RESUMO

Objective:To investigate the effect of SCN1A gene polymorphism (SCN1A-rs3812718) on the alterations of spontaneous brain activity using amplitude of low-frequency fluctuations (ALFF) of MR in patients with temporal lobe epilepsy (TLE).Methods:A total of 37 TLE patients (TLE group) admitted to the Epilepsy Center of the 900th Hospital of Joint Logistic Team from March 2018 to August 2019 were retrospectively analyzed, and another 28 healthy volunteers matched for gender, age, and years of education with the TLE group were selected as the healthy control group (HC group). Sixty-five subjects were divided into four groups by genotype and diagnosis: 34 cases in AA/AG-TLE subgroup, 3 cases in GG-TLE subgroup, 20 cases in AA/AG-HC subgroup and 8 cases in GG-HC subgroup. All subjects underwent sagittal 3D-T 1WI and resting-state functional MRI using a Siemens 3.0 T Trio Tim MR scanner. Then ALFF values of the four groups were calculated using DPABI by the MATLAB 2010 platform. The ALFF values between two groups were compared by independent samples t-test. The ALFF values of different genotypes at rs3812718 locus in TLE and HC group were analyzed by multivariate analysis of variance to find out the corresponding brain regions with interaction, and then post hoc simple effect analysis was performed. Results:The ALFF values in TLE group significantly increased in left marginal lobe, left parahippocampal gyrus, left fusiform gyrus, left hippocampus, right insular lobe and right inferior temporal gyrus (Alphasim corrected P<0.001) and decreased in the left superior frontal gyrus, left middle frontal gyrus, left inferior frontal gyrus, right middle frontal gyrus, right precuneus, left precuneus, bilateral cingulate gyrus and right angular gyrus (Alphasim correction P<0.05) compared with HC group. Subjects carrying the non-risk G allele had higher ALFF values in the right inferior temporal gyrus, right fusiform gyrus, and right cerebellum than subjects carrying the risk A allele ( t=3.30, Alphasim corrected P=0.002). There was a significant interaction effect on posterior cerebellar lobe, left anterior cerebellar lobe, left inferior temporal gyrus, left superior parietal lobule and right precuneus of TLE patients with SCN1A-rs3812718 genotype. Post-hoc simple effect analysis showed that ALFF significantly increased in the left posterior cerebellar lobe, left anterior cerebellar lobe, left inferior temporal gyrus and left fusiform gyrus in GG-TLE subgroup ( t=5.97, P<0.001), but significantly decreased in the right superior parietal lobule, right precuneus, right posterior cerebellar lobe in AA/AG-TLE subgroup compared to the HC group. Compared with GG-TLE subgroup, ALFF in left posterior cerebellar lobe, left fusiform gyrus and left inferior temporal gyrus decreased in AA/AG-TLE subgroup. Conclusion:SCN1A gene polymorphism in the rs3812718 locus affects spontaneous neural activity in resting state, which may be one of the pathophysiological mechanisms of TLE.

RESUMO

Objective Magnetic resonance spectroscopy(MRS)was applied to analyze the metabolism of the frontal lobe and caudate nucleus of rats underwent severe burn stress ,so as to improve the understanding of early brain damage caused by severe burn. Methods A total of 35 healthy male Sprague-dawley(SD)rats weighted 250 to 300 g were randomly divided into 7 groups, including the control group and 3,6,12,18,24 and 48 hour burn group post-burn stress, with 5 rats in each group. The modified standard rat burn model proposed by Walker and Manson was established with second-degree burn wound covering 20 percent of body surface. The rats in each group were examined by MRI at corresponding time points to observe change of MRS. The potential change of Cho/Cr and NAA/Cr of bilateral frontal lobe and caudate nucleus were recorded. Results (1)Compared with the con-trol group, the NAA/Cr of post-burn rats were significantly reduced, while Cho/Cr was significantly increased(P<0.01,respectively).(2)Significant differences in NAA/Cr,Cho/Cr values were observed among different sampling time points(P < 0.01,respectively).(3)The NAA/Cr value of the frontal lobe was significantly lower than cau-date nucleus (P < 0.01). The Cho/Cr value of frontal lobe was significantly higher than caudate nucleus (P <0.01). The NAA/Cr value of the left and right sides of brain were similar ,and the Cho/Cr value of the right brain was significantly higher than the, left side(P = 0.018). Conclusions MRS can detect the abnormality of burn stress-induced rat brain damage in the early stage ,with certain regularity.

