RESUMO

ObjectiveTo establish the quality standard of Liangditang benchmark samples. MethodUltra performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-QTOF-MS) was used to qualitatively analyze the chemical composition of Liangditang on the basis of molecular and fragment ion peak information with cracking law. The mobile phase was methanol (A)-0.05% phosphate aqueous solution (B) for gradient elution (0-10 min, 5%-23.5%A; 10-20 min, 23.5%A; 20-58 min, 23.5%-63%A; 58-60 min, 63%-90%A), the flow rate was 0.8 mL·min-1, and the detection wavelength was 254 nm. Electrospray ionization was employed under positive ion mode, the detection range was m/z 100-1 700. Key quality attributes and sources were determined by comparing with single medicine and reference substances. Through mass transfer analysis of multiple batches from decoction pieces to benchmark samples, high performance liquid chromatography (HPLC) for determining the contents of index components and HPLC detection of characteristic maps were established. Through the determination of 15 batches of benchmark samples, the content range of the index components and the common peaks of the characteristic map were determined. Thin layer chromatography (TLC) was applied to the identification of 5 medicines in the formula. Moisture and dry extract yield of the benchmark samples were determined by drying method. ResultA total of 27 compounds were inferred from the benchmark samples of Liangditang, among which 9 compounds were confirmed by comparison with the control, including catalpol, harpagide, gallic acid, albiflorin, paeoniflorin, verbascoside, angoroside C, cinnamic acid and harpagoside. A method for determining the characteristic maps of the benchmark samples were established and 13 peaks were assigned, and the characteristic peaks were mainly derived from wine-processed products of Rehmanniae Radix, Scrophulariae Radix and wine-processed products of Paeoniae Radix Alba. The similarity between the characteristic map of 15 batches of benchmark samples and the control characteristic map was >0.9. Methods for the determination of paeoniflorin, harpagoside, L-hydroxyproline and glycine were established, and the contents of these four components in 15 batches of benchmark samples were within ±30% of the corresponding mean value, and the transfer rate of decoction pieces to the benchmark samples was stable and controllable. TLC was established to identify 5 prescription drugs (except Ejiao) with two kinds of test solutions, and the results showed that the method had good specificity. The average dry extract yield was 48.06%, and the average moisture was 5.58%, which were within the range of ±10% and ±30% of their mean values, respectively. ConclusionThe quality standard of Liangditang benchmark samples was as follows:the similarity between the benchmark samples and the control characteristic map is >0.9, the contents of paeoniflorin, harpagoside, L-hydroxyproline and glycine are 217-403, 24-46, 634-1 178, 1 253-2 328 mg per dose, the dry extract yield is 43.0%-53.0%, the moisture is 4.0%-7.0%, under the set detection conditions, the benchmark samples have corresponding characteristic spots by comparing with the control herbs of 5 medicines. This quality standard is stable and reliable, which fills the gap in the quality control of Liangditang, and can provide a reference for the establishment of the quality standard of Liangditang granules.

To evaluate the correlation of the serum uric acid and free fatty acid (FFA) levels in Shandong coastal residents. To investigate the correlation between serum uric acid and FFA based on 3860 individuals who have been long staying in Qingdao, Yantai, Weihai, Rizhao with a randomized, stratified cluster sampling method. According to FFA quartile, subjects were divided into four groups: group Q1 of 908, group Q21016, group Q3958, and group Q4978 cases. The prevalence of hyperuricemia and serum uric acid levels increased with the increasing FFA quartile. Compared with Q1, Q2, and Q3 groups, the prevalence of hyperuricemia in Q4 group and the increase of serum uric acid were statistically significant(P<0. 05). And in the group Q4, hyperuricemia prevalence is twice as the group A. According to the serum uric acid level, subjects were divided into the normal uric acid group(n=3331) and the hyperuricemia group ( n = 529). In the hyperuricemia group, their systolic blood pressure, diastolic blood pressure, waist circumference, hip circumference, triglycerides, total cholesterol, low density lipoprotein-cholesterol (LDL-C), glucose, uric acid, FFA, body mass index etc. were significantly higher than those of the normal uric acid group (all P<0. 01), while the high density lipoprotein-cholesterol ( HDL-C), cystatin, glomerular filtration rate (eGFR) are significantly lower than those of the normal uric acid group( all P<0. 01). Serum uric acid levels are positively correlated with systolic and diastolic blood pressures, waist and hip circumferences, triglycerides, total cholesterol, LDL-C, FFA, blood glucose, body mass index (all P<0. 01); and negatively correlated with eGFR (P<0. 01). Multiple regression analysis showed that systolic blood pressure, FFA, total cholesterol, triglycerides, LDL-C, blood glucose, body mass index, eGFR were factors influencing serum uric acid independently. Multivariate binary logistic regression analysis showed that systolic blood pressure, waist circumference, total cholesterol, blood glucose, and FFA are independent risk factors to predict hyperuricemia onset while eGFR is a protective factor. Serum uric acid level is closely related to the free fatty acid, and FFA seems to be involved in the development and progression of hyperuricemia.

