RESUMO

Objective:To establish a global health talent competency index system in China as a scientific basis for improving the global health talent team construction.Methods:Such keywords as global health, competence, global competence and public health talent competence, were used to search for literature published from 2012 to 2022 in the China National Knowledge Infrastructure database. Through literature research and job responsibility analysis and the competency iceberg theory model, the authors established the initial Index set. In October 2022, the expert consultation method was used to identify indexes, with their weight evaluated. Based on the results of expert consultation, a survey questionnaire was developed to evaluate the importance of global health talent competency indexes. A questionnaire survey was conducted in November 2022 through random convenient sampling to select relevant personnel from domestic medical institutions, universities, and pharmaceutical enterprises, and exploratory factor analysis and regression analysis on questionnaire data were made to explore the internal structure, weights and quantitative relationships between indexes of the index system.Results:The global health talent competency index system consisted of 8 level-1 indexes and 28 level-2 indexes. The 8 level-1 indexes were professional accumulation, foreign language communication, international exchange, self-management and growth, influence, achievement drive, globalization literacy, and professionalism. The weights of the level-1 indexes were 0.110, 0.104, 0.153, 0.121, 0.106, 0.171, 0.139, and 0.096, respectively. Knowledge and skill related competencies included professional accumulation and foreign language communication; Behavioral competencies included international communication, self-management and growth, and influence; Personality traits and competencies included internal drive by achievements, global literacy, and professionalism. The regression analysis results showed that all the level-2 indexes, except for creativity, team collaboration, and independent thinking, had a significant predictive effect on their level-1 indexes ( P<0.01). Conclusions:The global health talent competency index system established in this study proves scientific, and practical for the selection, training, and evaluation of global health talents.

RESUMO

Objective:To determine whether subpopulations may exist which are related to regulation of immunization or inflammation in bone marrow mononuclear cells (BMMNCs) from 2 elderly patients with hip fracture and whether there might be any difference in the subpopulations between them.Methods:Two elderly patients with hip fracture were enrolled in this study. Their venous blood was harvested to determine subpopulations of complement (C)3, C4, interleukins (IL)-2, IL-6, IL-10, and lymphocytes. Single cell RNA sequencing (scRNA-Seq) was used to group their BMMNCs. Gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) analyses were performed of the top 20 differentially expressed genes for each subpopulation to judge the main function of each subpopulation. The subpopulations and the key genes related to regulation of immunization or inflammation were found out. The relationships were explored between subpopulations and prognosis of the patients.Results:By the venous blood indexes, IL-10 was slightly high in patient A; C3 below normal, C4 close to the lower normal limit, IL-2, IL-6 and IL-10 were significantly high, CD8 +T % was low, and CD4 +/CD8 + high in patient B. After scRNA-Seq and bioinformatics analyses, the BMMNCs in the 2 patients were divided into 5 subpopulations. GO and KEGG enrichment analyses showed that the functions of subpopulation 2 and subpopulation 4 were related to immunization or inflammation. CCL4, CCL5, LTB and CXCR4 in subpopulation 2 and C1QA, C1QB, CD14 and SPP1 in subpopulation 4 were related to the regulation of immunization or inflammation. The final prognosis of patient A was much better. The proportions of BMMNCs involved in subpopulation 2 and subpopulation 4 from patient A were higher than those from patient B [47.00% (1,431/3,045) versus 29.28% (882/3,012); 5.88% (179/3,045) versus 3.85% (116/3,012)]. Conclusions:The BMMNCs from elderly patients with hip fracture can be divided into subpopulations by scRNA-Seq. Some of the subpopulations may be related to regulation of immunization or inflammation, which may affect the post-injury immune inflammatory state and prognosis of the patients.

The outbreak of COVID-19 has spread quickly across 114 countries/territories/areas in six continents worldwide and has been announced as a pandemic by WHO. This study analyzed global COVID-19 epidemiological trends, examined impact of the pandemic on global health security, diplomacy, and social environment in China, and provided short- and long-term strategic policy recommendations for China’s subsequent preparedness and responses.

Objective Through investigating the clinicians’ scientific research level and weakness,we can carry out targeted clinical research training systematically,and then strengthen their utilization of clinical resources and data,and finally promote the transformation of scientific research resuits.Methods Filling and submitting the online questionnaires through WeChat,a popular chatting tool in China,physicians from 13 affiliated hospitals of a university in Shanghai have joined this research.Results 507 valid questionnaires have been received online:In terms of clinical research capacity,there seemed to be more barriers in the following issues:comprehension of the types and applications to design a clinical trial,formulation of the details including PICO elements(Patient-Intervention-Comparison-Outcome),methodological application including the category and principle of randomization,the category and principle of blind method and how to control the confounding elements and probable bias,and statistical problems in scientific research including discrimination of the different definition between FAS(Full Analysis Set) based on the principle of ITT(Intention-To-Treat) and PPS(Per-Protocol Set),how to write a standardized SAP(Statistical Analysis Plan) and how to calculate the sample size of a trial),and various management of clinical trials including data management,follow-up management,adverse event management and so on,and writing a protocol and a CRF (Case Report Form) in a standard and professional way.As for the needs for training,the top three topics were how to design a clinical research in a standardized style,how to practice the methodology and how to utilize the statistical skills into clinical trials.Conclusions Standardized design and writing of clinical trial protocols according to the CONSORT (Consolidated Standards of Reporting Trials) and SPIRIT (Standard Protocol Items:Recommendations for Interventional Trials),and the application of epidemiological and statistical methods are still short boards that most clinicians need to improve urgently,also are what they most concerned about at the same time.So it is necessary for physicians to receive systematic clinical research training to enhance their scientific research capacity.

RESUMO

OBJECTIVE: To observe the therapeutic effects of the conventional therapy plus Melitracen and Flupentixol in the treatment of functional dyspepsia(FD).METHODS: 320 FD cases were divided into gastrointestinal dyskinesis type(Type A,n=120 cases),ulceration type(Type B,n=100 cases),and nonspecific type(Type C,n=100 cases) based on their clinical symptoms.Patients of different clinical types were randomly assigned to receive conventional therapy alone or in combination with Melitracen and Flupentixol for 4 weeks.The curative effects were observed using gastrointestinal symptom rating scale(GSRS) and the total effective rate were computed.RESULTS: After treatment,the GSRS score of 3 different clinical types of FD in the combination therapy group versus the conventional therapy alone were 1.53?1.68 vs.3.10?1.78,2.02?1.63 vs.3.48?1.80 and 3.46?2.95 vs.5.18?3.21 respectively(P