RESUMO

Objective To explore early aberrant change of the function and grey matter volume within default-mode network in patients with type 2 diabetes mellitus (T2DM) by Independent component analysis and voxel-based morphometry. Methods Twenty-nine T2DM patients and thirty-three healthy control subjects were recruited in this study between May 2014 and January 2015,and the demographic and clinical data (fasting plasma glucose and HbA1c level) were obtained.General cognition was assessed by MMSE and MoCA in all subjects.Resting-state fMRI and T1-weighted imaging were performed in all subjects. Then the z-maps of default-mode network were obtained from Resting-state fMRI by independent component analysis, and the grey matter volumes were extracted by VBM.Finally,two-sample t-test was performed in the function and grey matter volume of DMN between two groups, and pearson correlation coefficients among zFC values extracted from significantly different brain regions, grey matter volumes and durations of T2DM patients were calculated. Results Patients with T2DM showed decreased Z value around the right anterior cingulated cortex and the right medial pre-frontal cortex(cluster=121,t=-3.81,P< 0.05).There was no significant difference in grey matter volume between the two groups, and no correlation between Z value or duration and grey matter volume was found. Conclusions It suggests the abnormality of connectivity of default-mode network may precede the abnormality of grey matter volume in type 2 diabetes mellitus patients. The DMN might be a new potential tool in monitoring the change of cognition in patients with type 2 diabetes mellitus.

RESUMO

Objective To compare the diagnostic value of DWI and ultrasound elastography(UE) in staging hepatic fibrosis in patients with chronic viral hepatitis B(CHB). Methods Fifteen healthy volunteers (control group) and 69 patients with CHB (disease duration more than 1 year) (patient group) were prospectively recruited. All of the subjects underwent DWI and UE experiments twice with the interval of less than 3 days. Liver ADC and shear wave velocity(SWV) values were obtained for subsequent analysis. Sixty?nine patients who had biopsy were grouped according to their pathological grading of fibrosis, from S0 to S4. One?way ANOVA was used to compare the ADC and SWV values between the five different fibrosis groups and control group. Spearman correlation analysis was used to analyze the correlations between the ADC and SWV values and those staging factors. Finally, receiver operating characteristic (ROC) curve analysis was performed to compare the diagnostic performance of ADC and SWV values in discriminating different stages of hepatic fibrosis. Results Based on the pathological results, the 69 patients were reclassified to five subgroups with 11, 13, 12, 15 and 18 patients in the S0, S1, S2, S3 and S4 groups respectively. ADC values were(1.39±0.09)×10-3,(1.39±0.08)×10-3,(1.38±0.10)×10-3,(1.20±0.06)×10-3,(1.12±0.07)×10-3 and(1.01±0.07)×10-3mm2/s for the control group and stages S0 to S4 respectively.SWV values were(1.17±0.07),(1.16±0.08),(1.23±0.10),(1.48±0.14),(1.85±0.14)and(1.97±0.12)m/s for stages S0 to S4 and the control group respectively. Statistically significant differences were observed among them (P<0.01). Both ADC (r=-0.894,P<0.01) and SWV (r=0.904,P<0.01) values were highly correlated with the stages of liver fibrosis. The area under ROC(AUC) for predicting fibrosis stages (≥S1, ≥S2, ≥S3 and S4) with ADC values was 0.893, 0.991, 0.966 and 0.952 respectively. Accordingly, the AUC for SWV values were 0.937, 0.993, 0.994 and 0.914. Conclusions The two imaging methods of DWI and UE showed good and similar diagnostic performance in discriminating the different stages of hepatic fibrosis.