<p><b>OBJECTIVE</b>To explore the therapeutic effect of enhancer of Zeste homolog 2 (EZH2) inhibitor GSK343 on periodontitis by regulating microphage differentiation.</p><p><b>METHODS</b>Macrophage RAW264.7 cells were divided into the blank (A group), control (B group), lipopolysaccharide (LPS) stimulation (C group), and LPS+GSK343 (D group) groups. Phenotype transformations was determined through Western blot analysis and enzyme-linked immunosorbent assay by detecting the differentiation of phenotypic biological markers, including tumor necrosis factor-α (TNF-α), inducible nitric oxide synthase (iNOS), interleukin-10 (IL-10), and Arginase-1 (Arg-1). Metergasis was identified by performing a phagocytosis test on Escherichia coli (E. coli).</p><p><b>RESULTS</b>Macrophage RAW264.7 cells produced classical phenotypic biomarkers (M1) TNF-α and iNOS under LPS stimulation. The expression levels of IL-10 and Arg-1 increased after adding GSK343 into the culture medium. GSK343 also induced the conversion of M1 macrophages into M2 macrophages. Macrophage RAW264.7 cells exerted a phagocytic effect on E. coli, and this effect was enhanced after adding LPS into the culture medium. GSK343 regulated the macrophage RAW264.7 phagocytosis of E. coli.</p><p><b>CONCLUSIONS</b>GSK343 possibly participates in the regulation of macrophage differentiation and, consequently, in the latent treatment of periodontitis.</p>

Objective To observe the relationship between serum lipid concentration profiles and thyroid function after thyroid hormone withdrawal( THW) in patients with differentiated thyroid carcinoma( DTC) .Methods Sixty-five post-operative DTC patients who prepared to receive radioiodine ablation were included in this study.Serum thyroid hormones ( TSH, FT4 , FT3 ) and lipid profile [ total cholesterol ( TC ) , triglycerides ( TG ) , low-density lipoprotein-cholesterol(LDL-C), and high-density lipoprotein-cholesterol(HDL-C)] levels were measured before and 3 weeks after THW.The relationship between serum lipid profile concentrations and thyroid hormone levels were measured.Results Before surgery, HDL-C level showed a significant positive correlation with TSH level(r=0.273, P=0.041), whereas other lipid profile did not show any significant correlation with thyroid hormones levels.Before THW, no correlation was observed between thyroid hormones and lipid profile.After 3 weeks′THW, the TC level was correlated with FT4 and FT3 levels(r were-0.321 and-0.415, respectively, both P0.05).Conclusion The serum lipid profile varies with the change of serum thyroid hormones after THW in postoperative DTC patients.

