RESUMO

Osteomesopyknosis is a rare sclerosing bone disorder of autosomal dominant inheritance. We report a first case of osteomesopyknosis in Korea. A 16-year old girl complained of diffuse back pain for 1 year. We performed physical examination, biochemical investigations and imaging studies. A radiograph of spine revealed rugger-jersey vertebra and sandwich vertebra. Bone specific alkaline phosphatase, osteocalcin and C-terminal telopeptides of type I collagen were normal. Only an axial skeleton involvement was shown on the whole body bone scan. This patient was diagnosed to have osteomesopyknosis. Osteomesopyknosis is characterized by normal level of bone turnover marker and an axial bone involvement. Osteomesopyknosis can be occurred in Korea and needs to be considered when patients, especially young patients, suffer from back pain and have only axial osteosclerosis.

Las enfermedades osteoesclerosantes que comprometen el esqueleto axial son un grupo de enfermedades poco frecuentes que se caracterizan por aumento en la densidad ósea, producto de alteraciones primarias en la osificación endocondral o membranosa. En este artículo, realizamos un análisis crítico de la clasificación de las osteoesclerosis axiales y proponemos una nueva clasificación con el objetivo de simplificar el abordaje de estas patologías. Así mismo, describimos las características clínicas, radiológicas y de laboratorio que permiten hacer su diagnóstico diferencial.


Osteosclerosant diseases with axial involvement are a rare group of disorders characterized by a high bone mineral density secondary to impairment in the endochondral and membranous bone formation process. In this article, we analyzed the current classification of these diseases and propose a new and simple approach. In addition, we describe the main radiologic, laboratory and clinical keys to arrive to the differential diagnosis.