RESUMO

Objective To investigate the diagnostic value of ADC histogram obtained from MR DW imaging for stage ⅠB cervical cancer. Methods Seventy three patients diagnosed by cervical smear screening as cervical cancer without priortreatment were included prospectively in the patient group, and staged according to the international federation of gynecology and obstetrics (FIGO) staging system. Forty three patients with uterine leiomyoma detected by gynecologic examination, ultrasonography or CT and with negative result of cervical smear screening who were scheduled for hysterectomy were included prospectively in the control group. The patients of both groups underwent routine pelvic MR sequences, dynamic contrast enhanced imaging and DWI before hysterectomy. ADC histograms of the entire tumor and cervix volume were generated by post-processing software. Features of ADC histogram for the 2 groups were observed. Histogram parameters such as mean ADC (ADCmean), median ADC (ADCmedian), the 25th percentile of ADC (ADC_25th), the 75th percentile of ADC (ADC_75th), skewness and kurtosis were recorded. Student's t test or Mann-Whitney U test depending on homogeneity of variance was employed for the comparison of those parameters. ROC analysis was employed for assessing the diagnostic performance of ADC histogram in distinguishing the 2 groups. Results Thirty five patients in the patient group were staged as FIGO IB. Five patients in the control group ended up with pathologic findings of cervical intraepithelial neoplasia grade 3. Therefore 38 patients in the control group were investigated. ADC histograms of the patient group were mostly skewed positively, while the curves were largely skewed negatively. ADCmean, ADCmedian, ADC_25th, ADC_75th, skewness and kurtosis for the IB stage patient group were (1.10±0.21)×10-3mm2/s, (1.05±0.21)× 10-3 mm2/s, (0.90 ± 0.19) × 10-3mm2/s, (1.26 ± 0.23) × 10-3mm2/s, 0.83 (median) and 1.25 (median) respectively. ADCmean, ADCmedian, ADC_25th, ADC_75th, skewness and kurtosis for the control group were (1.62 ± 0.25)×10-3mm2/s, (1.64±0.24)×10-3mm2/s, (1.42±0.24)×10-3mm2/s, (1.84±0.27)×10-3mm2/s,-0.11(median) and 0.29 (median) respectively. All parameters showed statistically different (t values were -9.693,- 11.117, -10.255, and -9.988 for ADCmean, ADCmedian, ADC_25th and ADC_75th respectively;Z values were -6.360 and -4.445 for skewness and kurtosis respectively; P< 0.01). ROC analysis indicated that ADCmedian had the highest diagnostic accuracy for differentiating the 2 groups, with the area under the curve being 0.97, a cutoff value of 1.21×10-3mm2/s, and a sensitivity of 95.6%and a specificity of 89.3%. Conclusion ADC histogram of DWI may be valuable for diagnosing stage IB cervical cancer by distinguishing stage IB cervical cancer from normal cervix or cervical benign lesions.

RESUMO

Objective:To explore the effect and molecular mechanism ofSPHK1 in the invasion and metastasis process of non-small-cell lung cancer cells(A549).Methods:Recombinant retrovirus was used to mediate the production ofA549/vector,A549/SPHK1,A549/scramble, andA549/SPHK1/RNAi that stably expressed or silencedSPHK1.The invasion and migration capacities of A549 cells overexpressing or silencingSPHK1 were determined usingTranswell invasion assay and scratch wound repair experiment. The protein and mRNA expression levels ofE-cadherin, fibronectin, vimentin inA549/vector,A549/SPHK1,A549/scramble,A549/SPHK1/RNAi were detected withWestern blot(WB) and quantitativePCR(QPCR) methods, respectively. Results:Transwell invasion assay and scratch wound repair experiments showed that over-expression of SPHK1 obviously enhanced the invasion and migration capacities ofA549 cells.WB andQPCR detection results showed that, the expression ofE-cadherin(a molecular marker of epithelial cells) and fibronectin, vimentin(molecular markers of mesenchymal cells) inA549 cells was upregulated after overexpression ofSPHK1; whileSPHK1 silencing significantly reduced the invasion and metastasis capacities ofA549cells, upregulated the expression of molecular marker of epithelial cells, and downregulated the expression of molecular marker of mesenchymal cells. Conclusions:SPHK1 promotes epithelial mesenchymal transition of non-small-cell lung cancer cells and affects the invasion and metastasis capacities of these cells.

