RESUMO

Abstract@#Vertebral column defects can have adverse effects on the physical and mental health of children and adolescents. Early screening and diagnosis is conducive to the correction of the disease, which can effectively slow down or stop the process of abnormal spinal curvature in children and adolescents. However, in terms of cost-effectiveness, there are different opinions around the world on the inclusion of school screening for abnormal spinal curvature into routine health services. This article reviews the concept and classification of vertebral column defects in children and adolescents, the epidemic situation, screening methods and significance by consulting the relevant literature at home and abroad, so as to provide reference for the prevention and treatment of the disease.

Objective:To evaluate the changes in subfoveal choroidal thickness (SFCT) in amblyopic eyes.Methods:A evidence-based medicine study. Chinese and English as search terms for amblyopia and choroid was used to search literature in Wanfang, CNKI, and PubMed of National Library of Medicine. Incomplete or irrelevant literature and review literature were excluded. The literature was meta-analyzed using STATA 15.0. The weighted mean difference (WMD) and 95% confidence interval ( CI) were selected as the estimated value of effect size, and subgroup analysis and sensitivity analysis were used to detect the source of heterogeneity. Results:According to the search strategy, 75 articles were initially retrieved, and 15 articles were finally included for meta-analysis. A total of 650 patients with amblyopia, aged 3 to 65 years old, were included. The enhanced depth imaging technology of spectral domain optical coherence tomography was used to measure SFCT. The results of meta analysis showed that SFCT of amblyopic eyes was more effective than the contralateral eye (WMD=18.89 μm, 95% CI 14.81-22.98 μm, P<0.001) and normal eyes were thicken (WMD=39.49 μm, 95% CI 33.88-45.09 μm, P<0.001). There was no statistically significant difference in SFCT between anisometropic and strabismic amblyopia eyes (WMD=-5.03 μm, 95% CI -19.50-9.44 μm, P=0.495). Conclusions:The SFCT of amblyopic eyes in amblyopic patients is thicker than that of the contralateral eye and normal eyes. There is no difference in SFCT between anisometropia and strabismus amblyopia.

Objective@#To investigate the relationship between childhood abuse experience and non-suicidal self-injury in middle school students, and gender differences of the relationship.@*Methods@#Stratified cluster sampling method was used to select 14 500 middle school students from Shenzhen, Zhengzhou, Nanchang and Guiyang as participants. Questionnaire survey was conducted to collect information on demographic indicators, childhood abuse experiences and non-suicidal self-injury, etc. Logistic regression was used to analyze the relationship between childhood abuse experience and non-suicidal self-injury, and gender differences.@*Results@#The prevalence of NSSI was 9.5%,while it was higher among male students (10.7%) than female students(8.3%) (χ2=22.77,P<0.01). The rate of NSSI was higher with the increasing exposure of childhood abuse. High levels of multiple forms of childhood abuse had independent effects on NSSI among total samples after controlling for confounding factors (P<0.01). In middle school students with high levels of sexual abuse, emotional neglect and childhood abuse, females versus males had higher risk to have NSSI (P<0.01).@*Conclusion@#Childhood abuse experiences were associated with increased risk of NSSI among middle school students, especially among girls. Groups at highest risk of NSSI in relation to certain types of childhood abuse should be targeted with interventions accordingly.

Objective@#The aim of this paper is to understand the associations between childhood abuse experience,parent-child relationship and non-suicidal self-injury (NSSI) in middle school students.@*Methods@#Stratified cluster sampling method was used to select 14 500 middle school students from Shenzhen,Zhengzhou,Nanchang and Guiyang as participants. Questionnaire survey was conducted to collect information on childhood abuse exposure,parent-child relationship and NSSI.@*Results@#The prevalence of NSSI was 14.81%,while it was higher among male students (15.95%) than female students (13.64%) (χ2=15.29,P<0.01). A multivariate Logistic regression models showed that the moderate and high level of childhood abuse (OR=2.23,3.27) and the low level of parents-child relationship (OR=1.80,1.36) were associated with high NSSI detection rate among middle school students (P<0.01). In each group of childhood abuse,low level of father-child relationship was associated with high NSSI detection rate among middle school students (OR=1.36,1.34,1.56,P<0.05). In the group with high level of childhood abuse experience,low level of mother-child relationship was associated with high NSSI detection rate (OR=1.20,P<0.05). Among male students,those with high level of childhood abuse experience,low level of parentchild relationship were associated with high NSSI detection rate (OR=1.46,1.35,P<0.01); regardless of the level of childhood abuse experience,low level of father-daughter relationship was associated with NSSI detection rate (OR=1.64,1.54,1.68,P<0.01) among females.@*Conclusion@#Childhood abuse experience and poor parent-child relationship are associated with NSSI in middle school students, with distinct gender difference. Prevention of childhood abuse and the improvement of parent-child relationship are beneficial to the prevention and control of NSSI.

