RESUMO

ABSTRACT Neuroleptic malignant syndrome (NMS) is a neurologic emergency potentially fatal. This rare side effect is most commonly associated with first-generation antipsychotics and less frequently with atypical or second-generation antipsychotics. The diagnosis relies on both clinical and laboratory criteria, with other organic and psychiatric conditions being ruled out. CASE REPORT: A 39-year-old female patient, who is institutionalized and completely dependent, has a medical history of recurrent urinary infections and colonization by carbapenem-resistant Klebsiella pneumoniae. Her regular medication regimen included sertraline, valproic acid, quetiapine, risperidone, lorazepam, diazepam, haloperidol, baclofen, and fentanyl. The patient began experiencing dyspnea. Upon physical examination, she exhibited hypotension and a diminished vesicular murmur at the right base during pulmonary auscultation. Initially, after hospitalization, she developed high febrile peaks associated with hemodynamic instability, prompting the initiation of antibiotic treatment. Despite this, her fever persisted without an increase in blood inflammatory parameters, and she developed purulent sputum, necessitating antibiotherapy escalation. The seventh day of hospitalization showed no improvement in symptoms, suggesting NNMS as a differential diagnosis. All antipsychotic and sedative drugs, as well as antibiotherapy, were discontinued, after which the patient showed significant clinical improvement. CONCLUSION: Antipsychotic agents are commonly employed to manage behavioral changes linked to various disorders. However, their severe side effects necessitate a high degree of vigilance, the cessation of all medications, and the implementation of supportive care measures. A prompt and accurate diagnosis of NMS is crucial to alleviating the severe, prolonged morbidity and potential mortality associated with this syndrome.

RESUMO

OBJECTIVE To mine the adverse drug event (ADE) signals of selinexor, and to provide reference for its clinical safety medication. METHODS ADE data for selinexor reported from July 3rd, 2019 to March 31st, 2023 were collected from the FDA adverse event reporting system (FAERS). Data mining was performed by using the reporting odds ratio (ROR) and proportional reporting ratio (PRR) methods, and categorization statistics were performed by using the system organ class (SOC) and preferred term (PT) from drug ADE terminology set in the MedDRA (version 26.0). RESULTS A total of 3 084 ADE reports were obtained for selinexor, with a total of 134 ADE-positive signals. Among the reported genders, there were 127 males and 124 females, with a predominant age of ≥65 years old (4.12%); the United States had the highest number of reports (96.53%), with consumers being the main reporters (77.27%); severe ADR was mainly characterized by hospitalization/prolonged hospitalization (26.26%), followed by death (17.15%). The top 3 ADE in the list of frequency were nausea (1 162 times), fatigue (790 times) and anorexia (610 times), all of which were mentioned in the selinexor’s instructions. The top 3 signals in the list of strength were device-associated bacteremia (ROR=115.07, PRR=114.94), blepharospasm dysfunction (ROR=106.70, PRR=106.54), and salmonella sepsis (ROR=99.90, PRR=99.81), all of which were not mentioned in the selinexor’s instructions. CONCLUSIONS In addition to the ADE of nausea mentioned in the instruction manual, attention should also paid to device-associated bacteremia, blepharospasm dysfunction, salmonella sepsis, and other ADE not mentioned in the instruction manual when using selinexor in clinical practice; weekly rechecking of the patient’s blood routine should be done to monitor the patient’s blood indexes, symptoms of infection, and so on, to ensure that the safety of 1661962346@qq.com drug use.

OBJECTIVE To provide reference for clinically safe drug use by mining oxaliplatin-related adverse drug events (ADE) of the nervous system. METHODS Oxaliplatin-related neurologic ADE data reported by the FDA adverse event reporting system (FAERS) between January 1st, 2004 and December 31st, 2022 were collected. The reporting odds ratio and proportional reporting ratio were used for data mining. The data were classified statistically by using systematic organ classification, high-level group term (HLGT) and preferred term (PT) in the MedDRA (version 26.0). RESULTS A total of 7 266 cases of oxaliplatin- related ADE, which were classified as various neurological, were retrieved, and 100 PT were identified. Of these, fifty-seven PT were unspecified adverse reaction signals in the manual. Among these reports, males (46.85%) were more than females (42.98%), the age of patients was 45-<75 years (65.22%), the number of reports was highest in Italy (16.32%), and the severe type was hospitalization or prolonged hospitalization (38.16%). The top 5 PT reports in the list of case number were peripheral neuropathy, paresthesia, neurotoxicity, loss of consciousness and dysarthria. The top 5 PT reports in the list of signal intensities were cold- induced paresthesia, neuromuscular rigidity, acute polyneuropathy, neuronal neuropathy, axonal and demyelinating polyneuropathy. A total of 13 HLGT were involved, with neurological diseases (not classified separately) having the highest number of signals (29). CONCLUSIONS When using oxaliplatin in clinical practice, it is not only necessary to monitor the high incidence of acute and chronic peripheral neuropathy, but also to pay attention to the patient’s consciousness state and neurological symptoms. We should pay attention to the rare types of adverse reactions, such as guillain-barre syndrome, Lhermitte sign, posterior reversible encephalopathy syndrome, and hyperammoniacal encephalopathy, so as to ensure the safety of medication.

