RESUMO

Parkinson's disease(PD)is a degenera-tive disease of the central nervous system characterized by the loss of dopamine(DA)neurons in the dense sub-stantia nigra and the depletion of DA neurons.Clinically,the treatment of PD is mainly supplementing dopamine deficiency or using DA receptor agonists,but these drugs can only alleviate the symptoms of PD patients,but cannot prevent neuronal loss and delay disease progres-sion.Natural bioactive polysaccharides have the advan-tages of multi-target,low toxicity and synergistic effect,and have great potential in the prevention and treatment of PD.Numerous studies have shown that polysaccha-rides can be involved in neuronal protection and preven-tion of neurodegenerative diseases through mechanisms such as oxidative stress,reducing neuroinflammation,and inhibiting anti-apoptosis.①Anti-oxidative stress.Oxi-dative stress is caused by increased reactive oxygen spe-cies(ROS)products and weakened antioxidant capacity,resulting in destruction of lipids,proteins,and DNA.Oxi-dative stress-induced mitochondrial dysfunction is thought to be an important cause of DA neuronal loss in PD mice.Polysaccharides reduce the damage of DA neurons in the substantia nigra by increasing the activity of antioxidant enzymes and inhibiting the formation of reactive oxygen species.② Reduce neuroinflammation.Neuroinflamma-tory response is the main causative factor of neurodegener-ation,microglia are innate immune cells present in the central nervous system,and their continuous activation is a key link in central nervous system neuroinflammation.Polysaccharides can regulate the expression of inflamma-somes,reduce the levels of pro-inflammatory cytokines and cytotoxic factors,inhibit the excessive activation of microglia,reduce PD neuroinflammatory damage,and then exert neuroprotective effects.③ Inhibiting apopto-sis.Apoptosis(APO)is the process of cell death caused by the activation of cell death procedures by various fac-tors.During the pathogenesis of PD,due to changes in the internal and external environment of DA neurons,some apoptosis-related genes cause DA neuronal death by regulating cell death signaling pathways.Polysaccha-rides can reduce the Bax/Bcl2 ratio,weaken the activa-tion of caspase-related proteins,improve the viability of PC12 cells,reduce apoptosis,and protect the activity of dopamine neurons.In summary,traditional Chinese med-icine polysaccharides can effectively treat and improve PD,and its mechanism of action involves anti-oxidative stress,reducing neuroinflammation and apoptosis.There-fore,traditional Chinese medicine polysaccharides have great development potential in the field of medicine and health.

RESUMO

Objective:To explore the effectiveness of positive cognitive therapy in improving anxiety and depression, psychological distress, positive awareness, and quality of life in cancer patients, with a view to providing an experience for practical clinical application.Methods:This study was a quasi experimental study, in which 96 patients with cancer radiotherapy attending Shanghai Proton Heavy Ion Center from April 2019 to January 2021 were selected as study subjects by convenience sampling, and were divided into 47 cases in the intervention group and 49 cases in the control group according to the district group randomization method. The control group was given conventional psychological care and health education, and instructed to learn the audio and video of positive cognitive therapy at the end of the study; the intervention group was given 8 sessions of positive cognitive therapy over a period of 4 weeks. At the end of the intervention, the Pdistress Thermometer, the Generalized Anxiety Inventory, the Patient Health Questionnaire Depression Inventory, the Brief Version of the Five Factor Positive Thinking Inventory, and the European Organization for Research and Treatment of Cancer, Quality of Life Questionnaire C30 were used to compare the psychological distress, anxiety and depression, positive cognitive awareness and quality of life of the two groups.Results:After the implementation of the positive cognitive therapy intervention, the incidence rate of anxiety and depression in the intervention group was 12.8% (6/47) and 14.9% (7/47), respectively, both of which were significantly lower than the 30.6% (15/49) and 32.7% (16/49) in the control group, with statistically significant differences ( χ2=3.80, 3.89, both P<0.05); after intervention the scores on the observation, description and perceived action dimensions of the Brief Version of the Five Factor Positive Thinking Scale were (23.8 ± 6.5), (28.6 ± 5.4) and (31.3 ± 5.3) respectively, all significantly higher than those of the control group (20.0 ± 5.1), (23.7 ± 5.5) and (26.9±6.2), the differences were statistically significant ( t=2.79, 3.59, 3.21, all P<0.05); the post-intervention score on the Emotional Functioning dimension of the European Organization for Research and Treatment of Cancer Quality of Life Scale (94.3 ± 7.4) was significantly higher than that of the control group (88.8 ± 11.4), with a statistically significant difference ( t=-2.28, P<0.05). Conclusions:Positive cognitive therapy is effective in relieving anxiety and depression, increasing positive awareness, and improving emotional distress in cancer patients. It can be incorporated into the daily psychological care process of tumor patients, giving them appropriate guidance and support to help improve their psychological and quality of life.