BACKGROUND:Fibroin is a natural macromolecular material with Arg-Gly-Asp peptide structure that is a special tripeptide structure closely related to cel adhesion, and it can promote cel migration, adhesion, and proliferation and influence cel morphology and function. OBJECTIVE:To compare the effects of different silk fibroin scaffolds to repair buccal mucosa defects in rats. METHODS:Ninety Sprague-Dawley rats were selected to make unilateral buccal mucosa defect models, and randomly divided into three groups, 30 rats in each group: porous silk fibroin scaffold was implanted into the buccal mucosa defect in experimental group, multi-layered crosslinked silk fibroin film was implanted into the buccal mucosa defect in control group, and vaseline gauze was used to cover the buccal mucosa defect folowed by suturing in blank control group. After 15 days, wound diameter was detected; after 30 days, bone defect tissues were taken for hematoxylin-eosin staining. RESULTS AND CONCLUSION:At postoperative 15 days, the wound diameter was significantly smaler in the experimental group than the control and blank control groups (P < 0.05), as wel as smaler in the control group than the blank control group (P < 0.05). Hematoxylin-eosin staining showed that at 30 days after operation, there were more epithelial spikes and fibroblasts, but less inflammatory cels in the experimental group than the other two groups (P < 0.05), and fibroin fibers were partialy absorbed and degraded in the experimental group. These findings indicate that porous silk fibroin scaffold for buccal mucosa defect repair can accelerate epithelialization and wound healing.

Objective To investigate the association between SLCO1B1/ApoE gene polymorphisms and lipid-lowering efficacy and safety of rosuvastatin.Methods DNA samples were extracted from blood using nano paramagnetic particle method.The SLCO1B1 521T>C and ApoE gene polymorphisms were screened by PCR-pyrophosphate sequencing method.Totally 152 patients received rosuvastatin orally at a dose of 10 mg/d.The lipidlowering efficacy was evaluated through detecting serum low-density lipoprotein cholesterol (LDL-C) level before and 8 weeks after the treatment.The incidence of myopathic adverse effect was assessed by follow-up of the occurrence of myalgia.Results The gene distribution of SLCO1B1 521T>C was 73.7%,23.7% and 2.6% respectively for TT,TC and CC in 152 patients,and the distribution of ApoE gene was 65.8%,13.2% and 21.0% respectively for ε3/ε3,ε3/ε2 and ε4/ε3.The genotype ε4/ε4,ε2/ε2 and ε4/ε2 were not detected.After orally receiving rosuvastatin 10 mg daily for 8 weeks,the decreased LDL-C levels showed significant differences (P<0.05) among ApoE genotype ε3/ε2,ε3/ε3 and ε4/ε3 groups,and the frequencies of myalgia showed significant differences in the three genotype groups of SLCO1B1 521T>C (P<0.05).Conclusion The gene polymorphism of SLCO1B1/ApoE was correlated with efficacy and safety of rosuvastatin.The combined detection of SLCO1B1/ApoE genes can be utilized to predict efficacy and risk,and then realize individualized medication.

BACKGROUND:The sensitivity andmucus secretion of theoral mucosamake oral soft tissues difficult to repair, so patients cannot achieve satisfactory outcomes after treatment. Nano-celulose protein mainly composed of glycine, alanine and serine has good histocompati bility. However, there is a lack of comparative study about the effect of nano-celulose protein and acelular matrix in oral mucosa repair, and the clinical effects of the two materials are stil under discussion. OBJECTIVE:To investigate the effects of nano-celulose proteinver susacelular matrix in oral mucosa repair. METHODS:Oral mucosadefect models were preparedinrats, and these rat models were randomly divided into four groups:oral mucosa defectswere repaired by vaseline (control group), nano-celulose protein, bovine skin acelular matrix and human skin acelular matrix, respectively. Repair effects were compared among different materials within 2 months after surgery. RESULTS AND CONCLUSION:The diameter of oral mucosa defect measured using a vernier caliper, had no significant differences among groups at 1 day after surgery (P> 0.05); the diameter of oral mucosa defect in the nano-celulose protein group was significantly lower than that in the other groups at 3, 5 and 7 days after surgery (P 0.05);but there were significant differences in the number of newborn capilary endothelialcels between the control group and the other three groups (P< 0.05). Furthermore, at 21 days after surgery, closely aligned and thicker new epithelialtissuecould be found in the nano-celulose protein group; in the bovine acelular matrix group,thedefect regionwasrepaired wel, new epithelialtissueappeared andthe number of inflammatory cels decreased; in the human acelular matrix group, inflammatory cels appearedobviously, and new epithelialtissueformed with the normal thickness. In contrast,abundant inflammatory cels and thinner epithelial tissues appeared in the control group. To conclude,bothnano-celulose protein and acelular matrix can accelerate wound healing by promoting oral mucosal epithelial hyperplasia.

