Introducción: La interrupción voluntaria del embarazo es un asunto político que polariza las opiniones y genera controversias entre el sector salud, social, económico y cultural. Objetivo: Determinar el nivel de conocimiento y actitudes sobre la interrupción voluntaria del embarazo en estudiantes de dos programas de Ciencias de la Salud de Cartagena. Materiales y métodos: Estudio descriptivo transversal, se aplicó el cuestionario adaptado por Marceliano y Medrano a 231 estudiantes de Enfermería y Medicina, los datos se analizaron obteniendo frecuencias relativas y absolutas. Resultados: El nivel de conocimiento de los estudiantes fue alto con 92,20% (213). Sin embargo, el 37,2% (86) no sabía cuáles eran las tres causales para que el aborto no se considere un delito. El 56,7% asume actitudes negativas hacia firmar una petición para legalizar el aborto y el 18,8% considera que si una mujer se realiza un aborto debe ir a la cárcel. Conclusión: El nivel de conocimiento de los estudiantes de medicina y enfermería es alto, sin embargo, los patrones muestran desconocimiento sobre las causales de legalización del aborto de acuerdo con la norma vigente y una variabilidad de actitudes positivas y negativas ante la interrupción voluntaria del embarazo.


Introduction: Voluntary interruption of pregnancy is a political issue that polarizes opinions and generates controversy within the health, social, economic, and cultural sectors. Objective: To determine the level of knowledge and attitudes regarding voluntary interruption of pregnancy in university students from two Health Sciences programs of Cartagena. Materials and methods: A descriptive cross-sectional study; the questionnaire adapted by Marceliano and Medrano was given to 231 students within the Nursing and Medical programs; the data was analyzed through relative and absolute frequencies. Results: The level of knowledge of the students was high with a 92.20% (213). Yet, a 37.2% (86) did not know the three reasons why abortion is not considered a crime. 56.7% have negative attitudes regarding signing a petition to legalize abortion and an 18.8% believe that a woman who has an abortion should go to jail. Conclusion: The level of knowledge in medical and nursing students is high, yet the patterns show lack of knowledge of the reasons for legalization of abortion according to the current laws and a variety of positive and negative attitudes regarding the voluntary interruption of pregnancy.

Purpose: To examine the historical process for institutionalization of therapeutic abortion in Peru and to explain its current condition. Methodology: Review of bibliographic information available in the country regarding the process conducted by many institutions to have the Ministry of Health approve the National Guide for Therapeutic Abortion. Subsequently, the lead researcher met with 10 heads of the Gynecology and Obstetrics Service/Department from the same number of hospitals, from whom he requested information, as they have been regulating the provision of therapeutic abortion services by means of local Guides or Protocols for the last 5 years. Results: The formalization of the National Guide for the provision of therapeutic abortion services by the Ministry of Health has been a persistent endeavor of many institutions of civil society, but the 10 convened hospitals have already been responding to the demand for the past 5 years. In these 5 years, 257 therapeutic abortions were performed. The frequency of therapeutic abortions is still low; however, there is a progressive increase over the years. The frequency of therapeutic abortions is predominant in the second trimester of pregnancy, with congenital anomalies incompatible with life being the most frequent cause. Misoprostol to evacuate the uterus has been used in 95% of the cases, and only 2% minor complications have been verified.


