RESUMO

Objective:To investigate the effects of hemodialysis (HD) and hemoperfusion (HP) combination treatment on maintenance hemodialysis (MHD) patients. Methods:A total of 80 MHD patients in Chongming Branch of Xinhua Hospital, Shanghai Jiao Tong University School of Medicine from July 2017 to July 2018 were randomly divided into two groups, i.e., HD+HP group (n=40) and HD group (n=40). The patients were followed up every 3 months for 1 year. The changes of laboratory indexes, dialysis adequacy indicators and quality of life scores of Kidney Disease Quality of Life-Short Form (KDQOL-SF) were compared between the two groups, and the prognosis, causes of death and adverse events were recorded. Results:At the end of one-year treatment, levels of parathyroid hormone (PTH), β2-microglobulin (β2-MG), high sensitive C-reactive protein (hsCRP), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and left ventricular mass index (LVMI) were significantly lower in HD+HP group than those in HD group (P0.05). The overall mortality rates of HD+HP group and HD group were 12.5% and 32.5%, respectively. No significant adverse events were observed during the follow-up. Conclusion:The effects of HD combined with HP on clearing middle and large molecular toxins, reducing microinflammation status, and improving renal anemia and left ventricular hypertrophy are better than those of only HD. There may be potential advantages of HD and HP combination in improving quality of life in MHD patients as well.

Objective To explore the clinical value of serum ANCA (antineutrophil cytoplasmic antibody),and ACLA (anticardiolipin antibodies) combined with β2-MG (β2 microglobulin) in the diagnostis of DN (early diabetic nephropathy).Methods Fifty cases of patients with early diabetic nephropathy treated in our hospital from February 2016 to February 2017 were selected,and 50 normal volunteers were collected,as the observation group and control group.Positive rates of ANCA,ACLA and β2-MG expression of the patients were observed and recorded.The correlation of ANCA,ACLA,β2-MG and DN was explored and analyzed,and ANCA,ACLA,β2-MG and ANCA,ACLA combined with β2-MG diagnostic efficiency were evaluated.Results The positive rates of ACLA,ANCA,and β2-MG antibodies in the observation group were significantly higher than those in the control group (P < 0.05).There was difference in DN incidence in different levels of ANCA,ACLA,β2-MG (P <0.05).There was a significant correlation in ACLA,ANCA,β2-MG and myocardial infarction.Combined diagnostic sensitivity ACLA + ANCA + β2-MG was significantly higher than that of pure ACLA,ANCA,β2-MG (P < 0.05).The specificity was lower than that of simple RBP,Hcy and ultrasonic scoring diagnosis.The consistency was higher than that of pure ACLA,ANCA,and β2-MG (P < 0.05).According to the results of ROC curve,the diagnostic efficiency of ACLA + ANCA + β2-MG combined with early diagnosis of DN was significantly better than that of pure ACLA,ANCA and β2-MG.Conclusion Combined use of ACLA + ANCA + and β2-MG can improve diagnostic accuracy of early diabetic nephropathy,which is more effectively than pure ACLA,ANCA,and β2-MG for diagnosis,and it is worthy of clinical promotion.

Objective To investigate the value of combined detection of serum CA125 ,VEGF‐C andβ2‐MG levels on early diag‐nosis of retroperitoneal lymph node metastasis in the patients with ovarian cancer .Methods Fifty‐one patients with ovarian cancer undergoing retroperitoneal lymph node cleaning operation were included as the experimental group ,including 29 cases of positive retroperitoneal lymph node metastasis .Contemporaneous 32 cases of benign ovarian tumor were selected as the control group .The serum CA125 level was detected by using electricity chemiluminescence (electrocheminescence) ,serum VEGF‐C level by using en‐zyme‐linked immunosorbent assay (ELISA) and serumβ2‐MG level by using the latex enhanced immune turbidimetric method .The serum CA125 ,VEGF‐C andβ2‐MG levels were compared among various groups .Results The serum CA125 ,VEGF‐C andβ2‐MG levels in the experimental group were (1 682 .5 ± 261 .5)μg/mL ,(2 125 .6 ± 96 .7)pg/mL and (2 .52 ± 0 .61)mg/L respectively , which in the control group were (30 .5 ± 6 .3)μg/mL ,(1 738 .0 ± 79 .8)pg/mL and (1 .87 ± 0 .56)mg/L respectively ,the difference between them was statistically significant (P< 0 .01) .The serum CA125、VEGF‐C、β2‐MG levels in 29 cases of retroperitoneal lymph node metastasis were more significantly higher ,which were (1 997 .3 ± 376 .8)μg/mL ,(2 895 .2 ± 126 .8)pg/mL and (4 .95 ± 0 .69)mg/L respectively ,and the difference was statistically significant compared with the control group (P<0 .01) .The sensitivity ,specificity and accuracy rate of the combined detection of serum VEGF‐C ,β2‐MG and CA125 for diagnosing ovarian cancer retroperitoneal lymph node metastasis were 95 .8% ,97 .3% and 98 .5% respectively .Conclusion The combined detection of serum VEFG‐C ,β2‐MG and CA125 has an important clinical value in early diagnosing retroperitoneal lymph node metastasis of o‐varian cancer .

