RESUMO

We report a case of naturally occuring anti-Xga in a 22-year-old man suffering from multiple traumas, who had no past history of transfusion or transplantation. The positive reaction was detected at 37 degrees Cin the antibody screening test with LISS/Coombs card (DiaMed AG, Cressier, Morat, Switzerland), which disappeared at the enzyme phase with the NaCl/Enzyme card (DiaMed AG). The Xga antigen was negative on his red cell surfaces. The unexpected antibody was finally identified as anti-Xga. We found four of twelve units of packed red blood cells that were compatible with the patient's serum. Anti-Xga is a very rare unexpected antibody in Korea, so we need more study in order to determine incidence and the significance of this antibody.

RESUMO

A 49 years old male patient suffering from liver cirrhosis and diabetes mellitus was admitted with complaints of chest pain and hematemesis. Three years ago, the patient received a transfusion of unknown blood components, presumed platelet concentrates. During the pretransfusion testing, we found irregular antibodies with the antibody-screening test and identified the antibody as anti-Xga. An antiglobulin test was done as a part of a cross-match and all compatible units of packed red cells were transfused. No transfusion reaction was observed.

RESUMO

Anti-Xga is a rarely occurring antibody, and only two cases with this antibody have been reported in Koreans. We describe here another example of anti-Xga which was detected in a 47-year-old male. The patient suffered from burn injury on the 20% of body surface area, acute renal failure, and ulcer bleeding. Unexpected antibody identification test demonstrated anti-Xga, which was reactive only by antiglobulin phase, and the reaction disappeared after the treatment with bromelin. The red cell phenotype of this patient was Xg(a-). Fourteen units of packed red blood cell were transfused without any adverse reactions. It has an important clinical concern in safe transfusion practice to determine the specificity and the clinical significance of blood group alloantibodies.

RESUMO

Authors experienced a patient with anti-Xga antibody in the serum. To our knowledge, this is the first report of anti-Xga antibody in Korea. The patient was a 65-year-old male with chronic renal failure who had received many hemodialyses. Anti-Xga antibody has not been implicated in hemolytic disease of the newborn or hemolytic transfusion reactions, but the antibody can be misinterpreted as clinically significant antibody in crossmatching because it usually reacts only on antiglobulin testing. In this case, no transfusion reactions happened.