Objective@#To observe the conditions of the eighth costal cartilage in ear reconstructed patients, and accomplish individualized aesthetic helix fabrication according to the texture and features of the cartilage.@*Methods@#From May 2013 to May 2016, the conditions of the eighth costal cartilages in 415 patients who received ear reconstruction with autogenous costal cartilage in our hospital were analyzed, during which, the length, width, flexibility, and plasticity that were closely related with the reconstruction of the helix attracted predominant attentions. During the surgery, based on the features of the above-mentioned aspects, 4 different methods were used to achieve reasonable length and strength of the helix as well as natural jointing between the helix and ear lobe.@*Results@#Four possible methods of using the eighth costal cartilage to reconstruct the helix were summarized and an autogenous costal cartilage framework that was individualized and with natural appearance was obtained.@*Conclutions@#According to the conditions of the eighth costal cartilage, a more satisfactory appearance of the reconstructed ear than ever before is obtained by choosing relevant helix fabrication programs.

Objective To evaluate the efficacy and safety of oral propranolol versus atenolol in the treatment of infantile hemangioma(IH). Methods A total of 75 infants with IH aged 5-24 weeks were randomly divided into two groups: propranolol group(n = 30)orally administrating propranolol 2 mg · kg?1 · d?1 in 3 divided doses daily for 24 consecutive weeks, atenolol group(n=45)orally administrating atenolol 1 mg · kg?1 · d?1 once a day for 24 consecutive weeks. After 1?, 4?, 12?, 24?week treatment, the infants with IH were followed and adverse reactions were recorded. In addition, the activity of IH was assessed by hemangioma activity score(HAS)before and after 24?week treatment, and changes of HAS were compared between the propranolol group and atenolol group. Results There was no significant difference in the proportion of patients experiencing satisfactory regression of hemangioma between the propranolol group and atenolol group(70%[21/30]vs. 75.6%[34/45], P>0.05). Treatment failure occurred in one patient in the propranolol group because of severe airway hyperreactivity, and in another patient in the atenolol group because of drug resistance. The incidence rates of gastrointestinal reactions, central nervous system adverse effects, chills on the extremities and bronchiolitis complicated by airway hyperreactivity were all significantly higher in the propranolol group than in the atenolol group(all P<0.05). None of hypotension, hypoglycemia and bradycardia occurred in the two groups. Conclusion Compared with propranolol, atenolol shows similar efficacy but less adverse effects in the treatment of IH.

BACKGROUND:There is no effective treatment for muscle atrophy caused by peripheral nerve damage. Skeletal muscle cel s, a structural unit of muscle contraction, can be used for studies on muscle atrophy when cultured in vitro. OBJECTIVE:To investigate the promotion effect of neuron-secreted factors on the growth of skeletal muscle cel s in vitro. METHODS:Skeletal muscle cel s primary cultured in vitro were divided into two groups:experimental group with neuron-secreted factors, and control group with common culture medium, respectively. Afterwards, the number of skeletal muscle cel s and expression level of alpha actin were detected by hematoxylin-eosin staining and immunohistochemical staining, respectively. RESULTS AND CONCLUSION:The number of skeletal muscle cel s and expression level of alpha actin in the experimental group were significantly higher than those in the control group (P<0.05). In conclusion, neuron-secreted factors have the ability of promoting the growth of skeletal muscle cel s and may be helpful for denervated muscle atrophy.

Objective To establish a rabbit model of lumbar laminectomy and bone grains replantation and provide experimental evidence for the clinical application.Methods Eighteen healthy male New Zealand rabbits were selected and randomly divided into two groups:the control group (n=6) and experimental group (n=12).The rabbits of control group were given general anesthesia, and taking the L5 spinous process as the center to perfom left L5 laminectomy, using a micro lancet forceps to slowly bite the lamina and ligamentum flavum for fenestration and exposed to an approximately 0.8 cm x 0.3 cm sized bone window and then sutured the skin.The rabbits of experimental group were exposed to an approximately 0.8 x 0.3 cm sized bone window as well, and bone fragments were cut into small grains.Then the small bone grains were embedded in medical collagen sponge, to form an arch shape, and replanted them to the site of epidural fenestration.CT scan and histological changes were observed at 4, 8 and 12 weeks after operation.Results At 8 weeks after operation, CT examination showed that in the experimental group, a thin bone plate was formed by the bone grains.At 12 weeks after operation, the bone plate became thicker and was connected with the vertebral bone, and with continuous bone trabeculae.The spinal canal and volume were not obviously changed, and no spinal cord compression was observed. The rabbits of control group showed segment lamina defects, a small scar protruding into the spinal canal, and the vertebral canal was not completely reconstructed. Conclusions The bone grains replantation can effectively promote bone reconstruction in the laminectomized rabbits, and the formed bone plate can prevent epidural scar from intruding into the spinal canal, and can reduce the postlaminectomy adhesion.

