RESUMO

Objective:To analyze the results of the joint screening of newborn hearing and deafness genes in Dalian to provide a reference for the prevention and control of hereditary deafness. Methods:Eight hundred and forty-two neonates born in Dalian Women and Children′s Medical Group from January 1, 2022 to May 30, 2022 were screened retrospectively, using AABR (automatic brainstem evoked potential). And 20 mutation sites of common genetic deafness 4 genes , including GJB2, GJB3, SLC26A4 (PDS) and mitochondrial genes associated with drug-induced deafness (MT-RNRI)(12SrRNA), were detected by high-throughput sequencing. Results:Among the 842 newborns, 840 passed hearing screening (99.8%); 36 cases (4.3%) passed the hearing screening but not the hearing loss gene screening; 804 cases passed through the both screening (95.5%); 2 cases (0.24%) failed in the both screening. 38 cases of deafness gene mutations were detected, with a total carrying rate of 4.51% (38/842). Among them, the carrying rates of heterozygous mutations in GJB2, GJB3, SLC26A4 (PDS), MT-RNRI (12SrRNA) were 1.90%, 0.24%, 1.30%, and 0.95%, respectively. The carrying rates of GJB2/GJB3 composite heterozygous mutations were 0.12%.Conclusions:The combined screening of neonatal hearing and deafness genes can reduce the missed rate of hearing screening. The carrier rate of neonatal deafness gene in Dalian is 4.51%, with the highest GJB 2 carrier rate, followed by SLC26A4 (PDS) carrier rate.

RESUMO

@#Objective    To verify the feasibility of a self-designed magnetic anchoring and traction device (MATD) for assisting two-port video-assisted thoracoscopic esophagectomy. Methods    Three Beagle dogs were selected as animal models with age ranging from 1-6 years and weight ranging from 8-12 kg, and they underwent two-port video-assisted thoracoscopic esophagectomy after general anesthesia. We used the MATD to retract the esophagus to different directions, which assisted mobilizing esophagus, detecting the nerves along esophagus and dissecting paraesophagus lymph nodes. The operation time, blood loss and feasibility of the MATD were recorded. Results    With the aid of the MATD, we successfully retracted and mobilized the esophagus, detected the nerves and dissected the lymph nodes in three Beagle dog models. During the operation, the MATD provided sufficient and steady traction of esophagus to achieve a good exposure of the operative field, effectively decreasing the interference between working instruments. The MATD worked well. The mean operation time was 30 min, and the mean intraoperative blood loss was about 10 mL. Conclusion    It is effective to use the MATD to assist retracting esophagus during video-assisted thoracoscopic esophagectomy. The magnetic anchoring and traction technique can assist to expose the surgical field, decrease the interference between the working instruments and have the potential clinical application.

ABSTRACT: In orthodontics, miniplates are used as a Temporary Anchoring Device (TAD) for the purpose dental movements, allowing the use of orthopedic forces. In comparison with mini-screws, miniplates have the advantage of a very low rate of failure. Nonetheless, their removal requires the same surgical procedure as during installation, which is an obvious disadvantage. The aim of this study is to review the indications of miniplates in patients with open bite, class II and class III anomalies, and review how miniplates improved orthodontics treatments. Information was obtained by a search in PubMed with the keywords listed below. Miniplates are indicated for retraction in mass of the arcade, where it was seen that the force of 150 g applied on maxillary molars, is sufficient not only to push the molars back into a corrected class I, but also to initiate retraction of premolars, canines, and incisors. In open-bite patients, mini plates, are achieved as a safe method, that is quick and a less expensive alternative to orthognathic surgery. Further, in class II and III patients they are used without producing dentoalveolar effects replacing extraoral devices as facemasks, with intraoral devices and elastics. (BAMP).

This paper aims to present the main theoretical and conceptual aspects of two theories: Collective Memory, a term coined by Maurice Halbwachs (1877-1945) and Social Representations Theory, developed by the psychologist Serge Moscovici (1928-2014), and how both fields converge, considering the sociological influence of Emile Durkheim (1858-1917). This study brings together theoretical and conceptual elements of Durkheim´s thought, which can broaden the discussion about the connections between the theoretical perspectives presented. Under this epistemological approach, the discussion between Collective Memory and Social Representations Theory has been little emphasized in the scientific productions that have proposed to concatenate the two theories, which brings originality to the present work. It is noticeable the similarity between the anchoring process of Moscovici and the social frames of memory, both of which depend on relational processes for the social construction of daily practices and the ability to theorize social reality.

RESUMO

ABSTRACT Introduction: The T-loop as designed by Burstone is a space closure spring used in the rational application of orthodontic biomechanics. Despite the diversity of studies, there is still no consensus on the optimal parametric characteristics for its conformation. Objective: This study aimed at reviewing the literature on the force systems released by different conformations of the T-loop, according to the type of anchorage and the main characteristics and factors that influence them. Results: Comparing the studies, the need for standardization was perceived in the methodology to shape the loops, regarding the variables that influence the force system. Most of the experimental studies with this loop do not report the vertical movement, nor the steps and angles that occur in the brackets. Clinical studies have obtained more variable results in relation to vertical acting forces, considering the influence of chewing. Conclusion: There is great potential for future studies with this type of loop, especially using nickel-titanium alloys, in order to achieve a pure translational movement without friction, with optimal and constant levels of force.