RESUMO

To investigate the clinical characteristics, diagnosis and treatment of intrahepatic biliary cystadenoma,a total of 7 patients with intrahepatic biliary cystadenoma admitted in our department from January 2008 to June 2013 were enrolled and the clinical data (i.e.clinical manifestations, imaging features and surgical treatment) were analyzed retrospectively. All the 7 patients were female and diagnosed as intrahepatic biliary cystadenoma.Five cases complained right upper abdominal pain of various severities.Color Doppler ultrasound,CT and MRI confirmed the diagnosis in all patients. The surgery included irregular fight hepatectomy (n =2),left lateral lobe hepatectomy (n =3),and left hepatectomy (n =2).Patients with intrahepatic biliary cystadenoma usually manifest nonspecific clinical symptoms, and thus are prone to be misdiagnosed as simple hepatic cysts. For such patients, surgery is of first choice, and complete resection is the key to avoiding the recurrence and malignancy.

RESUMO

Objective To observe the brain regions activated during calculation in patients with left temporal lobe epilepsy (TLE) and in normal subjects. Methods Functional magnetic resonance imaging (fMRI) was applied.Sixteen right-handed persons with left TLE were selected as the left TLE group, and sixteen healthy volunteers were recruited as a control group. The fMRI data was collected as each subject performed simple additions, subtractions and abdication subtractions.Statistical parametric mapping was used to compare the activated brain regions between the two groups. Results The error rate in the calculations was significantly higher in the left TLE group, and their average reaction time was significantly longer. There were aslo significant differences between the two groups in terms of brain activation patterns.Compared with the control group, the left TLE group exhibited hypo-activity in regions such as the left paracentral lobule, the posterior central gyrus, the bilateral inferior parietal lobule, the left angular gyrus, the bilateral supramarginal gyrus, the left middle frontal gyrus, the left superior gyrus, and also in the bilateral posterior cingulate, insular lobule, superior and inferior temporal gyrus, right hippocampus, parahippocampal gyrus, thalamus and cerebellum. The TLE subjects exhibited hyper-activity in the bilateral superior parietal lobule, the bilateral anterior cingulate,as well as in the right, middle and inferior frontal gyrus. Conclusion Our results support a significant functional reorganization of calculation-related neuronal networks within and between the hemispheres in TLE patients. The frontal and parietal lobes may play a compensatory role in the reorganization of the calculation function.Task-related fMRI technology can provide useful information for non-invasive assessment of mathematical computing and cognitive function in TLE patients.

RESUMO

Objective To evaluate the efficacy of PET-CT brain imaging and resting-state fMRI in preoperative localization of temporal lobe epileptic (TLE)focus. Methods PET-CT and resting-state fMRI were performed in 17 patients with refractory TLE,who then underwent surgical treatment.Seventeen healthy volunteers matched with gender and age were recruited as the control group. The resting-state fMRI images were post processed by SPM5 software.Regional homogeneity(ReHo) values of the whole brain and bilateral hippocampus were obtained and analyzed.PET-CT images were analyzed by visual analysis method and asymmetry index method and the standardized uptake value (SUV) of bilateral hippocampus were obtained. The ReHo values and SUV of the bilateral hippocampus were compared by two independent samples t-test,and analyzed by receiver operating characteristic curve (ROC) for optimized diagnostic threshold.Pearson correlation analysis was employed for evaluating the correlation between the SUV and ReHo values of bilateral hippocampus. The consistency between the diagnostic accuracy of PET-CT and resting-state fMRI was assessed by Kappa consistency test. The outcome of the patient group was compared with that of the control group, and with the pathological results,to evaluate the diagnostic value of the two modalities for preoperative localization of temporal lobe epileptic focus. Results Regional or comprehensive low metabolism of 18F-FDG in temporal lobes was presented in all 17 patients, and 11 patients out of 17 showed lateral decreased ReHo value. The diagnostic accuracy of the two examinations was 70.6% (12/17) and 64.7% (11/17) for PET-CT and resting-state fMRI respectively compared with pathological results, and could be increased to 76.5% (13/17) when the two methods were combined for diagnosis. The ReHo values of the TLE group (0.34 ± 0.12)were significantly lower than those of the control group (0.46 ± 0.07) (t =3.230,P =0.003).The sensitivity and specificity of resting-state fMRI were 88.2% and 94.1% respectively when the ReHo value was 0.36.There was significant difference between the SUV of the affected (4.17 ±0.63) and healthy side(4.77 ±0.56) of hippocampus in TLE group(t =2.930,P =0.006).The sensitivity and specificity of PET-CT were 88.2% and 64.7% respectively when SUV was 4.23.The two values could be used as a threshold in the localization of temporal lobe epileptic focus.Consistency of lesion detection was revealed between PET-CT and resting-state fMRI though it was not high, and the Kappa value was 0.49.However,no correlation was detected between the SUV and ReHo value using Pearson correlation test(r =0.280,P =0.314).Conclusion Combined PET-CT brain imaging and resting-state fMRI as a multi-modality imaging method might improve the diagnostic accuracy of the TLE focus's localization.

