RESUMO

Objective@#To investigate the value of acoustic parameters in the voice therapy for patients with unilateral vocal cord paralysis (UVCP).@*Methods@#From May 2015 to April 2018, 51 patients with UVCP and 59 healthy controls in Department of Otorhinolaryngology Head and Neck Surgery, Tianjin First Central Hospital, were involved in this research retrospectively. The UVCP patients were diagnosed with stroboscopic laryngoscopy. The minimum glottal area (MGA) was calculated by KIPS software when the people were pronouncing/i:/. The fundamental frequency (F0), Jitter, Shimmer and NHR were detected by CSL4500 multiple acoustic voice analyzer.@*Results@#MGA of UVCP patients was much higher than that of healthy control (male: 433.68±64.52 vs. 294.41±51.82, t=9.23, P=0.000; female: 498.80±73.42 vs. 302.03±76.54, t=13.21, P=0.000), which meaned vocal cord insufficiency.After voice therapy, MGA reduced significantly (male: 288.48±55.09, female: 258.22±57.17, t=24.41 and 31.22, P=0.000 vs. pre-therapy). MGA of untreated patients decreased in varying degrees. Compared with the voice therapy group, the MGA decreased in a significantly lower extent (24.25±22.91 vs. 188.31±54.37, t=8.97, P=0.000). The F0, Jitter, Shimmer and NHR raised significantly in UVCP patients group (P=0.000 vs. healthy control group), and they were reduced by voice therapy (all P<0.05). Each of the four acoustic parameters was relative with MGA, r=0.551, 0.867, 0.853 and 0.875 in turn, P=0.001, 0.000, 0.000, and 0.000.@*Conclusion@#MGA and acoustic parameters can reflect the acoustic features of UVCP patients, which are useful tools in the UVCP assessment and voice therapy.

BACKGROUND AND OBJECTIVES: The aim of this study was to compare between preoperative and postoperative voices by subjective auditory perceptual evaluation and objective acoustic analysis. The study also analyzed the acoustic parameters that best reflected the auditory perceptual characteristics in laryngeal microsurgery. SUBJECTS AND METHOD: The records of 49 patients who had been diagnosed as benign vocal fold mucosal diseases and had undergone laryngeal microsurgery were retrospectively analyzed. Preoperative and postoperative voices were assessed by auditory perceptual evaluation using the GRBAS Scale and acoustic analysis using Multi-Dimensional Voice Program (MDVP) and Analysis of Dysphonia in Speech and Voice (ADSV). Correlation between auditory perceptual characteristics and acoustic parameters was analyzed. RESULTS: Auditory perceptual evaluation showed that the patients' voices were improved after the operation. Several acoustic parameters of MDVP related to short-term and long-term variability showed significant improvement. Also, there were significant differences in several cepstral/spectral parameters of ADSV after operation. In the correlation analysis, the overall grade (G) of the GRBAS Scale showed highest correlation with the cepstral spectral index of dysphonia (CSID) (r=-0.700). The roughness (R) of GRBAS Scale was correlated with the smoothed amplitude perturbation quotient of long-term variability of MDVP (r=0.468), breathiness (B) with cepstral peak prominence (r=-0.703) and strain (S) with CSID (r=0.650) (p<0.01). CONCLUSION: Our results showed that the acoustic parameters were related to the long-term variability of MDVP, and the parameters of cepstrum/spectrum were useful for the objective assessment of voice quality in patients who received laryngeal microsurgery. These parameters were also useful for the quantification of voice quality and abnormality of phonation.

Objective To study the dynamic changes and potential impacts on the voice in postoperative pa-tients with different types of thyroid operation.Methods A total of 48 cases thyroidectomy,34 with thyroid isth-mus and lobectomy and 4 with total thyroidectomy,and 19 cases neck operation were included in this study for the examinations of electric laryngoscope,and acoustic voice analysis preoperative and postoperative,respectively.The characteristics of the acoustic parameters were analyzed.Results Vocal cord paralysis and cricothyroid joint disloca-tion in all the patients before and after operation were not observed.For male patients in the experimental group, postoperative parameter values of F0TD were higher than the day before surgery and 30 days after thyroidectomy. The difference was statistically significant (P 0.05).The parameter values of F0 in famale patients before the thyroidectomy,the 1st,2nd,3rd,7th day after the surgery and the parameter value of Jitter,Shimmer,HNR for 1st day after the operation were significantly dif-ferent(P 0. 05).Conclusion Voice parameter values alter in a way after thyroid surgery without apparent recurrent laryngeal nerve injury,especially in the female patients.The improvement of voice acoustic parameters can be observed on the 30th day after operation.