ObjectiveThis paper aims to analyze the clinical characteristics and medication rationality of liver injury related to Epimedii Folium preparation (EP) and explore the possible risk factors of liver injury, so as to provide a reference for the safe clinical application of Epimedii Folium (EF). MethodA retrospective analysis was conducted on liver injury cases related to EP from 2012 to 2016. ResultThe number of reported liver injury cases and the proportion of severe cases related to the use of EP show an increasing trend, indicating the objective existence of liver injury caused by EP. There are more cases of liver injury related to EP in women than in men, with an onset age range of 15-91 years old and a median onset age of 60 years old (median onset ages for men and women are 59 and 60 years old, respectively). The time span from taking EP alone to the occurrence of liver injury is 1-386 days, with a median of 38 days. The time span from taking both EP and Western medicine to the occurrence of liver injury is 1-794 days, with a median of 34 days. EF-related liver injury preparations are mostly composed of traditional Chinese medicines that promote immunity and tonify the liver and kidney, indicating that immune stress in the body may be the mechanism of liver injury caused by the use of EP alone or in combination. There is no increasing trend of toxicity with time or dose in the liver injury caused by EP. By further exploring its risk factors, it is found that patients have unreasonable medication methods such as excessive dosage, repeated use, and multi-drug combination, which may also be one of the important risk factors for EF-related liver injury. ConclusionEP has a certain risk of liver injury and should be emphasized in clinical diagnosis and treatment. Immune stress may be the mechanism of liver injury caused by EP, and in clinical use, it is necessary to be vigilant about the risk of liver injury caused by unreasonable use and combined use with Western medicine.

OBJECTIVE To explore the whole-process pharmaceutical care model of iodine contrast medium and promote the rational clinical use of iodine contrast medium. METHODS Clinical Professional Committee on Rational Drug Use of China Medical Education Association and Expert Committee on Drug Evaluation and Clinical Research of Guangdong Pharmaceutical Association organized domestic experts to establish a working group on the Consensus on the whole-process pharmaceutical care for iodine contrast medium. The working group conducted literature searches, evidence-based analysis, and discussions on the development process, indications, contraindications, adverse drug reactions, drug interactions, drug use for special population, pharmaceutical care, and other key topics to summarize the content and process of the whole-process pharmaceutical care for iodine contrast medium. This consensus was ultimately formed. RESULTS The consensus on whole-process pharmaceutical care for iodine contrast medium included an evaluation of the patient, renal function, combined drug use, and hydration regimen before examination, the presence of contrast agent extravasation or suspected acute adverse reactions during examination, observation time points and follow-up after examination, and the presentation of specific work in each stage through pharmaceutical care flowchart. The medication monitoring record form was also formed to record the work situation. CONCLUSIONS The consensus has established a whole-process pharmaceutical care system for iodine contrast medium, providing scientific evidence for clinical physicians and nursing staff in the rational use of such special drugs, and also serving as a reference for pharmacists in providing related pharmaceutical care.

