RESUMO

@#Abstract: This study aims to find out the causes for the dissolution of unqualified samples found during evaluation sampling of lansoprazole enteric-coated preparations by the laser infrared imaging system and orbitrap high resolution mass spectrometry, with suggestions for improvement. Lansoprazole enteric-coated preparations were tested by current standard, the dissolution of a batch of samples was below the limit and other items were in line with the standard. Considering that this product is unstable to acid and alkali, the following exploratory experiments were designed from different dimensions, based on the conclusion of the unannounced inspection of the company, to explore the reasons for the unqualified batch, including the influence of high-temperature and high-humidity storage environment on the key quality properties of the sample, the influence of 2-hour acid resistance test on dissolution result, the imaging of the core and the measure of coating layer thickness, the optimization of chromatographic conditions of related substances, and the analysis of the source of impurities. It was found that improper storage in circulation and poor coating process caused the low dissolution of this batch: the high-temperature and high-humidity storage environment possible in the circulation process led to the decreasing efficacy of disintegrating agent in the samples and thus the difficulty to release the active pharmaceutical ingredient fully; the coating solution could not be uniformly sprayed on the core, resulting in thin isolation layer and different thickness, which then affected the protection of the main drug against acid degradation. The above two reasons together resulted in unqualified dissolution of this batch. The overall quality of lansoprazole enteric-soluble preparation is good, but the formulation and process in some companies need to be optimized; and the temperature and humidity in the circulation process need to be controlled in strict accordance with the regulations.

RESUMO

ObjectiveTo observe the apoptosis induced by paeoniflorin (PF) in non-small cell lung cancer (NSCLC) cells and explore its mechanism. MethodCell counting kit-8 (CCK-8) was used to detect the inhibition rates of H1299, H292 and A549 cells with different concentrations of PF (2.5, 5, 10, 20, 25 µmol·L-1), and to screen suitable concentrations of PF and experimental cells. The inhibitory effect of PF on lung cancer cells was detected by clone formation assay. The effect of PF on cell apoptosis was detected by flow cytometry with annexin V-FITC/propidium iodide (PI) double staining. With the right concentration of drugs, levels of apoptosis-associated protein B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X protein (Bax), cleaved Caspase-3 and Caspase-3 were detected by Western blot. At the same time, the molecular expressions of hypoxia inducible factor -1α (HIF-1α) and Hippo signaling pathway were determined. ResultCompared with the blank group, PF significantly inhibited the growth of H1299, H292 and A549 cells of human lung cancer (P<0.01). PF significantly induced apoptosis in A549 cells (P<0.01), decreased the Bcl-2/Bax ratio (P<0.01), and significantly increased the cleaved Caspase-3 expression (P<0.01). Compared with those in the blank group, the expression levels of HIF-1α, transcriptional coactivator with PDZ-binding motif (TAZ), large tumor suppressor 1 (LATS1), Mps one binding 1 (MOB1) and Yes-associated protein (YAP) in A549 cells of the PF treatment group were significantly decreased (P<0.01), while the expressions of p-LATS1, p-MOB1 and p-YAP were significantly increased (P<0.01). At the same time, there was no significant effect on the expression levels of phosphorylated mammalian Ste20-like kinase 1 (p-MST1) and MST1, which did not reach a statistical difference. ConclusionAll data demonstrated that PF showed an anti-tumor effect by improving hypoxic conditions and inhibiting the abnormally activated Hippo signaling pathway, thereby inducing and promoting apoptosis in non-small cell lung cancer.

RESUMO

The essence of cirrhosis is the over-repairing reaction of liver tissue damage in the process of chronic liver disease.During repair, the liver parenchyma is gradually replaced by fibrosis tissue, resulting in changes in liver tissue morphology, followed by portal hypertension and other related manifestations.Liver failure are serious disorder of liver functions(synthesis, metabolism, transformation, regeneration, etc.)caused by various factors, often mainly manifested as jaundice, coagulation disfunction, hepatic encephalopathy, ascites, etc. The naming and typing of the two are different, and they can exist independently of each other or intersect with each other. In recent years, with the in-depth exploration of cirrhosis and liver failure, many new definitions and classification methods have been put forward in the research. However, due to the confusion of classification methods, there is still a lack of summary,so this article briefly reviews the current progress of clinical classification of liver cirrhosis and liver failure and their differences and intersections.