【Objective】 To explore a new treatment of primary bilateral macronodular adrenal hyperplasia (PBMAH) and its efficacy. 【Methods】 Clinical data of 20 PBMAH patients treated in our hospital during Mar.2010 and Apr.2021 were retrospectively analyzed. All patients underwent laparoscopic subcutaneous displacement of vascularized adrenal. The clinical symptoms, plasma free cortisol, adrenocorticotrophic hormone (ACTH), and 24 h urinary free cortisol were regularly monitored after surgery. 【Results】 Of all 20 patients, 19 were followed up for 18 to 120 months (median 60 months). Three months after surgery, reexamination showed 1 patient had decreased plasma free cortisol and increased ACTH, but had no symptoms of low corticosteroids. After another 3 months, the plasma free cortisol and ACTH returned to normal. After 4 to 48 months, the parameters recovered in all patients and the clinical symptoms disappeared. 【Conclusion】 Laparoscopic vascularized adrenal displacement is a new and effective method for the treatment of PBMAH. It can alleviate the Cushing syndrome with no obvious adverse reactions.

Objective:To explore the incidence and epidemiological features of workplace violence among medi-cal staffs in a Third - grade Class - A hospital in Xi'an,and to provide scientific basis for the hospital and relevant health management departments to make workplace violence prevention and control measures. Methods: Using Hospital Workplace Violence Questionnaire, a survey was conducted among medical staffs in a Third - grade Class - A hospital in Xi'an to investigate the occurrence of workplace violence over the past one year. Results:A-mong the 1174 respondents,655 suffered workplace violence in the past one year,with a rate of 55. 79% ;the vio-lence occurred mainly in day shift and wards;the perpetrators were mainly middle - aged,male and the families of patients;69. 42% of the respondents thought the workplace violence were unavoidable and 87. 31% of the respond-ents considered that the top measures that can be taken to prevent the workplace violence was "the correct media o-rientation";the main coping methods of medical staffs were patience explanation and forbearance,and after suffering workplace violence,the medical staffs would feel grievances,anger,declining enthusiasm for work and even didn't want to do medical care. The main measures that the hospital had taken to prevent workplace violence mainly in-cluded the security patrol,installing cameras in wards and keeping light bright at night in working area. Conclu-sions:Workplace violence is common among medical staffs. To prevent and control hospital workplace violence,it is recommended that:① Hospital employ more security personals and should particularly strengthen security work in high - risk period,high - risk departments;②it should strengthen the training of medical staff on the prevention and control of workplace violence and strengthen popularization of the relevant legal knowledge to the client;③it should strengthen the communication and cooperation with media and promote the correct media orientation;④it should smooth the patient feedback channels and timely correct the existing problems.

Objective To investigate the status of self-management of elderly patients with chronic heart failure (CHF) in Xinjiang Region,and to explore the effects of different demographic factors on self-management.Methods A total of 704 patients with chronic heart failure were randomly selected from Urumqi,Kashgar,Khotan,Changji,Shihezi,Yili Hazakh in Xinjiang Uygur Autonomous Region via convenience sampling method and were investigated using heart failure self-management scale.Results The total score of self-management of chronic heart failure elderly patients was(47.6±11.1) and scoring rate was 59.5%.There were statistically significant differences(P<0.01) in self-management score among patients with different ethnicity,religion,self-care ability,per capita income.The influencing factors of self-management were self-care ability,NYHA class and per capita monthly income(P<0.05).Conclusion The overall level of self-management of elderly patients with chronic heart failure in Xinjiang Region is low,and self-management is affected by self-care ability,NYHA class and per capita monthly income.Tailored culture-based education should be provided to improve patients' self-management.

