RESUMO

ObjectiveTo analyze the application of β-lactam antibiotics and bacterial drug resistance in our hospital for providing basis of the rational use of antibiotics.MethodsThe DDDs,total consumption sum,DDC and bacterial drug resistance of β-lactam antibiotics during 2008-2009 were analyzed statistically.ResultsCephem Antibiotics was the most extensive application,and the DDC of Benzylpenicillin Sodium was the minimum in the β-lactam antibiotics.There had bacterial drug resistance of most β-lactam antibiotics,but the differences were not significant.ConclusionThere was irrational use of β -lactam antibiotics,and it was important to enhance antibiotics supervisal and drug sensitivity before use for decreasing the abuse of antibiotics.

There have been conflicting data about the interactions between vancomycin and beta-lactam agents against Staphylococcus aureus strains with heterogeneous resistance to vancomycin. We evaluated the efficacy of these combinations against Mu 3 and heterogeneously vancomycin-resistant S. aureus (hetero-VRSA) strains which were isolated from Korean patients using a population analysis method. Antagonistic effects were observed when less than 1 g/mL of beta-lactam antibiotics was combined with vancomycin, whereas synergistic effects were noticed with more than 4 microgram/mL of beta-lactam antibiotics. The antagonistic effects at low concentrations of beta-lactams were most prominent at 2 microgram/mL of vancomycin, which were the vancomycin MICs of tested hetero-VRSA strains. This study showed the variable effects of vancomycin- beta-lactam combinations depending on the concentrations of beta-lactam antibiotics and this property could be used to develop screening methods for hetero-VRSA strains.

RESUMO

Se realizó un estudio prospectivo de 15 pacientes con empiema pleural, ingresados en el Hospital Clinicoquirúrgico Docente "Comandante Manuel Fajardo", en el período de enero de 1995 a diciembre de 1997. La forma aguda se presentó en 5 enfermos, mientras que la crónica se presentó en 10. La neumopatía inflamatoria fue la causa más frecuente. La mayoría fue tratada con combinaciones de antibióticos betalactámicos y aminoglucósidos, complementada con terapéutica quirúrgica. En la forma aguda resultó útil el lavado pleural, mientras que en la crónica el proceder más utilizado fue la decorticación, seguido de la ventana torácica y la resección pulmonar. Sólo falleció un enfermo con un cáncer de pulmón avanzado.


A prospective study of 15 patients with pleural empyema admitted at the "Comandante Manuel Fajardo" Clinical and Surgical Teaching Hospital from January, 1995, to December, 1997, was conducted. The acute form was observed in 3 patients and the chronic form in 10. The inflammatory neumopathy was the most frequent cause. Most of the patients were treated with combinations of lactam and amynoglycoside antibiotics, complemented with surgical therapeutics. In the acute form pleural lavage proved to be useful, whereas in the chronic form the most used procedure was decortication, followed by thoracic window and pulmonary resection. Only one patient died with advanced lung cancer.

RESUMO

patients were treated with mezlocillin/sulbactam, among them there were 59 cases of respiratory infection, 36 cases of urinary system infection,13 cases of abdominal infection, and 2 cases of septicemia. 84 patients were successfully cured, and 25 patients were markedly improved. The total effective rate was 99.9%, and the side effect rate was only 4.5%, which included one case of skin rash and two cases of abnormally high GPT. The study in vitro showed that antibacterial activity of mezlocillin was markedly enhanced against most ordinary pathogens with the help of sulbactam in the clinic.