Objective To observe the ameloblast morphology and ultrastructure changes of dental fluorosis in rats.Methods Forty SD rats were divided into experimental group and control group (20 rats in each group) by body weight using random number table method.Rats in experimental group were given drinking water with 50 mg/L fluoride to establish dental fluorosis model,and rats in control group were given drinking water without fluoride.All rats were killed at the 56th day,then ameloblast morphology and ultrastructure were observed by means of HE staining and transmission electron microscopy (TEM) at secretory,transition,maturation and post-maturation stages.Results Rats in experimental group showed typical symptoms of dental fluorosis,and lower incisors presented brown and white stripes on enamel surface,chalk color patches and other typical dental fluorosis changes,rats in control group no abnormality.HE staining results showed that differences were not observed between the two groups at the secretory and transformation stages,and few distortions and interstitial space widened were found in the experimental group at maturation and post-maturation stages.TEM displayed ultrastructure changes of ameloblasts,and no differences were observed between the two groups at secretory and transformation stages.Apoptosis was observed in some ameloblasts at the maturation stage in the experimental group,such as rough endoplasmic reticulum expansion,swelling,some ribosome particles exfoliating from endoplasmic reticulum,mitochondrial swelling and disappearance of crista,some nuclear membrane broken down,chromatin condensation and karyolysis.At post-maturation stage,intercellular space of the experimental group was wider than that of the control group.Conclusions Morphological and ultrastructure damage of ameloblasts may have occurred at the maturation stage in the process of dental fluorosis.Ameloblasts may be more sensitive to fluoride in dental enamel formation at maturation stage.

[Abstract] Objective To study the effects of resveratrol on chemokines CCR5,CXCL10 of serum and synovial fluid in acute gouty arthritis of rats.Methods According to random number table,36 Wista rats were divided into six groups:i.e.control group,model group,colchicine group,high dose resveratrol group (HR),medium dose resveratrol group (MR),and low dose resveratrol group (LR).The rats were respectively administrated with normal saline,colchicine suspension,high dose resveratrol,medium dose resveratrol,low dose resveratrol for 7 days,once a day.Rats acute gouty arthritis modles were estalished on the fourth day by monosodium urate solution (concentration of 25 mg/ml,0.05 ml) injected into the ankle cavity,while 0.05 ml normal saline was injected into joint cavity of rats in control group.2,9,24,48,72 hours after model estabilishment,blood samples were collected from inner canthus to detect levels of interleukin (IL)-1β,CCR5,CXCL10.72 hours after model estabilishment,synovial fluid were collected from ankel to detect levels of IL-1β,CCR5,CXCL10.The joint swelling index was observed periodically.Results The levels of IL-1 β in the low,medium,and high doses of resveratrol were (9.63 ± 0.71),(7.67 ± 0.48),and (6.66 ± 0.29) pg/ml respectively,being significantly lower than that in the model group (11.85 ± 0.45) pg/ml (P<0.05) in joint fluid.The levels of CCR5 in the medium and high doses of resveratrol groups were(4.49 ± 0.25) and (4.65 ± 0.25) ng/ml respectively,being significantly lower than that in the model group (5.64 ± 0.28) ng/ml (P<0.05).The levels of CXCL10 in the medium and high doses of resveratrol groups were significantly lower than that in the model group [(86.79 ± 2.90) and (89.61 ± 5.47) vs (110.13 ± 4.02) pg/ml,P< 0.05].The histopathologic results showed that resveratrol might reduce acute gouty arthritis joint edema,improve inflammatory cells infiltrating.Conclusions The levels of IL-1β,CCR5,and CXCL10 were raised in acute gouty arthritis.Resveratrol effectively controls acute gouty arthritis attack,and the curative effect is dose dependent.