RESUMO

Objective To investigate the correlation between the manual compression on injection point and the incidence of subcutaneous bleeding after subcutaneous injection of low molecular weight heparin(LMWH)in elderly patients with coronary artery disease. Methods A total of 131 elderly patients with coronary artery disease, who received subcutaneous injection of LMWH after percutaneous coronary intervention(PCI)at the Affiliated Nanjing Hospital of Nanjing Medical University of China between January 2019 and December 2021,were enrolled in this study.According to whether the manual compression on the injection point was employed or not after the injection of LMWH,the patients were divided into the study group(n=67)and the control group(n=64).The operation process of subcutaneous injection of LMWH was carried out in accordance with the"Supervision Standard for Nursing Quality of Hypodermic Injection of Low Molecular Weight Heparin"which was included in the norms formulated by authors'hospital. For the patients of the study group, the injection point was manually pressed for 3-5 min after the injection of LMWH,the manually-used force was to press the skin down for 1cm deep. The incidence of subcutaneous bleeding was compared between the two groups. Results In the study group and the control group, the incidence of subcutaneous ecchymosis was 9.0%and 7.8%respectively,the incidence of subcutaneous hard tubercle was 4.5%and 1.6%respectively,the differences between the two groups were not statistically significant(both P>0.05).The patient's age, gender, abdominal circumference and body mass index(BMI)carried no obvious correlation with the subcutaneous bleeding after LMWH injection(P>0.05),while a statistically significant correlation existed between the abdominal wall fat thickness and the subcutaneous bleeding(P<0.05),which could be used as an independent predictor for the occurrence of subcutaneous bleeding after LMWH injection. Conclusion No obvious correlation exists between the manual compression on injection point and the incidence of subcutaneous bleeding in elderly patients with coronary artery disease after subcutaneous injection of LMWH,therefore,no compression manipulation, used as a hemostatic measure,is required after subcutaneous injection of LMWH.The abdominal wall fat thickness is an independent predictor for subcutaneous bleeding after injection of LMWH.Standard operation procedures should be strictly followed so as to avoid the occurrence of subcutaneous bleeding after injection of LMWH.(J Intervent Radiol,2024,32:77-81)

RESUMO

Objective To determine the correlation between single-nucleotide polymorphisms of IL-17 with type 2 diabetes in Han population in southeastern Shanxi Provincein. Methods The basic information and related clinical data of the subjects were collected of normal healthy controls and T2DM.Whole blood DNA was extracted, and IL-17 polymorphisms(rs2275913,rs3819024,rs4711998 and rs8193036)were analyzed by using a polymerase chain reaction-high temperature ligase reaction. Results There was no significant difference in the genotype and allele frequency distribution of the four SNP loci of IL-17 gene between the two groups(P>0.05).IL-17 polymorphism was not linked with T2DM in multiple genetic models after controlling for age,BMI,WC,and dyslipidemia(P>0.05).Further analysis showed that the levels of AA genotype was substantially lower of FPG and HOMA-IR levels when compared with the AG and GG genotypes, and the differences among the three groups were statistically significant(P<0.05).However,BMI,FINS,HbA1c and HOMA-βwas no statistical significance among the three groups(P>0.05).Besides,WC and HOMA-IR of AA genotype(rs3819024)were notable higher than those of GG genotype in T2DM group(P<0.05).Conclusions The IL-17 gene polymorphisms rs2275913,rs3819024,rs4711998,and rs8193036 in the Han population of southeast Shanxi Province may not be associated with T2DM.In this region Han T2DM patients,AA genotype at rs2275913 of the IL-17 gene is associ-ated to FPG and HOMA-IR,while GG genotype at rs3819024 is related to WC and HOMA-IR,which could be the potential genotype of IL-17 impacting glucose metabolism.