OBJECTIVE To explore and analyze the adverse drug event (ADE) signals of cinacalcet and etelcalcetide, to provide a reference for safe drug use in the clinic. METHODS ADE reports related to cinacalcet and etelcalcetide were extracted from the FDA Adverse Event Reporting System from January 1st, 2004 to June 30th, 2023 using the OpenVigil online tool. The Bayesian confidence propagation neural network method was adopted to detect the signals of ADE from the key organ systems. The signals were encoded according to the preferred term in the ADE terminology set of the Medical Dictionary for Regulatory Activities (26.0 edition). RESULTS A total 41 709 and 1 710 ADE reports were extracted, and 29 and 45 safety signals were detected in key systems for cinacalcet and etelcalcetide, respectively; 20 and 36 positive signals were not included in the drug instructions. Hypocalcemia/decreased serum calcium, abnormal blood parathyroid hormone (PTH)/increased or decreased serum PTH were common ADEs of the two drugs, which were detected in the study. Among the signals not included in the drug instructions, new moderate and strong signals were detected, such as cinacalcet-induced calcification defense (metabolic and nutritional diseases), bone starvation syndrome and high conversion bone diseases (musculoskeletal and connective tissue diseases) as well as etelcalcetide-induced sudden death, necrosis and treatment of non-responders (general disorders, administration site), unstable angina pectoris, myocardial ischemia (cardiac diseases), intestinal perforation, gastric antrum vasodilation and gastric ulcer (gastrointestinal diseases). CONCLUSIONS In the clinical application of the two drugs, apart from the common ADEs such as hypocalcemia and abnormal blood PTH, the surveillance of some new potential ADEs should also be carried out, such as bone starvation syndrome, calcification defense, ventricular disease and other cinacalcet-induced ADEs, sudden death, myocardial ischemia, unstable angina pectoris, intestinal perforation, gastric ulcer and other etecalcetide-induced ADEs. If new ADEs appear, clinic should promptly assess the benefits and risks, and update the treatment plan and pharmacological monitoring plan to ensure the safety of patient medication.

OBJECTIVE To explore the predictive factors of cefoperazone/sulbactam-induced thrombocytopenia in adult inpatients, and to establish and validate the nomogram prediction model. METHODS Data of adult inpatients treated with cefoperazone/sulbactam in Xi’an Central Hospital from Jun. 30th, 2021 to Jun. 30th, 2023 were retrospectively collected. The training set and internal validation set were randomly constructed in a 7∶3 ratio. Singler factor and multifactor Logistic regression analysis were used to screen the independent predictors of cefoperazone/sulbactam-induced thrombocytopenia. The nomogram was drawn by using “RMS” of R 4.0.3 software, and the predictive performance of the model was evaluated by the receiver operating characteristic curve and C-index curve. Hosmer-Lemeshow goodness-of-fit test was used to evaluate the calibration degree of the model. Using the same standard, the clinical data of hospitalized patients receiving cefoperazone/sulbactam in Xi’an First Hospital in the same period were collected for external validation of the nomogram prediction model. RESULTS A total of 1 045 patients in Xi’an Central Hospital were included in this study, among which 67 patients suffered from cefoperazone/sulbactam-induced thrombocytopenia, with an incidence of 6.41%. After the false positive patients were excluded, 473 patients were included finally, including 331 in the training set and 142 in theinternal validation set. Multifactor Logistic regression analysis showed that age [OR=1.043, 95%CI (1.017, 1.070)], estimated glomerular filtration rate (eGFR) [OR=0.988,95%CI(0.977, 0.998)], baseline platelet (PLT) [OR=0.989, 95%CI(0.982, 0.996)], nutritional risk [OR=3.863, 95%CI(1.884, 7.921)] and cumulative defined daily doses (DDDs) [OR=1.082, 95%CI(1.020, 1.147)] were independent predictors for cefoperazone/sulbactam-induced thrombocytopenia (P<0.05). The C-index values of the training set and the internal validation set were 0.824 [95%CI (0.759, 0.890)] and 0.828 [95%CI (0.749, 0.933)], respectively. The results of the Hosmer-Lemeshow test showed that χ 2 values were 0.441 (P=0.802) and 1.804 (P=0.406). In the external validation set, the C-index value was 0.808 [95%CI (0.672, 0.945)], the χ 2 value of the Hosmer-Lemeshow test was 0.899 (P=0.638). CONCLUSIONS The independent predictors of cefoperazone/sulbactam-induced thrombocytopenia include age, baseline PLT, eGFR, nutritional risk and cumulative DDDs. The model has good predictive efficacy and extrapolation ability, which can help clinic identify the potential risk of cefoperazone/sulbactam-induced thrombocytopenia quickly and accurately.