RESUMO

Objective To explore the asymmetry of metabolic products of the auditory cortex in normal human brain after being stimulated by the pure tone with the proton magnetic resonance spectroscopy(1 H-MRS). Methods Twelve healthy volunteers, including 7 males and 5 females, were performed MRS tests before and after the stimulation of the pure tone of 90 dB in intensity and 1000 Hz in frequency. The the metabolites peak of the N-acetylaspartate (NAA) ,creatine(Cr) ,choline( Cho) , glutamine/glutamate ( Glx) and GABA in auditory cortex were investigated, The content of metabolic products were conducted with half-quantitative analysis. The differences of the NAA/( Cho + Cr) ,Glx/Cr and GABA/ Cr ratios before and after pure tone stimulation in the bilateral auditory cortex were analyzed. The difference of distribution of the metabolic products between the left and right auditory cortex was also analyzed to explore the asymmetry of the auditory cortex. Results The NAA/( Cho + Cr) and GABA/Cr ratios increased, the Glx/Cr ratio decreased in the left auditory cortex after being stimulated by the pure tone. There were significant differences in the NAA/( Cho + Cr) , Glx/Cr and GABA/Cr ratios before and after pure tone stimulation (P0.05). There were statistically significant differences in the Glx/Cr ratio of the auditory cortex between two sides after being stimulated by the pure tone. Conclusion The metabolic lateralization exists in auditory cortex of normal human brain after being stimulated by the pure tone, which may be the bases of the functional asymmetry.

RESUMO

Objective To investigate the changes of brain volumes in neuromyelitis optica (NMO)patients using voxel-based morphometry (VBM) method, and preliminarily explore the pattern of cerebral anatomical impairment. Methods Twenty-three clinically defined NMO patients and 15 gender and age matched healthy volunteers underwent 3-dimensional (3D) fast spoiled gradient echo (FSPGR) sequence scanning on 3.0 Tesla MR systen. Raw data was processed and analyzed using statistical parametric mapping (SPM) 5. Whole brain volumes included grey matter volume (GMV), white matter volume (WMV), total intracranial volume (TIV), grey matter fraction (GMF), white matter fraction (WMF),brain tissue fraction (BTF) and regional brain volumes between the two groups were compared by independent samples t-test and an Pearson were performed to compare the regional brain volumes and the ages. Results GMV of NMO group[(610. 2 ± 55.0) ml] was significantly decreased comparing to healthy control group[(657. 2 ± 36. 3) ml] (t = - 2. 915, P < 0. 05). The age of NMO patients [(40 ± 9) years old] showed negative correlation with GMF [(42. 5 ± 2. 6) %] (r = - 0. 673, P < 0. 05). Regional brain volume analysis showed decreased GMV in left insula and bilateral posterior cingutates in NMO patients, while decreased WMV was found in left frontal and left parietal white matter. Conclusion VBM could detect brain volume changes sensitively. Total grey matter volume in NMO patients was decreased comparing to HC group. Regional grey matter atrophy in NMO patients occurred in left insular and bilateral posterior cingutates, regional white matter atrophy occurred in left frontal and left parietal lobe.