Objective To identify small non-coding RNAs encoded by plasmid pPCP1 and investigate their roles in biofilm formation, stress tolerance and/or virulence in Yersinia pestis.Methods Seven plasmid pPCP1-encoded sRNAs were identified by RNA-seq results in Y.pestis in our previous studies.Northern blot was used to validate the presence of the seven sRNAs.The sRNA-deletion mutants were constructed via λ-Red homologous recombination system.The biofilm formation, high salt tolerance and virulence of the phenotypes were compared between Y.pestis WT strain and sRNA mutants.Results and Conclusion The expression of seven pPCP1-encoded sRNAs was validated and the transcript length detected by Northern blotting corresponded to the length observed by RNA-seq.On this basis, five sRNA-deletion mutants were obtained.The capacity of biofilm formation was weakened upon deletion of sR3446.The tolerance of sR3446, sR3457, sR4338 and sR4340 mutants was found weakened in vitro compared to that of wild-type strain,but the tolerance of sR6143 was found increased.Slight virulence attenuation was found in two sRNA mutants ( sR4338 and sR4340 ) .The results suggest that pPCP1-deriving sRNA might be implicated in stress response, biofilm and virulence in Y.pestis.

Objective:To explore the curative effect and adverse reactions of cisplatin combined with rIL-2 on malignant pleural effusion. Methods:Totally 60 cases of patients with malignant pleural effusion were randomly divided into the observation group ( 30 cases) and the control group (30 cases). In the observation group, 30 cases were given intrapleural injection of cisplatin and rIL-2 af-ter the pleural effusions were drained, and in the control group, the patients received intrapleural injection of cisplatin. Both of the two groups were observed of tumor response and drug adverse reactions, compared the quelity of life and immune function indicators before and after treatment. Results:The response rate in the observation group was 83. 3%, which was significantly higher than that in the control group (56. 7%) (P0. 05). Conclusion:Cisplatin combined with rIL-2 shows confirmed effects in the patients with malignant pleural effusion.

Aim To observe the damages induced by hydrogen peroxide in cultured bovine cerebral microvascular endothelial cells (BCMEC) and evaluate the protective effects of hydroxyethylpuerarin on hydrogen peroxide-injured BCMEC. Methods BCMEC were cultured and transferred into modified Eagle medium (MEM). The viability of cells was detected by MTT assay. Cell injury was determined by lactate dehydrogenase (LDH) activity in the extracellular medium. Flow cytometry was employed to observe the occurrence of apoptosis. Morphologic changes of cells were visualized under phase contrast and electron occurrence of apoptosis. Hydroxyethylpuerarin was shown to increase the survival rate and decrease the activity of LDH of BCMEC damaged by hydrogen peroxide. Hydroxyethylpuerarin was also found to protect BCMEC against apoptosis induced by hydrogen peroxide. Conclusion Hydrogen peroxide induces BCMEC injury either by apoptosis or through necrosis. Hydroxyethylpuerarin protects BCMEC against hydrogen peroxide-induced injury in a concentration-dependent manner. Its antioxidant effects might be involved as the mechanism protection.

Purpose The aim is to modify the pyrogenic pathol ogy mo del induced by injection of saccharomycetet water in rats,and to eliminate the t emperature decline period after injection of saccharomycete water sc.Metho ds It was measured that the anus temperature of both two groups of rats (one group was injected of incubated saccharomycete water and the other inj ected unincubated saccharomycete water sc) 1,2,3,4,6,8 h after injection respect ively.Results The anus temperature had no decline period and the temperature rose quickly in the group of injected with incubated saccharomyc ete water (in 34℃thermostasis water).There was significant difference(P<0.05 or P<0.01)between incubated group and unincubated group in temperature risin g by t-test.Conclusion No temperature declining peri od was observed in the pyrogenic pathology model of rat, if those rats were trea ted with saccharomycete water which was incubated at 34℃for 0.5 h.