OBJECTIVE To correctly identify and deal with the adverse drug reaction as pancytopenia caused by isavuconazonium and to provide reference for the safe use of isavuconazonium. METHODS Clinical pharmacists analyzed a case of severe infection and renal insufficiency who experienced pancytopenia after using isavuconazonium. Clinical pharmacists screened the drugs used during hospitalization and evaluated the relationship between this adverse drug reaction and isavuconazonium, as well as the possible mechanisms, based on the half-life of the drugs and relevant literature. RESULTS & CONCLUSIONS The relationship between pancytopenia and isavuconazonium was assessed as “possibly related”. When using isavuconazonium, attention should be paid to avoiding the combination of drugs with the same mechanism or potential interaction. For patients who have a course of treatment for more than 2 weeks, have hematological abnormalities or complicated with liver and renal insufficiency, or should use it combined with other drug with same mechanism, it may be considered to increase the frequency of blood routine monitoring.

RESUMO

Tenofovir disoproxil fumarate (TDF) is a first-line treatment for chronic hepatitis B. With increasing use worldwide, the adverse events of renal injury caused by this drug have also attracted industry attention. This article reports a 61- year-old patient with liver cancer complicated with hepatitis B virus (HBV) infection. The patient started using TDF in mid-March 2022 and developed kidney injury after 2 months of treatment, during which he received 2 courses of donafenib combined with sintilimab chemotherapy and irregular administration of diclofenac for pain relief. In this paper, Naranjo’s assessment scale was used to evaluate the drugs that may be associated with renal injury, including TDF and sintilimab, and the drugs that are suspected to be associated with renal injury are donafenib and diclofenac. The renal injury caused by TDF can be judged according to the changes in the patient’s condition, the incidence of drug-induced renal injury, clinical manifestations, occurrence time, occurrence mechanism, drug combination, and high-risk factors. The changes of serum creatinine in patients with liver cancer complicated with HBV infection after TDF should be dynamically monitored in the clinic, and the dose of antiviral drugs should be adjusted if necessary and other antiviral drugs with less impact on renal function can be selected, to provide individualized medication recommendations for tumor patients, reduce the incidence of TDF-related renal injury.

OBJECTIVE To explore the characteristics and regulations of adverse drug reactions (ADR) caused by apatinib, and to provide a reference for the safe use of apatinib in clinic. METHODS Case and group reports on ADR and safety evaluation of apatinib were retrieved from Chinese and English databases such as CNKI, Wanfang medical network, VIP and PubMed since its listing in 2014, literature data were extracted and statistically analyzed after screening. RESULTS Totally 101 cases were included, involving 221 ADR. In the above cases, the male-to-female ratio was 1.24∶1, with the highest proportion of patients aged 51 to 70 years, most of the patients were given a dose of 500 mg or more, and the patients given low dose of apatinib combined with other antitumor drugs were also likely to have ADR. One to two types of adverse reaction were the most common, while the types could reach up to six. Most ADR occurred within 30 days after medication, and the systems/organs involved were mainly the cardiovascular system damage,skin and its accessories damage, gastrointestinal system damage and urinary system damage; the main clinical manifestations were hypertension/aggravation,hand-foot syndrome,abdominal pain diarrhea and albuminuria, etc. Hypertension/aggravation, hand-foot syndrome and myelosuppression were the most common serious ADR. Most ADR could be improved/cured by suspension of administration, dose downregulation and symptomatic treatment. All 4 patients who died had underlying diseases, and their ECOG scores all ≥2 points. Special ADR (such as reversible posterior encephalopathy syndrome, psychiatric disorders, and cognitive impairment) were mostly caused by apatinib itself, or may be caused by apatinib in combination with the primary or underlying disease. CONCLUSIONS Advanced age, large dose, combination medication, underlying diseases and poor physical condition might be the high risks for ADR caused by apatinib. It is recommended to monitor the blood pressure,urine protein and skin of hands and feet of all patients with medication on a daily basis,pay attention to the occurrence of special ADR, and timely detect abnormal states and give effective intervention,so as to avoid the aggravation of ADR and other secondary ADR.