Objective To determine the prevalence and risk factors of hyperuricemia (HUA) and gout among residents aged≥20 years in Shandong coastal areas compared with the results of 2004. Methods A randomized stratified cluster sampling was conducted, and 5 191 inhabitants were investigated for prevalences of HUA and gout in Qingdao, Yantai, Weihai, Rizhao, and Dongying. Results (1)The level of serum uric acid had the tendency to increase in both males and females in 2009 compared with that in 2004. (2)According to the uniform population standardization, the standardized prevalence of HUA was 16.99%in 2009. The standardized rate increased by 3.72%compared to 2004, being 4.57%in males and 3.33%in females. (3) The standardized prevalence of gout was 1.36%. Compared with that in 2004, the standardized rate increased by 0.26%, decreased by 0.45%in males while increased by 0.85%in females. (4) Compared to 2004, the prevalence of HUA among males aged≤30 years was the highest, whereas aged between 50 years and 60 years was the lowest at present. (5) Currently, the prevalence of HUA among females aged≤30 years was significantly higher than that of 2004, and aged≥70 years was the highest. (6) Logistic regression analysis showed that the amount of daily intake of seafood, meat, beer, and liquor were independent risk factors of HUA and gout. Conclusions Compared to 2004, the level of serum uric acid and the prevalence of HUA and gout show a tendency to increase among younger in the population surveyed in 2009, significant in female. Dietary factors exert significant effects on the development of HUA and gout. The amount of daily intake of seafood, meat, beer, and liquor are independent risk factors of HUA and gout.

Objective To determinate the risk factors of gout in patients with hyperuricemia.Methods Patients detected with hyperuricemia both in epidemiological survey of Shandong coastal areas in 2004 and in health examination of our hospital were followed up for three years to observe the incidence of gout, relationship of diet and gout, and changes of biochemical indicators.Results During 3 years, 102 patients (19%) out of 536 patients with hyperuricemia developed gout. Age(OR=1.046, P<0.05), serum uric acid(OR=1.021, P<0.05), fasting plasma glucose(OR=1.021, P<0.05), triglyceride(OR=1.008, P<0.05), tony crab intake ( OR=5.992, P<0.05),and beer intake(OR=1.012, P<0.05) were the risk factors of gout attack in patients with hyperuricemia.Conclusions Excess intake of tony crab and beer resulting in fluctuation of serum uric acid is the main risk factor of gout in patients with hyperuricemia. Correcting metabolic disorder of glucose and lipid, reducing the intake of high-purine food, and controlling the level of serum uric acid are the measures to reduce gout attack.

BACKGROUND: Resin filling, full crown and veneer are used in cosmetic restoration of anterior teeth. Doctors prefer resin because of its perfect clinical effect and small hurt. Some patients complain of sensitivity after rsein filling and we are interest in choosing a better resin with less sensitivity.OBJECTIVE: To compare the incidence of postoperative sensitivity and the difference of the patients' self sensitivity alter cosmetic restoration of anterior teeth with vital pulp by 3M-Z350 Nano-resin and 3M-Z100 composite resin. METHODS: A total of 112 teeth with vital pulp because of caries or traumatic tooth fractures of 68 patients demanding cosmetic restoration were selected from the Department of Endodontics, Stomatological Hospital of Luzhou Medical College from August to December 2006. The teeth were random divided into 3M-Z350 group and 3M-Z100 group, with 56 teeth in each group. 3M-Z350 nano-rasin and 3M-Z100 composite resin were used for cosmetic restoration of anterior teeth with vital pulp. Ryge criteria were used to evaluate the sensitivity to the cold stimulation following cosmetic restoration, and the level of pain was detected by the pain ruler.RESULTS AND CONCLUSION: Compared with 3M-Z100 group, the incidence of sensitivity and self-pain level were decreased in the 3M-Z350 group at 1 day, 1 week, and 1 month after restoration (P < 0.01 or P < 0.05). There was no significant difference in the sensitivity and self-pain level between the two groups (P > 0.05). Anterior teeth with vital pulp were well restored by 3M-Z350 nano-resin which might reduce the incidence of postoperative sensitivity.