Objective To investigate the roles of A kinase anchoring protein1(AKAP1)in high-glucose induced mitochondrial fission in podocytes. Methods Conditionally immortalized human podocytes were cultured in serum-free medium for 24 hours, and then exposed to different glucose concentration conditions in different time periods. The protein expressions of AKAP1 were observed by immunofluorescence, and AKAP1,dynamin related protein1 (Drp1) and phospho Ser 637-Drp1 (p-Drp1)were analyzed by Western blotting.AKAP1 siRNA was transfected to block AKAP1 expression.Podocytes were then divided into normal control group (5 mmol/L glucose),hypertonic group (30 mmol/L mannitol+5 mmol/L glucose),high glucose group (35 mmol/L glucose),and high glucose+AKAP1 siRNA group.Mitochondrial morphological changes were assessed by mitotracker red staining.Podocyte apoptosis was assessed by flow cytometry. Results Compared with normal group,high-glucose induced more podocytes apoptosis (P < 0.05),more mitochondrial fission with decreased aspect ratio and form factor (all P < 0.05).Upregulated AKAP1 protein level, and increased ratio of p-Drp1/Drp1 (all P < 0.05) in time and concentration dependent manners were also obscrvcd.Compared with high glucose group, transfection of AKAP1 siRNA showed less apoptosis (P < 0.05),less mitochondrial fission with increased aspect ratio and form factor (all P < 0.05),and down-regulated AKAP1 protein level as well as p-Drp1/Drp1 ratio (all P < 0.05).Conclusion High glucose induced mitochondrial fission might be induced through AKAP1-Drp1 pathway.

ABSTRACT Based on the non-consensual approach of the Theory of Social Representations, the present study aimed to investigate the processes of social and psychological anchoring regarding the social representations of the gypsy woman, dimension which allows analyzing the modulation of the social objects from the interindividual positioning and the experiences shared by subjects of the representation according to their sociocultural context of insertion. The study included 643 non-gypsy, Brazilian and Italian individuals with mean age of 22.81 years (SD=5.73). Once lexical correspondence analysis was conducted, the process of psychological anchoring indicated the formation of eight clusters, which characterize different representations, according to the following dimensions: magical-religious, maternity and care of the family-group, social exclusion, and female figures. Through the analysis of the social anchoring process, we verified that variables such as contact, gender, and nationality act in the modulation of these different representations over the object, producing ambiguities that have historically led to processes of discrimination against gypsy groups.

RESUMO

Apoptosis, especially the intrinsic mitochondrial cell death pathway, is regulated by the BCL-2 family of proteins. Defects in apoptotic machinery are one of the main mechanisms that cells employ to evade cell death and become cancerous. Targeting the apoptotic defects, either by direct inhibition of BCL-2 family proteins or through modulation of regulatory pathways, can restore cell sensitivity to cell death. This review will focus on the aspects of BCL-2 family proteins, their interactions with kinase pathways, and how novel targeted agents can help overcome the apoptotic blockades. Furthermore, functional assays, such as BH3 profiling, may help in predicting responses to chemotherapies and aid in the selection of combination therapies by determining the mitochondrial threshold for initiating cell death.

RESUMO

Secure reconstruction of multiple hepatic ducts that are severely damaged by tumor invasion or iatrogenic injury is a challenge. Failure of percutaneous or endoscopic biliary stenting requires lifelong placement of one or more percutaneous transhepatic biliary drainage (PTBD) tubes. For such difficult situations, we devised a surgical technique termed cluster hepaticojejunostomy (HJ), which can be coupled with palliative bile duct resection. The cluster HJ technique consisted of applying multiple internal biliary stents and a single wide porto-enterostomy to the surrounding connective tissues. The technique is described in detail in the present case report. Performing cluster HJ benefits from three technical tips as follows: making the multiple bile duct openings wide and parallel after sequential side-to-side unification; radially anchoring and traction of the suture materials at the anterior anastomotic suture line; and making multiple segmented continuous sutures at the posterior anastomotic suture line. Thus, cluster HJ with radial spreading anchoring traction technique is a useful surgical method for secure reconstruction of severely damaged hilar bile ducts.

RESUMO

BACKGROUND: Despite the benefits of continuous peripheral nerve blocks, catheter dislodgment remains a major problem, especially in the ambulatory setting. However, catheter dressing techniques to prevent such dislodgment have not been studied rigorously. We designed this simulation study to test the strength of two commercially available catheter dressings. METHODS: Using a cadaver model, we randomly assigned 20 trials to one of two dressing techniques applied to the lateral thigh: 1) clear adhesive dressing alone, or 2) clear adhesive dressing with an anchoring device. Using a digital luggage scale attached to a loop secured by the dressing, the same investigator applied steadily increasing force with a downward trajectory towards the floor until the dressing was removed or otherwise disrupted. RESULTS: The weight, measured (median [10th–90th percentile]) at the time of dressing disruption or removal, was 1.5 kg (1.3–1.8 kg) with no anchoring device versus 4.9 kg (3.7–6.5 kg) when the dressing included an anchoring device (P < 0.001). CONCLUSIONS: Based on this simulation study, using an anchoring device may help prevent perineural catheter dislodgement and therefore premature disruption of continuous nerve block analgesia.