RESUMO

The primary objective of this work was to present the acoustical identification of humpback whales, detected by using an autonomous ambient noise measurement system, deployed in the shallow waters of the Southeastern Arabian Sea (SEAS) during the period January to May 2011. Seven types of sounds were detected. These were characteristically upsweeps and downsweeps along with harmonics. Sounds produced repeatedly in a specific pattern were referred to as phrases (PQRS and ABC). Repeated phrases in a particular pattern were referred to as themes, and from the spectrographic analysis, two themes (I and II) were identified. The variation in the acoustic characteristics such as fundamental frequency, range, duration of the sound unit, and the structure of the phrases and themes are discussed. Sound units were recorded from mid-January to mid-March, with a peak in February, when the mean SST is ~28°C, and no presence was recorded after mid-March. The temporal and thematic structures strongly determine the functions of the humpback whale song form. Given the use of song in the SEAS, this area is possibly used as an active breeding habitat by humpback whales during the winter season.

Objective To investigate the acoustic characteristics of the voice of 3~6 years old children with hearing-impairment. Meth-ods The voice samples were collected from 148 hearing-impaired children and 122 normal children, all aged 3~6 years. The acoustic param-eters including F0, F0SD, jitter, shimmer, NNE, OQ, SQ, AQ, STF were compared. Results The F0, SQ, STF, F0SD, jitter were more in the children with hearing-impairment than in the normal children (P0.05). There was no significant difference of all the acoustic parameters among chil-dren with different ages (P>0.05), except the NNE was less in 3~4 years old children than 5~6 years old children (P0.05). Conclusion The vibration of vocal cords is instable and ir-regular, saying overquick and close intensely in hearing-impaired children. The age and sex do not influence the acoustic parameters, except NNE is less in younger children.

Objective To investigate voicing changes of adult patients with obstructive sleep apnea hypopnea syndrome (OSAHS) before and after H-uvulopalatopharyngoplasty (H-UPPP). Methods 56 adult OSAHS pa-tients and 40 healthy people were included in the study. Acoustic parameters and formant frequencies were measured for each patient before and after H- UPPP, and also for the control group. Results Acoustic parameters: each group demonstrated no differences in all the parameters except for normalized noised energy (NNE). NNE increased after H-UPPP. Formant frequency: F1, B1, F2, B2, F3 of OSAHS patients were significantly lower than normal control. There was no significant difference in the formant frequency before operation and one week after; however, F1 and F2 were lower than the normal control one week after surgery. One month after surgery, F1 and F2 were ob-viously higher than that obtained in one week. All the other parameters compared with normal controls showed no significant discrepancies. Conclusion Acoustic characteristics of adult OSAHS patients were different from healthy person. After H-UPPP, the vocal tracts of patients changed, thus causing improvement to the acoustic parameters and voicing qualities, especially at the formant frequency. After the surgery, the formant frequencies of the patients increased gradually to the range of healthy people.

BACKGROUND AND OBJECTIVES: A number of studies have found that anatomic and physiological changes of the larynx with advancing age result in reduced control of the phonatory mechanism in elderly individuals. Although vocal signs of senescence are a small reflection of involution, it is important to understand the effects of normal aging on voice by differentiating changes precipitated by aging from those precipitated by physical abnormalities. The purposes of this study are to investigate the characteristics and examine the normative reference of the acoustic parameters with aging in Koreans. MATERIALS AND METHODS: The acoustic measurements in 300 normal persons of all age groups without laryngeal and audiological problems were assessed by multi-dimensional voice program (MDVP) 4305, KAY Elemetrics Corp., NJ, USA), and the changes due to advancing age were analysed. RESULTS: Fundamental frequency in women decreased with advanced age but it was noted with stability in men as they aged. Frequency stability decreased with advanced age in Koreans; amplitude stability decreased in men but not in woman. The normative reference of acoustic parameters in all age groups was also discussed. CONCLUSION: It may be suggested that the different normal thresholds for evaluating acoustic parameter in normal voice should be adjusted in different age groups in Korean.

BACKGROUND AND OBJECTIVES: The survival rate of laryngeal cancer has improved in recent day. But in case of advanced cancer, radical destructive surgery is required for survival. So early detection of laryngeal cancer prevent from this disastrous condition. In this point of view, mass screening test for early detection of laryngeal cancer is necessary. Screening tests using voice has many advantages of being simple, non invasive, and requiring less space. In this system, the most important factor is the selection of acoustic parameters to be used in voice analysis. Thus authors studied the acoustic parameters that can differentiate normal, benign, malignant laryngeal diseases by an acoustic analyzing system and we also checked the availability of parameters. MATERIALS AND METHODS: We evaluated the voice analyzed data from 25 laryngeal malignancy patients, 33 benign laryngeal disease patients, 35 normal control who visited PNUH otolaryngology department from October 1996 to May 1998. A computerized speech lab. 4300B (CSL) was used to carry out the analysis of each voice sample and statistical analysis, ANOVA. Canonical analysis and cumulative frequency curve were used. RESULTS: The statistically significant parameters that can differentiate normal and malignant laryngeal disease groups were 15 parameters and can differentiate normal and benign laryngeal disease group were 9 parameters and that can differentiate benign and malignant laryngeal disease group were 7 parameters. CONCLUSION: We consider that these parameters and detection programs may be effective in development of a screening system using voice only. Developing diagnostic tools and programs would need further study of phonetics and voice engineering.