Objetivo: Examinar el proceso histórico para institucionalizar el aborto terapéutico en el Perú y precisar su situación actual. Metodología: Revisión de la información bibliográfica existente en el país en relación al proceso seguido por muchas instituciones hasta conseguir que el Ministerio de Salud apruebe la Guía Nacional para el Aborto Terapéutico. Luego el investigador principal convocó a 10 jefes de servicio/departamento de Ginecología y Obstetricia de igual número de hospitales que tienen regulada la prestación de servicios de aborto terapéutico mediante Guías o Protocolos locales desde 5 años antes, a quienes se les solicitó información. Resultados: La oficialización de la Guía Nacional para la prestación de servicios de aborto terapéutico por parte del Ministerio de Salud ha sido un trabajo persistente de muchas instituciones de la sociedad civil, pero los 10 hospitales convocados ya venían respondiendo a la demanda desde 5 años atrás. En los 5 años se atendió 257 abortos terapéuticos. La frecuencia del aborto terapéutico es aún baja, sin embargo se aprecia un aumento progresivo conforme avanzan los años. Es predominante la frecuencia de abortos terapéuticos en el segundo trimestre del embarazo, siendo la causa más frecuente las anomalías congénitas incompatibles con la vida. El uso del misoprostol en la evacuación del útero ha alcanzado el 95% de los casos y no se ha verificado más de 2% de complicaciones leves. Conclusión: El aborto legal por causales de salud se viene ofertando en forma restringida en el Perú.

RESUMO

It is usual to understand conscientious objection as a doctor’s refusal to perform a legitimate intervention, for subjective personal reasons unrelated to medicine. It is then accepted only by respect to professional autonomy and freedom of beliefs. Understood in that way it would be thus reasonable to limit it, curtail it or delete it, since the objector would not be willing to grant benefits that correspond to his medical profession. This work aims to show exactly the opposite, taking as an example the project of law that pretends to decriminalize abortion in some situations in Chile. Is the objector who defends medical activity relying on ethic codes that seek to preserve the values and principles of our profession.

OBJECTIVE: To identify perceived barriers to accessing reproductive health care according to the women of Ocotal, Nicaragua; describe their understanding of their reproductive rights; and document their opinions about Nicaragua's total ban on abortion. METHODS: From May to June 2014, three focus group discussions were held in Spanish with 17 women from two different neighborhoods (barrios) in the city of Ocotal, Nicaragua. A semi-structured discussion guide with open-ended questions was employed to elucidate local perspectives regarding the focus group discussions themes. RESULTS: Serious obstacles including 1) violence against women, 2) machismo, 3) criticism from others, and 4) lack of communication and education limit women's ability to make their own reproductive health decisions. Women had a pervasive lack of knowledge about reproductive rights and the international human rights documents that define them. In addition, due to religious and cultural ideologies, most women supported the country's total ban on abortion in most circumstances, with the possible exception of rape. CONCLUSIONS: Both men and women in Ocotal should be encouraged to participate in community-level programs designed to reduce the impact of the following obstacles to receiving reproductive health care: 1) violence against women and machismo; 2) insufficient, non-standardized sexual education and information about reproductive rights; and 3) poor communication within families and the community at large. Any future public health campaigns to address women's reproductive health needs in Ocotal should implement these types of programs, at the neighborhood level, to reduce stigma surrounding sexual health and activity.


OBJETIVO: Determinar cómo perciben las mujeres de Ocotal, Nicaragua, las barreras de acceso a la atención de salud reproductiva; describir sus conocimientos acerca de los derechos reproductivos; y consignar sus opiniones acerca de la prohibición total del aborto en Nicaragua. MÉTODOS: De mayo a junio del 2014, se establecieron tres grupos de discusión en español en los que participaron 17 mujeres de dos barrios diferentes de la ciudad de Ocotal. Se utilizó una guía de discusión semiestructurada que constaba de preguntas de respuesta libre para dilucidar las perspectivas locales con respecto a los temas del grupo de discusión. RESULTADOS: Los obstáculos graves, incluidos 1) la violencia contra la mujer, 2) el machismo, 3) las críticas por parte de otros, y 4) la falta de comunicación y formación, limitan la capacidad de las mujeres para tomar sus propias decisiones de salud reproductiva. Las mujeres mostraron una carencia generalizada de conocimientos acerca de sus derechos reproductivos y los documentos internacionales de derechos humanos que los definen. Además, como consecuencia de sus ideas religiosas y culturales, la mayor parte de las mujeres apoyaron la prohibición total del aborto en el país en la mayor parte de las circunstancias, con la posible excepción de la violación. CONCLUSIONES: Se debe alentar a los hombres y mujeres de Ocotal a participar en los programas comunitarios diseñados para reducir la repercusión de los siguientes obstáculos para obtener atención de salud reproductiva: 1) la violencia contra la mujer y el machismo; 2) la educación sexual no estandarizada y la información acerca de sus derechos reproductivos insuficientes; y 3) la comunicación deficiente dentro de las familias y en la comunidad en general. Con objeto de reducir el estigma en torno a la salud y la actividad sexuales, las futuras campañas de salud pública orientadas a tratar las necesidades de salud reproductiva de las mujeres de Ocotal deben llevar a cabo en los barrios estos tipos de programas.