Objective To explore changes of urine transferrin (TFR),micro albumin (mALb),β2 microspheres protein (MG),α1MG and N-acetylβ-D amino group (NAG) in children with kidney injury induced by cytomegalovirus (CMV) infec?tion, and the outcome after treatment. Methods Fifty children with CMV infection were used as case group, and 35 chil?dren of convalescence stage of upper respiratory tract infection were used as control group. The serum levels of creatinine (Scr), blood urea nitrogen (BUN),β2MG, liver function, CMV-IgM, CMV-PCR and brainstem auditory (BAEP), head CT and urine routine test, urine TFR, mALb,β2MG,α2MG and NAG were detected. The sputum CMV-PCR was detected if childrencombined with CMV pneumonia. Ganciclovir (5mg/kg) was given to two groups, 1/12 h i.v. for 14 d. The urine TFR,mALb,β2MG, α2MG and NAG were detected again after treatment. Results There were no significant differences in the urine mALb and TFR between the two groups. The urine levels ofα1MG, NAG andβ2MG were higher in case group than those of control group. The urine levels ofα1MG, NAG andβ2MG were decreased after 2-week treatment in case group. There were no significant differences in urine mALb and TFR before and after treatment. Conclusion The combined detection ofβ2MG,α1MG and NAG can predict CMV kidney damage in children at a early stage.

Objective To explore the diagnostic values of serum cystatin(CysC),retinol binding protein(RBP),β2 microglobulin (β2-MG),and urine microalbumin(mALB)and transferrin(TRF )in the diagnosis of early diabetic nephropathy(DN).Methods 118 patients with diabetes mellitus(DM)were divided into three groups according to the urinary albumin excretion rate (UAER), simple DM group,early DN group and clinical stage DN group.The immunity turbidimetry method was applied to measure the ser-um CysC,RBP,β2-MG and urine mALB,TRF;the enzymatic method was applied to measure the serum Cr;the rate method was applied to measure the serum Bun.The detection results were analyzed statistically.Results serum CysC,RBP,β2-MG and urine mALB,TRF levels in the early DN group and the clinical stage DN group were significantly higher then those in the simple DM group,the differences had statistical significance(P 0.05 ),serum Cr and Bun levels in the clinical stage DN group were significantly higher than those in the simple DM group with statistical difference(P <0.05).Conclu-sion Serum CysC,RBP,β2-MG and urine mALB,TRF levels in the patients with DN are significantly increased,which has the im-portant clinical value for early diagnosing and monitoring DN.

Objective To research correlation of serum protein spectrometry and lymphoma markers for diffuse large B cell lymphoma (DLBCL) patients. Whether there is relative between the protein and prognosis will be further researched. Methods Serum protein spectrometry of 62 DLBCL patients was detected by the SELDI-TOF-MS technique and Weak cation exchange 2 (WCX2) chip. Lactate dehydrogenase (LDH) was detected by biochemistry method. Beta-2-microglobulin (β2-MG) and cancer antigen125(CA125)were detected by enzyme-linked immunosorbent assay (ELISA). The level of LDH, β2-MG and CA125 for DLBCL patients between 11×103~12×103 protein expressed positively and negatively was analyzed. Meanwhile,correlation analysis and survival analysis were done. Results LDH, β2-MG and CA125 in 11×103~12×103protein expressed positive group were (523.30±435.96)U/L, (3.23±1.24)mg/L, (81.07±61.39)U/L respectively,and they were higher than that in negative group (P<0.05). 11×103~12×103 protein was positive correlated to LDH, β2-MG and CA125 (P<0.01). The survival time in 11×103~12×103 protein expressed in positive group,in which median survival time was 11 months, was shorter than that in negative group(P <0.01). The survival time in LDH normal group was longer than that in increased group(P <0.01). The survival time of β2-MG and CA125 had no significant difference between increased group and normal group. Conclusion LDH and 11×103~12×103 protein are expected to be prognosis indicators for DLBCL patients.