RESUMO

Glutamate acting on AMPA-type ionotropic glutamate receptor (AMPAR) mediates the majority of fast excitatory synaptic transmission in the mammalian central nervous system. Dynamic regulation of AMPAR by post-translational modifications is one of the key elements that allow the nervous system to adapt to environment stimulations. S-palmitoylation, an important lipid modification by post-translational addition of a long-chain fatty acid to a cysteine residue, regulates AMPA receptor trafficking, which dynamically affects multiple fundamental brain functions, such as learning and memory. In vivo, S-palmitoylation is controlled by palmitoyl acyl transferases and palmitoyl thioesterases. In this review, we highlight advances in the mechanisms for dynamic AMPA receptors palmitoylation, and discuss how palmitoylation affects AMPA receptors function at synapses in recent years. Pharmacological regulation of S-palmitoylation may serve as a novel therapeutic strategy for neurobiological diseases.

RESUMO

Objective To evaluate the clinical outcome of artificial cervical disc arthroplasty combined with anchoring cervical intervertebral fusion cage (ACIFC) in the treatment of multilevel cervical disc herniation. Methods From October 2011 to September 2013, 0.26 patients with multilevel cervical disc herniation underwent artificial cervical disc arthroplasty combined with ACIFC, there were 10 male and 16 female with age from 32 to 60 years (mean 46.8 years). Two segment were involved in 15 patients, three segment were involved in 11 patients. Neck disability index (NDI) and Japanese Orthopaedic Association (JOA) score were used to evaluate neurofunctional recovery preoperatively and postoperatively. The postoperative stabilization and the range of motion (ROM) of implanted disc and the fusion of cage were observed on dynamic radiograph. Results The average operation time and amount of bleeding were (95±15) min and (100±20) ml respectively. The average postoperative follow-up was 15 months(range 6-20 months). At the end of follow up,the scores of JOA was increased and the scores of NDI was decreased:(9.08±1.72) scores vs. (14.88±1.37) scores, (40.31±4.97) scores vs. (25.23±4.10) scores, there had significant differences (P0.05). Solid fusion was achieved in all levels brought by cage. There was no cage subsidence or displacement. All artificial cervical disc achieved definite stabilization, no intraoperative and postoperative complication was noted. Conclusions Artificial cervical disc arthroplasty combined with ACIFC provides a feasible way for the treatment of multilevel cervical disc herniation. The definite stabilization and maintained ROM can reduce the fusion segments and avoid the increased stress of adjacent segment which can ensure a good preliminary clinical outcome.

There are currently many devices used for molar distalization when there is a Class II malocclusion; some of these devices include headgear, tooth-supported and bone-supported pendulums, the distal jet, and the sliding jig, among others. All of them have undesirable side effects. This case report aims to illustrate the results obtained by using a device with cortical and bone anchorage to treat a Class II malocclusion of a patient with a skeletal and dental class II malocclusion. A Cortical Dual Forcé Distalizer (C-DFD, a cortical-anchoring appliance designed at CES University in Medellín, Colombia was used. This device was modified by using a couple of mini-screws to reinforce anchorage, thus achieving upper distalization of molars and avoiding treatment with extractions. The C-DFD is a bone-supported distalizer that achieves a more controlled and in-body distal movement of the first upper molar, avoiding the inclinations produced by other distalizers.

This study presents reanalyzes of Cepeda et al. (2008)'s data using an Item Response Theory (IRT) model and Multiple Correspondence Analysis (MCA). Both internal consistency (α = 0.96) and fit to the unidimensionality assumption (per parallel analysis) were found to be satisfactory. Coincidences between both analyses are shown. The percentage of students affected by bullying in the sample is similar to that found in other studies in Colombia. Finally, the questionnaire appears to be a valid and reliable measure of bullying.

RESUMO

Single-port video-assisted thoracic surgery (VATS) has slowly established itself as an alternate surgical approach for the treatment of an increasingly wide range of thoracic conditions. The potential benefits of fewer surgical incisions, better cosmesis, and less postoperative pain and paraesthesia have led to the technique's popularity worldwide. The limited single small incision through which the surgeon has to operate poses challenges that are slowly being addressed by improvements in instrument design. Of note, instruments and video-camera systems that are narrower and angulated have made single-port VATS major lung resection easier to perform and learn. In the future, we may see the development of subcostal or embryonic natural orifice translumenal endoscopic surgery access, evolution in anaesthesia strategies, and cross-discipline imaging-assisted lesion localization for single-port VATS procedures.