A pesar de que el aborto, por razones de salud, no es considerado un delito en el Perú, el Estado niega su inclusión en la política pública, con lo cual infringe el derecho de las mujeres a interrumpir un embarazo cuando este afecta su salud. Al examinar el artículo del Código Penal que exceptúa de delito a este tipo de aborto, se identifican atribuciones que protegen a las mujeres y establecen condiciones para ofertar dicha atención. En el presente documento se pone en debate los argumentos que el Estado Peruano viene usando para no aprobar el Protocolo de Aborto Terapéutico, con el cual se reglamentaria su oferta y financiamiento en los servicios públicos y se fundamenta por qué se debería cumplir con esta obligación, en base al marco conceptual de la “causal salud”. Se presentan, además, dos casos judicializados, en que el Estado Peruano ha sido encontrado responsable de violar los derechos humanos de dos adolescentes a quienes se les negó el aborto terapéutico.


Although abortion for health reasons is not considered a crime in Peru, the State does not allow its inclusion in public policy, thus violating women’s right to terminate a pregnancy when it affects their health. When examining the article in the Criminal Code which decriminalizes this type of abortion, provisions are identified which protect women and set the conditions to offer this type of service. This document sets the debate about the arguments used by the Peruvian State for not approving a therapeutic abortion protocol which would regulate the provision and financing of therapeutic abortion in public services, and explains why this obligation should be complied with, based on the conceptual framework of “health exception”. In addition, it presents two cases brought before the judicial court in which the Peruvian State was found guilty of violating the human rights of two adolescents to whom a therapeutic abortion was denied.

Objective This paper aims at the therapeutic effect aft er the artifitial abortion by employing the TCM periodic treatment with respect to shortening the vaginal bleeding time and accelerating the uterine lining recovery. Methods Sampling 280 cases of artifitial abortion in early pregnancy, the author divides them at random the testing and control groups with each of the membership of 140 cases. Given the Nifuratel perorally same as the control group, the testing group is additionally under the TCM periodic treatment, that is, dosing Chinese medicine with the purpose of eliminating blood stasis,promoting tissue regeneration as well as invigorating the vital energy and nourishing the blood during the 1 to 3 days'postoperation period, giving Chinese medicine with a view of strenthening liver and kidney together with activating vital energy during the 4 to 14 days' postoperation period, instructing Chinese medicine in order to nourish the spleen and kidney as well as regulate Chong and Ren Vessels during the 15 to 25 days' postoperation period perorally and respectivelly. In contrast, the control group is given Xuefuzhuyu Capsules after the whole postoperation period. Results In terms of the time of vaginal bleeding, menstrual recovery and the regenerating condition of the uterine lining, the result (except for 3 cases in testing group and 2 cases in control group quiting the experiment) shows that after treatment (9 days after the abortion) the testing group achieves the effect superior to that of control group (P<0.01). Conclusion The conclusion is that the TCM periodic therapy is effective in the aspect of shortening the vaginal bleeding time, accelerating the uterine lining recovery and regulating the menstrual circle.
