Spondyloenchondrodysplasia with immune dysregulation related to ACP5 (SPENCDI, OMIM number 607944) is an uncommon immune-skeletal dysplasia with heterogeneous manifestations and variable severity. It is characterized by spondylar and metaphyseal lesions, immune dysfunction, and neurological involvement. Here we report the clinical, radiological and genetic aspects of 4 girls with SPENCDI treated at a children's hospital. They all had skeletal manifestations and 3 developed severe immune disease. In 3 patients, the likely pathogenic variant c.791T>A; p.Met264Lys (homozygous mutation) was observed, while 1 patient had variants c.791T>A; p.Met264Lys and c.632T>C; p.lle211Thr (variant of uncertain significance with pathogenic prediction based on bioinformatics algorithms) caused by a compound heterozygous mutation in ACP5. The repeated presence of variant c.791T>A suggests the possibility of a common ancestor in our population. The recognition and diagnosis of this disorder is important to achieve a timely approach, which should be multidisciplinary and aimed at preventing possible complications.

RESUMO

Introduction: Lumbar spine pathology is major healthcare burden encountered in district and urban hospitals. Diagnostic imaging in these patients is not always indicated. Imaging is considered in those patients who show little or no improvement in their LBP after approximately six weeks of medical management with or without physical therapy, the current study was undertaken to assess the appropriateness of LBP imaging in primary care following the ACP guidelines. The inclusion criteria for the study were adult patients with persistent low back pain with orMethod: without radiculopathy post six weeks of medical management affecting their activity of daily living. And the exclusion criteria were spinal trauma , neurological impairment, bladder and bowel involvement, spinal malformations, history of malignancy presenting at the time of index visit. The study included total 115 patients. Out of 115 patients 87Result: [70%] patients showed abnormal finding on the radiographs. The most common abnormal finding was disco vertebral degeneration such as spondylosis (osteophyte formation) in 49.6%. Routine x-ray imaging in low backConclusion: pain patients is not always warranted. Moreover routine radiological imaging methods are not associated with meaningful clinical outcomes for the patients. Unnecessary imaging can lead to harmful radiation exposure of the patients and can further lead to additional medical expenditure and needless surgical intervention. Diagnostic radiological imaging should be used judicially.

RESUMO

Objective:To summarize the clinical features and gene phenotype of children with spondyloenchondrodysplasia with immune dysregulation (SPENCDI) caused by ACP5 gene mutation. Methods:The medical data and genetic phenotype of a child diagnosed with SPENCDI in the Department of Pediatrics, the First Affiliated Hospital of Zhengzhou University in February 23, 2017 were analyzed retrospectively.Besides, " spondyloenchondrodysplasia" were taken as the search terms to perform the retrieval in CNKI, Wanfang Data, and PubMed, in an attempt to conduct the literature review.χ 2 test was used to compare the factors among children with different mutations. Results:The 4.5-year-old girl was admitted to hospital for complaint of " fever and chilblain-like rash" when she was 2 years old.She was diagnosed with systemic lupus erythematosus (SLE) concomitant with lupus nephritis.Methylprednisolone combined with cyclophosphamide, mycophenolate mofetil was used for the treatment. However, she experienced multiple infections, thrombocytopenia, limp, and growth retardation during the treatment.Genetic detection identified ACP5 gene compound hybrid mutation: c.779C>A and c. 770T>C.She was diagnosed with SPENCDI, and was subjected to follow-up.A total of 78 SPENCDI patients were retrieved from the databases, with various clinical manifestations of SPENCDI, commonly with skeletal involvement and immune phenotypes; 73.08% of the cases were positive for antinuclear antibodies, 57.69% of cases were positive for anti-double stranded-DNA antibodies and 34.62% of cases had neurological symptoms. In 58 cases, ACP5 gene mutations were detected, including 44 homozygous mutations and 14 compound heterozygous mutations.Patients with ACP5 gene homozygous mutation had a higher probability of consanguineous marriage in parents [56.82% (25/44 cases) vs.14.29% (2/14 cases)]; patients with ACP5 gene heterozygous mutation were more likely to develop SLE [64.29% (9/14 cases) vs.34.09% (15/44 cases)]( χ2=7.722, 3.992; all P<0.05). Conclusions:The majority of the ACP5 gene mutations are homozygous mutations in patients with SPENCDI, and heterozygous mutations are rare. The clinical manifestations of SPENCDI are various and complex, it is prone to develop autoimmune diseases, and there was no clear correlation between clinical features and gene phenotype in SPENCDI patients.