Objective To investigate the distribution of high pathogenicity island(HPI)in multiple-drug-resistance gram-negative bacilli and analyze the protein sequence.Methods To amplify thefyuA-irp2 gene cluster of the 84 isolates by multiple polymerase chain reaction(PCR),the product was subsequently sequenced.Results The positive rate ofirpl,irp2,irp3,irp4 and fyuA was 40.48%,41,67%,5.95%,O%and 16.67%,respectively.Theamino sequence offyuA comefromEC06748,Kp7151 and PAE7 was usedto compare with AL590842,there are 100%identities.Amino sequence ofirp2 come from Kp49 and Kp51 have 99%identities with AAA27636.1,but amino sequence of irp2 come from EC04 and EC07 only have 90%identities with 1176840.The GenBank accession number is FJ211852 and FJ211851.Amino sequence ofirpl come fromKp 10,Kp49 and Kp51 have 99%identities with AL590842。and amino sequence ofirp3 come from EC03,Kp51,Kp10 and Kp49 have 97%identities with CAA73128.There are the same mutation among the same species,and different mutation among different species.Conclusion There was different extant mutant lost in thefy~t-i,v2 gene cluster in multiple-drug-resistanee gram-negative bacilli.

BACKGROUND: A lot of researches have proved that polyglicolide acid (PGA), as a stent material, has been successfully used to construct engineered tissues, such as cartilage, bone and tendons, in nude mice or even big mammal. Whether the incubation of vascular endothelial cells and smooth muscle cells in the polyglicolide acid may subcutaneously construct vessel-like structure in nude mice needs a further study.OBJECTIVE: To verify the feasibility of forming vessel-like structure in the nude mice by subcutaneous implantation of polyglicolide acid cocultured with vascular endothelial cells and smooth muscle cells derived from newborn umbilical vein.DESIGN: Contrast study.SETTING: Tissue Engineering Key Laboratory, Medical College, Shanghai Jiao Tong University.MATERIALS: This study was performed at Tissue Engineering Key Laboratory, Medical College, Shanghai Jiao Tong University from January to June 2002. Belly band was derived from newborn babies in our department of obstetrics & gynecology. The parturien provided the informed consent, and this study was approved by the local research ethics committee. Twenty-six nude mice (3-4 weeks old, clean grade, irrespective of gender) were selected in this study. The animal experiment received confirmed consent from the local ethic committee. Polyglicolide acid was provided by Albany International Research Co.METHODS: Vascular endothelial cells and smooth muscle cells derived from newborn umbilical vein were incubated on a piece of polyglicolide acid to produce cell-material compound. In addition, the compound covered around the silicone tube to form a tube-like structural substance. Subsequently, the tube-like structural substance was subcutaneously implanted in 20 nude mice, which were regarded as an experimental group. And then, polyglicolide acid alone was subcutaneously transplanted in the rest 6 nude mice, which was regarded as a control group.MAIN OUTCOME MEASURES: Gross observations of cell-material compound by 2 and 6 weeks after transplantation;HE staining and immunohistochemical staining.detection; expression of factor Ⅷ and α-smooth muscle actin.RESULTS: Twenty-six nude mice were included in the final analysis. ① Gross observation: At 2 weeks after implantation,both the experimental and control groups formed tubular structures, however, at 6 weeks after implantation, the tubular structure still remained in experimental group but not in the controls. ② Histological observation and immunohistochemical detection: The histological examination of the engineered vessel showed that at 2 weeks, the vessels in both group contained mainly undegraded PGA fibers, while at 6 weeks, the PGA fibers were almost completely degraded in both groups and in the control group only fibrous-like tissue formed. Contrastly, in experimental group a typical vascular structure formed, Masson's trichrome stain, which stains collagen green, smooth muscle fibers red and cells purple, showed significant amounts of stainable collagen and smooth muscle fibers in the wall of the engineered vessel, furthermore,immunohistochemistry examination revealed that there were an endothelial cell layer formed in the inner surface of the engineered vessel which was confirmed by positive staining of yon Willebrand factor, meanwhile, the smooth muscle cells in the wall of the engineered vessel were confirmed by the positive staining of smooth muscle α-actin.CONCLUSION: The subcutaneous implantation of polyglicolide acid cocultured with vascular endothelial cells and smooth muscle cells may form vessels, which are similar to normal vascular histological structure.

RESUMO

Objective To explore the method and result of endoscopic thyroidectomy. Methods Thyroidectomy was performed endoscopically in 4 cases of thyroidoma. Results 4 patients underwent endoscopic thyroidectomy, whose operation time was 52,63,70,75 minutes respectively. An average blood loss during operation was 35ml and no complication occurred. They were discharged 4 days~5 days after operation. Conclusions The endoscopic thyroidectomy has the advantages of minimal invasion, less bleeding, less complication and quicker recovery.