OBJECTIVE To provide reference for safe drug use in clinic by mining the adverse drug events (ADE) of 3 kinds of anti-influenza A virus drugs (oseltamivir, zanamivir, baloxavir marboxil). METHODS The ADE data of oseltamivir, zanamivir and baloxavir marboxil were collected from the FDA adverse event reporting system (FAERS) between the first quarter in 2004 and the third quarter in 2022, and mined by using reporting odds ratio (ROR) method. The designated medical events (DME) were estimated. The system organ class (SOC) in the Medical Dictionary for Regulatory Activities (MedDRA, version 25.0) was used for the classification and statistics of drug ADE terminology. RESULTS A total of 12 636, 1 749 and 1 283 ADE reports were retrieved for oseltamivir, zanamivir and baloxavir marboxil, involving 26, 16 and 17 SOCs, respectively. Oseltamivir was strongly associated with sleep terror, abnormal behavior, hallucination and delirium. Zanamivir was implicated in abnormal behavior, delirium, incoherence, and altered state of consciousness with prominent signal intensity. Baloxavir marboxil was strongly associated with ischemic colitis, hemorrhagic cystitis, erythema multiforme and melaena. Erythema multiform was detected in the DME of three drugs with strong signals. CONCLUSIONS When clinically administering the three drugs, it is crucial to pay close attention to both common adverse reactions and those ADEs that are not explicitly mentioned in the drug instructions. For oseltamivir, clinicians should exercise caution due to the potential risk of acute kidney injury and fulminant hepatitis, necessitating regular monitoring of the patient’s liver and kidney function. When prescribing zanamivir, caution should be exercised due to ADEs related to the respiratory system, including acute respiratory distress syndrome and respiratory failure, necessitating close monitoring of the patient’s respiratory status. Similarly, for baloxavir marboxil, clinicians should be vigilant for potential ADEs such as erythema multiforme and rhabdomyolysis.

OBJECTIVE To provide reference for the clinically safe application of acalabrutinib by mining and analyzing the risk signals of adverse drug events (ADE). METHODS The acalabrutinib-induced ADE reports were extracted from the U.S. FDA adverse event reporting system using the OpenVigil 2.1 platform from November 1, 2017 to March 31, 2023. The reporting odds ratio (ROR) method and composite criteria method from the Medicines and Healthcare Products Regulatory Agency (MHRA) were used for detection of ADE signals. RESULTS There were 7 869 ADE reports of acalabrutinib as the primary suspect drug and 142 ADE positive signals were detected from them, involving 20 system organ classes, which was generally consistent with the ADE recorded in the drug instruction of acalabrutinib, mainly involving general disorders and administration site conditions, various inspection, blood and lymphatic system disorders, various neurological disorders and cardiac disorders. In addition, this study identified several new potential ADE signals that were not mentioned in the drug instruction, including sudden cardiac death, pulmonary toxicity, tumor lysis syndrome, pleural effusion, dyspepsia, gastroesophageal reflux disease, bone pain, decreased blood pressure, and abnormal blood sodium, etc. CONCLUSIONS When using acalabrutinib, in addition to paying attention to the ADE recorded in its instructions, the risk of serious ADE that may lead to death, such as sudden cardiac death and pulmonary toxicity, should also be evaluated to avoid or reduce the occurrence of ADE as much as possible.

Diarrhea caused by chemotherapy is called chemotherapy-related diarrhea (CRD). CRD can lead to reduced treatment effectiveness and compliance, affect the long-term outcome of tumor patients, and can be life-threatening in severe cases. In addition to conventional chemotherapy drugs, many molecularly targeted drugs are also associated with CRD, including small molecule epidermal growth factor receptor (EGFR) inhibitors, anti-EGFR monoclonal antibodies, phosphoinositide 3-kinase inhibitors, small molecule inhibitors of vascular endothelial growth factor receptor, BCR-ABL1 and KIT inhibitors, human epidermal growth factor receptor 2 target inhibitors, cyclin-dependent protein kinase inhibitors, antibody-drug conjugates and other molecularly targeted drugs. The occurrence mechanism may be related to the intestinal mucosal injury or enteritis caused by molecularly targeted drugs. The clinical manifestations are increased stool frequency and/or loose imposition, and patients are often associated with excess hyperproduction and/or colic. The incidence of CRD varies with different drugs. Great importance should be attached to collecting medical history and differential diagnosis, actively intervening and conducting dynamic evaluation, strengthening patient education, and timely detecting and preventing the occurrence of intestinal toxicity.