RESUMO

Low back pain (LBP) is one of the most common complaints of Japanese citizens, and is a symptom for which acupuncture has been applied since ancient times. We empirically recognize that acupuncture is effective for LBP, but in recent years, it became necessary to show evidence regarding its efficacy and effectiveness.Currently, to elucidate the effect of a medical intervention, randomized controlled trials (RCTs) are considered the gold standard and outcomes of RCTs are integrated as a systematic review (SR), and these are used to establish the degree of recommendation in the clinical practice guidelines (CPGs). Eventually, practitioners make their clinical decisions according to the recommendations in CPGs.In this report, the author compared CPGs of acupuncture for LBP between those of the United Kingdom (UK) and United States of America (USA). The NICE guideline development group of the UK did not fully understand Japanese-style acupuncture, and they considered clinical trials with placebo/sham as a control to be the most important, leading to underestimating the effect of acupuncture. On the other hand, the ACP guideline of the USA considers clinical trials with conventional treatment as a control to be more important, and accordingly the effect of acupuncture was evaluated realistically.

RESUMO

Objective To analyze the epidemiological characteristics of hepatitis B in Guangzhou from 2006 to 2018. Methods The data were retrieved from the National Infectious Disease Surveillance Information Report Management System. Descriptive methods were used to analyze and summarize the results. Chi-square test was used for statistical analysis of classification data, and Joinpoint software was used to analyze the annual change percentage and trend. Results A total of 292 674 cases of hepatitis B were reported in Guangzhou from 2006 to 2018, and the annual average reported incidence was 182.2 per 100,000. Chronic hepatitis B was the main type, accounting for 89.9%. The incidence rate of males (241.6 / 105) was higher than that of females (117.9 / 105), with a ratio of 2.0:1. During the period from 2006-2018, the reported incidence of hepatitis B in Guangzhou first increased, reached the highest in 2008, and then decreased year by year. There was no significant difference in the number of HBV cases among different months. The average onset age of hepatitis B reported in Guangzhou was 39.3 years old, and the average onset age increased year by year from 2006 to 2018. Conclusion The incidence rate of hepatitis B in Guangzhou has decreased since 2008, but it was still higher than the national average incidence rate. The hepatitis B infection in Guangzhou remains a big issue. It is suggested to further strengthen the prevention and control of hepatitis B in adults by maintaining a national immunization program for children, thereby reducing the epidemiological level of hepatitis B in Guangzhou.

RESUMO

Immunotherapy strategies targeting the programmed cell death ligand 1 (PD-L1)/programmed cell death 1 (PD-1) pathway in clinical treatments have achieved remarkable success in treating multiple types of cancer. However, owing to the heterogeneity of tumors and individual immune systems, PD-L1/PD-1 blockade still shows slow response rates in controlling malignancies in many patients. Accumulating evidence has shown that an effective response to anti-PD-L1/anti-PD-1 therapy requires establishing an integrated immune cycle. Damage in any step of the immune cycle is one of the most important causes of immunotherapy failure. Impairments in the immune cycle can be restored by epigenetic modification, including reprogramming the environment of tumor-associated immunity, eliciting an immune response by increasing the presentation of tumor antigens, and by regulating T cell trafficking and reactivation. Thus, a rational combination of PD-L1/PD-1 blockade and epigenetic agents may offer great potential to retrain the immune system and to improve clinical outcomes of checkpoint blockade therapy.