RESUMO

Objective To investigate the associations between preconception dietary patterns(DPs)among Chinese women of childbearing age and neonatal birth weight. Methods The subjects selected for the questionnaire survey and follow-up were women of childbearing age who underwent prenatal eugenic examination at Jiang'an Maternal and Child Health Hospi-tal.Dietary intake information was collected using a semi-quantitative food frequency questionnaire, dietary patterns were extrac-ted by principal component analysis, and the relationship between DPs and birth weight was analyzed by modified Poisson re-gression or linear regression models. Results The final analysis of 221 maternal and infant pairs showed that women who fol-lowed the"nuts-poultry"pattern,one of the four dietary patterns, had a lower risk of delivering large for gestational age(LGA)infants(RR:0.25;95%CI:0.08-0.79),which was more pronounced in those who delivered male infants(RR:0.14;95%CI:0.03-0.72).Conclusion The risk of having LGA newborn is decreased in woman who takes a preconception dietary pattern characterized by nuts and poultry, which is more pronounced in those delivering male infants.Females of childbearing age should maintain good dietary habits before conception to ensure proper growth and development of the fetus and reduce the risk of ad-verse birth outcomes.

RESUMO

Objective To study the regulation effects of Bazhen Decoction combined with Baizhu Fuzi Decoction on glucose metabolism and estrogen level in rats with polycystic ovary syndrome. Methods The rat model of polycystic ovary syndrome was established by subcutaneous injection of human chorionic gonadotropin(HCG)combined with insulin.Rats were randomly divided into the polycystic ovary syndrome group, the Baizhu Fuzi Tang group(6.4 g/kg),the Bazhen Tang group(9.2 g/kg),the Bazhen Tang combined with Baizhu Fuzi Tang group(11.55 g/kg)and the diethylstilbestrol group(0.5 mg/kg).A blank control group(unmodulated rats)was set up with 10 rats in each group. The uterine index of rats was determined and calculated.Fasting blood glucose(FBG),2 h postprandish blood glucose(2 h PBG)and fasting insulin(FINS)were determined and insulin sensitivity index(ISI)and insulin resistance index(IR)were calculated.Serum estradiol(E2),testosterone(T),luteinizing hormone(LH),follicle stimulating hormone(FSH),prolactin(PRL),anti-Mullerian tube hormone(AMH)and insulin-like growth factor 1(IGF-1),C-reactive protein(CRP),interleukin(IL)-6,tumor necrosis factor(TNF)-α,total peroxidase activity(T-AOC),superoxide dismutase(SOD)and malondialdehyde(MDA)levels in ovarian tissue were detected by enzyme-linked immunosorbent assay(ELISA).The pathological changes of ovarian tissue were detected by hematoxylin-eosin(HE)staining.Real-time quantitative polymerase chain reaction(qPCR)was used to determine mRNA levels of ARA70,CBP,SCR1 and HOXA10 in endometrium and mRNA expressions of AMPK,GLUT4 and PPARγ in ovarian tissue.Western blot assay was used to determine expression levels of AMPK,GLUT4 and PPARγ in ovarian tissue. Results Compared with the polycystic ovary syndrome group, uterine index,FINS,FBG,2 h PBG,IR levels, serum T,LH,FSH,PRL,AMH,IGF-1 levels, ovarian tissue CRP,IL-6,TNF-α,T-AOC and MDA level, endometrial ARA70,CBP,SCR1 mRNA level of rats decreased significantly in the Baizhu Fuzi decoction group, the Bazhen decoction group and the Bazhen decoction combined with Baizhu Fuzi decoction group(P<0.05).ISI level, serum E2 level, ovarian tissue SOD level, endometrial HOXA10 mRNA level, ovarian tissue AMPK,GLUT4 and PPARγ mRNA and protein levels were significantly increased(P<0.05).The above indexes were significantly changed in the Bazhen decoction and Bazhu Fuzi decoction group than those of the Bazhu Fuzi decoction group and the Bazhen decoction group(P<0.05).Conclusion Bazhen Decoction combined with Baizhu Fuzi Decoction can regulate glucose metabolism, inhibit ovarian tissue oxidation and inflammatory damage, improve endometrium tolerance, regulate estrogen level, and improve the progression of polycystic ovary syndrome in rats. The mechanism may be related to the regulation of AMPK/GLUT4/PPARγ pathway.