OBJECTIVE To analyze the occurrence of adverse drug reactions (ADR) between bevacizumab biosimilars and original drugs, and to provide data support for rational use of drugs in clinical. METHODS ADR reports of bevacizumab biosimilars and original drugs reported by Jiangsu Cancer Hospital from January to December 2022 were retrospectively analyzed. RESULTS A total of 6 818 patients were treated with bevacizumab, and 136 ADR patients were reported. The incidence of ADR caused by bevacizumab biosimilars was higher than original drugs (2.18% vs. 0.71%, P=0.004). In ADR reports, the main treatment plan was bevacizumab combined with other tumor drugs (129 patients); 118 patients were cured and improved; there were 108 general reports and 28 serious reports; the main system/organ involved in ADR was the cardiovascular system; there were no statistical significance in the incidence rates of hypertension/blood pressure increase, leukocyte/platelet decrease, diarrhea and fever caused by bevacizumab biosimilars and original drugs. CONCLUSIONS The incidence of ADR related to bevacizumab biosimilars is significantly higher than that of the original drugs, but there is no significant difference in the clinical manifestation of ADR. Clinicians can use bevacizumab biosimilars or original drugs based on the willingness of patients and their families.

Introducción: La adherencia al tratamiento farmacológico favorece la supresión viral y reduce la resistencia a la terapia antirretroviral de gran actividad a largo plazo. Objetivo: Determinar la relación entre los aspectos farmacológicos y la adherencia al tratamiento antirretroviral de una IPS colombiana. Metodología: Estudio analítico transversal en pacientes con diagnóstico de VIH en tratamiento antirretroviral entre los años 2012 a 2020. Se utilizó un modelo de regresión logística binaria múltiple con fines explicativos. Resultados: Se analizaron 9835 pacientes donde la proporción de adherencia fue de 90 % y en el modelo ajustado se evaluó su relación con los antecedentes de no adherencia (ORa:0,52 IC95 °/o:0,40-0,66), grupo farmacológico (2 ITIAN + 1 IP u otro) (ORa:1,22 IC95 %:0,99-1,76), dos tomas al día (ORa:1,02 IC95 %:0,74-1,40), unidades al día (≥ 3) (ORa:0,69 IC95 %:0,47-1,02), reacciones adversas a medicamentos (ORa:0,56 IC95 °%:0,40-0,78), polimedicación (ORa:1,36 IC95 %:1,00-1,85), tiempo TAR (1 a 2 años) (ORa:1,63 IC95 %:1,27-2,09),tiempo TAR (6 a 12 meses) (ORa:1,66 IC95 %:1,27-2,18), tiempo TAR (<6 meses) (ORa:1,36 IC95 %:1,03-1,78), tasa de reclamación de los medicamentos (ORa:0,42 IC95 %:0,32-0,55) y antecedentes PRUM (ORa:0,11 IC95 %:0,09-0,14). Discusión: La proporción de adherencia obtenida es superior a lo descrito para otros países (entre 60-77 %); sin embargo se encuentra que los hallazgos correspondientes al efecto de las variables farmacológicas analizadas son acordes a lo descrito en estudios previos en el tema Conclusión: Los antecedentes de no adherencia, reacciones adversas, tasa de reclamación de los medicamentos y antecedentes de problemas relacionados con el uso de medicamentos son aspectos que reducen la probabilidad de adherencia; mientras que el mayor tiempo de uso del tratamiento aumenta la misma.


Introduction: Adherence to drug treatment promotes viral suppression and reduces long-term resistance to highly active antiretroviral therapy (HAART). Objective: To determine the relationship between the pharmacological aspects and adherence to antiretroviral treatment in a Colombian IPS. Methodology: Cross-sectional analytical study in patients with HIV on antiretroviral treatment between 2012 and 2020. A multiple binary logistic regression model was used for explanatory purposes. Results: A total of 9,835 patients were analyzed where the proportion of adherence was 90 % and in the adjusted model its relationship with history of non-adherence was assessed (ORa: 0,52 95 % CI: 0,40-0,66), pharmacological group (2 NRTI + 1 PI or other) (ORa: 1,22 95 % CI: 0,99-1,76), two doses per day (ORa: 1,02 95 % CI: 0,74-1,40), units per day (≥ 3 ) (ORa: 0,69 95 % CI: 0,47-1,02), adverse drug reactions (ORa: 0,56 95 % CI: 0,40-0,78), polypharmacy (ORa: 1,36 95 % CI : 1,00-1,85), ART time (1 to 2 years) (ORa: 1,63 95 % CI: 1,27-2,09), ART time (6 to 12 months) (ORa: 1,66 95 % CI: 1,27-2,18), ART time (<6 months) (ORa: 1,36 95 % CI: 1,03-1,78), inconsistency in the claim (ORa: 0,42 95 % CI: 0,32-0,55) and PRUM history (ORa: 0,11 95 % CI: 0,09-0,14). Discussion: The proportion of adherence obtained is higher than that described for other countries (between 60-77 %); however, the findings corresponding to the effect of the pharmacological variables analysed are in line with those described in previous studies on the subject. Conclusion: The history of non-adherence, adverse reactions, inconsistencies in the claim fill history and problems related to the use of medications are aspects that reduce the probability of adherence. While the longer time of use of the treatment increases adherence.