The general purpose of this study was to evaluate in vitro the association of treatments with fluoridated toothpastes and supplemented with sodium trimetaphosphate (TMP) and casein phosphopeptide-amorphous calcium phosphate (CPP-ACP) (MI Paste Plus®) in promoting remineralization and reduce demineralization, respectively, of tooth enamel. Bovine enamel blocks (12/group) were selected through the initial surface hardness (SH) and divided into 5 experimental groups: 1) Toothpaste without F (Placebo); 2) Toothpaste with 1100 ppm F (1100F), 3) MI Paste Plus®, 4) Toothpaste with 1100 ppm F associated with MI Paste Plus® (1100F-MI Paste Plus®) and 5) Toothpaste with 1100 ppm F + 3% TMP associated with MI Paste Plus® (1100F-TMP-MI Paste Plus®). For Remineralization Manuscript 1 (RE>DES), blocks of bovine enamel were selected for the surface hardness after artificial caries lesion (SH1) and subjected to 6 pH cycles for 6 days. After pH cycling, final surface hardness (SH2) was determined to calculate the percentage of surface hardness recovery (%SHR), integrated loss of subsurface hardness (ΔKHN), profile analysis and depth of the lesions of subsurface through polarized light microscopy (PLM), confocal laser scanning microscope (MCVL), scanning electron microscopy (SEM), dispersive energy spectroscopy (EDS), fluoride (F), calcium (Ca) and phosphorus (P) concentration in the enamel. The data were submitted to ANOVA (1-criterion), followed by the Student-Newman-Keuls test (p<0.001). 1100F and 1100F-TMP-MI Paste Plus® groups showed similar values of %SHR (p = 0.150). The lowest depth of lesion (ΔKHN and PLM) was observed for the 1100F-TMP-MI Paste Plus® group when compared to the others (p<0.001). The 1100F-TMP-MI Paste Plus® group showed a more uniform and complete surface in relation to the other treatments (MCVL and SEM). The F concentration was similar between the 1100F, 1100F-MI Paste Plus® and 1100F-TMP-MI Paste Plus® groups (p>0.001). The treatment with 1100F-TMP-MI Paste Plus® promoted an increase in the concentration of Ca in the enamel by ⁓ 51% and ⁓ 21% respectively, when compared to the 1100F and MI Paste Plus® groups (p<0.001). Similar values of P in the enamel were observed in the MI Paste Plus®, 1100F-MI Paste Plus® groups (p>0.001), except for the 1100F-TMP-MI Paste Plus® group, which presented high concentration (p<0.001). For demineralization Manuscript 2 (DES> RE), bovine of enamel blocks were selected for their initial surface hardness (SHi) and then subjected to 5 pH cycles for 7 days. After final surface hardness (SHf), percentage of loss of surface hardness (%SH), integrated loss of subsurface hardness (ΔKHN), analysis of the profile and depth of subsurface lesions through polarized light microscopy ( PLM), confocal laser scanning microscopy (MCVL), scanning electron microscopy (SEM), dispersive energy spectroscopy (EDS), fluoride (F), calcium (Ca) and phosphorus (P) concentration in the enamel. The data were submitted to ANOVA (1-criterion), followed by the Student-Newman-Keuls test (p<0.001). For %SHR, the Placebo group had the lowest values (p>0.001). The 1100F-TMP-MI Paste Plus® group remineralized the enamel surface by ~ 38% compared to MI Paste Plus® (p<0.001). The lowest depth of the lesion (ΔKHN) was observed for the 1100F-TMP-MI Paste Plus® group when compared to the others (p<0.001), being 32% lower when compared to the 1100F group. The F, Ca and P concentration was higher for the 1100F-TMP-MI Paste Plus® group (p>0.001). In view of the partial results obtained, it is possible to conclude that the combination of treatments with fluoridated toothpastes and supplemented with sodium trimetaphosphate (TMP) and amorphous calcium phosphate casein-phosphate (CPP-ACP) (MI Paste Plus®) (1100F-TMP -MI Paste Plus®) promoted a significant additional effect in the de-remineralization process, and could be an alternative treatment for patients at risk and caries activity(AU)

Proctoporus machupicchu is a semi-fossorial lizard, which was discovered in two locations in the Historic Sanctuary of Machu Picchu (SHMP), Urubamba province, Cusco, Peru. The type series consists of two females and a male, however, the male paratype does not possess femoral pores which is incongruous with the males mentioned in the literature. In this paper, we report a new location outside the SHMP and re-examine the type series. Internally, the male paratype has no male vestiges (testicles and seminiferous ducts), so we conclude that there was an error in sexing. In addition, we report a new locality based on a juvenile male collected in the San Luis Private Conservation Area (ACP San Luis) of La Convención province. This new record increases the range of distribution of the species by 20.9km to the northeast and its altitudinal range from 2760 - 2800 to 2760 - 2966 m of altitude.