RESUMO

Objective To compare the safety and clinical efficacy of lesion removal combined with percutaneous pedicle screw fixation with classical posterior lesion removal in the treatment of lumbar brucelli spondylitis(LBS)by unilateral biportal endoscopic technique with transforaminal lumbar interbody fusion(UBE-LIF)technique.Methods The clinical data of 32 patients with LBS admitted by the Department of Spine and Orthopedics of Gansu Provincial Hospital of Traditional Chinese Medicine from January 2020 to January 2022 were retrospectively analyzed, and the clinical data of the 32 LBS patients were divided into 15 cases in the UBE-LIF group and 17 cases in the posterior group. The general data,surgery-related indexes, and postoperative pathological HE staining of the two groups were recorded and analyzed. The patients'clinical recovery was assessed according to their erythrocyte sedimentation rate(ESR)and C-reactive protein(CRP),low back pain visual analogue score(VAS),Japanese Orthopaedic Association(JOA)score, and Oswestry Dysfunction Index(ODI)preoperative,1 week after surgery,1,3,6 months and 1 year after surgery.Lumbar lordosis angle(LL)and intervertebral space height(DH)were measured by imaging before surgery and at the last follow-up,and intervertebral bone graft fusion was assessed using Suk grading criteria. Results Both groups successfully completed the operation and no serious postoperative complications occurred. There were no significant differences in gender, age, surgical segment, operation time, preoperative ESR and CRP,preoperative VAS,JOA score and ODI index, preoperative LL and DH(P>0.05).The intraoperative blood loss, postoperative drainage, postoperative getting out of bed,and postoperative hospital stay in UBE-LIF group were significantly lower than those in the posterior group(P<0.001).Pathological examination of diseased tissues was performed during surgery, all of which was consistent with brucellosis changes.Patients in both groups were followed up for 12-18 months, with an average of 14.8 months. The VAS,JOA score, and ODI index at all postoperative time points in the two groups were significantly improved compared with the preoperative period(P<0.05).The difference between the two groups was significantly greater than that in the postoperative group:VAS score was lower in UBE-LIF group than in the posterior group(P<0.01),CRP in both groups was higher than that in the preoperative group, and the elevation level was significantly lower in UBE-LIF group than in the posterior group(P<0.001).There was no significant difference in ESR between the two groups compared with that before surgery(P>0.05).There were no significant differences in VAS,JOA score,ODI index,CRP or ESR between the remaining time points after surgery(P>0.05).At the last follow-up,imaging examination showed that the overall fusion rate of intervertebral bone graft in UBE-LIF group was 93.3%and 94.1%in the posterior group, without significant difference(x2=0.246,P=0.884).LL and DH were significantly improved in both groups compared with preoperative ones(P<0.01),and the two groups did not significantly differ before and after surgery(P>0.05).Conclusion Both surgical treatments for LBS are safe effect.Compared with posterior lesion removal bone graft fusion internal fixation,UBE-LIF technology combined with percutaneous pedicle screw internal fixation has the advantages of clear intraoperative vision, less blood loss, faster early postoperative recovery, and shorter postoperative hospital stay, and thus is a feasible surgical method for the minimally invasive treatment of LBS.

RESUMO

BACKGROUND:In recent years,it has been found that some traditional Chinese medicine monomers can alleviate oxidative stress and apoptosis of the skin flap, promote vascular regeneration of the skin flap and prevent skin flap necrosis by activating autophagy. OBJECTIVE:To review the research progress of traditional Chinese medicine monomer regulating autophagy in preventing flap necrosis. METHODS:The Chinese and English key words were"traditional Chinese medicine(TCM),autophagy, skin flaps".The first author searched the relevant articles published in CNKI and PubMed databases from January 2010 to October 2022.A total of 196 articles were retrieved in the preliminary screening and then screened according to the inclusion and exclusion criteria. The quality assessment was conducted by reading the literature titles and abstracts.Finally,55 articles were summarized. RESULTS AND CONCLUSION:(1)The regulation of autophagy is mediated by AMPK/mTOR,PI3K/AKT and other signaling pathways.Activation of autophagy can alleviate the oxidative stress and apoptosis of the flap, promote the regeneration of blood vessels in the flap, and prevent flap necrosis.(2)Terpenoids(Betulinic acid,Andrographolide,Notoginseng Triterpenes,Catalpa),phenolic compounds(Resveratrol,Curcumin,Gastrodin),phenolic acids(Salvianolic acid B)and steroid compounds(Pseudoginsenoside F11)in traditional Chinese medicine monomers can alleviate oxidative stress and apoptosis of skin flap by regulating related signaling pathways to activate autophagy, promote skin flap angiogenesis and promote skin flap survival.(3)Studying the research progress of traditional Chinese medicine monomer to prevent flap necrosis by regulating autophagy can provide a reference and theoretical basis for traditional Chinese medicine to prevent flap necrosis and promote flap healing in the clinic.