Background: Adverse drug reactions (ADRs) is one of the important factors contributing to morbidity and mortality among patients and is a major public health burden. Spontaneous ADR reporting plays an important role in detection of ADRs, reducing their incidence and improving patient safety. India has one of the lowest ADR reporting rates in the world. Aim and Objective: The aim of the study was to assess the knowledge, attitude and practice (KAP) of doctors towards pharmacovigilance. Materials and Methods: A KAP questionnaire validated by subject experts was given to the doctors outside the teaching profession within 50 km of Government TD Medical College, Alappuzha, who consented to be a part of the study. The questions were structured to obtain the demographic details of the doctors, their KAP toward pharmacovigilance. No identifiable information regarding the participants was collected and the participants were assured of their confidentiality. A week’s time was given to the participants to answer the questions. Results: During the period of 1 year from June 2019 to May 2020, a total of 121 doctors responded to the questionnaire. The respondents aged from 26 to 67 years with the mean age being 37.31 ± 14.15 years. 94 (77.7%) thought that anyone could report an ADR while 16 (13.2%) thought that only doctors could report ADRs. Only 76 (62.8%) were aware of the existence of National Pharmacovigilance Centre in India. Ninety-eight (81%) did not know how to submit the ADR form to the nearest pharmacovigilance center. A vast majority 99 (81.8%) thought that ADRs to drugs of any system of medicine could be reported, while 20(16.5%) felt that only ADRs to modern medicines need to be reported. Among the factors discouraging doctors from reporting ADRs, lack of training to report an ADR 99 (81.8%) and lack of time during practice 83 (68.5%) were cited as the common reasons. A vast majority 116 (95.9%) had not received any formal training on ADR reporting and 114 (94.2%) opined that they would like to receive a formal training on the same. Conclusion: Doctors were largely aware of pharmacovigilance but had lack of KAP of reporting ADRs and filling up an ADR form. It emphasizes the need for regular mandatory education and training on ADR reporting among healthcare workers, and also the need to run continuous awareness campaigns on spontaneous reporting of ADRs to enhance reporting rate.

Background: Geriatric patients are at a greater risk of developing adverse drug reaction (ADR) because of changed metabolic activity and renal status associated with aging. As most of the elderly patients suffer from multiple chronic diseases (Diabetes, Hypertension, Arthritis, constipation, peptic ulcer disease, etc.), they are under polypharmacy and the risk of ADRs is exacerbated by multiple medications. Metabolic ADRs include weight gain, hypoglycemia, hyponatremia, hypokalemia, and the endocrinal abnormality are the frequent cause of hospital admission in geriatric patients. Most metabolic ADRs due to drugs are treatable. There are few such studies conducted in India regarding metabolic ADR in geriatric patients. Hence, this study aims to evaluate the metabolic ADRs encountered in geriatric patients with the following objectives in Odisha. Aim and Objectives: The prevalence and spectrum of metabolic ADRs in geriatric population were studied. The causality assessment, severity assessment, and preventability were done using standard scales. Materials and Method: This was a prospective and observational study conducted from September 2016 to September 2018 in Department of Pharmacology in collaboration with Department of Geriatric Medicine and Department of Medicine of SCB Medical College and Hospital. All geriatric patients (?60 years) with ADR, reporting to the above departments, were included in the study. The detailed information of type of ADRs and its characteristics was filled up in Suspected ADR Reporting Form designed by Indian pharmacopeia commission version 1.3. The incidence and spectrum of metabolic ADRs were observed. Their causality, severity, and preventability were evaluated by the WHO-UMC System, Hartwig’s Severity Scale, and Schumock Thornton Preventability Scale, respectively. Result: A total of 236 ADRs were reported in 2 years, out of which 28.8% were metabolic ADRs. Metabolic ADRs were more reported in young old patients and often encountered ADR was hypoglycemia 73.5% due to use of oral anti diabetics. About 29% of the metabolic ADRs had a causal relation of probable association. About 94.2% were moderate in severity and 89.7% metabolic ADRs could be probably preventable. Conclusion: There was a probable causal association of metabolic ADR with the drug used and they were moderate verity. The metabolic ADRs found in our study can be preventable with appropriate means.

