stain were used to observe the Lewy body like inclusion. The expression of tyrosine hydroxylase (TH) or ASN protein was determined by anti-TH or anti-alpha-synuclein immunohistochemistry, respectively. Results In control rats, ASN protein distributed widely in brain, especially in hippocampus, cortex and striatum. Rotenone obviously increased TH positive neurons and fibers loss in substantia nigra and striatum (P < 0.05). In rotenone treated rats, ASN positive cells increased in global brain but not distributed in an even manner. In substantia nigra, ASN positive stuff was found aggregate in both cytoplasm and nucleus, and some formed spherical inclusion; in striatum, ASN positive neurites end aggregated and agglomerated around neurons; and in hippocampus, few dot-like ASN were aggregated in cell body, and no notable change was found in nucleus. Conclusion In rotenone administrated PD rats, ASN protein aggregated in several brain regions but most obviously in striatum and substantia nigra, and the distribution region of ASN was changed from peri-synapse to the cytoplasm and nucleus of dopaminergic neuron.

RESUMO

In this study, the general bacterial probe and specific cellulolytic bacterial probes were used to quantify the bacteria in rumen. The total RNA were extracted and then hybridized with general bacterial probe after a dilution of concentration. The result showed that there was a high correlation between the hybridization signal and the dilution of total bacterial RNA. Based on the result above, the quantities of three cellulolytic bacteria in rumen sample were detected. The comparative RNA percentage of three cellulolytic bacteria to total bacterial RNA were similar to the previous reports. It can be concluded that the quantification of bacteria in rumen could be conducted by this approach, and which could be used in future research.