Knowledge about reproduction of white-lipped peccary is of great importance to assist with the conservation of this species and enable its rational use in captivity. This study aimed to evaluate the effect of ACP-103®, ACP-116® and BTS semen extenders on sperm viability during cooling of Tayassu pecari semen. Five ejaculates from four adult males were chilled. The animals were submitted to the protocols of sedation and anesthesia for semen collection by the electroejaculation method. After collection, the semen was macro- and microscopically assessed and diluted to reach 35x106 spermatozoa/mL in each of the three different extenders tested. The fresh-extended semen was packed in a BotuFLEX® thermal box to keep samples at 15°C for 24 hours. After cooling, the following semen parameters were analyzed: sperm motility, functional and structural integrity of sperm membranes, mitochondrial activity, chromatin condensation, and the thermoresistance test was performed. The parameters sperm motility, structural and functional integrity of sperm membranes, mitochondrial activity, and chromatin condensation were preserved after use of the extenders tested, and were similar to those of in natura semen (p>0.05). Curvilinear velocity (VCL) (p<0.05) was the only parameter with reduced values after cooling regardless of the extender used. The percentage of sperm with normal morphology was greater in samples cooled using the BTS extender (p<0.05). The ACP-103®, ACP-116® and BTS extenders can be used for the cooling and preservation of white-lipped peccary semen at 15°C for 24 hours.(AU)

OBJECTIVES: This study was conducted to evaluate fluoride release and the micro-shear bond strength of resin-modified glass ionomer cement (RMGIC) in casein phosphopeptide-amorphous calcium phosphate (CPP-ACP)-remineralized caries-affected dentin (CAD). MATERIALS AND METHODS: Exposed dentin surfaces of 30 human third molar teeth were divided into 2 equal groups for evaluating fluoride release and the micro-shear bond strength of RMGIC to CAD. Each group was subdivided into 3 equal subgroups: 1) control (sound dentin); 2) artificially demineralized dentin (CAD); 3) CPP-ACP remineralized dentin (remineralized CAD). To measure fluoride release, 15 disc-shaped specimens of RMGIC (4 mm in diameter and 2 mm in thickness) were bonded on one flat surface of the dentin discs of each group. Fluoride release was tested using ion chromatography at different intervals; 24 hours, 3, 5, 7 days. RMGIC micro-cylinders were built on the flat dentin surface of the 15 discs, which were prepared according to the assigned group. Micro-shear bond strength was measured after 24 hours water storage. Data were analyzed using 1- and 2-way analysis of variance and the post hoc least significant difference test (α = 0.05). RESULTS: Fluoride detected in solutions (at all intervals) and the micro-shear bond strength of RMGIC bonded to CPP-ACP-remineralized dentin were significantly higher than those bonded to artificial CAD (p < 0.05). CONCLUSIONS: Demineralized CAD consumes more fluoride released from RMGIC into the solution for remineralization than CPP-ACP mineralized dentin does. CPP-ACP increases the micro-shear bond strength of RMGIC to CAD.

In a previous study on subjects with cardiovascular diseases (CVD) we have observed that in obese patients with ACP1*B/*B genotype the proportion of those with type 2 diabetes (T2D) is significantly lower as compared to other ACP1 genotypes. We have now carried a new study in subjects with T2D without CVD and in non diabetic subjects without CVD. We have studied 277 subjects with T2D without CVD and 137 healthy subjects without T2D and without CVD. Iinformed consent was obtained from these subjects to participate to the study that was approved by the Council of Department. ACP1 genotype was determined by DNA analysis. Statistical tests were carried out by SPSS programs. ACP1*B/*B genotype which is associated with the highest concentration of F isoform exerts a protective effect on susceptibility to T2D in obese subjects. A negative correlation is observed between F isoform concentration and odds ratio for susceptibility to T2D in obese subjects The present observation confirms the association previously observed in subjects with CVD making unlikely the possibility of a mere sampling chance artifact. The expression of ACP1 isoforms in adipose tissue trough an action on adipocytes lipid binding protein and lipid metabolism may exert an important role in the susceptibility to T2D in obese subjects