BACKGROUND: It has been demonstrated that the C-106T polymorphism at promotor region of aldose reductase gene and GLU298ASP (894G→T) polymorphism at exon 7 of endothelial nitric oxide synthase (eNOS) gene are associated with diabetic nephropathy, it should be further studied wnether the risk for diabetic nephropathy will increase when the above polymorphic sites coexist.OBJECTIVE: To investigate the association between the coexistence of the polymorphisms of both the C-106T at promotor region of aldose reductase gene and GLU298ASP (894G→T) at axon 7 of eNOS gene in chromosome 7q35 region and the genesis of diabetic nephropathy in northern Chinese Hah people.DESIGN: A case-controlled, comparative observation.SETTING: Department of Endocrinology, the Affiliated Hospital of Medical College, Qingdao University.PARTTCIPANTS; Totally 139 inpatients with type 2 diabetes mellitus were selected from the Department of Endocrinology, the Affiliated Hospital of Medical College, Qingdao University from November 2002 to April 2005,including 54 males and 85 females, (64±8) years of age. Inclusive criteria: Accorded with the standards of diabetic diagnosis and classification by WHO in 1999. According to the 24-hour urinary albumin excretion rate (UAER), they were divided into diabetic nephropathy group (n =61) and non-diabetic nephropathy group (n =78). Meanwhile, 63 healthy controls were selected as the control group, including 24 males and 39 females, 50-78 years of age. All the enrolled subjects were Han people. Informed contents were obtained from all the subjects.METHODS: The genotypes and alleles of polymorphisms of GLU298ASP(894G→T) at exon 7 of eNOS gene and C-106T at promotor region of aldose reductase gene were detected by polymerase chain reaction restriction-fragment length polymorphism technique (PCR-RFLP), DNA sequencing technique and agarose gel electrophoresis separation technique.Then the frequency of genotypes and alleles were compared.MAIN OUTCOME MEASURES: ① Polymorphisms of eNOS gene and aldose reductase gene; ② Results of the multivariant stepwise regression analysis of risk factors for diabetic nephropathy; ③ Polymorphisms of aldose reductase C-106T and eNOS 894G→T and the relative risk of diabetic nephropathy.RESULTS: All the 139 patients with type 2 diabetes mellitus and 63 healthy adults were involved in the analysis of results. ① The frequencies of the T allele and TG genotype at exon 7 894G→T polymorphic site of eNOS gene in the diabetic nephropathy group were significantly higher than those in the non-diabetic nephropathy group and control group (χ2=8.261, 19.629, P < 0.05). ② The frequencies of the T allele and CT genotype at promotor region of aldose reductase gene in the diabetic nephropathy group were significantly higher than those in the non-diabetic nephropathy group and control group (χ2=6.343, 8.940, P < 0.05, 0.01). ③ After associated analysis on the above gene polymorphisms, it was found that the frequency of TG/CT genotype in the diabetic nephropathy group were significantly higher than that in the non-diabetic nephropathy group and control group (χ2=6.972, P < 0.01); whereas the frequency of GG/CC genotype was significantly lower than that in the non-diabetic nephropathy group and control group (χ2=13.304, P < 0.05). ④Glycosylated hemoglobin (GHbA1c), systolic blood pressure, total cholesterol, polymorphisms of 894G→T at exon 7 of eNOS gene and C-106T at promotor region of aldose reductase gene were all the independent risk factors for diabetic nephropathy (Wald =5.627, 4.92, P < 0.05). ⑤ GG/GC genotype might be the protective genotype for diabetic nephropathy (OR=0.25, P < 0.01); The risk for diabetic nephropathy in the carrier of GG/CT or TG/CC genotype increased by 2.3 times and risk for diabetic nephropathy in the carrier of TG/CT genotype increased by 4.8 times.CONCLUSION: The coexistence of polymorphisms of 894G→T at exon 7 of eNOS gene and C-106T at promotor region of aldose reductase gene can increase the risk of diabetic nephropathy in patients with type 2 diabetes mellitus. And the TG/CT haplotype formed by the coexistence of polymorphism of these two genes is probably the predisposing genotype for diabetic nephropathy.