Background: Cutaneous adverse drug reactions (CADRs) range from mild-to-severe types and occasionally can become fatal. Hence, these incur additional financial burden both to patients and community. Aim and Objective: The aim of the study was to describe the characteristics of CADRs reported to ADR monitoring center (AMC) of a tertiary care center. Materials and Methods: CADRs reported to the AMC over a period of 2 ½ years were retrospectively studied. This study mainly focused on affected age group, gender, various pattern of CADRs, the group and name of drugs causing CADRs, and severity and causality assessment. Results: CADRs contributed 31.6% of the total ADRs reported to the AMC. Among these, 51.7% were females and 40% were of 51–60 years age group. About 37.9% of CADRs were pruritus. Antibacterial drugs were the most common cause of CADRs and beta-lactam antibiotics were responsible for 30% of CADRs. Stevens Johnson syndrome (SJS) constituted 4.9% of CADRs and 20% of this was due to Paracetamol. Drugs were withdrawn in 89% of cases and 85% cases recovered. On causality assessment, 94% were of probable category. Conclusion: Pruritus was the most commonly observed CADR and antibacterial drugs were the most common cause. Beta lactam antibiotic was the most frequent antibacterial drug to cause CADRs. The most common serious CADR was SJS and Paracetamol was the most frequent culprit drug.

Background: Lack of knowledge of pharmacovigilance and underreporting of adverse drug reaction (ADR) are worldwide problems of big concern. Gross underreporting of ADR in India led to increased hospital admissions, increase in length of hospital stay, and increase in healthcare cost burden. Aims and Objectives: This study was planned to assess knowledge, attitude, and practice on ADR and pharmacovigilance among nursing staff in Gandhi Medical College and Associated Hospitals, Bhopal Madhya Pradesh. Materials and Methods: A questionnaire-based, cross-sectional, and observational study was conducted among nursing staff in a tertiary care Centre. Data were collected in the form of a validated questionnaire comprised of 17 questions regarding knowledge, attitude, and practice of nursing staff toward ADR and Pharmacovigilance. Data were analysis by descriptive statistics and result was computed. Results: Around 33% of respondents were aware of Pharmacovigilance program of India and 99% of them had knowledge about main objective of pharmacovigilance. Out of total filled questionnaire received, only 12% nurses practiced reporting ADR. The major factor discouraging participants from reporting ADR was lack of time to report. The majority of nursing staff had a positive attitude toward awareness of pharmacovigilance and ADR reporting. Conclusion: Our study has shown that there is lack of correct knowledge regarding ADR reporting and pharmacovigilance among staff nurses. Although, the attitude toward ADR reporting was adequate, the actual practice of ADR reporting is unsatisfactory. Background: For decades, it has been observed that mental health is shrouded in stigma and discrimination. The scope, severity, and expense of impairment and costs to people, families, and societies are staggering. Mental illnesses are among the most frequent illnesses, affecting over a quarter of the population in any given year. According to national institute of mental health and neurosciences, Bangalore, the prevalence of schizophrenia has been considered as 4/1000 for all ages and both sexes. Aim and Objectives: The objectives of this study were to as follows: (1) To evaluate adverse drug reactions (ADRs) in patients with schizophrenia who received antipsychotic treatment and (2) to compare ADRs in typical versus atypical antipsychotic agents in schizophrenic patients. Materials and Methods: A total of 50 schizophrenic patients were enrolled for evaluating adverse effects to antipsychotic drugs. During the research, all ethical precautions were taken. All patients were followed up by medical history, history of drugs, and any severity of adverse drug reaction. Causality assessment was graded by Naranjo scale. Result: Among all of the antipsychotic drugs, risperidone (05%), quetiapine (04%), and aripiprazole (04%) have shown lowest propensity to cause serious adverse event. These drugs are most commonly prescribed drugs and are least likely to affect quality of life of patient. However, the risk of extrapyramidal symptoms is lower with olanzapine (05%) than haloperidol (34%) and even in case with risperidone at higher dose (20%). Although atypical antipsychotics such as olanzapine (46%) have shown maximum potential to produce metabolic side effect such as dyslipidemia and hyperglycemia compared to that of other antipsychotics. Conclusion: The most common adverse effects were found with typical and atypical antipsychotics such as weight gain, drowsiness, constipation, sedation, dyslipidemia, and hypotension.