The objective was achieved by dividing the research into two experiments: (1) improving the freezing protocol using powdered coconut water (ACP-104) as a diluent for the cryopreservation seminal of common carp; (2) evaluating the effect of supplementation of vitamins C (ascorbic acid) or vitamin E (α-tocoferol) with the best extenders tested in experiment 1 on the quality of post-thawed. For experiment 1, semen pools from 14 selected males were formed. Seminal samples were evaluated for total motility, velocity, percentage of normal sperm and sperm vitality before and after the seminal cryopreservation. This was done in ACP-104 extender plus dimethyl sulfoxide (DMSO), or ethylene glycol (EG), or glycerol or methanol all at concentration 10% diluted in 1:3 (semen:extender). The samples were frozen in vapors of nitrogen into dry shippers and stored in liquid nitrogen (-196 °C). For experiment 2, eight pools were formed from the 15 males. The semen samples were evaluated following the same analysis of experiment 1 adding duration of total motility. The sperm cryopreservation was performed in extenders ACP-104 plus DMSO or EG supplemented or not with vitamin C or E. The best results found in Experiment 1 were obtained with DMSO and EG. They do not differ significantly for total motility (24% and 28%; P>0.05) and normal sperm (32% and 26%; P>0.05) respectively. For experiment 2, EG supplemented with vitamin E, produced significantly better results overall motility, sperm normality and duration of motility relative to DMSO. In conclusion, EG should be the cryoprotectant of choice for use with the ACP-104 supplemented or not with vitamin E.(AU)

RESUMO

Bryconamericus is the most diverse genus within Stevardiinae, comprising 61 valid species distributed in Cis- and Trans-Andean basins from Panama in Central America to northern Argentina in South America. Three species are known from the upper rio Paraná basin: B. exodon, B. iheringii, and B. turiuba. Herein we describe a new species of Bryconamericus from the upper rio Paraná basin inhabiting tributaries of Ivaí, Piquiri, and Tibagi basins, Paraná State, Brazil. The new species differs from its congeners by the presence of unaligned teeth in the outer tooth row of the premaxilla; a single, vertical, dorsally expanded and rounded humeral spot; 36-39 pored scales in the longitudinal series; body depth 31.6-37.9% SL; anal-fin base length 24.8-30.1% SL; number of branched anal-fin rays 19-22, and bony hooks on pelvic- and anal-fin rays of sexually dimorphic males. The new species is syntopic with other Stervadiinae in the upper rio Paraná basin such as B. iheringii, B. turiuba, Piabarchus stramineus, and Piabina argentea.(AU)

RESUMO

ABSTRACT Tuberculosis is leading cause of death among the global bacterial infections. The main causative for tuberculosis is Mycobacterium tuberculosis, which will survive in its host human being for decades in latent or chronic levels. In addition, the late multidrug resistance at a disturbing rate accompanies the appearance of tuberculosis. The quick spread of resistance to initial stage treatment medications has redirected the focus of the medical community in the creation of an array of new drug against Mycobacterium tuberculosis. The InhA protein is a component of Fatty acid synthetase (FAS) II and exhibits an NADH reliant enoyl-ACP reductase activity. InhA is a vital enzyme of M.tuberculosis in control of cell wall synthesis, which can turn out to be a great focus for the synthesis of anti-tubercular treatment. Inspired from the offering biological actions of phytoconstituents from Allium sativum, the current research concentrates on looking at novel lead compounds from the plant. Molecular docking studies were carried out employing specific phytoconstituents from A.sativum with the protein InhA target. Ajoene shows much more encouragingresults with a Mol Dock rating of 80.6047Kcal/mol, as opposed to the typical initial line drug isoniazid (Moldock score: -58.7028 Kcal/mol). Molecular docking prediction indicate that Ajoene could be formulated into a possible treatment drug for Mycobacterium tuberculosis.

This manuscript presents an unobtrusive method for sleep apneahypopnea syndrome (SAHS) detection. The airflow is indirectly measured through a sensitive mattress (Pressure Bed sensor, PBS) that incorporates multiple pressure sensors into a bed mattress. The instantaneous amplitude of each sensor signal is calculated through Hilbert transform, and then, the information is reduced via principal component analysis. The respiratory events (ERs -apneas/hypopneas) are detected as a reduction in the resulting instantaneous amplitude and accounted in the respiratory event index (IER), which is a severity indicator similar to the offcial apnea-hypopnea index (AHI). The respiratory signals extracted from PBS are analyzed first by clustering the information coming from channel pairs, and then using the eight channels. The IER performance is compared with the AHI for different severity categories. For the diagnosis of healthy and pathological patients we obtain a sensitivity, specificity and accuracy of 92%, 100% and 96%, respectively using two or eight PBS channels. These results suggest the possibility to propose PBS as an alternative tool for SAHS diagnosis in home environment.