BACKGROUND: Recent studies have found that hyperuricemia and gout are closely correlated with the occurrence and development of diabetes, coronary heart disease, hypertension and cerebrovascular diseases. It is of significance to investigate their prevalence so as to find way of early interventions.OBJECTIVE: To determine the prevalence and risk factors of gout and hyperuricemia among residents above 20 years old in Shandong coastal area.DESIGN: A randomized, stratified cluster sampling survey.SETTING: Department of Endocrinology, Affiliated Hospital of Qingdao University Medical College.PARTICIPANTS: A random, stratified cluster sampling was conducted in Shandong coastal area including Qingdao,Yantai, Weihai, Rizhao and Dongying. Residents lived in these areas for 5 years or more, aged between 20 to 80 years, were selected, and they were surveyed by family as a unit.METHODS: A randomized, stratified cluster sampling survey was conducted. The prevalence of gout and hyperuricemia were investigated among about 5 000 residents in Qingdao, Yantai, Weihai, Rizhao and Dongying. The serum uric acid, lipids, glucose and creatinine were detected with Sysmex chemix-180 total automatic biochemical analyzer. Those with uric acid higher than reference level were reexamined by collecting fasting blood sample on the third day. The comparison between rates was taken with the Chi-square test, means between two groups with the t test, means between multiple groups with analysis of variance, correlation between dependent and independent variables with logistic regression analysis.MATN OUTCOME MEASURES: Prevalance of hyperuricemia; Level of serum uric acid; Prevalence of gout in patients with hyperuricemia; Influencing factor of hyperuricemia.RESULTS: This investigation planned to include 5 500 subjects while in fact 5 003 subjects were investigated and the response rate was 91%, in which males were 2 395 (47.87%) and females were 2 608 (52.13%). ① The prevalence of hyperuricemia was 13.19% with standardized rate of 13.27% according to the Shandong population in 2000; The pevalence was higher in males than in females (18.32%, 8.56%, x2=108.52, P< 0.01). The risk in males was 2.5 times higher than that in females (OR =2.5). The prevalence of gout was 1.14% with standardized rate of 1.10%; and the prevalence in males was higher than that in females (1.94%, 0.42%, x2=30.38, P < 0.01). The risk in males was 5.3 times higher than that in females (OR =5.3). ②The average value of serum uric acid in normal males was higher than in normal females [(343.40±84.54), (258.90±70.90) μmol/L, t =48.03, P < 0.01]. It was obviously higher in male patients with hyperuricemia than in female ones [(469.43±48.08), (399.73±104.91) μmol/L, t =11.70, P < 0.01]. It was higher in male patients with gout than in female ones [(502.44±106.76), (403.48±52.72) μmol/L, t =2.07, P < 0.05]. ③The prevalence of gout in patients with hyperuricemia was 8.34%. ④ The prevalence of hyperuricemia and gout were climbing up with age after 40 years old in females and those elder than 70 years old were of high risk; while in males,the prevalence of hyperuricemia and gout increased with age before 60 years old and those aged 50-59 years were of high risk, yet after 60 years, it climbed up with age again. Nevertheless, the mean ages of hyperuricemia and gout in females were older than male. The average episode ages of hyperuricemia and gout in females were later than in males respectively by 7.5 and 8.5 years. ⑤ Non-conditional Logistic regression analysis showed that drinking frequency,drinking quantity, the quantity and frequency of seashell intake, BUN, Cr, TG, TC, BMI and WHR were the independent risk factors of male patients with hyperuricemia [OR =1.016-30.217, 95%C/ (1.010-1.023)-(9.955-214.869)]; while HDL-C and heavy physical labour were the protective factors (OR =0.492, 95%C/ 0.339-0.713; OR =0.755, 95% CI 0.575-0.991).As for females, age, hypertension, the quantity of seashell food intake, BUN, Cr, TG, WHR and light physical labour were the independent risk factors of hyperuricemia [OR =1.022-27.34, 95%CI (1.006-1.040)-(9.955-214.869)]. Similarly, HDL-C was a protective factor (OR =0.428, 95%CI0.223-0.820).CONCLUSION: ① The prevalence of hyperuricemia and gout are different between genders: ② The risk factors of hyperuricemia and gout among residents in Shandong coastal area include the high intake of marine products such as seashell foods, less physical activity, abdominal obesity and kidney insufficiency, as well as the existence of metabolic syndrome. Drinking is also involved in the increased prevalence in males, and age in females. ③ Higher risk for hyperuricemia and gout are noticed in all age groups in males, whereas in females after 50 years old.