RESUMO

Objective To investigate the effects of ribs in high intensity focused ultrasound (HIFU)beam path on formation of coagulative necrosis in cattle liver in vitro in the same acoustic power. Methods The HIFU ablation was carried out in the manner of a single dot in the liver in vitro. The liver was ablated by HIFU in free field, then the ribs were placed between the focal plane and transducer, directing the beam axis through the intercostals space, lied at the rib edge and on the centre of middle rib. For each of these axial arrangements, the plane of the front surface of ribs was placed 3 cm,6 cm or 9 cm in front of the focus. After HIFU exposure, the tissue necrosis rate(TNR),the maximum length, width, depth and position of the coagulated necrosis in the liver were measured. Results ①The TNR was 100% in the free field. When the ribs were placed 3 cm in front of the focal plane, directing the beam axis through the intercostals space, the TNR was 100%. When directing the beam axis lied at the rib edge and on the centre of middle rib,the TNR decreased sharply. When the ribs were placed 6cm and 9cm in front of the focal plane, the TNR was more than 50%. ②When the ribs were placed in HIFU beam path, the volume of necrosis decreased compared with those in free field. The relationship between the ribs and beam axis, the location of ribs placed away from the focal plane different, the volume, length, width and position of the coagulated necrosis different. ③ When the ribs were close to the focal plane, unacceptable damage could been observed on the surface of ribs. When the ribs were far away from the focal plane, the ribs were safe. Conclusions The volume of necrosis decrease when the ribs are placed in HIFU beam path. The relative location of ribs and HIFU beam axis, the location of ribs away from the focal plane are relative with the volume of the coagulated necrosis. When directing the beam axis through the intercostals space, the volume of necrosis is large, and the shape of coagulated necrosis is regulated. When the ribs are closed with the focal plane, ribs are damaged probably.

RESUMO

Objective BOLD-fMRI was used to observe the activated auditory cortex evoked by pure tone stimuli in patients with sensorineural hearing loss and this paper is to discuss the objective measure for patient with sensorineural hearing toss. Methods BOLD-fMRI examinations were taken in 22 patients with unilateral moderate to severe sensorneural hearing loss and 15 control subjects. The volumes and intensities of the two hemispheres of the activated auditory cortex were analyzed quantitatively. Results Significant activation was found in the temporal lobe in control subjects, and significant differences in the volume and intensity were noted between the contralateral and ipsilateral activated auditory cortexes in them (P<0.01), exhibiting clearly eontralateral predominance. When the normal ear of patients with sensorineural hearing loss received signals, there was no significant difference be-tween contralateral and ipsilateral activated auditory cortexes (P>0.05). Conclusion When the normal ear of pa-tients with sensorneural hearing loss was stimulated by pure tone, the contralateral hemisphere predominance disap-peared. This result seems to show the plasticity of auditory cortex of patients with unilateral hearing loss.

RESUMO

Objective To evaluate the rehabilitating effect of hyperbaric oxygen on visual pathway lesions with blood oxygen level dependent functional magnetic resonance imaging (BOLD-fMRI) and diffusion tensor imaging (DTI). Methods Sixteen patients with visual pathway lesions (the study group) and twelve healthy volunteers (the control group) were assessed using BOLD-fMRI and DTI. After hyperbaric oxygen therapy, the patients in the study group were again assessed using BOLD-fMRI and DTI. The activated regions of the BOLD-fMRI scan and the fractional anisotropy (FA) value determined from the DTI were calculated. Results Before hyperbaric oxygen treatment, there were significant differences between control and study groups in their BOLD-fMRI activated regions and the FA values of their radiation optics (P≤0.01). After hyperbaric oxygen treatment, there were no significant differences. Conclusion Combining BOLD-fMRI with DT1 could be used to evaluate the rehabilitation effect of hy-perbaric oxygen treatment in patients with visual pathway lesions.