RESUMO

Several lines of evidence are implicating an increased persistence of apoptotic cells in patients with asthma. This is largely due to a combination of inhibition, or defects in the apoptotic process and/or impaired apoptotic cell removal mechanisms. Among apoptosis-inducing genes, an important role is played by p53. In the present study, we have investigated the possible relationship between p53 codon 72 polymorphism and asthma and the interaction with ACP1, a genetic polymorphism involved in the susceptibility to allergic asthma. We studied 125 asthmatic children and 123 healthy subjects from the Caucasian population of Central Italy. p53 codon 72 and ACP1 polymorphisms were evaluated using a restriction fragment length polymorphism-polymerase chain reaction (RFLP-PCR) method. There is a statistically significant association between p53 codon 72 polymorphism and allergic asthma: Arg/Arg genotype is more represented in asthmatic patients than in controls (P=0.018). This association, however, is present in subjects with low ACP1 activity A/A and A/B only (P=0.023). The proportion of children with A/A and A/B genotype carrying Arg/Arg genotype is significantly high in asthmatic children than in controls (OR=1.941, 95% C.I. 1.042-3.628). Our finding could have important clinical implications since the subjects with A/A and A/B genotypes of ACP1 carrying Arg/Arg genotype are more susceptible to allergic asthma than Pro/Pro genotype.

RESUMO

Objective: To compare the efficacy of casein phosphopeptide-amorphous calcium phosphate nanocomplexes (CPP-ACP) and Duraphat varnish on preventing radiation caries and dentin hypersensitivity (DH) in irradiated head and neck cancer pa-tients. Methods:A total of 112 patients with head and neck cancer who will undergo radiotherapy were selected and randomly divided into the CPP-ACP group (experimental group) and the Duraphat varnish group (control group). A day prior to the scheduled radiothera-py, CPP-ACP containing paste had been applied once a day for 5 weeks to the teeth surface of the experimental group. Duraphat varnish was applied with the same frequency in the control group. The decay missing filling tooth (DMFT), decay missing filling surface (DMFS), and DH of two groups before radiotherapy and 12 months after radiotherapy were observed and statistically analyzed. Re-sults:Before radiotherapy, DMFT, DMFS, and DH showed no significant differences between the groups (P>0.05). However, signifi-cant increase in DMFT, DMFS, and DH was noted in the two groups 12 months after the radiotherapy (P<0.05). A higher increase was observed in the DMFT, DMFS, and DH of the control group compared with those in the experimental group (P<0.05). Conclusion:CPP-ACP effectively reduced radiation caries and sensitivity in irradiated head and neck cancer patients and deservesclinical applica-tion.

RESUMO

OBJECTIVE: The purpose of this study was to evaluate the effect of casein phosphopeptide amorphous calcium phosphate (CPP-ACP) on the shear bond strength (SBS) of brackets bonded to non-demineralized teeth with either phosphoric acid etching or self-etching primer. METHODS: Sixty human premolars were randomly assigned to 1 of 4 treatment groups (n = 15 each): phosphoric acid etching (group 1); self-etching primer (group 2); CPP-ACP for 2 weeks + phosphoric acid etching (group 3), and CPP-ACP for 2 weeks + self-etching primer (group 4). After bonding of the maxillary premolar metal brackets, specimens were subjected to shear forces in a testing machine. Scanning electron microscopy was used to observe etching patterns on the enamel surfaces of all teeth. A 2-way analysis of variance was used to test for effects of CPP-ACP and etching system on SBS. RESULTS: Significantly higher mean SBSs were observed in groups subjected to phosphoric acid etching (i.e., groups 1 and 3; p 0.05). We observed a uniform and clear etched pattern on the enamel surface of the phosphoric acid etching groups. CONCLUSIONS: CPP-ACP does not significantly affect the SBS of orthodontic brackets bonded to non-demineralized teeth, regardless of which adhesive method is used to bond the brackets.