OBJECTIVE: To study the therapeutic effects of Chaiqin Chengqi Decoction (CQCQD) in treating severe acute biliary pancreatitis. METHODS: Ninety patients with severe acute biliary pancreatitis were treated with CQCQD, and they were divided into two groups: early-treated group (54 patients treated with CQCQD within 3 days after the onset of severe acute biliary pancreatitis) and late-treated group (36 patients treated with CQCQD between 3 and 7 days after the onset of severe acute biliary pancreatitis). The complication incidence rate, operation rate, mortality rate and hospitalization period were examined. RESULTS: The incidence rates of encephalopathy, infection and gastrointestinal hemorrhage were lower in the early-treated group than those in the late-treated group (P<0.05). The hospitalization periods of the early- and late-treated groups were (24.9+/-18.4) days and (51.6+/-45.9) days respectively (P<0.05). The general mortality rate was 14.4%. The mortality rate of the early-treated group (7.4%) was significantly lower as compared with that of the late-treated group (25.0%) (P<0.05). The operation rate of the early-treated group (11.1%) was also significantly lower as compared with that of the late-treated group (27.8%) (P<0.05). CONCLUSION: Treating severe acute biliary pancreatitis with CQCQD in early stage may reduce the complication incidence rate, shorten the hospitalization period, and decrease the operation rate and mortality rate.

OBJECTIVE: To investigate the occurring mechanism and clinical characteristics of severe acute pancreatitis (SAP) associated with hypoalbuminemia in early stage and its influence on prognosis of SAP and the preventive and therapeutic management of this disease. METHODS: One hundred and thirty-eight cases diagnosed as SAP complicated by hypoalbuminemia in early stage were accepted in our hospital from August 1, 2003 to December 31, 2004, and they were divided into 2 groups according to the level of plasma albumin: mild hypoalbuminemia (30 to 35 g/L) group and severe hypoalbuminemia (0.05). The incidence rate of infection and the mortality in the severe hypoalbuminemia group were higher than those in the mild hypoalbuminemia group (P<0.01) in the later stage of SAP. CONCLUSION: Hypoalbuminemia in the early stage can accelerate the deterioration in pathophysiology of SAP. The lower level of the plasma albumin is in the early stage, the more complications and the higher incidence rate of infection and mortality will be in the later stage. To relieve the extent of systemic inflammatory response syndrome (SIRS) and abundant supplement of albumin, amino acid and lipid in time may be crucial to prevent the occurrence and deterioration of hypoalbuminemia.

RESUMO

Objective:To observe the effect of thrombolysis in treating acute deep vein thrombosis (DVT) in the lower limbs.Methods:For the 14 patients diagnosed by colour Doppler ultrasonographic examination,intravenous drip of urokinase,dextran and mailuoning was ministered alternatively through the superficial venosus dorsalis pedis in the involved side.Meanwhile,oral intake of aspirin and dipyridamole was also prescribed.Results:Ten patients healed and four significantly improved.Conclusion:The efficacy of thrombolysis for treatment of DVT in the lower limbs is valid.This intervention is especially suitable for those patients who missed the optimum time window of operation.
